Associations Between Dietary Glycemic Index, Glycemic Load, and Chronotype in Overweight and Obese Women: A Cross-Sectional Study.

UNLABELLED: Anemia affects 1.62 billion people globally, with a high prevalence in vulnerable groups, including 67.1% of children and 59.1% of women in India. Traditional anemia screening methods are invasive, costly, and impractical in remote areas. This study aims to evaluate the EzeCheck non-invasive device for estimating hemoglobin levels, comparing its efficacy with conventional hematology analyzers. The goal is to provide a more accessible and efficient solution for anemia screening, particularly in resourcelimited settings. This hospital-based cross-sectional study was conducted at Medi

Heart rate variability to objectively measure fear of falling in post fall patients: a protocol study.

BACKGROUND: At least one-third of older people experience falls each year, often resulting in a fear of falling that leads to a cycle of negative consequences. Fear of falling activates the amygdala, which triggers a complex response involving cognitive, physiological, and behavioral components. Physiologically, the stress response includes autonomic changes like increased heart rate and reduced Heart Rate Variability (HRV). Behaviorally, heightened alertness leads to fight, flight, or freeze responses, influenced by amygdala and Periaqueductal Gray activations. In conditions like post-fall sy

The underrated value of non-motor symptoms of Parkinson's disease in making diagnostic and treatment decisions.

Parkinson's disease (PD) is identified clinically by the presence of prominent motor signs; however, in recent years there has been growing recognition of non-motor symptoms (NMS) causing a similar or even greater impact on patients' quality of life. Wide spectrum of non-motor problems includes cognitive, neuropsychiatric, sensory, gastrointestinal, urinary, and cardiovascular issues, among others. Some of them are integral parts of the disease, while others may be side effects of applied therapies. Non-motor symptoms like hyposmia, cognitive decline, autonomic symptoms, and sleep disorders ma

Orthostatic blood pressure changes among adults in Ekiti state, Nigeria: Impact of socio-demographic, anthropometric, and biophysical factors.

Orthostatic intolerance is the inability to maintain upright posture, a key manifestation of autonomic dysfunction that can lead to orthostatic hypotension (OH). We aimed to investigate orthostatic blood pressure (BP) changes among cohorts living in Ekiti State and to explore plausible underlying factors. We encompassed 140 participants (28% male; 72% female; age range: 18-70 years) in our preliminary study. Socio-demographic and anthropometric indices were collected using appropriate tools. Orthostatic responses were assessed at baseline, standing, and sitting following standardized protocols

Postural Orthostatic Tachycardia Syndrome, Menopause and Hormone Replacement Therapy: Clinical Decisions in Times of Uncertainty.

Postural orthostatic tachycardia syndrome (POTS), characterized by a rise in heart rate of at least 30 beats per minute from supine to standing position without accompanying orthostatic hypotension, is one of the most common autonomic disorders with disabling cardiovascular and neurologic manifestations. Hormonal influence has been long recognized by the disorder predominantly affecting women of reproductive age, with frequent onset around menarche, exacerbation of symptoms before or during menses, and pregnancy being one of POTS triggers. Hormone replacement therapy (HRT) and menopause in wom

Negative [99mTc]Tc -DPD Scintigraphy, Presence of Monoclonal Protein and Biopsy Suggestive of AL Amyloidosis in a Patient With Homozygous p.Ala101Val Transthyretin Gene Variant.

Amyloidosis is a rare disease associated with the deposition of misfolded proteins that damage multiple organs, leading to a wide range of symptoms. The most frequently implicated proteins in amyloidosis include immunoglobulin Free Light Chains (FLC), related to AL amyloidosis, and transthyretin (TTR), which is responsible for ATTR amyloidosis. Here, we report the case of a 52-year-old patient with a history of chronic diarrhea, loss of weight, and orthostatic hypotension with imaging-confirmed cardiac amyloidosis (CA). The diagnostic process was notably complex due to elevated serum FLC, nega

Deep Learning-based Classification of Patients with Postural Orthostatic Tachycardia Syndrome using Wearable ECG and Accelerometer Data.

Postural Orthostatic Tachycardia Syndrome (POTS) is a chronic autonomic disorder characterized by chronic (> 3 months) orthostatic intolerance and an increase in heart rate (HR) of ≥ 30 beats per minute (bpm) without orthostatic hypotension. Traditional diagnostic approaches, such as the active standing or tilt-table test, are typically conducted under controlled clinical conditions, limiting their ability to capture the natural variability of symptoms and the intricate physiological responses occurring in daily life. These tests may cause patient discomfort, dizziness, nausea, or syncope. Fur

Autonomic dysfunction in patients with wild-type transthyretin amyloidosis.

BACKGROUND: Autonomic dysfunction is well recognized in hereditary transthyretin amyloidosis (ATTRv), but it has not been systematically studied in wild-type transthyretin amyloidosis (ATTRwt). Because ATTRwt primarily presents with cardiomyopathy, autonomic symptoms may mimic heart failure and lead to inappropriate treatment. Here we aimed to investigate the presence and extent of autonomic dysfunction in ATTRwt. METHODS: In ATTRwt patients and controls, we performed an extensive autonomic examination, including standardized questionnaires, passive and active orthostatic challenges, Valsalva

Keeping Patients on Their Feet: How Fall Prevention Clinics Can Be More Effective.

BACKGROUND: Although fall prevention programs in real-world clinical settings often generate recommendations to reduce fall risk factors, evidence that they improve clinical outcomes remains limited. To better understand this gap, we examined implementation rates of recommendations. METHODS: This retrospective cohort study evaluated patients who received care at an interdisciplinary (nurse, physical therapist, geriatrician) fall prevention consultation clinic from November 2020 to December 2022. Data were collected on patient demographics, screening assessments (including self-reported falls,

When caution becomes neglect: Deep brain stimulation and cognitive impairment in Parkinson's disease.

Over the past 25 years, a prevailing clinical dogma has held that significant cognitive impairment constitutes a contraindication to the use of Deep Brain Stimulation in Parkinson's disease. Whilst multiple studies, excluding such patients, have repeatedly emphasised the motoric benefits of this approach, less consideration has been given to the consequences of excluding this cohort. However, emerging evidence suggests that Deep Brain Stimulation in Parkinson's Disease patients with moderate cognitive impairment not only allows for significant reductions in dopaminergic therapy (typically alle

How to Approach and Manage Orthostatic Hypotension.

Orthostatic hypotension (OH) is a frequently undiagnosed condition, particularly affecting older adults and individuals with autonomic dysfunction. This comprehensive review offers a unique synthesis of current evidence on the aetiology, prevalence, diagnosis, and management of OH, integrating perspectives from neurology, cardiology, and geriatric medicine. It critically appraises the current limitations within clinical guidelines, highlighting the lack of standardised diagnostic protocols and the inadequate recognition of delayed OH and postural hypertension. Particular focus is given to the

Multiple System Atrophy Combined Outcome Assessment (MuSyCA): process, format, and validation plan.

PURPOSE: The Unified Multiple System Atrophy Rating Scale (UMSARS) is widely used as an outcome measure in MSA trials, but it has limitations for clinical trial use. To address these, we developed the Multiple System Atrophy Combined Outcome Assessment (MuSyCA), a comprehensive multimodal tool for disease-modifying MSA trials. The purpose of this manuscript is to describe the development and validation plan for MuSyCA, with emphasis on its structure, intended use, and assessment of reliability, validity, and sensitivity in tracking disease progression. METHODS: The development of MuSyCA follo

Prevalence and incidence of orthostatic hypotension in patients with Parkinson's disease: an updated systematic review and meta-analysis.

AIM: Systematically quantifying the prevalence and incidence of orthostatic hypotension in Parkinson's disease patients. BACKGROUND: Orthostatic hypotension is a common non-motor symptom in Parkinson's disease patients. Although systematic reviews have been published previously, existing data are outdated, and there is a lack of research examining differences in the prevalence and incidence of this symptom across various etiologies. An update of current studies is urgently needed. DESIGN: Systematic review and meta-analysis. METHODS: A systematic search was conducted across the following da

Patients with Ehlers-Danlos syndrome experience reduced effectiveness of lidocaine local anesthetic: a randomized cross-over clinical trial.

BACKGROUND: The Ehlers-Danlos syndromes (EDS) are a collection of heritable connective tissue disorders. Local anesthetic resistance is commonly reported by patients with EDS, but there is little clinical evidence to support this finding. We hypothesized that patients with EDS would have increased anesthetic resistance using lidocaine compared with healthy controls. METHODS: An interventional study design was used. Participants (total N=135: EDS n=91, healthy n=44) completed a series of sensation tests using a standardized 10 g monofilament following subcutaneous injection with saline or lido

Putative sympathetic-predominant subtype in body-first Parkinson's disease is associated with accelerated cognitive decline.

BACKGROUND: Parkinson's disease (PD) can be classified into brain-first and body-first subtypes based on the initial site of α-synuclein pathology. Postmortem studies further suggest that body-first PD may be divided into two phenotypes: sympathetic-predominant subtype (SPS) and parasympathetic-predominant subtype (PPS). However, studies on longitudinal clinical characteristics of the two putative body-first subtypes are limited. OBJECTIVES: We aim to investigate the clinical features of these subtypes. METHODS: In a cohort of 73 body-first PD patients, we identified 14 patients with orthost

Oral Agents for the Treatment of Refractory Hypotension in the Acute Care Setting - A Narrative Review.

Refractory hypotension is a common occurrence in the acute care setting and is associated with a variety of conditions and underlying disease states. Persistent hypotension is associated with negative outcomes including organ failure and mortality. While initial management of hypotension often involves parenteral agents such as vasopressors, long term use is not desirable or feasible in many cases. The use of oral agents for refractory hypotension in the acute care setting gives providers options in these clinical scenarios, specifically vasopressor weaning, intradialytic hypotension, and orth

Development and validation of a clinical prediction model for first fall in early Parkinson's disease: a study of two fall-naive cohorts.

BACKGROUND: Falls are frequent and debilitating complications in Parkinson's disease (PD), with a substantial risk present even in early stages. Predicting the first fall is critical for preventive interventions, yet existing models are often unsuitable for fall-naive, early PD patients due to their reliance on fall history. OBJECTIVE: This study aimed to develop and externally validate a clinical prediction model for first falls in a population of fall-naive, early PD patients. METHODS: This prognostic model study used data from two cohorts: the Parkinson's Progression Markers Initiative (P

High intensity interval training for Parkinson's disease: A scoping review of systemic effects and physiological adaptations.

BackgroundHigh intensity interval training (HIIT) involves vigorous intensity exercise bouts interspersed with low intensity bouts. Despite growing interest, the optimal dosage and clinical adaptability of HIIT in Parkinson's disease (PD) remain unclear. This scoping review synthesized the literature on systemic adaptations underlying HIIT in PD and developed a clinical framework while considering chronotropic incompetence, orthostatic hypotension, and disease progression.MethodsThree databases were searched for studies that incorporated HIIT interventions in PD.
