Substance Abuse as a Trigger for Myocardial Infarction in Young Adults: An Underrecognized Major Risk.

Myocardial infarction (MI) remains a major public health problem worldwide. The use of illicit substances is increasing. These substances may be associated with an increased incidence of MI through various mechanisms, regardless of the presence of pre-existing risk factors. Although data on this subject are limited, it is important to highlight this issue. The aim of this review is to establish the association between substance misuse and MI, particularly in young adults, and to evaluate the underlying mechanisms based on the current literature. This review was prepared using publications iden

Outcome of Pediatric, Adolescent, and Young Adult Oncology Patients With Non-neutropenic Fever: A Scoping Review of the Literature.

BACKGROUND: There are established guidelines for pediatric oncology patients with neutropenic fever. However, half of pediatric oncology patients present with non-neutropenic fever. For this group, no established guidelines exist. OBJECTIVE: In this scoping review, we have explored the outcomes of non-neutropenic fever in pediatric, adolescent, and young adult patients with cancer-directed treatment. The results of this scoping review should assist in the creation of a guideline for the management of non-neutropenic fever in this group of patients. METHODS: Multiple electronic databases and

Regional anesthesia alone is associated with reduced intraoperative hypotension compared with combined general and regional anesthesia in shoulder arthroscopy performed in the beach-chair position.

BACKGROUND: Shoulder arthroscopy in the beach-chair position is frequently associated with intraoperative hypotension, especially under general anesthesia (GA). Regional anesthesia (RA) alone may offer improved hemodynamic stability and facilitate ambulatory management. This study compared intraoperative hemodynamic effects of RA alone versus combined GA and RA. METHODS: We conducted a monocentric, retrospective cohort study including all adult patients undergoing elective shoulder arthroscopy between October 1, 2023, and October 1, 2024, at a private surgical center in Paris, France. Patient

A Comprehensive Framework for the Prediction of Intra-Operative Hypotension.

In this paper, the problem of triggering early warning for intra-operative hypotension (IOH) is addressed. Recent studies on the Hypotension Prediction Index have demonstrated a gap between the results presented during model development and clinical evaluation. Thus, there is a need for better collaboration between data scientists and clinicians who need to agree on a common basis to evaluate those models. In this paper, we propose a comprehensive framework for IOH prediction: to address several issues inherent to the commonly used fixed-time-to-onset approach in the literature, a sliding wind

Multiple Synchronous CSF-Venous Fistulas in Spontaneous Intracranial Hypotension: A Multi-Institutional Case Series.

CSF-venous fistulas (CVFs) are a common cause of spontaneous intracranial hypotension. Due to the more routine use of decubitus myelography and advancements in various imaging techniques, recognition of CVFs has increased in recent years. Most commonly, patients harbor only 1 fistula at the time of myelography (although additional de novo fistulas can arise after treatment). Occasionally, 2 synchronous CVFs may be seen on a single myelogram. The coexistence of more than 2 CVFs, however, is quite rare and has only been previously described in 2 instances. Here, we present a multi-institutional

Improved Conspicuity of CSF-Venous Fistulas with Saline Pressure Augmentation: A Multi-Institutional Case Series.

CSF-venous fistulas are a common and increasingly recognized cause of spontaneous intracranial hypotension. Most CSF-venous fistulas occur in the thoracic spine and usually arise from nerve root sleeve diverticula. Myelography in the lateral decubitus position is necessary to detect and localize these fistulas, because this technique maximizes contrast density within diverticula, thereby permitting visualization of draining veins. Many modifications to decubitus myelography have been employed in an attempt to improve the conspicuity of CSF-venous fistulas. In theory, maximizing the subarachnoi

Circadian mechanisms underlying post-exercise blood pressure responses in healthy adults.

STUDY OBJECTIVES: We aimed to determine whether the endogenous circadian system modulates blood pressure (BP) and its hemodynamic mechanisms during the post-exercise period. METHODS: Ten healthy adults (mean age: 24 ± 2 [SD] years; 4 males, 6 females) completed a 30-hour circadian protocol in dim light (<8 lux), while all behaviors and measurements were distributed across the 24-hour circadian cycle. Participants underwent five recurring 6-hour cycles of 2-hour sleep opportunities and 4-hour standardized wake episodes. After 90 minutes of awakening during each wake episode, participants perfo

Haemodynamic monitoring and management of the hypotensive out-of-hospital cardiac arrest patient in the adult intensive care unit: a clinical consensus statement of the Association for Acute CardioVascular Care of the ESC.

AIMS: Out-of-hospital cardiac arrest (OHCA) represents a major public health challenge, with high mortality and significant neurological impairments among survivors. Haemodynamic instability, particularly hypotension (a mean arterial blood pressure <65 mmHg), may be a key contributor to post-resuscitation morbidity and mortality. METHODS AND RESULTS: After return of spontaneous circulation, hypotension can result from various causes, including arrhythmias, mechanical complications, thromboembolism, or different types of shock, as well as sedation, temperature control and positive pressure ven

The Diagnostic Burden of Spontaneous Intracranial Hypotension: Imaging Volume and Specialists' Involvement prior to Diagnosis.

BACKGROUND AND PURPOSE: Spontaneous intracranial hypotension (SIH) is a disabling condition that is frequently underdiagnosed as a result of diagnostic challenges. Delays in diagnosis can be attributed to under-recognized MRI findings or a lack of clinical suspicion, given the wide range of symptoms. This study aims to explore the diagnostic burden of SIH by examining the number of imaging examinations and clinician visits before diagnosis. MATERIALS AND METHODS: This retrospective single-institution study included 71 patients with spinal CSF leaks who had a confirmed diagnosis of CSF-venous

Sex-specific multimorbidity clusters and all-cause mortality among community-dwelling older adults: findings from the ASPREE cohort.

BACKGROUND: Multimorbidity is common in older adults, but sex differences in chronic condition clustering remain unclear. This study explored multimorbidity clusters and their associations with all-cause mortality among community-dwelling adults aged 70 years and over. METHODS: This was a secondary analysis of data from 16,095 Australian ASPREE participants aged ≥70 years without prior dementia or cardiovascular disease. Fifteen baseline chronic conditions were grouped using latent class analysis (LCA). Observed-to-expected (O/E) ratios characterized conditions over-represented within cluster

Microcatheter Intrathecal Selective Myelography for Intraprocedural CSF-Venous Fistula Evaluation Pre- and Postembolization.

Recognition of CSF-venous fistulas (CVFs) as a cause of spontaneous intracranial hypotension is increasing. Currently, lateral decubitus digital subtraction myelography and lateral decubitus CT myelography are the mainstays for diagnosing CVFs. Although various methods exist for diagnostic assessment of spinal CVFs, real-time intraprocedural evaluation during endovascular treatment has yet to be described. We report our experience with real-time intraprocedural evaluation of a CVF before and after transvenous embolization using the technique of microcatheter intrathecal selective myelography.

Navigating the epidural plexus: a venographic classification system to predict technical feasibility in cerebrospinal fluid-venous fistula embolization.

BACKGROUND: Navigating the epidural plexus (EP) is a key step in transvenous embolization of cerebrospinal fluid-venous fistulas (CSFVFs) in spontaneous intracranial hypotension (SIH). However, no standard venographic classification currently exists to predict procedural complexity. This study proposes a venographic classification of the EP and evaluates its correlation with technical outcomes. METHODS: From November 2022 to May 2025, 85 patients underwent transvenous embolization for CSFVF. In 75 cases, detailed EP venography was available and categorized as: Type I (55%), homogeneous EP, Ty

Incidence and Predictors of Rebound Intracranial Hypertension after Transvenous Embolization of CSF-Venous Fistulas.

BACKGROUND AND PURPOSE: CSF-venous fistula (CVF) is a common cause of spontaneous intracranial hypotension (SIH) and can be treated with transvenous embolization (TVE). Rebound intracranial hypertension (RIH) is an important periprocedural consideration after TVE and CSF leak closure. The purpose of this study was to determine the incidence and severity of postprocedural RIH among patients with CVFs treated with TVE. MATERIALS AND METHODS: This investigation was a single-center, retrospective cohort of consecutive patients treated with TVE for SIH due to a CVF. Patients were included who had

Spine MRI Diverticular Patterns Predict CSF-Venous Fistula Location: A 100-Patient Study.

BACKGROUND AND PURPOSE: CSF-venous fistulas (CVFs) are an increasingly recognized cause of spontaneous intracranial hypotension and require invasive myelography for localization. Whether spine MRI can noninvasively predict CVF origin remains unclear. The purpose of our study was to determine if spine MRI features, particularly the size and location of spinal meningeal diverticula, are predictive of the CVF location identified on myelography. MATERIALS AND METHODS: Retrospective review was conducted of 100 patients with a CVF confirmed on decubitus CT myelography who underwent preprocedural sp

Efficacy and safety of intensive antihypertensive strategy in patients with low diastolic blood pressure: a secondary analysis of a cluster randomised trial.

BACKGROUND: Intensive blood pressure (BP) control <130/80 mm Hg could bring additional benefits for cardiovascular disease (CVD). However, it is unclear whether intensive BP control remains effective and safe in the general hypertensive population with low diastolic BP (DBP) since a J-shaped relationship between DBP and CVD was observed. METHODS: This is a post hoc analysis of the China Rural Hypertension Control Project. Mixed-effect Cox proportional regression and generalised estimating equation models were used to determine HRs of outcomes by intensive BP control, stratified by baseline DB

Scrub Typhus Meningoencephalitis With Concurrent Brucellosis in Rural Nepal: Diagnostic Challenges With Negative PCR in a Low-Resource Setting.

Scrub typhus is an important cause of acute febrile illness in Nepal and can rarely present with meningoencephalitis and multiorgan involvement. We report a 32-year-old female farmer from rural Nepal who presented during the monsoon season with high-grade fever, myalgia, vomiting, and an eschar following an unidentified forest bite. She developed meningismus, hypotension, mild acute respiratory distress syndrome, and transaminitis. Cerebrospinal fluid showed lymphocytic pleocytosis and elevated protein, while bacterial culture, acid-fast staining, and GeneXpert were negative. Serum scrub typhu
