Efficacy and safety of pazopanib in patients with paclitaxel-pretreated primary cutaneous angiosarcoma in Japan: Results of the Japan Clinical Oncology Group single-arm confirmatory trial (JCOG1605).

BACKGROUND: Paclitaxel is a standard first-line chemotherapy for primary cutaneous angiosarcoma. However, there is no established second-line treatment after paclitaxel. OBJECTIVES: The aim of the JCOG1605 study was to evaluate the efficacy and safety of second-line pazopanib for primary cutaneous angiosarcoma. METHODS: This multi-institutional, single-arm, confirmatory trial was conducted in Japan. Patients with cutaneous angiosarcoma not associated with lymphedema or radiation exposure were included in this study. Pazopanib was administered orally at an initial dose of 800 mg, once daily.

Physiotherapy in the primary care oncology team.

As the incidence of cancer rises, medical advancements have made it possible to have increased survival rates among cancer patients. However, many experience persistent treatment-related side effects that negatively affect their quality of life. At the first international Oncology Physiotherapy Conference in 2018, it was agreed that undergraduate physiotherapy training provides a strong foundation for oncology rehabilitation. However, management of specific side effects such as cancer-related fatigue (CRF), chemotherapy-induced peripheral neuropathy (CIPN) and lymphoedema, necessitates additio

Regional Nodal Irradiation Impact on Lymphedema, Surgical Outcomes, and Quality-of-Life Following Mastectomy, Axillary Dissection, and Immediate Lymphatic Reconstruction.

Radiation therapy following axillary lymph node dissection (ALND) is a key risk factor for lymphedema, with regional nodal irradiation (RNI) posing a higher risk. Immediate lymphatic reconstruction (ILR) with microsurgical lymphovenous anastomosis performed concurrently with ALND aims to prevent lymphedema, but its efficacy in the setting of RNI is unclear. This study compares lymphedema incidence, complications, and LYMPH-Q patient-reported outcomes (PROs) after ILR based on receipt of RNI.We retrospectively studied consecutive patients who underwent mastectomy and ALND with ILR between 2017

Moving beyond cancer: longitudinal changes in 24-hour movement behaviors and their association with quality of life in breast cancer survivors.

BACKGROUND: Many breast cancer survivors experience persistent symptoms after treatment, impairing quality of life (QoL). At the same time, maintaining healthy levels of 24-hour movement behaviors (24h-MBs) i.e. engaging in 150 min of moderate-to-vigorous physical activity (PA) a week, several hours of light PA a day, limiting sedentary behavior during the day, and achieving restorative sleep, remains challenging. While these behaviors influence QoL individually, little is known about the combined impact of 24h-MBs in breast cancer survivorship. Therefore, this study aimed to 1) examine longit

Oncological Feasibility of Conservative Axillary Surgery (Opinion Article): Tailored Axillary Surgery vs. Axillary Reverse Mapping-Guided Axillary Lymph Node Dissection.

BACKGROUND: Tailored axillary surgery (TAS) and axillary reverse mapping (ARM)-guided axillary lymph node dissection (ALND) have been developed to avoid arm lymphedema without increasing a risk of axillary recurrence. However, the oncological feasibility of TAS and ARM-guided ALND remains a crucial consideration. METHODS: This article reviewed the oncological feasibility of TAS and ARM-guided ALND based on the current literature. RESULTS: For ALND performed after TAS, additional involved nodes were found in 70% of upfront surgery patients and 60% of neoadjuvant chemotherapy (NAC) patients. A

Divergent vascular courses in the submental lymph node flap revealed by cadaveric study.

Vascularized submental lymph node (VSLN) flap transfer is a promising treatment for lymphedema, yet its venous anatomy remains underexplored. This study investigated variations of the submental artery and vein and the courses of hilar arteries and veins connecting lymph nodes in 40 VSLN flaps from 20 Thai cadavers. Colored latex was injected in both sides of the internal carotid arteries, external carotid arteries, internal jugular veins, and external jugular veins of each cadaver. The mandibles were removed en-bloc and then sliced coronally for investigation under stereomicroscopy. The submen

Prophylactic Buried Dermal Flap with Immediate Lymphatic Reconstruction: Effect on Functional Outcomes after Axillary Lymph Node Dissection.

BACKGROUND: Axillary lymph node dissection (ALND) may result in breast cancer-related lymphedema in up to one-third of patients. ALND creates axillary dead space and contributes to decreased range of motion. Immediate lymphatic reconstruction (ILR) is a preventive microsurgical axillary lymphovenous anastomosis to reduce postsurgical lymphedema. This study evaluates the effect of a buried dermal flap as an adjacent local flap on functional limb outcomes in patients after ALND. STUDY DESIGN: A single-center retrospective review was performed for patients who underwent mastectomy, ALND, and ILR

Prophylactic LYMphatic Reconstruction (LYMbR) to prevent lymphedema after node dissection for cutaneous malignancies: a randomized controlled trial.

BACKGROUND AND AIMS: Cancer-related lymphedema (CRLE), a chronic complication of cancer treatment, affects 39-73% of patients post-lymph node dissection, impacting physical health, social participation, and finances. Prophylactic immediate lymphatic reconstruction (ILR) via lymphaticovenous anastomosis (LVA) has shown potential in reducing CRLE incidence by two-thirds following axillary and inguinal node dissection. However, rigorous phase III studies with long-term follow-up are still needed to confirm these promising results. This study aims to evaluate the efficacy, safety, and long-term ou
