940-nm Photobiomodulation Enhances ADSC-Mediated Tracheal Cartilage Repair through TRPV1-Associated Calcium Signaling and Mitochondrial Redox Remodeling.

BACKGROUND: : Azathioprine and mercaptopurine are commonly used in chronic active Crohn's disease. They share the disadvantage of a delayed onset of action and potentially serious side-effects, and are metabolized to thioguanine nucleotides which are thought to be the active metabolites. The direct use of 6-thioguanine may offer a more rapid and safer alternative. We conducted an open prospective study to investigate the efficacy and safety of 6-thioguanine in chronic active Crohn's disease. METHODS: : Thirty-seven patients with chronic active Crohn's disease and a Crohn's disease activity in

Kikuchi-Fujimoto Disease: A Rare Cause of Fever and Generalized Lymphadenopathy.

Kikuchi-Fujimoto disease (KFD), recognized as histiocytic necrotizing lymphadenitis, is an uncommon, non-threatening, and self-resolving condition mainly seen in young adults. It frequently appears with cervical lymphadenopathy and fever, often resembling infectious, autoimmune, or malignant disorders like lymphoma. KFD poses a diagnostic challenge because of its nonspecific clinical and radiological characteristics, potentially resulting in unwarranted tests and intense treatments. We report the case of a 25-year-old male patient who presented with a 12-day history of ongoing high-grade fever

IgA Vasculitis Presenting As Isolated Monoarthritis Mimicking Transient Synovitis: A Diagnostic Challenge With Delayed Purpura.

IgA vasculitis (IgAV), previously known as Henoch-Schönlein purpura, is a small-vessel vasculitis that may involve the skin, joints, gastrointestinal tract, and kidneys. While purpura is a key clinical feature, presentations without rash at onset can occur and may lead to diagnostic uncertainty. We report the case of a two-year-old boy who presented with a three-day history of acute right knee pain, swelling, and reduced weight-bearing ability following a recent febrile respiratory illness. Ultrasound of the knee demonstrated mild periarticular fluid, and a provisional diagnosis of transient s

Hepatic hilar tuberculosis lymphadenitis diagnosed by endoscopic ultrasound-guided fine needle aspiration: a case report and literature review.

BACKGROUND: Hepatic hilar tuberculosis lymphadenitis is a rare condition that often mimics other diseases, leading to diagnostic challenges due to its nonspecific clinical presentation. It can be easily mistaken for a malignant tumor, complicating the diagnostic process. CASE PRESENTATION: A 34-year-old woman was incidentally found to have a space-occupying lesion in the porta hepatis during a routine physical examination. Imaging revealed an irregular mass located posterior to the portal vein and in the head of the pancreas. Endoscopic ultrasonography-guided fine-needle aspiration (EUS-FNA)

Diagnostic Imaging of Extrapulmonary Tuberculosis Across Organ Systems.

Extrapulmonary tuberculosis (EPTB) is an infectious disease characterized by the invasion of Mycobacterium tuberculosis beyond the lungs. Diagnosis is frequently delayed due to nonspecific clinical presentations that vary by organ system, making diagnostic imaging essential for disease detection, characterization, and treatment monitoring. The objective of this review is to examine and summarize imaging-based approaches for the diagnostic evaluation of EPTB across multiple body systems, including the central nervous system, spine, cardiovascular system, lymphatic system, abdominal and hepatic

Study on the Association Network of Tuberculosis Lesions in Adult Extrapulmonary Tuberculosis in China: A Large-Scale Multicenter Observational Study.

Background: Extrapulmonary tuberculosis (EPTB) is a significant health problem which can lead to severe morbidity and mortality. In clinical practice, EPTB can have a variety of nonspecific clinical manifestations and can be concurrent with other types of EPTB. As information pertaining to concurrent EPTB is scarce, research efforts are needed to find concurrent EPTB types and to explore the association networks and rules of concurrent EPTB. Materials and Methods: An observational multicenter study was carried out at 21 hospitals from 15 provinces in China from Jan 1, 2011, to Dec 31, 2017. Al

Mediastinal Lymphadenopathy Presenting With the "Medu Vada Sign".

The "Medu Vada" (or "doughnut") sign is a compelling radiological finding often noted on axial chest CT scans, characterized by a circular or ring-like appearance. This distinct morphology arises from enlarged mediastinal adenopathy, specifically involving lymph nodes such as those in the subcarinal or right paratracheal areas, which encircle the adjacent airway or esophagus. We report the case of a 55-year-old female who presented with a constellation of non-specific constitutional symptoms, including chronic cough, significant weight loss, diminished appetite, and recurrent evening fevers. Practical Imaging Approach to Determining the Cause of Nonneoplastic Lymphadenopathy.

In the daily clinical practice of radiologists, unexpected lymphadenopathy is frequently encountered, the majority of which is nonneoplastic. The causes of nonneoplastic lymphadenopathy are variable, and nonspecific histologic findings may challenge the accurate diagnosis. Therefore, inferring or identifying the cause based on imaging findings carries substantial clinical relevance. The causes of nonneoplastic lymphadenopathy can be broadly categorized as follows: (a) infections that lead to lymphadenitis; (b) systemic disorders such as sarcoidosis, Kawasaki disease, rheumatoid arthritis, syst

Fine needle aspiration diagnosis of Kikuchi's lymphadenitis in axillary lymphadenopathy: A rare condition with an unusual presentation site.

Kikuchi's lymphadenitis (KL) is a benign, self-limited, reactive condition with unknown etiology, usually seen in young women of Asian descent. It is most commonly seen in the cervical lymph nodes; however, in rare cases, axillary lymph nodes are involved. Cytological diagnosis is possible with adequate lymph node sampling by fine needle aspiration in the proper clinical setting. Pathologists face some difficulties in making the diagnosis of KL due to overlapping findings among other conditions, such as tuberculosis, lupus lymphadenitis, non-specific reactive conditions, and even malignant lym

A Rare Differential for Myalgia and Fever Associated With Cervical and Axillary Lymphadenopathy Presenting via Same Day Emergency Care.

Kikuchi-Fujimoto disease (KFD) is a rare, self-limiting necrotising lymphadenitis, mainly affecting young adults, and commonly presenting with fevers and lymphadenopathy. Diagnosis is confirmed by histopathological examination via lymph node biopsy, and management is primarily supportive with non-steroidal anti-inflammatory drugs and/or a tapering course of steroids. We herein describe a case of a 23-year-old Caucasian female with severe KFD. She presented to the same day emergency care (SDEC) with a six-week history of systemic symptoms, including myalgia, arthralgia, fevers, and weight loss.

Paediatric Cervical Osteomyelitis With an Atypical Presentation: A Case Report and Literature Review.

Cervical osteomyelitis is an uncommon condition in children. Diagnosis can prove difficult due to non-specific symptoms. We describe a case of paediatric cervical osteomyelitis that initially manifested with neck spasms, later progressing to fever and lymphadenitis, illustrating the variable clinical course of the condition. Investigations showed elevated inflammatory markers. The initial blood culture was reported as a contaminant, with subsequent blood cultures remaining negative. Magnetic resonance imaging (MRI) confirmed the diagnosis of cervical osteomyelitis at the C3-C4 level. The patie

Abdominal Sarcoidosis: A Mystery Revisited.

Unexplained lymphadenopathy is an often-encountered symptom or abnormal finding during physical examinations in practice, and it can stem from a wide range of causes. These require multiple investigations to reach a diagnosis. One of the rare causes is Sarcoidosis, which is a multi-system disease characterized by the infiltration of various organs by non-necrotizing granulomas with intrathoracic involvement being the most common presentation. It has a global prevalence of approximately 5-64 cases per 100,000 people, depending on the region. Abdominal sarcoidosis accounts for approximately 30%

Oropharyngeal Tularemia: A Case From Rural Armenia Highlighting Diagnostic Challenges and Treatment Outcomes.

Tularemia, a life-threatening zoonotic infection caused by Francisella tularensis, is a significant public health concern. Diagnosis can be challenging due to nonspecific symptoms, including fever, tonsillitis, and lymphadenopathy. We present the case of a 48-year-old male from a rural area of the Tavush region admitted to our hospital, initially presenting with left-sided tonsillitis, followed by the development of enlarged lymph nodes in the neck and a mild fever. Despite initial treatment with amoxicillin, tularemia was confirmed through positive serological tests, including antibody titers

A Case of Necrotizing Lymphadenitis Misdiagnosed as a Neck Abscess.

Histiocytic necrotizing lymphadenitis (HNL), also known as Kikuchi disease, is a relatively rare sub-acute necrotic localized lymphadenitis. This benign, self-limiting condition is characterized by fever, painful lymphadenopathy, skin rash, and other systemic symptoms. Due to its nonspecific presentation, unclear etiology, and pathogenesis, HNL has a low incidence rate in clinical practice. Insufficient awareness among clinicians and pathologists can easily lead to misdiagnosis. This article reported a 27-year-old female patient who was admitted to the hospital with fever, neck pain, and enlar

Syphilis in Hematopathology Practice: A Diagnostic Challenge.

CONTEXT.—: Syphilis, a reemerging disease caused by the spirochete Treponema pallidum, is becoming more frequent in surgical pathology and hematopathology practices. Hematopathologists typically receive lymph node biopsies from patients with syphilis who have localized or diffuse lymphadenopathy. Occasionally, syphilis infection in the aerodigestive tract can show a prominent lymphoplasmacytic infiltrate and mimic lymphoma. Besides the varying and occasional atypical morphology, the fact that clinical suspicion tends to be low or absent when histologic evaluation is requested adds to the impor

Cat Scratch Disease of the Breast/Axilla: Recognition of a Rare Disease and Approaches for Differential Diagnosis.

Cat scratch disease rarely presents as a breast or axillary mass mimicking carcinoma both clinically and radiologically. Diagnosing breast/axillary cat scratch disease is challenging due to its rarity and nonspecific findings. Here, we reported 2 patients with breast cat scratch disease and reviewed 14 patients with cat scratch disease involving breast/axilla from the past 30 years. It mainly affects women (median age: 48), consistently presenting as axillary lymphadenopathy, and demonstrates ipsilateral breast mass in half of patients (50%, 8/16). The breast mass was most commonly located in
