Adult Medullary Diffuse Midline Glioma Presenting with Orthostatic Hypotension: A Case Report.

Diffuse midline glioma, H3K27-altered, is a rare and aggressive central nervous system tumor that typically affects children, whereas adult cases are uncommon and less well characterized. We report a 43-year-old woman who experienced progressive lightheadedness for 10 months and was diagnosed with orthostatic hypotension on the basis of a positive tilt-table test result. Despite treatment with midodrine, her symptoms worsened, and dysarthria, dysphagia, sensory disturbance, and gait instability later developed. Magnetic resonance imaging confirmed an intra-axial mass in the dorsal medulla oblo

Emerging Role of Liquid Chromatography-Mass Spectrometry in the Clinical Laboratory Evaluation of Chronic Autonomic Failure.

Liquid chromatography with electrochemical detection (LC-ED) after batch alumina extraction has been the mainstay for assaying levels of catecholamines and related 3,4-dihydroxy compounds (catechols) as part of the clinical laboratory workup of patients with neurogenic orthostatic hypotension, especially in the setting of the autonomic synucleinopathies Parkinson disease with orthostatic hypotension (PD + OH), pure autonomic failure (PAF), and multiple system atrophy (MSA). Liquid chromatography with tandem mass spectrometry (LC-MS/MS) is faster and measures catechols and non-catechol metaboli

Cardiovascular Signs and Symptoms: Syncope and Presyncope.

Syncope is an abrupt, transient, and complete loss of consciousness associated with an inability to maintain postural tone, followed by rapid and spontaneous recovery. Syncope is caused by temporary cerebral hypoperfusion. Presyncope describes symptoms such as lightheadedness and vision changes that may or may not precede syncope. Syncope can be classified by its mechanism: reflex (neurally mediated), orthostatic hypotension, or cardiac. Initial evaluation of loss of consciousness should include a careful history and detailed physical examination with orthostatic vital signs and 12-lead electr

Spinal cord involvement and cardiovascular autonomic dysfunction in Parkinson's disease.

Patients with Parkinson's disease (PD) frequently present autonomic cardiovascular dysfunction. This study investigated the involvement of autonomic centers in the upper thoracic spinal cord in cardiovascular dysfunction in patients with PD using multimodal MRI and markers of orthostatic hypotension. We recruited 26 patients with PD, stratified based on the presence (PDRBD(+), n = 11) or absence (PDRBD(-), n = 15) of rapid-eye movement sleep behavior disorder (RBD), and 22 matched healthy controls (HC). Participants underwent multimodal MRI of the cervical and upper thoracic spinal cord. Quant

Children and adolescents with disorders of gut-brain interaction with comorbid hypermobility and orthostatic intolerance have worse outcomes.

OBJECTIVES: Disorders of gut-brain interaction (DGBI) affect about 40% of children and are often comorbid with hypermobility spectrum disorders (HSDs) and orthostatic intolerance (OI). However, how these comorbidities impact outcomes in pediatric DGBI is not well understood. This study aimed to compare outcomes in DGBI patients with HSD, OI, both, or neither. METHODS: We reviewed records of patients aged 9-21 years from a multidisciplinary DGBI clinic. Patients met Rome IV criteria for DGBI and had documented HSD and/or OI diagnoses from specialists in gastroenterology, rheumatology, genetics

Orthostatic blood pressure trajectories characterize heterogeneous disease progression in Parkinson's disease.

BACKGROUND: Identifying biomarkers that reflect disease progression in Parkinson's disease (PD) is of substantial clinical relevance. Accumulating evidence suggests a potential link between orthostatic hypotension and disease progression in PD. However, prior studies have largely relied on single baseline assessments, thereby overlooking longitudinal fluctuations in orthostatic blood pressure changes. OBJECTIVES: This study aimed to identify distinct longitudinal trajectories of orthostatic systolic blood pressure change (∆SBP) and to investigate their associations with disease progression in

Blunted response of caudal locus coeruleus to arousing stimuli in Parkinson's disease.

Parkinson's disease (PD) causes progressive degeneration of noradrenergic neurons in the locus coeruleus (LC), contributing to non-motor symptoms. Using neuromelanin-sensitive ultra-high field (7T) MRI, we previously identified a reduction in neuromelanin signal in the caudal LC, indicating a rostro-caudal gradient of noradrenergic cell loss. Caudal LC degeneration was associated with greater severity of non-motor symptoms such as orthostatic hypotension and apathy. In the current case-control study, we expanded the PD cohort to further corroborate the structure-symptom relationships within th

Hallucinations and cognitive impairment are associated with blood pressure fluctuations in Parkinson's disease.

BACKGROUND: Patients with Parkinson's disease (PD) frequently develop cardiovascular autonomic dysfunctions, including blood pressure (BP) fluctuations and orthostatic hypotension (OH) in the early stage, and hallucinations or cognitive impairment in the middle stage to late stage. The current study aimed to determine whether OH, BP fluctuations and cerebral white matter hyperintensities (WMHs) on MRI are associated with the development of hallucinations and cognitive impairment. METHODS: In this joint cross-sectional case-control study conducted across two institutions, we analysed the systo

Hypotensive Episodes Precede Cognitive Fluctuations in Parkinson's Disease Dementia.

BACKGROUND: Cognitive fluctuations (CFs) are a disabling feature in Parkinson's disease dementia (PDD) and contribute to caregiver burden. The relationship between CFs and neurogenic orthostatic hypotension (nOH), a common autonomic complication in PDD, has not been adequately examined. CASES: We describe four patients with PDD and nOH with caregiver-reported CFs manifesting as impaired attention, comprehension, language, and visuospatial skills. Each patient underwent bedside assessment for orthostatic vital signs and 24-hour ambulatory blood pressure monitoring (ABPM). CFs were consistently

Cardiocerebral hemodynamic characteristics of vasovagal syncope associated with cerebral autoregulation impairment.

BACKGROUND: It is generally believed that cerebral hypoperfusion in patients with vasovagal syncope (VVS) is secondary to hypotension; however, current evidence suggests that this is not always the case. Thus, this study aimed to analyze the hemodynamic characteristics of patients with VVS and dynamic cerebral autoregulation (CA) dysfunction (referred to as CA-impaired VVS) to improve the diagnosis and treatment of this VVS subtype. METHODS: This retrospective study included 143 patients with VVS who underwent the head-up tilt test (HUTT) using transcranial Doppler (TCD). Patients were divide

Treating the Patient, Not Only the Amyloid: Symptomatic Management in Transthyretin Amyloidosis.

Transthyretin amyloidosis (ATTR) is a progressive multisystem disorder characterized by extracellular deposition of misfolded transthyretin fibrils, leading to neurological, cardiac, gastrointestinal, urogenital, sexual, and ophthalmological involvement. While disease-modifying therapies have significantly improved survival and slowed disease progression, a substantial proportion of patients continue to experience a high symptomatic burden that markedly impairs quality of life. Symptomatic manifestations often occur early, may precede the diagnosis, and frequently persist despite etiological t

Cardiac Syncope: An Underestimated Cause of Unexplained Syncope in the Elderly-Data from a Single High-Volume Syncope Unit.

Background/Objectives: Syncope remains a common problem in the elderly, adversely affecting quality of life, morbidity and mortality. Diagnosis is challenging due to the atypical presentation, multifactorial aetiology, overlap with non-syncoptic falls and increased prevalence of cardiac disease. This study aims to investigate the impact of cardiac syncope in this high-risk population. Methods: A retrospective single-centre observational cohort study, including 171 patients ≥65 years old with syncope of unknown origin or other falls, was conducted. Different diagnostic tests and strategies were

Cardiovascular Safety Signals of Oral Versus Topical Minoxidil in FAERS: A Disproportionality Analysis (Analytic Cohort 2012-2025).

Oral minoxidil, including low-dose regimens, is increasingly used off-label for alopecia, but cardiovascular safety remains a clinical concern. We compared cardiovascular adverse event reporting patterns for oral versus topical minoxidil using a disproportionality analysis of the FDA Adverse Event Reporting System (FAERS). FAERS data (2004Q1-2025Q3) were imported and deduplicated; minoxidil reports were restricted to primary/secondary suspect (PS/SS) drugs and eligible reports from 2012 to 2025. Exposure was classified as ORAL, TOPICAL, BOTH, or UNKNOWN using a standardized route/dose-form dic

Association of neurogenic orthostatic hypotension with cognitive decline in Parkinson's disease: a longitudinal cohort study.

INTRODUCTION: Neurogenic orthostatic hypotension (nOH), a common non-motor feature of Parkinson's disease (PD), is defined as a sustained drop in blood pressure (BP) upon standing due to autonomic dysfunction. Although prior studies support an association between nOH and cognitive impairment, its longitudinal impact on cognitive decline in PD remains insufficiently explored. We aimed to determine to what extent baseline nOH is associated with an accelerated decline in global and exploratory domain-specific cognitive functions. METHODS: A retrospective longitudinal cohort study was conducted u

Evaluation of Orthostatic Hypotension in Patients With Idiopathic Parkinson's Disease Utilizing the Head-Up Tilt Test.

INTRODUCTION: Parkinson's disease (PD) is a neurodegenerative disorder, affecting individuals aged 60 and older, with rapidly increasing prevalence in aging populations, particularly in India. Apart from classical motor symptoms, nonmotor symptoms, particularly orthostatic hypotension (OH), significantly affect the quality of life, increasing the fall risk that contributes to increasing morbidity and mortality. This single-center, prospective, cross-sectional observational study evaluates OH, a key cardiovascular autonomic dysfunction, in idiopathic PD patients using the head-up tilt test (HUT

Single-Pill Combination Olmesartan/Amlodipine/Hydrochlorothiazide Safety and Effectiveness in Older Patients with Hypertension: Pooled Analysis of Post-Approval Observational Studies.

INTRODUCTION: Despite needing intensive blood pressure (BP) control, elderly patients with hypertension may be undertreated because of concern about potential hypotension-related adverse events (AEs). Many older patients need combination therapy to achieve BP targets. This study investigated whether single-pill combination (SPC) antihypertensive therapy may benefit such patients. METHODS: This study analyzed pooled data from patients treated with olmesartan/amlodipine/hydrochlorothiazide (O/A/H) SPC therapy in three multicenter, prospective, observational studies (RESOLVE, RESOLVE-PRO and RES

Postprandial Hypotension-Methods for the Evaluation and Management.

Postprandial hypotension (PPH) is a common but underrecognized condition in older adults that affects nearly half of the population and is frequently associated with autonomic dysfunction. Unlike orthostatic hypotension (OH), PPH often presents with sleepiness and syncope. Its prevalence and severity are influenced by meal composition, particularly carbohydrate content, and the time of day, with morning meals posing a greater risk. Its pathophysiology involves splanchnic blood pooling, altered gastric emptying, impaired baroreflex function, and intestinal peptide activity, including those of g
