Treating hearts and minds: adverse cardiovascular effects of psychiatric medications.

Cardiovascular disease (CVD) is one of the leading causes of death in patients with severe mental illness (SMI), being ~3.3 times higher than in the general population. The adverse cardiovascular effects of psychiatric medications have short- and long-term consequences that contribute to higher rates of cardiovascular death in patients experiencing SMI. By understanding these adverse effects, clinicians can better address cardiovascular health inequalities in patients with SMI from a pharmacological perspective. This review highlights both the short- and long-term adverse cardiovascular effect

Patient with AL amyloidosis with recurrent syncope as the first symptom: case report and literature review.

BACKGROUND: Immunoglobulin light-chain (AL) amyloidosis is a rare systemic disease caused by the extracellular deposition of misfolded light chains. Nonspecific manifestations, especially those dominated by severe autonomic neuropathy, frequently delay diagnosis. METHODS AND RESULTS: A 68-year-old woman was admitted after five months of recurrent syncope. The clinical picture was dominated by refractory orthostatic hypotension, nausea and vomiting alternating with diarrhea and constipation, stocking-glove paresthesias, and proteinuria. Bone marrow examination showed the presence of clonal pla

Management of Orthostatic Hypotension: A Review.

IMPORTANCE: Orthostatic hypotension is a common but underrecognized condition that increases with age and is associated with a lower quality of life, falls, and increased mortality. The frequent coexistence of supine hypertension and postprandial hypotension with orthostatic hypotension makes it a challenging condition to manage. OBSERVATIONS: Testing for orthostatic hypotension should be done in patients with orthostatic symptoms (eg, vision changes and dizziness that occur only when upright and improve when seated or lying down), as well as asymptomatic patients in high-risk groups such as

Older peoples' perspectives of postural hypotension in general practice: a qualitative study.

BACKGROUND: Postural hypotension (PH) affects 20-30% of community-dwelling older adults, but largely under-recorded in general practice. An important contributor to under-detection of PH is a lack of symptoms, awareness of symptoms and patient under-reporting. AIM: To explore older peoples' perceptions and experiences of PH DESIGN & SETTING: Qualitative interview study with older adults living in urban or rural areas of England. METHOD: One-to-one semi-structured interviews with older people aged ≥65 years who either had a diagnosis of PH or were identified as at high-risk of PH. Participant

Dysautonomia in Long COVID is Prevalent and Could Explain the Frequency of Symptoms.

Background: Long COVID presents with a variety of symptoms, some of which could be related to autonomic dysfunction. Our aim was to evaluate the prevalence of autonomic dysfunction in long COVID patients. Methods: We conducted a cross-sectional study and included all consecutive patients enrolled in several clinical research studies. We performed the following autonomic dysfunction markers: heart rate variability, heart rate, systolic and diastolic blood pressure changes during NASA Lean Test, cardiopulmonary exercise testing and a Composite-Autonomic-Symptom-Score (COMPASS)-31 scale. We used l

Arterial Stiffness Mechanisms and Orthostatic Hypotension in SPRINT: A Randomized Controlled Trial.

BACKGROUND: Hypertension is a leading cardiovascular disease risk factor. Intensive blood pressure (BP) control reduces cardiovascular disease events but may be limited by treatment-related serious adverse events (SAEs). OBJECTIVES: The purpose of this study was to determine if baseline carotid to femoral pulse wave velocity (PWV)-including the component of structural stiffening, due to remodeling of the vessel wall, and load-dependent stiffening, due to the BP load on the arterial wall-is independently associated with orthostatic hypotension (OH) and SAEs in the SPRINT (Systolic Blood Pressu

The combined association of STOPPFall medication use and orthostatic blood pressure abnormalities with future falls and fractures in community-dwelling older people.

PURPOSE: STOPPFall(Screening Tool of Older Persons Prescriptions in older adults with high fall risk) defines fall-risk-increasing drugs(FRIDs) associated with orthostatic hypotension(OH)/falls. This study assesses the association between the cumulative effect of STOPPFall medications and OH, on future falls/fractures, over 4-year follow-up among community-dwelling older people ≥ 65 years using The Irish Longitudinal Study on Ageing(TILDA) data. METHODS: STOPPFall medications were recorded at Wave1. Orthostatic blood pressure(BP) was measured by active stand using a Finometer at Wave1, record

Associations of Poincaré Plot-Derived Parameters with Heart Rate Variability and Autonomic Reflex Testing in a Real-World Clinical Population.

Background/Objectives: Poincaré plot analysis represents a nonlinear approach to heart rate variability (HRV) assessment, but the physiological meaning of several derived parameters remains unclear. This study aimed to evaluate associations between selected Poincaré plot-derived parameters, conventional HRV indices, and cardiovascular autonomic reflex tests in a real-world clinical population. Methods: This observational study included 269 adult patients referred for evaluation of suspected autonomic dysfunction. All participants underwent short-term resting ECG, cardiovascular autonomic refle

Testing a Personalised Dysautonomia Management Protocol in Patients with Orthostatic Intolerance and a Diagnosis of Myalgic Encephalomyelitis/Chronic Fatigue Syndrome or Long COVID.

Background/Objectives: Myalgic Encephalomyelitis/Chronic Fatigue Syndrome (ME/CFS) and Long COVID (LC) are complex multisystem conditions with significant functional disability. Many patients experience symptoms of orthostatic intolerance, which can be captured in some cases as Orthostatic Hypotension (OH) or Postural orthostatic Tachycardia Syndrome (PoTS) on objective testing. Conservative treatments are recommended for first-line symptom management, but there is a lack of efficacy evidence. This study aims to assess the feasibility of an 8-week clinically supervised, personalised Dysautonom

Associations between orthostatic hypotension and related factors in hospitalised older adults: a cross-sectional study in Turkey.

BACKGROUND: Orthostatic hypotension is a prevalent condition in older adults. In light of its potential association with serious complications, the routine assessment of orthostatic blood pressure changes and the evaluation of factors associated with orthostatic hypotension in hospitalised older adults remain of current importance. Raising awareness among nurses, who are healthcare professionals closely involved in patient care, on this issue is essential. OBJECTIVE: To investigate the prevalence of orthostatic hypotension and its associated factors in hospitalised older adults. METHODS: This

Why do many elderly Japanese die suddenly during winter bathing?

Japanese people enjoy bathing, and often soak in deep hot bathtubs up to their shoulders for long periods, from evening to night. However, there have been numerous reports of unexpected deaths related to bathing, especially among older individuals and during the winter months. This review explores the reasons behind these seasonal and age-specific fatalities. Japanese detached houses typically have low insulation and rely on partial, intermittent heating, resulting in cold dressing rooms and bathrooms during winter. Elderly individuals, who often bathe under such conditions, experience signifi

Gender differences in non-motor symptoms in Parkinson's disease: a multicenter longitudinal study.

BACKGROUND: Non-motor symptoms (NMSs) are highly prevalent in Parkinson’s disease (PD) and affect patients’ quality of life. Data on gender differences in NMSs are largely cross-sectional and derived from chronically treated populations. Longitudinal evidence in early, levodopa-naïve PD patients remains limited. This study aims to longitudinally investigate gender differences in a wide range of NMSs in early-stage levodopa-naïve PD patients during the first 2 years following levodopa initiation. METHODS: This multicenter, prospective study enrolled 216 levodopa-naïve PD patients (139 men, 77 w

Cardiovascular Safety of Parkinson's Disease Therapies: A Comprehensive Review.

Parkinson's disease (PD) is a neurodegenerative disorder characterized by progressive motor dysfunction and a host of additional systemic manifestations. PD is managed via a broad range of pharmacological and procedural treatments aimed at ameliorating motor and nonmotor symptoms. As PD occurs with a high prevalence of cardiac comorbidities, investigation and awareness of the cardiac safety of this range of treatments are essential. This review aims to synthesize current consensus on the cardiotoxic risk associated with the most prevalent treatments for PD, including levodopa, catechol-O-methy

Elevated blood viscosity is associated with dysautonomia in long COVID symptoms.

BACKGROUND: Long COVID is associated with elevated inflammatory and antibody levels. Elevated plasma proteins can increase blood viscosity and decrease blood flow. Our aim is to evaluate if blood viscosity is associated with long COVID outcomes. METHODS: We conducted a cross-sectional study and included a sample of patients enrolled in our long COVID clinic. We estimated whole blood viscosity (WBV) using two previously validated formulas and compared it with the NASA lean test, COMPASS-31 scale as measures of dysautonomia and the symptom burden as measured by the modified COVID-19 Yorkshire s

Symptoms are an optimal target for orthostatic hypotension screening and management: an argument in favor.

Orthostatic hypotension (OH), defined as a sustained reduction of ≥ 20 mmHg in systolic or ≥ 10 mmHg in diastolic blood pressure within 3 minutes of standing, represents one of the most clinically significant manifestations of autonomic failure. Beyond its hemodynamic definition, OH is associated with disabling symptoms, falls, syncope, reduced quality of life, increased healthcare utilization, and excess mortality. In clinical practice, management often requires balancing objective blood pressure measurements with the patient's lived experience of orthostatic intolerance. This viewpoint argue

Unveiling orthostatic hypotension in precapillary pulmonary hypertension.

BACKGROUND: In addition to critical alterations in the pulmonary arterial network, pulmonary arterial hypertension (PAH) and chronic thromboembolic pulmonary hypertension (CTEPH) may exhibit cardiovascular abnormalities that impair systemic arterial pressure regulation during gravitational challenges. Therefore, we investigated the prevalence, subtypes and haemodynamic mechanisms of orthostatic hypotension (OH) in PAH or CTEPH. METHODS: OH was assessed using a 10-min active standing manoeuvre from supine baseline, with heart rate (HR; ECG) and arterial pressure (volume-clamp finger photopleth
