Variations in adverse events associated with different infusion modalities of parenteral nutrition: A pharmacovigilance study.

BACKGROUND: Parenteral nutrition can be administered through various infusion modalities. However, limited research has examined the differences in adverse reactions associated with these modalities. METHODS: Data from the Food and Drug Administration adverse event reporting system database were analyzed using disproportionality analysis to identify and compare adverse event signals across different infusion modalities. RESULTS: A total of 1642 adverse event reports related to parenteral nutrition were included: 356 in the all-in-one group, 85 in the two-in-one group, 1086 in the fat emulsio

Enhancing outcomes in severe lymphedema through combined treatment strategies.

Lymphedema, particularly in its advanced stages, presents significant challenges in treatment, often necessitating a combination of therapies to manage symptoms effectively and improve patient outcomes. This article reviews the findings of Wang et al, regarding the use of lymphovenous anastomosis and complex decongestive therapy in treating severe, deformed stage III lymphedema with recurrent infections. The case report details the promising results achieved through this combined therapy, highlighting substantial reductions in limb volume and the complete resolution of recurrent lymphangitis.

Pulmonary Lymphangitis Carcinomatosis in Rapid Progressive Recurrent Pheochromocytoma: A Case Exhibiting the Peer Diagnostic Value of 99mTc-FAPI-46 and 99mTc-HYNIC-Octreotide Scans.

A 43-year-old woman with a history of left adrenal pheochromocytoma was suspected for recurrence 2 months after adrenalectomy. Imaging assessments revealed metastatic lesions in the lungs and abdominal lymph nodes. Despite initial treatment with long-acting octreotide, the patient's symptoms worsened. Consequently, the patient was referred to the nuclear medicine department for theragnostic purposes. Comprehensive re-evaluation revealed a recurrent tumor in the left adrenal bed and multiple metastatic lesions, including lung, liver, lymph nodes, and skeletal metastases. In this patient, 99mTc-

Pseudomonas-Enterobacter Co-Infection Drives Cellulitis and Lymphangitis in Equines: A Case Report.

This case report detailed a rare co-infection of Pseudomonas asiatica and Enterobacter hormaechei in a 9-year-old warmblood mare, leading to severe cellulitis and secondary lymphangitis following traditional hoof blood-letting therapy. The mare exhibited acute limb swelling, fever, cutaneous ulceration, lymphatic dysfunction and unknown anemia. Comprehensive diagnostics, including bacterial culture, whole-genome sequencing, anti-elastin antibody (AEAb) ELISA, and diagnostic imaging, confirmed the pathogens causing cellulitis and secondary lymphangitis. AEAb levels were elevated, correlating wi

Genomic epidemiology of Histoplasma in Africa.

Histoplasmosis, the disease caused by thermally dimorphic fungi in the genus Histoplasma, is usually associated with pulmonary involvement in healthy individuals and a disseminated syndrome in immunocompromised patients. Among African patients, lung disease is less commonly reported than skin, lymph node, or bone disease. Because different species or strains may be associated with different disease presentations and outcomes, understanding genetic and phenotypic variation in the genus Histoplasma is important for accurate diagnosis and treatment. We sequenced the genomes of 36 Histoplasma isol

Characteristics and Outcomes of Diffuse Interstitial Pneumonias Discovered in the ICU: A Retrospective Monocentric Study-The "IPIC" (Interstitial Pneumonia in Intensive Care) Study.

Background/Objectives: Interstitial lung disease (ILD) is a heterogenous group of disorders characterised by an association of inflammatory and fibrotic abnormalities of the lung. Acute respiratory failure (ARF) may represent the initial picture of the disease. This study aims to highlight the diagnosis of ILD in the intensive care unit (ICU) and to describe the epidemiological, prognostic, and imaging features of patients diagnosed for the first time with ILD in the ICU. Methods: We conducted a single-centre retrospective study. We screened all 2459 patients admitted to our ICU from October 2

Unusual presentation of sclerosing lymphangitis of the penis with large ulcerations.

Sclerosing lymphangitis of the penis (SLP) is a rare, benign condition typically presenting as a cord-like swelling. We report an unusual case of SLP in a 75-year-old circumcised man with marked penile edema and deep ulcerations. STI screening was negative. After 2 weeks of sexual abstinence and local care, the ulcerations healed, revealing the characteristic SLP cord. The penile swelling resolved completely after 2 months. This presentation is unique due to the patient's age and the prominent edema and ulcerations, suggesting these features may indicate a severe clinical form of SLP that only

Impact of diabetes mellitus on adenolymphangitis episodes in patients with filarial lymphedema-Towards an integrated care approach.

BACKGROUND: Filarial lymphedema, caused by lymphatic filariasis, is characterized by chronic swelling and recurrent skin infections. Acute adenolymphangitis (ADL) episodes significantly exacerbate morbidity. Diabetes mellitus (DM) increases susceptibility to infections; however, the relationship between diabetes and ADL frequency and severity in filarial lymphedema patients remains unclear. This study investigated the influence of diabetes on ADL attacks and identified predictors influencing ADL episodes in patients with filarial lymphedema. METHODOLOGY/PRINCIPAL FINDINGS: This case-control s

Implementation of Customized Homogenizing Devices in Radiotherapy for the Treatment of Non-Melanoma Skin Cancer: Three Case Reports.

Background and Clinical Significance: In most cases, the success of radiotherapy in the treatment for skin cancer is limited, particularly due to the irregularities of the neoplasm's surfaces or even tissue discontinuity. Based on a comprehensive clinical assessment, the therapeutic approach for radiotherapy was established for the patients included in this study. Wax-paraffin (50:50) devices were custom-designed for radiotherapy treatment, confirming adequate homogeneity and conformity indices for doses of 55-66 Gy, and chemotherapy when necessary. Toxicity and treatment response were also as

Rare case of extralymphatic dirofilariasis presenting as a facial subcutaneous nodule in non-endemic region of Spain.

BACKGROUND: Filarial infestation is highly prevalent in tropical countries, lymphangitis and chronic lymphedema being the most common complaints in these regions. Yet, extralymphatic filariasis outside endemic areas is an extremely rare scenario. CASE PRESENTATION: We report a case of filariasis in the south of Spain. A previously healthy 49-year-old male patient reported a subcutaneous facial swelling. Surgery showed an encysted solitary filarial worm. CONCLUSION: To our knowledge, this is a rare clinical manifestation of filariasis in non-endemical regions in Spain. Still, it should be kep

Trastuzumab Deruxtecan Significantly Reduces Human Epidermal Growth Factor Receptor 2-Positive Pancreatic Metastasis from Advanced Breast Cancer: A Case Report.

INTRODUCTION: Pancreatic metastases from breast cancer are rare, with only 61 reported cases. They are often difficult to differentiate from primary pancreatic cancer based on clinical presentation and imaging. The median overall survival after the diagnosis of pancreatic metastases has been reported to be short (17 months). CASE PRESENTATION: A 50-year-old woman was diagnosed with human epidermal growth factor receptor 2 (HER2)-positive advanced right breast cancer, with right axillary lymph node and multiple bone metastases. After palliative irradiation for thoracolumbar metastases, the pat

Obscure Skin Infection in a Morbidly Obese Patient: The Challenges of Diagnosis.

Obesity is a complex and increasingly prevalent condition associated with significant morbidity and mortality. Increased susceptibility to skin and soft tissue infections is attributable to multiple factors, including immune dysregulation, impaired mobility, and the anatomical challenges posed by excess adipose tissue. In morbidly obese individuals, limited access to intertriginous areas, difficulties with repositioning, and overlapping comorbidities can delay the recognition and treatment of infections, potentially worsening clinical outcomes. This case report describes a 50-year-old morbidly

Early Pseudoprogression Mimicking Pneumonitis After Tarlatamab Therapy: A Case Suggestive of Immune Cell-Associated Respiratory Syndrome (ICARS).

Tarlatamab, a DLL3-targeting bispecific T-cell engager, is an emerging option for previously treated small-cell lung cancer (SCLC). Pseudoprogression with tarlatamab is poorly defined and can mimic drug-induced pneumonitis. A 77-year-old man with extensive-stage SCLC developed fever and hypoxia by Day 3 after the first 1-mg dose, with rapid tumour enlargement and diffuse left-lung infiltrates. High-dose methylprednisolone and tocilizumab produced prompt clinical and radiographic improvement; follow-up imaging showed resolution of infiltrates and tumour regression below baseline, consistent wit

Two Cases of Epidermal Growth Factor Receptor L861R Mutation-Positive Lung Adenocarcinoma Treated With Osimertinib and Afatinib.

BACKGROUND: Epidermal growth factor receptor (EGFR) mutations are detected in approximately 40%-50% of patients with lung adenocarcinoma in Asian populations and in around 10% of patients in Western populations. Among these, the EGFR L861R mutation is uncommon and undetectable using conventional polymerase chain reaction methods. Recently, clinically available next-generation sequencing (NGS) has been used to detect L861R mutations, potentially increasing the number of identified cases of L861R-positive non-small cell lung cancer. Herein, we present two cases of EGFR L861R-mutated lung adenoca
