Untangling profiles of postthrombotic syndrome using unsupervised machine learning.

Postthrombotic syndrome (PTS) is a chronic condition that can develop after deep vein thrombosis (DVT) and is diagnosed using the Villalta scale. This study applied unsupervised machine learning to investigate the heterogeneity of PTS among patients and within the Villalta scale. In 818 patients from the IDEAL DVT study, clustering identified 4 clinical profiles: (1) younger patients with provoked DVT, (2) women with joint pain, (3) men with isolated popliteal DVT, and (4) older men with diabetes and femoral vein involvement. Clustering of Villalta items revealed a distinction between signs an

Utility of venoactive compounds in post-thrombotic syndrome: A systematic review.

BACKGROUND: Post-thrombotic syndrome (PTS) has been highly prevalent; over 50% of the patients develop PTS after lower extremity acute deep vein thrombosis. Venoactive compounds (VACs) have been recommended for decades for patients with chronic venous insufficiency, including PTS. The objective of our study was to perform a systematic review to determine the quality of evidence on the utility of VACs for both prevention and treatment of PTS. METHODS: A systematic review was conducted to search the literature between January 1, 1980, and July 14, 2023, for venoactive drugs or medications, deep

Endovascular Exclusion of a Refluxing Segment of Femoral Vein in Post-Thrombotic Syndrome, Characterized by a Vicious Shunt with the Main Upward Draining Pathway.

PURPOSE: To describe a novel endovascular technique to treat ilio-femoral reflux in post-thrombotic syndrome (PTS). It consists of regulated exclusion of a femoral vein (FV) segment with indication based on Doppler ultrasound (DUS) scan and ascending and descending venogram. TECHNIQUE: Through lower limb venous DUS, we identify a post-thrombotic refluxing FV, characterized by a duplicated FV and/or ascending collateral veins draining into a common trunk with a re-entry point into the refluxing FV itself. The above findings indicate a second-level venogram. If the descending phase confirms the

[Deep vein thrombosis in the upper extremities focusing on Paget-Schrötter syndrome].

Among the three main forms of upper extremity deep vein thrombosis (UE-DVT), catheter-related UE-DVT has the highest incidence, cancer-associated UE-DVT the highest mortality, and primary UE-DVT (Paget-Schrötter syndrome) the most varied treatment proposals. This review finds that nticoagulant treatment (AC) for 3-6 months leads to symptom freedom or mild postthrombotic syndrome in most patients. Still, several studies show better results after surgical treatment if performed less-than 6 weeks after symptom onset. A randomized study of AC alone versus AC + surgery less-than 6 weeks in patients

Editor's Choice - A Core Outcome Set for Clinical Studies on Chronic Venous Disease Involving the Deep Veins.

OBJECTIVE: Chronic venous disease (CVD) is a debilitating disease that results in significant morbidity and costs. A lack of standardised outcome reporting has made it difficult to evaluate the impact of interventions for CVD involving the deep veins. This study aimed to develop a core outcome set (COS) for studies evaluating interventions for this subset of CVD. METHODS: The COS was developed using the Core Outcome Measures in Effectiveness Trials (COMET) methodology. A systematic review and interviews with 19 patients experiencing post-thrombotic syndrome after deep vein thrombosis generate

Association of post-thrombotic syndrome with metabolic syndrome and inflammation - a systematic review.

INTRODUCTION: Post-thrombotic syndrome (PTS) is a chronic complication of deep vein thrombosis (DVT). Given its impact on vascular health, understanding risk factors for the development of PTS, as well as conditions such as metabolic syndrome that may contribute to vascular inflammation, is crucial. Metabolic syndrome is a constellation of factors that increase cardiovascular disease risk, insulin resistance, diabetes mellitus (DM), and cerebrovascular disease. Despite the established connection between metabolic syndrome and venous thromboembolism (VTE), the association between metabolic synd

Ultrasound Guided Needle Penetration Technique for Recanalisation of Short Segment Chronic Femoral Vein Occlusion.

INTRODUCTION: This case report presents an ultrasound guided needle penetration technique as a novel method for the recanalisation of short segment chronic femoral vein occlusion in a patient with a history of intravenous drug use. CASE REPORT: A 41 year old male presented with a painful calf ulcer. Initial ultrasound imaging identified a chronic short segment occlusion in the proximal femoral vein, with extensive scarring from repeated intravenous drug injections. Traditional endovascular approaches were unsuccessful due to dense tissue scarring. The ultrasound guided needle penetration tech

Three-Year Outcomes of Surgical Implantation of a Novel Bioprosthetic Valve for the Treatment of Deep Venous Reflux.

BACKGROUND: Chronic venous insufficiency (CVI) affecting the deep venous system is most often caused by postthrombotic syndrome (PTS), which leads to reflux, blood pooling, and venous hypertension in the lower extremities. Researchers have sought to restore deep valvular competence, with numerous attempts at primary repair and valve replacement yielding limited results. This article presents the mid-term outcomes (3 years) of the first-in-human study of the bioprosthetic VenoValve surgically implanted in patients with CVI. METHODS: The VenoValve is a 10-mm × 21-mm monocusp bioprosthetic valve

A Novel Approach to an Under-Recognized Entity: Percutaneous Thrombectomy of IVC Filter Thrombosis Complicated With Pulmonary Emboli Using the INARI FlowTriever Device: A Case Report.

Inferior vena cava (IVC) thrombosis, a rare complication of IVC filter insertion, is usually managed with anticoagulation, often insufficient for extensive thrombus. Surgical thrombectomy, effective but invasive, is less favored. Endovascular options like the Inari FlowTriever system are rising in popularity for their mechanical thrombectomy capability without requiring fibrinolytics or ICU admission.

Revisiting the Open Vein Hypothesis to Reduce the Postthrombotic Syndrome: Implications for Multidisciplinary Care and Research: A Scientific Statement From the American Heart Association.

The "open vein hypothesis" postulates that early thrombus clearance and restoration of venous blood flow may prevent postthrombotic syndrome after proximal deep vein thrombosis. Since its proposal several decades ago, new insights from basic and clinical studies have motivated a re-evaluation and refinement of this hypothesis. According to data from these studies, susceptibility to postthrombotic syndrome occurs as a result of differences in genetic composition, thrombophilic conditions, predilection to inflammation and fibrosis, endogenous fibrinolytic capability, timing of symptom presentati

Improvement of rheolytic thrombectomy for acute deep vein thrombosis of the whole lower limb by primary popliteal vein thrombosis clearance: protocol for a prospective, multicenter, randomized controlled trial (the Reformation study).

INTRODUCTION: Pharmacomechanical thrombectomy (PMT) can be a useful treatment for restoring vein patency quickly, especially for extensive acute deep vein thrombosis (DVT). However, previous evidence failed to validate the effectiveness of PMT in reducing the incidence of post-thrombotic syndrome (PTS). To address this controversy, the reformation study aims to improve rheolytic thrombectomy for acute DVT of the lower limb through primary popliteal vein thrombosis clearance. METHOD AND ANALYSIS: Reformation is a prospective randomised multicentre trial. It has 160 patients in two groups: the
