Crystalloid vs colloid for Hemodynamics during Anesthesia INduction in TAVR patients (CHAIN-T): study protocol for a randomized controlled trial.

OBJECTIVE: To assess whether preload with colloids could reduce hypotension during induction in patients undergoing transcatheter aortic valve replacement (TAVR) compared to crystalloids. METHODS: This study is a single-center, prospective, randomized, double-blind, parallel-controlled trial conducted at Xijing Hospital. A total of 116 patients scheduled for elective TAVR under general anesthesia will be randomly assigned in a 1:1 ratio to either the colloid group or the crystalloid group, receiving either 5 ml/kg of colloid or crystalloid solution before anesthesia induction infused over 15

Circulatory context determines the anti-inflammatory efficacy of post-sepsis vagus nerve stimulation.

Activation of the cholinergic anti-inflammatory pathway (CAP), including vagus nerve stimulation (VNS), suppresses inflammatory cytokine production and has shown efficacy in experimental sepsis when applied prior to sepsis induction. However, VNS before sepsis onset is rarely feasible in clinical practice, and whether VNS remains effective after sepsis onset is unclear. Using a lipopolysaccharide-induced mouse sepsis model, we evaluated VNS applied before and after sepsis onset. VNS applied before sepsis onset significantly reduced plasma TNF-α levels, whereas VNS applied after sepsis onset fa

A gastric dieulafoy lesion presenting with life-threatening upper gastrointestinal hemorrhage during recovery from hypertriglyceridemic acute pancreatitis: a case report.

BACKGROUND: Dieulafoy lesions are an uncommon but potentially life-threatening cause of non-variceal upper gastrointestinal bleeding, caused by rupture of a large-caliber submucosal artery through a minute mucosal defect; diagnosis is difficult because bleeding is intermittent and the surrounding mucosa appears nearly normal. Hemorrhage from a gastric Dieulafoy lesion during recovery from hypertriglyceridemic acute pancreatitis (HTG-AP) has rarely been reported. CASE PRESENTATION: A man in his twenties with severe obesity presented with acute epigastric pain after alcohol and a high-fat meal.

[Idiopathic systemic capillary leak syndrome - a rare but serious differential diagnosis in the hypovolemic patient].

Idiopathic systemic capillary leak syndrome (SCLS) is a rare, life-threatening cause of hypovolemic shock characterized by hemoconcentration, hypoalbuminemia, and acute hypovolemia due to plasma extravasation. It should be suspected in patients presenting with unexplained hypotension and markedly elevated hemoglobin levels, often following nonspecific prodromal symptoms such as malaise. Management is supportive and includes aggressive crystalloid resuscitation, vasopressor therapy, corticosteroids, and intravenous immunoglobulins. Owing to the risk of rapid hemodynamic deterioration and multi-

Primary Adrenal Insufficiency in a Patient with Mosaic Klinefelter Syndrome.

Klinefelter syndrome (KS) is a common sex chromosome disorder characterized by hypergonadotropic hypogonadism. Primary adrenal insufficiency (PAI) has rarely been reported in association with KS, particularly in mosaic cases. We report a case involving a 39-year-old man with mosaic KS (47,XXY/46,XY) and PAI. He presented with hyperkalemia and subtle clinical features without hypotension. Endocrinological evaluation confirmed PAI, and elevated gonadotropin levels with low testosterone levels indicated primary hypogonadism. Steroid profiling using liquid chromatography-tandem mass spectrometry s

Early outcomes of transcatheter vacuum-assisted mass extraction in the left heart and the aortic arch.

BACKGROUND: Transcatheter vacuum-assisted mass extraction (TVME) in the systemic circulation is increasingly being reported. However, its feasibility, effectiveness, and safety remain poorly defined. AIMS: We sought to investigate the feasibility, effectiveness and safety of TVME in the left heart and aortic arch. METHODS: Consecutive patients from 6 centres in the United States and Canada were included. Feasibility was defined as technically successful mass extraction without unplanned cardiac surgery or intraprocedural mortality. The effectiveness endpoint was feasibility with successful d

Association of Iron Deficiency Anemia with Outcomes in Atrial Fibrillation: A Nationwide Analysis.

BACKGROUND: Patients with atrial fibrillation (AF) frequently have iron deficiency anemia (IDA), but the mechanism by which this condition affects clinical outcomes is not well understood. The purpose of this study was to assess whether IDA is associated with mortality in patients with AF. METHODS: Data from the National Inpatient Sample (NIS) for 2021-2022 examined IDA and inpatient outcomes in a nationally representative cohort of hospitalized adults diagnosed with AF. Patients were categorized by IDA status. The factors assessed were mortality, hospital and intensive care unit (ICU) length

Age-related differences in clinical manifestations, neuroimaging features, and treatment of spontaneous intracranial hypotension: a retrospective cohort study.

BACKGROUND: The influence of age on the management of spontaneous intracranial hypotension (SIH), particularly epidural blood patch (EBP), remains unclear. This study aims to characterize age-related differences in clinical manifestation, neuroimaging features, and treatment outcomes. METHODS: In this retrospective cohort study, we analyzed 54 SIH patients who underwent EBP at our center. Patients were classified into two groups based on the cohort median age (35 years). Demographic, clinical, neuroimaging features, and treatment outcomes were compared between the two groups. We further utili

Potential sphenopalatine ganglion block efficacy as an alternative to epidural bloodpatch for post-dural puncture headache: a monocentric french study.

BACKGROUND: Post-dural puncture headache (PDPH) is a common complication of lumbar puncture caused by cerebrospinal fluid leakage and intracranial hypotension. Epidural blood patch (EBP) is considered the gold standard treatment but remains invasive. Sphenopalatine ganglion block (SPGB), a non-invasive technique, has shown promising results in recent studies, but it was not evaluated as an alternative of EBP. This study aimed to evaluate the effectiveness of SPGB in PDPH by determining whether it could reduce the need for EBP in clinical practice. METHODS: We conducted a prospective monocentr

Advanced CT Myelography for Spontaneous Cerebrospinal Fluid Leaks.

Spontaneous intracranial hypotension is a frequently debilitating condition that is most commonly caused by loss of cerebrospinal fluid (CSF) from the spine. The most frequently occurring types of spontaneous spinal CSF leaks that cause spontaneous intracranial hypotension are ventral dural tears, lateral dural tears, and CSF venous fistulas, but other leak types are recognized including sacral dural tears or fistulas into vascular malformations. Localization and characterization of spinal CSF leaks with CT myelography facilitates subsequent targeted treatment. A tailored approach based on the

Neonatal Common Atrioventricular Valve Replacement After Failed Staged Biventricular Repair.

Balanced complete atrioventricular septal defect (cAVSD) with aortic coarctation (CoA) is generally considered amenable to staged biventricular repair (BVR). However, anatomic suitability does not necessarily guarantee functional stability. We report a 3.5-kg neonate with balanced cAVSD (Rastelli type A), severe common atrioventricular valve regurgitation (CAVVR), and CoA. Preoperative echocardiography indicated suitability for BVR. At 9 days of age, arch repair and pulmonary artery banding (PAB) were performed. However, weaning the patient from cardiopulmonary bypass was impossible because of

Frespaciguat (MK-5475): A Novel Inhaled Soluble Guanylate Cyclase Stimulator for Pulmonary Hypertension Associated with COPD.

Soluble guanylate cyclase (sGC) stimulators are a validated therapeutic approach for pulmonary vasodilation; however, oral agents can cause systemic hypotension and may worsen ventilation-perfusion mismatch in pulmonary hypertension associated with chronic obstructive pulmonary disease (PH-COPD). To address these limitations, we applied a property-driven medicinal chemistry strategy to develop an inhaled sGC stimulator that maximizes pulmonary exposure while minimizing systemic exposure. Structure-activity relationship and pharmacokinetic studies identified high systemic clearance and low perm

Falciform Ligament Inflammation Complicated by Umbilical Vein Thrombophlebitis, Left Portal Vein Thrombosis, and Multiple Hepatic Abscesses: A Case Report.

Falciform ligament pathology is a rare cause of acute upper abdominal pain and may resemble more common pancreatic and hepatobiliary disorders. A woman in her 50s presented with sudden severe epigastric pain radiating to her back and marked hyperamylasaemia of 1,747 U/L. This combination fulfilled clinical and biochemical diagnostic criteria for acute pancreatitis, although computed tomography (CT) demonstrated no pancreatic inflammation and instead showed severe inflammatory change centred on the falciform ligament, with associated focal fat infarction, left portal vein thrombosis, and gas lo

Effect of Etomidate Induction on Intraoperative Hemodynamics, Postoperative Inflammation, and Recovery After Laparoscopic Sleeve Gastrectomy: A Retrospective Cohort Study.

BACKGROUND: Etomidate is commonly used for anesthetic induction because of its favorable hemodynamic profile but may cause adrenocortical suppression. Whether its use is associated with hemodynamic stability, and postoperative inflammatory and recovery outcomes after laparoscopic sleeve gastrectomy (LSG) remains uncertain. METHODS: This single-center retrospective cohort study included adults undergoing elective LSG from January 2014 through December 2024. The exposure was etomidate use during anesthetic induction. The primary outcome was time-weighted average MAP (TWA-MAP). Secondary outcome

A Case of Suspected Scombroid Poisoning in Intensive Care Staff: Is It Anaphylaxis?

Scombroid poisoning (histamine fish poisoning) is an uncommon foodborne illness that frequently mimics immunoglobulin E-mediated anaphylaxis. We report a case of a previously well 28-year-old intensive care nurse who developed abrupt facial flushing, wheeze, dizziness, and hypotension after consuming canned tuna. Initial observations demonstrated sinus tachycardia and hypotension. She was treated according to established anaphylaxis guidelines with two doses of intramuscular adrenaline, intravenous crystalloid resuscitation, and subsequent adrenaline infusion in intensive care. She recovered r

A Hidden Catastrophe: Ruptured Abdominal Aortic Aneurysm Presenting as Acute Ischemic Stroke.

A ruptured abdominal aortic aneurysm (AAA) is a life-threatening emergency that typically presents with hypotension or abdominal pain but rarely with primary neurological deficits. We report the case of a 74-year-old woman presenting with acute focal neurological deficits concerning for ischemic stroke, whose preceding abdominal pain and lower extremity mottling ultimately pointed toward an alternative diagnosis. Point-of-care ultrasound and subsequent computed tomography angiography revealed a ruptured infrarenal AAA. We discuss how consideration of these additional clinical features led to t

Hemodynamic Effects of Interfacility Dexmedetomidine Infusions During Transport of Patients with Alpha-2-adrenergic Agonist Withdrawal.

BACKGROUND: In the Philadelphia region, there has been an increase in the potency of illicit opioids as well as the addition of nonopioid adulterants. Currently, medetomidine, a veterinary anesthetic that acts as an alpha-2-adrenergic agonist, is a common adulterant in the illicit opioid supply, causing both unique overdose and withdrawal syndromes. Although opioid withdrawal alone is not deadly, with added adulterants, especially medetomidine, this is no longer the case. Dexmedetomidine is an alpha-2-adrenergic agonist frequently used for sedation in the emergency department and intensive car
