Factors associated with lymphedema therapist willingness to refer to surgery.

PURPOSE: Treatment options for lymphedema have expanded in the last decade through the development of complex surgical interventions, whose innovations have transformed these options from a "last resort" to a potential way to restore lymphatic function, cosmesis, and reduce daily patient burden. Lymphedema therapists, who serve as a primary contact point for many individuals with lymphedema, may exhibit varying levels of acceptance of surgical intervention, potentially impacting patient education and access to these procedures. The current study seeks to identify factors associated with therap

Lymphedema: A Narrative Review of Recent Literature.

Lymphedema is a common condition often resulting from cancer treatment, among other causes, and poses significant impacts on the patient's quality of life. Given the lack of curative treatments for lymphedema as well as its detrimental and prevalent nature, much growth is needed in the medical understanding and management of this condition. This review seeks to explore the recent literature on lymphedema, focusing on the most novel research. The aim is to identify the latest research on etiology, diagnosis and monitoring, risk factors, prevention, and treatment of lymphedema. A narrative liter

NF-κB-driven lymphangiogenesis affects kidney function via a VEGFR-3-mediated pathway.

The lymphatic system maintains fluid homeostasis and orchestrates immune cell trafficking throughout tissues. While extensively studied in cancer and lymphedema, its role in nonlymphoid organs, particularly the kidney, remains an emerging area of investigation. Previous research established molecular connections among NF-κB, VEGFR-3, and PROX-1 in regulating lymphatic growth during inflammation, and studies using global knockout mice revealed that the NF-κB1 subunit (p50) influences lymphatic vessel density. However, the role of RelA - a key component of the canonical NF-κB heterodimer - in re

High-frequency ultrasound combined with microbubbles for preoperative lymphatic mapping for lymphedema with a non-linear pattern in indocyanine green lymphography.

OBJECTIVES: Lymphatic-venous anastomosis (LVA) is an effective surgical treatment for lymphedema, which requires accurate identification of lymphatic vessels. Indocyanine Green (ICG) lymphography, the most common method for lymphatic mapping, cannot always successfully identify lymphatic vessels. We aimed to explore high-frequency ultrasound (HFUS) and contrast-enhanced ultrasound (CEUS) as a reliable alternative for lymphatic mapping when ICG lymphography is not feasible. MATERIALS AND METHODS: We performed combined HFUS and CEUS for lymphatic mapping on the patients who exhibited no obvious

Self-management of lower limb lymphedema at home following gynecologic cancer surgery: a qualitative study of women's experiences and challenges.

INTRODUCTION: As a common chronic complication after gynecological cancer surgery, lower limb lymphedema significantly compromises patients' quality of life, both physically and psychologically. However, patients' adherence to home-based CDT, the internationally recommended first-line treatment, was frequently inadequate. This study aimed to explore the lived experiences and core challenges faced by women managing lower limb lymphedema at home after gynecologic cancer surgery. The insights gained can guide the formulation of improved nursing care strategies. METHODS: A descriptive phenomenolo

Integrated surgical treatment: a new model for treating secondary extremity lymphedema based on algorithms.

UNLABELLED: Secondary extremity lymphedema (SEL) is a chronic and progressive disorder resulting from impaired lymphatic drainage, most commonly following oncologic interventions such as breast or gynecological cancer surgery. Characterized by the accumulation of lymphatic fluid, progressive inflammation, adipose hypertrophy, and tissue fibrosis, SEL poses significant therapeutic challenges, particularly in its advanced stages. While conservative management remains the first-line treatment for mild cases, surgical intervention becomes essential in moderate to severe disease. Surgical approache

Impact of Body Mass Index on the Outcomes of Intensive Complex Decongestive Therapy for Lower Limb Lymphedema.

BACKGROUND: Complex decongestive therapy is recognized as the primary treatment of lymphedema. While the influence of weight on the lymphedema appearance and evolution is well known, the relationship between weight and complex decongestive therapy outcome remains underexplored. PURPOSE: The purpose of this research is to evaluate the impact of body mass index on the effectiveness of intensive complex decongestive therapy in patients with primary and secondary lower limb lymphedema. METHODS: We conducted a prospective study on 159 patients who underwent 551 complex decongestive therapy progra

Decreased Kinase Activity of the VEGFR3 Variant c.3175G>C Associated with Primary Lymphedema.

Vascular endothelial growth factor receptor 3 (VEGFR3) assumes a pivotal role in regulating the development and maintaining the structural integrity of the lymphatic system. Decreased activity of VEGFR3 can precipitate aplasia or hypoplasia of lymphatic system components, culminating in primary lymphedema. To date, numerous genetic variants have been identified within the FLT4 gene, which encodes VEGFR3; however, the majority of these remain uncharacterised and are classified as 'variants of uncertain significance'. In preceding investigations involving FLT4 sequence analysis conducted on indi

Extending eligibility criteria for sentinel lymph node biopsy in vulvar cancer: An update on the GroSNaPET study.

INTRODUCTION: Sentinel node biopsy (SNB) is the standard approach for nodal staging in clinically node-negative (cN0) patients with unifocal vulvar cancer ≤4 cm. Patients outside these criteria undergo lymphadenectomy, although many prove node-negative. The prospective GroSNaPET study evaluated SNB followed by lymphadenectomy in these patients. This report presents updated outcomes, long-term follow-up, and complication data. MATERIALS AND METHODS: This single-center prospective study included cN0 patients ineligible for standard SNB due to ≥1 of the following: tumor >4 cm, multifocality, pri

Establishment of a long-term secondary lymphedema animal model in the rodent hindlimb.

INTRODUCTION: Secondary lymphedema is a chronic swelling of the extremities caused by physical disruption of the lymphatic system, leading to impaired lymph drainage. It frequently develops in cancer patients after surgical removal of lymph nodes and vessels during tumor resection, when reconnection of lymphatic pathways fails. Current clinical therapies are mainly palliative or conservative, with limited therapeutic effects. Therefore, an animal model that closely mimics the chronic and severe characteristics of secondary lymphedema in patients is required to enable mechanistic and therapeuti
