Rapidly Growing Malignant Peripheral Nerve Sheath Tumors Arising From Neurofibromatosis Type 1: A Case Report by Rehabilitation Physicians.

We describe a case of a 40-year-old Japanese woman with rapidly growing malignant peripheral nerve sheath tumors (MPNSTs) arising from neurofibromatosis type 1 (NF-1). The patient presented numbness in both legs, back pain, and gait disturbances. Magnetic resonance imaging (MRI) revealed a spinal tumor at the thoracic level. To resolve her symptoms, a laminectomy and intradural tumor resection were performed. The tumor was diagnosed as a neurofibroma with no malignant characteristics. After the surgery, she participated in a rehabilitation program aimed at promoting independence in daily activ

Impact of stress and preoperative psychological preparation on immunity, inflammatory responses, and surgical outcomes in adults undergoing elective surgery with general anesthesia in Palestine.

Stress is a normal human physiological reaction that can unfavorably affect the body, such as cardiac and immunological disturbances. Several articles have shown how stress can affect the immune status. This study was designed to assess the impact of stress on the immune system using the WBC count as an indicator of immune status in the human body. In a tertiary care teaching hospital, a prospective observational study was carried out with a total of 309 samples collected; two complete blood counts were observed for each patient, one at the anesthesia clinic one week before the surgery and the

A Review of Intracardiac Mass Cases Encountered Over a Two-Year Period.

Intracardiac mass is a rare condition that poses significant diagnostic and therapeutic challenges. Regardless of the pathological diagnosis, these cases are often difficult to manage, as patients typically present with urgent clinical symptoms at their initial visit. Most symptoms result from blood flow disturbances caused by a large intracardiac tumor. Furthermore, patients face a high risk of thromboembolism. Herein, we present five cases of intracardiac masses, including three malignant tumors, encountered over approximately two years. Each case had a different pathological diagnosis: angi

Improvement in Upper Limb and Systemic Myalgic Encephalomyelitis/Chronic Fatigue Syndrome (ME/CFS) Symptoms After Surgical Treatment of Neurogenic Thoracic Outlet Syndrome.

Thoracic outlet syndrome (TOS) is characterized by compression of nerves or blood vessels as they pass through the scalene triangle and the costoclavicular space, and under the pectoralis minor. Common symptoms include arm fatigue and heaviness, paresthesias, and neck and upper back pain, provoked by arm extension or elevation. We have recently reported that some myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS) patients report symptoms suggestive of TOS, specifically with respect to overhead activity, but there is uncertainty whether this overlap in symptoms is more related to ME/CF

Association of Gla-Rich Protein (GRP) with Inflammatory Markers in Critically Ill Patients: A Cross-Sectional Observational Study.

OBJECTIVES: Gla-rich protein (GRP), a vitamin K-dependent protein, has been increasingly recognized for its dual role in modulating inflammation and inhibiting pathological calcification. Despite its emerging importance in chronic conditions, limited evidence exists regarding its behavior during acute critical illness. This study aimed to investigate the association between GRP, systemic inflammatory markers, oxidative stress (via total thiol oxidation-reduction ratio, TORR), and calcium metabolism in critically ill patients. MATERIALS AND METHODS: This cross-sectional observational study inc

Cerebral Hemodynamic Changes and Postoperative Brain Function in Patients Undergoing Cardiac Valve Surgery With Cardiopulmonary Bypass: A Prospective Cohort Study.

Background Cardiopulmonary bypass (CPB) during cardiac surgery disrupts cerebral hemodynamics, potentially leading to postoperative neurological complications such as delirium. Transcranial Doppler (TCD) offers non-invasive monitoring of cerebral blood flow alterations, yet its predictive value for delirium remains underexplored. This study aims to investigate the association between intraoperative cerebral hemodynamic changes monitored by TCD and postoperative delirium in patients undergoing cardiac valve surgery with CPB. Methods This prospective cohort study enrolled 184 elective cardiac va

Presenting symptoms and outcomes of cervical and thoracic ependymomas compared to conus and filum ependymomas.

OBJECTIVE: Prior studies on spinal ependymoma typically group patients based on tumor pathology, often combining myxopapillary ependymomas and other ependymoma subtypes into a single cohort. This study aimed to evaluate differences in patient presentation and long-term functional outcomes between cervical/thoracic and conus/filum ependymomas. The authors hypothesized that these variations are location specific and warrant further stratification. METHODS: The medical records of adult patients who underwent resection for spinal ependymoma at a single tertiary-care institution between 2006 and 2

Arrhythmias in Systemic Sclerosis: A Call for Interdisciplinarity Teams.

BACKGROUND: Systemic sclerosis (SSc) is a rare autoimmune disease characterized by progressive fibrosis, systemic inflammation and vascular dysfunction, with manifestations that can affect multiple organs, including the heart. Cardiac involvement in SSc is often underdiagnosed, although it can have serious consequences on the prognosis, especially the occurrence of arrhythmias. These rhythm disturbances can result from direct damage to the myocardium, the conduction system, or the coronary microcirculation. Equally, the medication used can have iatrogenic consequences manifested by severe arrh

Longitudinal Trends in Myocardial Metabolic Uptake Following Stereotactic Arrhythmia Radioablation Using 2-Deoxy-2-[18F]Fluoro-D-Glucose Positron Emission Tomography-Computed Tomography.

PURPOSE: Ventricular tachycardia (VT) is a life-threatening rhythm disturbance of the heart that has been shown to be amenable to treatment with stereotactic body radiation therapy, known as stereotactic arrhythmia radioablation (STAR). The metabolic impact of directly targeting arrhythmogenic regions of scarred left ventricle (LV) is unknown. In this study, the relationship between radiation dose and 2-deoxy-2-[18F]fluoro-D-glucose positron emission tomography (18FDG-PET) uptake in LV segments following STAR was interrogated. METHODS AND MATERIALS: Patients with VT in the ENCORE-VT trial rec

Breaking the Rhythm: Extracorporeal Membrane Oxygenation (ECMO) as a Life Saver in a Case of Resistant Arrhythmia and Cardiac Arrest.

Refractory ventricular arrhythmias and cardiac arrest in younger patients are rare, life-threatening events, particularly when standard resuscitation fails. Extracorporeal membrane oxygenation (ECMO) provides temporary circulatory support, enabling hemodynamic stabilization, myocardial recovery, and time for diagnostic and therapeutic interventions. We report the case of a 40-year-old East African woman, medically free at baseline, who developed refractory ventricular fibrillation (VF) and cardiogenic shock during elective myomectomy on 12th August 2024. She underwent 45 minutes of cardiopulmo

Mechanisms and Clinical Implications of the Post-traumatic Stress Disorder (PTSD)-Cardiovascular Disease Link.

Post-traumatic stress disorder (PTSD) is increasingly recognized as a significant contributor to cardiovascular morbidity and mortality. The aim of this review was to synthesize current evidence on the biological, behavioral, and clinical mechanisms linking PTSD with cardiovascular disease (CVD) and to highlight clinical implications for screening, prevention, and management. To achieve this, relevant studies were identified through targeted searches of PubMed, Scopus, Web of Science, and Google Scholar using combinations of terms related to PTSD, CVD, autonomic dysfunction, inflammation, endo

Clinical implications and feasibility of cerebral autoregulation-based precision blood pressure monitoring in major noncardiac surgery: A protocol for AUTOREGULATE-NONCARDIAC, a multicentre prospective cohort study and peri-operative precision medicine platform.

BACKGROUND: Peri-operative hypotension is strongly associated with organ injury following noncardiac surgery, however hypotension avoidance trials have not shown meaningful improvements in cardiovascular outcomes and only inconsistent improvements in renal and neurological outcomes. The true haemodynamic drivers of peri-operative organ injury are probably falls in BP below individual autoregulatory boundaries and not below population-based harm thresholds. Novel methods of personalising peri-operative blood pressure (BP) management are needed. Cerebral autoregulation (cAR)-guided precision BP
