An insight into short- and long-term mechanical circulatory support systems.

Cardiogenic shock due to acute myocardial infarction, postcardiotomy syndrome following cardiac surgery, or manifestation of heart failure remains a clinical challenge with high mortality rates, despite ongoing advances in surgical techniques, widespread use of primary percutaneous interventions, and medical treatment. Clinicians have, therefore, turned to mechanical means of circulatory support. At present, a broad range of devices are available, which may be extracorporeal, implantable, or percutaneous; temporary or long term. Although counter pulsation provided by intra-aortic balloon pump

Postcardiotomy syndrome: recurrent cardiac tamponade and an exquisite steroid response.

A 26-year-old woman presented moribund with fever and pleuritic chest pain 3 times in 4 months following elective aortic root surgery. She was admitted 41 days after surgery with cardiac tamponade requiring surgical drainage twice within 1 week. Despite this, she was re-admitted for a second time 4 days after discharge with persistent pericardial effusion. High fevers and an incidental regurgitant murmur were extensively investigated for and treated as possible endocarditis or graft infection without conclusive results. The patient spent a total of 61 days in hospital during this period, recei

Prediction of survival in patients with cardiogenic shock and multiorgan failure treated with biventricular assist device.

Mechanical circulatory support (MCS) is valuable in saving the lives of patients with severe cardiogenic shock. However, their survival is limited if multiorgan failure (MOF) proves to be irreversible. Although ventricular assist devices (VADs) have been shown to reverse end-organ failure in some patients, the pathophysiological mechanisms of end-organ failure and its regression are not fully understood, and clinical markers and thresholds for the "point of no return" are lacking. We investigated predictors of 30-day survival in patients supported with a biventricular assist device (BVAD). We

Early and late outcomes of cardiac operations in patients with cirrhosis: a retrospective survival-rate analysis of 47 patients over 8 years.

OBJECTIVES: Patients with liver cirrhosis are considered as high-risk population for cardiac surgery. The aim of this study was to review mortality and mid-term outcome of patients with liver cirrhosis requiring coronary artery bypass graft (CABG), valve replacement, or combined procedures. METHODS: Between July 1997 and December 2006, 47 patients (mean age 65.4 ± 11.7 years) with liver cirrhosis were operated for CABG (21 patients), aortic valve replacement /mitral valve replacement (14 patients), CABG/VR (9 patients) or aortic dissection/tumorexstirpation (3 patients) (group I). Thirty-thre

[Postcardiotomy syndrome outside a cardiosurgical clinic].

Postcardiotomy syndrome (PCTS) is an idiopathic inflammatory disease affecting pericardium and pleura; it complicates the postoperative period in cardiosurgery. High efficiency of corticosteroid therapy suggests autoimmune nature of PCTS. As a rule, this condition develops within two weeks after surgery although cases of its delayed onset (after 1 year) were reported too. PCTS may proceed both as an acute disease and as a mild disorder characterized by general inflammation and obscure serositis making difficult its diagnosis. The existence of such "restricted" forms of PCTS is not universally

[Aortocoronary bypass and cardiac valve surgery].

In Germany, approximately 100,000 patients underwent cardiac surgery in 2005. The most important postoperative problems with prognostic impact for these patients were acute heart failure, acute renal failure, infections, neurological complications, postcardiotomy syndrome, cardiac tamponade and atrial fibrillation. The "EuroSCORE" is a relevant predictor of immediate and long-term outcome in these patients. The patency rates of coronary artery bypass grafts (CABG) are important for the long-term outcome in such surgery. There has been a significant increase in the number of patients aged 80 ye

[Integrated case fees in cardiosurgery--a pilot project for fast-track rehabilitation].

BACKGROUND: Lengthy recovery and treatment times following cardiosurgical interventions were the motivation for introducing a pilot procedure to integrate acute and rehabilitative treatment structures. The advantage of such a pilot procedure is the medico-economic link between direct transition from acute care to rehabilitation treatment and cutting average case costs. With this in mind, shared case fees for patients following cardiosurgery are being agreed in a pilot project between health insurance companies, acute-care hospitals and rehabilitation clinics. The aim of this study was thus to

Cardiac assist devices for end-stage heart failure.

Cardiac assist devices have become an important therapy for patients with end-stage cardiac failure. These devices continue to evolve. Most current devices provide temporary, left-sided support for patients with cardiogenic shock from postcardiotomy syndrome or for patients awaiting cardiac transplantation. The two most frequently used left ventricular assist devices as a bridge to transplant are the Thermocardiosystems (HeartMate I) and the Novacor devices. The selection criteria for the use of these devices and treatment of these patients will be reviewed. Additionally, other currently avail

Cardiac tamponade after removal of atrial intracardiac monitoring catheters in a pediatric patient: case report.

The incidence of cardiac tamponade after cardiac surgery is reported as ranging from 0.04% to 7%. Although a relatively infrequent complication, tamponade is associated with significant morbidity and mortality. Reports of tamponade after pediatric cardiac surgery are few and generally associated with postcardiotomy syndrome or, less commonly, removal of left atrial or pulmonary artery catheters after surgery. A case is presented of cardiac tamponade in a pediatric patient resulting from removal of a direct atrial and a pulmonary artery catheter after cardiac surgery. The pathophysiology of car

[Transitory left ventricular outflow tract obstruction after mitral valve reconstruction].

HISTORY AND ADMISSION FINDINGS: A few days after uneventful surgical reconstruction of the mitral valve a 43-year-old man was found to have a systolic murmur due to prolapse of the posterior leaflet, suggesting renewed mitral regurgitation. INVESTIGATIONS: Echocardiography revealed haemodynamically significant left ventricular outflow tract obstruction (LVOT) with a left ventricle to aorta systolic gradient of 83 mm Hg. In addition there was moderately severe mitral regurgitation as well as a pericardial effusion but no signs of tamponade. TREATMENT AND COURSE: The obstruction was at first t

Influence of different assist devices on survival after orthotopic heart transplantation.

BACKGROUND: In 1995, a risk factor of 1.88 was indicated for one-year mortality in connection with bridging to heart transplantation. Both one-year and three-year survival rates in patients bridged to transplantation were less than 80%. METHODS: From 3/89 to 12/98, 903 orthotopic heart transplantations were performed at our center in 888 recipients. Bridging was necessary in 142 patients. RESULTS: The one-year survival rate was 76% in pts without VAD, 86% in pts bridged with VAD and 66% in pts with VAD due to postcardiotomy syndrome. The three-year survival rates were 73%, 80% and 55% respec

Constrictive pericarditis after coronary artery bypass surgery as a cause of unexplained dyspnea: a report of five cases.

Constrictive pericarditis after coronary artery bypass grafting (CABG) is rare and can present as unexplained dyspnea. We report five consecutive cases of post-CABG constrictive pericarditis seen within a period of 17 months at our institution. All patients presented with heart failure of unknown etiology within a period of 8-84 months after surgery. During the initial post-CABG period, two patients had developed postcardiotomy syndrome that was successfully treated with steroids. They were all assessed noninvasively and invasively. In all patients, the diagnosis of constriction was initially

Delayed cardiac tamponade in a patient with penetrating chest trauma.

We describe a case of cardiac tamponade due to pulmonary artery laceration as a late sequela in a patient who had sustained penetrating chest trauma. A 35-yr-old man presented to our emergency department complaining of pleuritic left chest pain, shortness of breath, and fever 19 days after being hospitalized for a stab wound to the left chest. During his first hospitalization, chest X-ray study, echocardiogram, and central venous pressure determination were all normal. On second presentation, he had a cardiac tamponade and underwent a median sternotomy. A pulmonary artery laceration was discov

[Surgical aspects of pulmonary thrombendarterectomy].

Pulmonary thromboendarterectomy is an accepted operative procedure for treatment of pulmonary hypertension due to chronic embolism. Despite its proven value this procedure has been established at very few centers worldwide. In this paper we report our actual operative concept and operative results. Between 8'89 and 4'96 127 patients were operated with use of extracorporeal circulation, deep hypothermia and circulatory arrest. After analysis of the initial high perioperative mortality (26%, 29/108) our operative and postoperative concept changed since 11'94: 1. central incision of both pulmonar

Postcardiotomy syndrome following transvenous pacemaker insertion.

We describe a case of postcardiotomy syndrome following permanent pacemaker insertion. A 79-year-old woman presented with complete heart block. Following temporary pacing, a dual chamber permanent pacemaker was inserted. Two weeks later she developed fevers and malaise with an acute rise in inflammatory markers. After systemic infection had been carefully excluded, she was commenced on prednisolone with immediate benefit. Prednisolone was withdrawn over 2 months without recurrence of symptoms. Although the postcardiotomy syndrome is rare following transvenous pacemaker insertion it should be c

The postcardiac injury syndromes.

Late pericarditis following myocardial infarction, cardiac surgery, or trauma is referred to as postmyocardial infarction syndrome (PMIS) or postcardiotomy syndrome (PCS), respectively. The term postcardiac injury syndrome (PCIS) is used to encompass both these entities. PCIS is characterized by fever, pleuropericardial pain, pericarditis, and pulmonary involvement. Abnormal laboratory findings include leukocytosis, high sedimentation rate, and chest x-ray abnormalities of pleural effusion with or without pulmonary infiltrates. Evidence supports an immunopathic etiology; viruses may play a con

[Care of patients following coronary bypass surgery from the internist's viewpoint].

Follow-up of bypass patients in the early postoperative phase involves the management of complications such as perioperative myocardial infarction, postoperative arrhythmias, pericarditis, postcardiotomy-syndrome, fever, infection and chest pain. The longterm management has to focus on changes in lifestyle with particular regard to risk factors for coronary atherosclerosis. Diagnostic tools for work-up of postoperative chest pain include stress testing and radionuclide techniques; ultrafast computerized tomography is superior in the evaluation of bypass function to cine-NMR. Sclerosing mediastinitis: improved management with histoplasmosis titer and ketoconazole.

Recognition that many patients with benign sclerosing mediastinitis have smoldering disease responsible for failure of surgical procedures or for development of collateral circulation in patients with superior vena caval obstruction has markedly improved management of these difficult patients. Histoplasmosis complement fixation titers have been used to detect unsuspected subacute disease and to follow the therapeutic adjunctive management with ketoconazole, an oral antifungal agent. Twenty-two patients with benign sclerosing mediastinitis demonstrated a variety of symptoms relating to the area
