Orthostatic blood pressure reduction is associated with renal damage in functionally preserved elderly hypertensive patients.

BACKGROUND: Blood pressure (BP) exhibits specific characteristics in elderly patients. We evaluated the association between positional BP changes during orthostatism and the presence of hypertension-mediated organ damage (HMOD) in functionally preserved elderly hypertensive patients. METHODS: This was a multicenter, cross-sectional study of 156 hypertensive patients with a mean age of 75 ± 5.8 years (49% women). Central and peripheral office and 24-h BP were measured using an oscillometric device. Orthostatic BP changes were recorded according to the conventional definition of orthostatic hyp

Outcome-Based Cardiovascular Risk Framework is Required After Spinal Cord Injury.

Blood pressure (BP) instability is a hallmark of disrupted autonomic cardiovascular control after spinal cord injury (SCI). Individuals frequently experience hypertensive surges during autonomic dysreflexia and hypotensive drops during orthostatic hypotension, yet the commonly used thresholds for defining these events are derived from expert consensus rather than outcome-based evidence. Similarly, arterial stiffness, typically assessed by pulse wave velocity, is consistently elevated in SCI, but no validated cut points exist to guide clinical intervention. This lack of outcome-anchored thresho

Best Practice Expert Opinion and Guidelines for Assessing Blood Pressure Stability in Clinical Trials and Foundational Research After Spinal Cord Injury.

Spinal cord injury (SCI) and the associated autonomic dysfunction often lead to unique and severe blood pressure instability, including low resting blood pressure, orthostatic hypotension, postprandial hypotension, and transient episodic hypertension. Despite the high prevalence of these complications and the associated negative clinical outcomes, standardized protocols for measuring blood pressure stability in SCI clinical trials and research remain lacking. This guidelines article outlines best practices for measuring blood pressure stability in the SCI population. The guidelines were develo

Revealing Non-Motor Symptom Profiles of Orthostatic Hypotension Subtypes in Parkinson's Disease: Insights From a Multidimensional Case-Control Study.

Orthostatic hypotension (OH) represents a cardinal non-motor manifestation in Parkinson's disease (PD), yet the clinical relevance of its subtypes remains unclear. This study aimed to characterize the clinical and blood pressure (BP) profiles of PD-OH and its subtypes. We enrolled 186 PD patients, including 59 with OH, and collected clinical, postural BP, autonomic, cognitive, emotional, sleep, and REM sleep behavior disorder (RBD) data. Analyses used t-tests, nonparametric tests, and chi-square tests as appropriate. Compared to PD-nOH, PD-OH patients had later onset, higher RBD prevalence, po

Cognitive and behavioral disturbances in PSP and MSA: clinical features and imaging correlates.

Cognitive and behavioral disturbances are increasingly recognized as core features of Progressive Supranuclear Palsy (PSP) and Multiple System Atrophy (MSA), with important clinical and prognostic implications. In PSP, cognitive impairment-often an early sign-is dominated by a dysexecutive syndrome linked to dorsolateral prefrontal atrophy and fronto-subcortical circuit disruption. Deficits also extend to attention, memory, language, visuospatial abilities, and social cognition. Apathy is the most common behavioral symptom, associated with medial prefrontal and subcortical atrophy, while impul

Exploring the Link Between Pharmacological Management of Orthostatic Hypotension and Cognitive Performance: A Scoping Review.

BACKGROUND: Orthostatic hypotension is recognized as a potential contributor to cognitive impairment in older adults. It is still unknown whether pharmacological treatment of orthostatic hypotension can affect cognition. OBJECTIVE: The aim of this scoping review was to identify whether treatment of orthostatic hypotension with midodrine, droxidopa, and fludrocortisone had any effect on cognitive performance. METHODS: We searched the databases of Embase and MEDLINE. Key eligibility criteria included observational studies or clinical trials using midodrine, droxidopa, or fludrocortisone to tre

Validating the ADFSCI hypotension symptom domain as a scalable patient reported outcome measure in spinal cord injury.

Hypotension is an under-recognized consequence of spinal cord injury (SCI). Clinical assessments, such as head-up tilt testing, are resource-intensive and carry safety risks. In this study, we evaluated the validity of the hypotension symptom domain of the Autonomic Dysfunction Following Spinal Cord Injury (ADFSCI) questionnaire as a patient-reported outcome measure for hypotension-related symptoms in individuals with SCI. Known-groups validity and convergent validity were supported by higher hypotension symptom scores in participants with physiologically defined orthostatic hypotension (OH).

Orthostatic intolerance following total joint arthroplasty remains a significant barrier to successful same-day discharge: a narrative review of an underreported complication.

Early mobilisation following total joint arthroplasty is crucial in considering patients for successful same-day discharge (SDD). Recent years have seen an increase in SDD after total joint arthroplasty, with complication rates comparable to those with prolonged hospital stays. Orthostatic intolerance (OI) is a significant barrier to successful SDD after total hip and knee arthroplasties, with up to 52% of patients experiencing it. Patient-related factors include being female, being elderly, having multiple comorbidities, possessing a higher American Society of Anesthesiologists (ASA) status,

Haemodynamic effects of low versus high dialysate temperature in haemodialysis patients: a single-blinded, randomised, cross-over study.

BACKGROUND: Cooling of the dialysate may improve intradialytic haemodynamics. This study compared low dialysate temperature (LDT) of 35 °C with high dialysate temperature (HDT) of 37 °C (standard) in terms of blood pressure (BP), intradialytic haemodynamics, and orthostatic BP. METHODS: In a single-blind, randomised, controlled, crossover study fourteen haemodialysis (HD) patients underwent a 4-h HD session with LDT (35 °C) or HDT (37 °C) separated by a 1- week interval. BP was monitored throughout HD. Intradialytic haemodynamics were assessed with ultrasound dilution technique (Transonic). O

Understanding nocturnal polyuria in cardiovascular autonomic failure: Pathophysiological mechanisms and clinical implications.

BACKGROUND: Nocturnal polyuria (NP), characterized by excessive nighttime urine production, is a common non-motor symptom in synucleinopathies, particularly in the presence of cardiovascular autonomic failure (AF). NP contributes to sleep fragmentation, increased fall risk, and exacerbation of morning orthostatic hypotension. Diagnosis requires a comprehensive assessment of lower urinary tract symptoms, comorbidities and medication use. METHODS: We conducted a scoping review of the existing literature on PubMed up to December 2024 regarding NP associated with synucleinopathies and AF. RESULT

Electrocardiographic and Hemodynamic Changes During Tilt-Table Testing in Patients With Syncope.

Background Syncope is a common clinical condition with diverse underlying mechanisms, making its evaluation challenging. Tilt-table testing is widely used to assess autonomic and cardiovascular responses; however, the role of electrocardiographic (ECG) parameters in differentiating syncope types remains unclear. Methodology This retrospective study analyzed patients who underwent tilt-table testing for syncope evaluation in 2023-2024. A total of 59 patients were included and divided into three groups: orthostatic hypotension (n = 20), vasovagal syncope (n = 19), and negative test results (n =

What are we treating? The need for broader provider understanding of POTS, its nature, and care.

Postural orthostatic tachycardia syndrome (POTS) as conventionally defined is a chronic condition (typically > 3 months duration) incorporating reproducible symptoms of orthostatic intolerance (including dizziness/lightheadedness, and near-syncope) in the absence of orthostatic hypotension (ie, absence of a sustained systolic blood pressure drop > 20mmHg with upright posture). More recently, however, the :POTS: landscape as applied by many clinicians has broadened; the term :POTS: has become increasingly used to categorize a multisystem disorder the underlying etiologies of which remain unclea

Synucleinopathy-like clinical features and biologically defined synuclein seeding in PSP-Parkinsonism: an imperfect match.

Whether synucleinopathy-like (syn-like) symptoms match biologically defined synuclein seeding in PSP-parkinsonism (PSP-P) vs. Parkinson's disease (PD) remains unexplored. We studied 80 subjects: 20 PSP-P, 20 PSP-Richardson-syndrome(PSP-RS), 20 PD, and 20 controls(CS). Procedures included specific smell-testing, structured interview by a sleep specialist, orthostatic-stress test, specific dysautonomia scale, CSF-α-synuclein seed-amplification-assay (asyn-SAA) and neurofilament-levels (NfL). Frequency of syn-like symptoms was low in CS and PSP-RS, intermediate in PSP-P, and high in PD. Premotor

Osteopathic Treatment of Postprandial Cardiovascular Symptoms Suggestive of Altered Autonomic Regulation: A Case Report.

Postprandial cardiovascular symptoms may be associated with alterations in autonomic regulation, particularly when the postprandial decrease in blood pressure is not accompanied by an adequate compensatory increase in heart rate. We report the case of an 84-year-old man with a long-standing history of postprandial symptoms, characterised by dizziness, generalised weakness, fatigue, and presyncope occurring between 10 and 30 minutes after food intake, with improvement in the supine position. The patient presented with postprandial hypotension without an appropriate increase in heart rate. A str

Association between orthostatic blood pressure change and masked and white coat hypertension: the Nagahama study.

BACKGROUND: Exaggerated blood pressure (BP) response to orthostatic stimuli is indicative of cardiovascular frailty and may be associated with masked and white coat hypertension, which are BP abnormalities associated with cardiovascular outcomes. We aimed to clarify this possible association in a cross-sectional analysis of a large general population. METHODS: We enrolled 7618 community residents (mean age: 57.7 years). Orthostatic BP change was calculated as the difference between systolic BP measured in a seated position and at 3 min after standing. Orthostatic hypertension and hypotension

Orthostatic hypotension: new frontiers in pharmacotherapeutic management.

BACKGROUND: Orthostatic hypotension remains one of the most disabling manifestations of autonomic failure. Current therapies can improve upright blood pressure and orthostatic tolerance, but treatment is often limited by modest efficacy, short duration of action, and worsening of coexisting supine hypertension. AREAS COVERED: This review summarizes the contemporary management of orthostatic hypotension in adults with autonomic failure. After briefly reviewing epidemiology, hemodynamic physiology, and major etiologic categories, the discussion focuses on reviewing current non-pharmacological a

Effect of midodrine on orthostatic intolerance after hip and knee arthroplasty: A randomized controlled trial (the MObILE study).

BACKGROUND: Orthostatic intolerance (OI), characterized by dizziness and nausea during postural change, is common after hip and knee arthroplasty and can delay mobilization and discharge. Midodrine, an oral alpha-1 agonist, may reduce OI by improving vascular tone, but evidence in this setting is limited. METHODS: A double-blind, randomized, placebo-controlled trial was performed in adults undergoing primary unilateral total hip or knee arthroplasty. Patients received oral midodrine hydrochloride 10 mg or placebo (four to five doses over 24-30 h). Mobilization included a standardized orthosta
