Management of dual antiplatelet therapy following covered femoropopliteal stenting - Why size matters.

Background: Endovascular therapy has emerged as an effective minimally invasive approach for treating peripheral artery disease (PAD). Postprocedural dual antiplatelet therapy (DAPT) is commonly used to prevent adverse events, but optimal management remains uncertain. Patients and methods: This international, multicentre retrospective study synthesized data from three studies on covered stents (VIABAHN® Endoprosthesis) in 437 PAD patients (2001-2015). A dimension index (total stent length/smallest diameter) was developed and externally validated in a second cohort (n=148). Results: Among 321 p

Misplacement of a femoral venous catheter into the ascending lumbar vein: A case report and first use of balloon catheter compression hemostasis.

RATIONALE: This case report aims to highlight a rare but life-threatening complication of femoral venous catheterization and to describe a novel endovascular technique for its management. Non-tunneled femoral catheters provide rapid vascular access for emergency dialysis (e.g., arteriovenous graft [AVG] occlusion, hyperkalemia) but carry risks of vascular injury, potentially causing fatal bleeding. This is the first report of hemorrhagic shock due to ascending lumbar vein rupture from femoral catheter misplacement, successfully managed by balloon compression hemostasis. PATIENT CONCERNS: A 48

18F-FDG PET/CT in lymphangiosarcoma: A case report and review of literature.

OBJECTIVES: Lymphangiosarcoma is a rare tumor that affects the upper limbs of patients who have undergone breast cancer surgery, including axillary dissection, followed by radiation therapy (RT) to the axilla and has a poor prognosis. 18F-FDG PET/CT may enable the earlier detection of malignant transformation in a setting of chronic lymphedema and help evaluate the extent and staging of the tumor, allowing earlier initiation of treatment options. CASE PRESENTATION: We herein report a case of cutaneous lymphangiosarcoma in a 47-year-old breast carcinoma patient, which occurred 9 years after in

Alternative percutaneous management of iatrogenic femoral artery pseudoaneurysms using plug-based closure devices after failed us-guided compression and evaluation of risk factors for compression failure.

PURPOSE: Pseudoaneurysms (PAs) arise from the disruption of the arterial wall, resulting in extravasation of blood into the surrounding tissue. Iatrogenic sources are a leading cause of PA formation. There is an upward trend in the incidence of PAs due to the growing number of endovascular procedures. This study identifies the risk factors associated with the failure of Ultrasound-guided compression repair (UGCR) and evaluates the efficacy of vascular closure devices (VCDs) in the percutaneous treatment of iatrogenic femoral artery PAs. There are case reports in the literature, with a few case

Forequarter Amputation for Breast Cancer. Systematic Review and Survival Analysis.

INTRODUCTION: Forequarter amputation (FA) or Interscapulothoracic disarticulation is indicated in patients with invasive breast carcinoma (BC) in selected cases: primary resection of a locally advanced or recurrent tumor, Stewart & Treves syndrome (STS), or sarcoma secondary to radiation due to breast cancer (radiation-induced sarcoma, RIS). However, no studies have robustly evaluated the indications, results, complications, recurrence and overall survival in the context of FA in patients with BC. MATERIAL AND METHODS: We performed a systematic review of 8 databases according to the PICOS and

Breaking the Rhythm: Extracorporeal Membrane Oxygenation (ECMO) as a Life Saver in a Case of Resistant Arrhythmia and Cardiac Arrest.

Refractory ventricular arrhythmias and cardiac arrest in younger patients are rare, life-threatening events, particularly when standard resuscitation fails. Extracorporeal membrane oxygenation (ECMO) provides temporary circulatory support, enabling hemodynamic stabilization, myocardial recovery, and time for diagnostic and therapeutic interventions. We report the case of a 40-year-old East African woman, medically free at baseline, who developed refractory ventricular fibrillation (VF) and cardiogenic shock during elective myomectomy on 12th August 2024. She underwent 45 minutes of cardiopulmo

Using Transmitral Pressure Gradients and Residual Mitral Regurgitation to Optimize Outcome After Transcatheter Edge-to-Edge Repair.

BACKGROUND: Although reducing mitral regurgitation (MR) after mitral transcatheter edge-to-edge repair (M-TEER) improves outcomes, the impact of increased transmitral mean pressure gradient (TMPG) remains controversial. OBJECTIVES: This study aimed to evaluate the clinical significance of MR reduction and TMPG elevation in patients with functional mitral regurgitation (FMR) after M-TEER. METHODS: A total of 2,360 FMR patients were evaluated using postdischarge echocardiography after M-TEER. The relationship between TMPG and outcomes was assessed using spline analysis and group-based comparis

Psychosocial Impact of Postmastectomy Lymphedema Syndrome: Insights From a National Claims Database Analysis of Antidepressant Prescription Fills.

BACKGROUND: Postmastectomy lymphedema syndrome (PMLS) has been associated with lower quality of life due to symptoms and financial consequences. This study assessed the impact of PMLS on psychosocial well-being using antidepressant fill patterns. We hypothesized that PMLS increased the odds and duration of antidepressant fills. METHODS: Mastectomy encounters in the Merative MarketScan Research Databases, 2007-2022, were extracted using Current Procedural Terminology codes. Among these, patients who developed PMLS and filled antidepressant prescriptions were identified using the International

Sex differences in hemodynamics and outcomes after transcatheter aortic valve replacement.

BACKGROUND: Women remain underrepresented in transcatheter aortic valve replacement (TAVR) trials, despite well-recognized anatomical differences that necessitate tailored procedural strategies. This study assessed sex-specific differences in baseline characteristics, procedural approaches, hemodynamic outcomes, and their impact on 5-year all-cause mortality. METHODS: We analyzed data from 20,094 patients in the IMPPACT TAVR registry. Hemodynamic outcomes, including prosthesis-patient mismatch (PPM), were defined according to Valve Academic Research Consortium-3 criteria. Kaplan-Meier and Cox

Clinical Impact of In-Hospital Hemoglobin Decline Without Overt Bleeding After Transcatheter Aortic Valve Replacement.

BACKGROUND: The Valve Academic Research Consortium recently expanded its bleeding endpoint definition, recommending that any procedure-related blood loss should be classified as bleeding, even in the absence of a clinically evident source. However, the lack of specific reference thresholds for hemoglobin decline limits the ability to discriminate clinically relevant events and the prognostic utility of these criteria. OBJECTIVES: To evaluate the incidence, predictors, and prognostic implications of hemoglobin declines without overt bleeding in patients undergoing transcatheter aortic valve re

Clinical implications and feasibility of cerebral autoregulation-based precision blood pressure monitoring in major noncardiac surgery: A protocol for AUTOREGULATE-NONCARDIAC, a multicentre prospective cohort study and peri-operative precision medicine platform.

BACKGROUND: Peri-operative hypotension is strongly associated with organ injury following noncardiac surgery, however hypotension avoidance trials have not shown meaningful improvements in cardiovascular outcomes and only inconsistent improvements in renal and neurological outcomes. The true haemodynamic drivers of peri-operative organ injury are probably falls in BP below individual autoregulatory boundaries and not below population-based harm thresholds. Novel methods of personalising peri-operative blood pressure (BP) management are needed. Cerebral autoregulation (cAR)-guided precision BP

Renal Denervation in Asia: 2025 Asia Renal Denervation Consortium (ARDeC) Consensus Statement Endorsed by the Hypertension Cardiovascular Outcome Prevention and Evidence in Asia (HOPE Asia) Network.

The second Asia Renal Denervation Consortium consensus conference shared information and developed updated recommendations for renal denervation (RDN). Current evidence confirms that RDN significantly reduces blood pressure across all metrics (office, home, and ambulatory) throughout 24 hours. Modern RDN approaches target the distal main renal artery and branches where nerves more closely approximate the vessel wall. Understanding renal artery anatomy is crucial; the main renal artery typically divides into anterior and posterior divisions as first-order branches, which further subdivide into
