Spectrum of cytological findings in patients with neck lymphadenopathy--experience in a tertiary care hospital in Pakistan.

INTRODUCTION: Lymph adenopathy is of great clinical significance as underlying diseases may range from a treatable infectious etiology to malignant neoplasms. In fact it is also essential to establish that the swelling in question is a lymph node. Fine needle aspiration cytology (FNAC) plays a vital role in solving these issues, nowadays being recognized as a rapid diagnostic technique because of its simplicity, cost effectiveness, early availability of results, accuracy and minimal invasion. FNAC is particularly helpful in the work-up of cervical masses and nodules because biopsy of cervical

Insight into the epidemiology of laboratory confirmed human tuberculosis in Ibadan, Nigeria.

Nigeria being one of the leading nations with the highest burden of tuberculosis (TB) urgently needs more impetus in the diagnosis of this disease judging from the increased incidence of human immunodeficiency virus/acquired immune deficiency syndrome (HIV/AIDS). As culture of mycobacteria provides the definitive diagnosis of TB, this study reviews its use in the confirmation of laboratory samples from patients presented at different clinics in Ibadan from April, 2004 to April, 2008. Fine needle aspirates, sputum and stool samples were collected from suspected tuberculous patients who were eit

Typhus and other rickettsioses: emerging infections in Germany.

BACKGROUND: Rickettsioses are diseases caused by rickettsiae, obligate intracellular bacteria that are transmitted by arthropods to humans. They cause various types of spotted fever and typhus. METHODS: A review of the literature is presented along with the authors' own findings. RESULTS: Six indigenous species of rickettsiae have been found in Germany to date, five of which have been described as human pathogens in other countries. Rickettsia slovaca causes tick-borne lymphadenitis (TIBOLA). Rickettsia helvetica is a known pathogen of nonspecific fever; its role in endocarditis is still und

Lymph node biopsies: a hospital based retrospective study.

INTRODUCTION: Lymphadenopathy is a common clinical finding at the outpatient setting that may be caused by a vast array of disease processes. METHODS: A retrospective study to analyze the histopathological diagnosis of lymph node biopsies with respect to age, sex, and site was conducted. All the lymph nodes sent for histopathological examination from August 2003 to 2007 were included. RESULTS: Of all patients with enlarged lymph nodes, 49% were males (mean age: 34.52 years) and 51% were females (mean age: 38.1 years). The most common diagnosis was tubercular lymphadenitis (42%), followed by

Familial tularaemia.

Tularaemia is a zoonotic disease caused by Francisella tularensis . In this report, we have presented an early stage case of tularemia with fever and pharyngitis and two cases from the same non-endemic region with typical lymphadenitis. All three patients were treated with non-specific medications in healthcare centres, the treatment being directed towards symptoms resembling those of upper respiratory tract infections. However, there was no regression in their complaints. Because the first case had been treated earlier, his lymphadenopaties regressed and there was no suppuration. The other tw

Left unilateral breast autoinflation and intraprosthetic collection of sterile pus: an unusual operative finding of silicone gel bleed with silicone lymphadenitis.

Implant rupture is a known complication after augmentation mammoplasty. Saline implant failure may present with few diagnosable problems due to the loss of volume. On the other hand, intracapsular silicone gel implant rupture or failure can be silent, asymptomatic, and difficult to diagnose in the absence of loss of breast volume. Silicone leak and migration to regional nodes has been reported but is uncommon and lymph node enlargement can be nonspecific in patients with intact prostheses. Causes, timing, and diagnosis of implant failure sites and mechanisms have been extensively studied and r

Necrotizing cervical lymphadenopathy: Kikuchi-Fujimoto disease in a young male.

Kikuchi-Fujimoto disease (KFD) or histiocytic necrotizing lymphadenitis is a rare benign disease, more common in Japanese women, with only a few cases reported from other parts of the world. The pathogenesis is poorly understood. A 22-year-old man noticed a node growth a month before his admittance. At first he noticed one enlarged node in the left axilla, later also the nodes on his neck and in his right axilla. Before that he had felt shivery and reported night sweats. Physical examination revealed bilateral enlarged (up to 2 cm) painful but not inflamed lymph nodes on the neck and in both a

The clinical importance of lymphadenopathy in systemic lupus erythematosus.

Lymphadenopathy (LAP) is a frequent and usually nonspecific feature of Systemic Lupus Erythematosus (SLE). In spite of the fact that LAP usually carries no risk to the patient, SLE patients with LAP were found to have higher disease activity levels. Besides the disease itself, other causes of LAP such as infections, immunological or malignant diseases must be considered in the differential diagnosis. LAP is a prominent clinical characteristic of Kikuchi-Fujimoto's disease which is associated with fever, arthralgia, and leukopenia, features also found in SLE. As the prognosis and treatment of K

Differentiation of sarcoidosis from tuberculosis using real-time PCR assay for the detection and quantification of Mycobacterium tuberculosis.

BACKGROUND: The clinical and pathological features of sarcoidosis and tuberculosis may mimic each other, and when the caseous necrosis is not seen in tuberculosis tissue, differentiation is not easy. OBJECTIVE: This study evaluates the ability of real-time PCR quantification and sets the quantitive value to differentiate sarcoidosis from TB. METHODS: Formalin-fixed and paraffin-embedded sections of biopsy samples, from 104 patients with sarcoidosis, 31 patients with tuberculosis, and 55 controls with other respiratory diseases (26 with nonspecific lymphadenitis and 29 with emphysema bullae),

Fine needle aspiration cytology (FNAC) and neck swellings in the surgical outpatient.

BACKGROUND: Fine Needle Aspiration Cytology (FNAC) is a simple, quick and inexpensive method that is used to sample superficial masses like those found in the neck and is usually performed in the outpatient clinic. It causes minimal trauma to the patient and carries virtually no risk of complications. Masses located within the region of the head and neck, including salivary gland and thyroid gland lesions can be readily diagnosed using this technique. The objective of this descriptive study was to see the frequency of various pathological conditions detected on FNAC in patients presenting with

[Cat scratch disease and other human infections caused by Bartonella species].

The different Bartonella species can cause various human infections such as cat scratch disease, chronic bacteremia (homeless patient with nonspecific symptom), endocarditis, bacillary angiomatosis, peliosis, and Carrion's disease. Diagnostic approaches include serology, culture and molecular approaches. PCR is especially useful on lymph nodes biopsies from patients with cat-scratch disease and on valvular samples taken from culture-negative endocarditis. Serology exhibits a very high sensitivity in the latter situation. The treatment should be chosen according to the clinical presentation.

Interleukin-6 and lipopolysaccharide-binding protein in acute appendicitis in children.

The aim of the study was to evaluate the diagnostic accuracy of interleukin-6 (IL-6) and lipopolysaccharide-binding protein (LBP) in children with acute appendicitis (AA) and to compare this with the diagnostic accuracy of routinely used C-reactive protein (CRP) and white blood cell (WBC) count. Eighty-two consecutive children admitted to our Department because of suspected AA were enrolled in this prospective study and classified into two groups: group 1 (49 children who underwent surgery for AA) and group 2 (33 children with no surgery with diagnosis of non-specific abdominal pain or sonogra

[Diagnosis and management of periodic fever with aphthous pharyngitis and adenitis (PFAPA)].

PFAPA is non-hereditary syndrome characterized by periodic episodes of high fever, aphthous stomatitis, pharyngitis and cervical adenitis. It manifests usually in early childhood, especially before 5 years of age, and last for several years. Its etiology is unknown, but some recent reports suggest a dysregulation of the immune response with continuous pro-inflammatory cytokine activation and a reduced anti-inflammatory response both during and between febrile attacks. The diagnosis is clinical and it is important to exclude other entities of similar presentation with periodic fever and orofaci

Aetiology of peripheral lymphadenopathy in adults: analysis of 1724 cases seen at a tertiary care teaching hospital in southern India.

BACKGROUND: In patients presenting with peripheral lymphadenopathy, excision biopsy of the most accessible lymph node provides material to establish an early diagnosis, and is important in the management of these patients. METHODS: A retrospective study was done of 1724 lymph node biopsy specimens obtained from adult patients and submitted for histopathological examination over a 12-year period. RESULTS: About one-third (n = 614; 35.6%) of these patients had non-specific lymphadenitis. This included a heterogeneous group of disorders comprising benign follicular hyperplasia, reactive hyperpl

Expression of BIRC7 protein and mRNA in non-Hodgkin's lymphoma.

Studies have shown that BIRC7, a new member of inhibitor of the apoptosis protein family, is expressed in fetal tissues and most solid tumors in humans. However, there are no reported data concerning BIRC7 expression in lymphomas. We investigated the expression of BIRC7, survivin, Bcl-2, Bax, p53 and p170 proteins in 167 cases of non-Hodgkin's lymphoma (NHL) and 10 cases of non-specific lymphadenitis by tissue microarray-based immunohistochemistry. BIRC7 mRNA in three cell lines and 16 cases of NHL were detected by reverse transcriptase-polymerase chain reaction. BIRC7 protein was exhibited in

Role of diffusion weighted MR in the discrimination diagnosis of the cystic and/or necrotic head and neck lesions.

PURPOSE: The purpose was to determine whether the diffusion weighted imaging (DWI) was able to differentiate necrotic tumor or metastatic lesions from infected necrotic lesions such as abscesses and necrotic lymphadenitis in the neck. MATERIALS AND METHODS: DWI was performed on 37 consecutive patients with 85 head and neck necrotic and cystic lesions. The lesions were classified into four categories: metastatic lymph node involvement including lymphoma, necrotic tumor, abscesses and necrotic lymphadenitis. Each lesion was histopathologically studied and proved. RESULTS: In 12 patients, there

[Kikuchi-Fujimoto disease (histiocytic necrotizing lymphadenopathy) clinicopathologic and immunohistochemical study of 14 cases and its differential diagnosis with other reactive and neoplastic necrotizing lymphadenopathies].

Kikuchi-Fujimoto disease also known as histiocytic necrotizing lymphadenopaty (HNL) is a rare entity, originally described in Japanese population, although currently it has been described all over the world. It is more frequent in young women and it is usually located in cervical lymph nodes. We report 14 cases of HNL in Mexican population, their clinicopathological and immunohistochemical study as well as a comparative study with other necrotizing lymphadenopaties due to B or T-cell lymphomas, tuberculosis, Epstein Barr virus infection, and non-specific necrosis. In our study we found that th
