Oedema: causes, management and relationship to wounds.

Oedema is a complex condition, and understanding its underlying causes is essential for safe and effective management. The most common aetiology is poor venous return, and venous disease develops when the veins are unable to withstand the hydrostatic pressure within them, leading to a cascade of tissue changes including lipodermatosclerosis, haemosiderin deposition and venous leg ulceration. The most disabling consequence is often secondary lymphoedema. Under normal circumstances, most interstitial fluid that is not reabsorbed at the venous end of the capillaries enters the lymphatic system an

Screening for Posttraumatic Lower Extremity Lymphedema: Patient Characteristics, Risk Factors, and Quality of Life Outcomes.

Lower extremity lymphedema (LEL) is a frequent, under-recognized, and chronic condition. The prevalence of this condition in patients with lower limb trauma is unclear. A self-reported lower extremity lymphedema screening questionnaire (LELSQ) was recently validated for screening LEL. The primary aim of this study was to investigate the clinical and demographic characteristics of posttraumatic LEL (PTLEL) in patients with a history of severe lower extremity trauma using the LELSQ. Secondary objectives were to identify risk factors for PTLEL, to investigate the relationship between LELSQ scores

Continuous Quadroiliac Plane Block for Postoperative Analgesia After Total Hip Arthroplasty: A Case Report.

The quadroiliac plane block (QIPB) is a novel posterior fascial plane technique targeting the quadratus lumborum fascia at the iliac crest. We describe a 68-year-old, 88-kg woman with breast cancer-related lymphedema who underwent total hip arthroplasty under general anesthesia and declined neuraxial anesthesia. A continuous QIPB was performed postoperatively using an epidural catheter inserted into the posterior thoracolumbar fascial plane. After an initial 40 mL of 0.25% bupivacaine, infusion continued at 10 mg/h. Sensory spread progressed from L2-L4 to L1-L5 within 1 hour. Pain scores remai

A retrospective cross-sectional study comparing health-related quality-of-life in females with lipoedema and bilateral leg lymphoedema.

BACKGROUND: Lipoedema is a condition of abnormal accumulation of painful adipose tissue, usually in the lower body of women. The disproportionate subcutaneous adipose tissue may negatively impact health-related quality of life (HRQoL). There are currently no patient reported outcome measures (PROM) specifically designed to assess the HRQoL in individuals with lipoedema. The aim of this study was to compare scores on validated lower limb lymphoedema PROMs between females with lipoedema and lymphoedema. METHODS: In a private lymphoedema clinic in Australia between 1 October 2021 and 22 August 2

Systemic Elimination Rather than Redistribution: Segmental Body Water Analysis after Leg Lymphovenous Anastomosis.

Lymphovenous anastomosis (LVA) is a microsurgical procedure that redirects stagnant lymph flow into the venous circulation, reducing fluid stasis in patients with secondary limb lymphedema. Although previous studies have reported that complex decongestive therapy redistributes fluid from the affected leg to other body segments, whether fluid reduction after LVA results from systemic elimination or intersegmental redistribution remains unclear. This study aimed to clarify the mechanism by analyzing the changes in segmental body water (SBW) volume using bioelectrical impedance analysis (BIA).We

Self-regulation in patients with cancer-related lymphedema: A concept analysis.

PURPOSE: Although sustained self-management behaviors are important for improving or preventing the worsening of cancer-related lymphedema (CRL), the definition of self-regulation, a core element of self-management behavior, remains unclear. These findings provide a foundation for the development of self-management behavior measurement scales, research advancements, and the development of effective interventions. This study aimed to define the concept of self-regulation in patients with CRL. METHODS: Walker and Avant's eight-step concept analysis method was used to clarify the attributes, ant

"Phoenix Rising": A scoping review on the impacts of dragon boating exercise on well-being among breast cancer survivors and factors affecting their participation.

OBJECTIVE: Breast cancer survivors (BCS) often experience declines in physical and psychosocial well-being post-treatment, making supportive interventions essential. Dragon boating(team-based paddling in a long boat) has emerged as a promising, popular exercise for BCS. However, no comprehensive reviews exist on its impacts or participation factors. This scoping review addressed these gaps by examining two research questions (RQ): RQ1) the impacts of dragon boating on BCS' well-being, and RQ2) facilitators and barriers influencing their participation. METHODS: Seven databases (PubMed, Embase,

Targeting excessive cholesterol deposition alleviates secondary lymphoedema.

Lymphoedema is a chronic debilitating disease caused by impaired lymphatic drainage and is characterized by tissue swelling, fat expansion, inflammation and fibrosis1,2. However, the exact mechanisms that drive lymphoedema are poorly understood. Although lymphatic vessels are known to transport cholesterol from peripheral tissues back to the systemic circulation3, the importance of impaired lymphatic drainage for cholesterol clearance in humans and its relevance to lymphoedema remain unknown. Here we show that lymphatic drainage insufficiency in human lymphoedema leads to excessive cholesterol

Assessing Epidermal Permeability in Lymphedema Skin with Sulfo-NHS-Biotin Immunofluorescent Penetration Assay.

Skin epidermal permeability is a critical barrier function, and its disruption is implicated in various dermatological conditions and in secondary lymphedema, leading to increased fluid loss and dermal complications. Many techniques exist to assess this permeability. However, these methods frequently require specialized instruments or lack the spatial resolution needed to identify specific cellular layers involved in barrier dysfunction. Here, we describe a step-by-step protocol for an immunofluorescence-based dye penetration assay that provides high spatial resolution for visualizing the perm

Identifying Genetic Predisposition and Clinical Risk Factors for Breast Cancer-Related Lymphedema: Toward Personalized Prevention.

BACKGROUND: Breast cancer-related lymphedema (BCRL) remains a major chronic complication following axillary lymph node dissection (ALND), particularly in regions where locally advanced breast cancer is prevalent. While several clinical factors have been identified, the role of genetic predisposition in BCRL development remains underexplored. This study aimed to identify genetic and clinical factors associated with the development of BCRL, focusing on the Gap Junction Protein Alpha-4 (GJA4) rs705193 mutation as a potential biomarker. METHODS: This prospective cohort study was conducted on 106

Lymphatic‑specific magnetic resonance lymphangiography biomarkers for grading lymphedema in animal models.

Magnetic resonance lymphangiography (MRL) using gadolinium-based contrast agents is valuable for evaluating lymphedema but is limited by its inability to selectively visualize lymphatic structures, hindering accurate diagnosis and severity grading. To overcome this, we demonstrated imaging biomarkers for lymphedema grading using iron oxide-based T1 MRI contrast agent–enhanced MRL (INV-MRL) in animal models. Lymphedema was induced in the hindlimbs of rats by lymph node dissection and radiation, and was successfully established in 29 of 40 animals. Of these, 26 underwent both INV-MRL and near-in
