Intravascular Ultrasound-Guided Iliofemoral Venous Stenting: In-Stent Mural Thrombus as a Possible Early Sign of Occult Malignancy.

Dedicated venous stents became available in Japan only recently, and reports of their use and follow-up in Japanese practice remain scarce. A 77-year-old woman developed acute left iliofemoral deep vein thrombosis (DVT) after prolonged recumbency and was treated with edoxaban. Eighteen days later she underwent endovascular therapy of the right lower limb for chronic limb-threatening ischaemia, and aspirin was added. Limb swelling and pain persisted despite three months of anticoagulation and graduated compression, and imaging then showed chronic occlusion of the left common and external iliac

Machine learning-based prediction of post-thrombotic syndrome in unprovoked lower extremity deep vein thrombosis: a two-center study incorporating uric acid and clinical risk factors.

BACKGROUND: Post-thrombotic syndrome (PTS) is a common and debilitating complication after lower extremity deep vein thrombosis (LEDVT). Early risk assessment of PTS patients is still a clinical challenge. METHODS: The retrospective, multicenter cohort study included 265 patients with unprovoked LEDVT. Baseline clinical and biochemical data including serum uric acid, body mass index (BMI) and treatment delay were noted. Ten machine learning (ML) models (e.g., support vector machine [SVM], XGBoost and LightGBM) were constructed for predicting PTS occurrence in a 70:30 training-validation split

Monocyte depletion reduces late experimental post-thrombotic fibrotic injury in a stasis mouse model.

BACKGROUND: Post thrombotic syndrome is a fibrotic disease related to inflammation resolution. There are no direct therapies that can ameliorate this disease. Monocyte/macrophages (Mo/MØ) are the primary leukocyte involved with later venous resolution, and likely direct vein wall responses and healing. Herein, we explored the vein wall response with Mo/MØ depletion by two methods. METHODS: Using two mouse models of venous thrombosis (VT), complete stasis and a flow restricted model, Mo/MØ depletion was accomplished using CD11b-DTR mice administered diphtheria toxin, and clodronate micelle adm

Long-term outcomes of infrapopliteal versus popliteal venous access for percutaneous mechanical thrombectomy in acute deep vein thrombosis: A propensity score-matched retrospective cohort study.

BackgroundDespite anticoagulation, 50% of proximal deep vein thrombosis (DVT) cases progress to post-thrombotic syndrome (PTS). While percutaneous mechanical thrombectomy (PMT) reduces thrombotic burden, optimal venous access selection (popliteal vs infrapopliteal) remains unestablished. This study compares long-term outcomes between these approaches.MethodsA retrospective cohort of 56 acute DVT patients (28 per group) underwent propensity score matching for age, sex, symptom duration, and thrombus extent. All received PMT via either infrapopliteal access or popliteal access at a tertiary cent

Midterm clinical outcomes of mechanical versus rheolytic thrombectomy for iliofemoral or iliocaval deep vein thrombosis.

OBJECTIVE: In patients with iliofemoral deep vein thrombosis (DVT), early thrombus removal reduces the risk of post-thrombotic syndrome (PTS). It remains uncertain if mechanical thrombectomy (MT) using the ClotTriever system may offer advantages as compared with rheolytic thrombectomy (RT) using the AngioJet ZelanteDVT. METHODS: In our multicenter, retrospective, observational study, we included 122 patients (mean age, 48 years; 57% women) with iliofemoral (78%) or iliocaval DVT (22%). All underwent early thrombus removal with either MT (n = 40) or RT (n = 82) and had a minimum of 3 months of

Postthrombotic syndrome: risk after deep vein thrombosis and estimates of its incidence and prevalence in Europe.

BACKGROUND: Postthrombotic syndrome (PTS) is a common complication of deep vein thrombosis (DVT) of the lower extremities, associated with reduced quality of life and high health care costs. No reliable estimates exist of PTS risk by DVT location and its burden in the general population. OBJECTIVES: This study estimated the risk of PTS by anatomical location of DVT and the age- and sex-specific incidence and prevalence of PTS in Europe. METHODS: We performed a systematic review and mixed-effects meta-analysis of studies reporting the cumulative incidence of PTS defined by Villalta score in p

[Acute venous disorders: deep vein thrombosis and superficial vein thrombosis of the lower extremity : Diagnosis and treatment in dermatology practice].

BACKGROUND: Deep vein thrombosis (DVT) is associated with potentially serious complications such as pulmonary embolism (PE) and postthrombotic syndrome. Superficial vein thrombosis (SVT) is also clinically relevant, as concomitant DVT and/or PE may already be present at initial diagnosis or may develop during follow-up. OBJECTIVE: Which diagnostic and therapeutic steps are most relevant for dermatologists in clinical practice? METHODS: Overview of the German S2k guideline and selected literature, supplemented by two case vignettes. RESULTS: DVT diagnostics are guided by clinical pretest pro

[Postthrombotic syndrome: an update].

Postthrombotic syndrome (PTS) is the most common chronic late complication after deep vein thrombosis (DVT): approximately 20-50% develop symptoms and a severe disease course with venous ulceration occurs in roughly 5-10%. The trigger is persistent venous hypertension caused by residual obstruction from incomplete recanalization and/or valve destruction leading to reflux; inflammatory processes further damage the vein wall and valves. Clinically, patients report heaviness, pain, tightness, edema, cramps and skin changes (hyperpigmentation, eczema, lipodermatosclerosis), potentially progressing
