Treatment of acute maxillary sinusitis. Erythromycin base and phenoxymethyl-penicillin (penicillin V).

One hundred patients with acute maxillary sinusitis have been studied. The diagnosis was made clinically and confirmed radiologically, using a roentgenological 6-point score for mucous membrane thickening and secretion. Fifty patients in each group were treated with either erythromycin base or phenoxymethyl-penicillin (penicillin V). The roentgenological state of each sinus was classified on the 1st, 5th, 10th, and 15th day, giving an objective evaluation of the treatment effect. There was no significant difference between the two treatment modes as to therapeutic outcome, and the results are

Comparison of subjective and radiological findings during the course of acute maxillary sinusitis.

Some of the most common symptoms and signs of acute maxillary sinusitis were graded and compared with the radiological findings during the course of the disease. Four different treatment modes were used in 176 patients. The radiological and the subjective findings did not differ among the groups, which consequently were evaluated as an entity. The radiographic results demonstrated that the healing course during the first 15 treatment days follows an even regression line. The subjective findings show a more marked improvement during the first five days than subsequently, and also a more rapid i

Effects of drainage in the treatment of acute maxillary sinusitis.

24 patients with maxillary sinusitis were studied with regard to the effect of therapy. 8 of the patients were treated with a single dose of antibiotics and repeated antral aspirations at close intervals. 16 other patients were treated with repeated aspirations only. In the first group, 'cure', in the sense of freedom from retained secretion, was achieved after 3-5 aspirations within 2-10 hours. In the second group, changes in the proteolytic activity and the concentrations of albumin, IgG, IgM and IgA were followed. In purulent secretions the proteolytic activity was high and the concentratio

[Transport of cefotetan into the otorhinolaryngeal tissues].

The tissue specimens were taken to evaluate the efficacy of cefotetan (CTT), a new cephamycin antibiotic, for otorhinolaryngological infectious diseases. The concentrations of CTT in palatine tonsils, maxillary sinus mucosa, wall of maxillary sinus cysts, discharge from otitis media and saliva were studied. The serum level determined at 3 hours following intravenous injection of 1 g of the drug was 49.2 micrograms/ml (n = 4). The serum level at 12 hours following intravenous injection of 2 g of the drug was 14.4 micrograms/ml (n = 4). 15-25% of the serum levels of CTT were demonstrated in pala

Acute maxillary sinusitis--a comparison between 27 different treatment modes.

When comparing the outcome of different treatment modes for acute maxillary sinusitis, the roentgenological examination can be recommended for the objective evaluation as a complement to the clinical examination. Using the occipito-mental projection and an additional occipito-mental side view, the diagnosis can be established in a cheap and simple way, and the treatment effect can be followed during the course of treatment in the form of radiological sinus changes. The treatment outcome of 27 different treatment modes in 1320 cases of acute maxillary sinusitis (2039 maxillary sinuses) was comp

Bacampicillin in acute maxillary sinusitis: concentration in sinus secretion and clinical effect. A randomized, double-blind study of two dosage regimens.

The clinical effect and the penetration of ampicillin into sinus secretion after oral administration of bacampicillin was studied in 47 patients with acute maxillary sinusitis allocated at random in treatment with 400 mg bacampicillin 3 times or 1200 mg twice daily. The overall clinical effect was similar and was assessed as "good" in 23/25 patients treated with 400 mg doses and in 19/22 treated with 1200 mg. Adverse reactions were reported by 4 patients. The concentrations of ampicillin obtained in sinus secretion were proportional to the dose given. The highest values after 400 mg bacampicil

Sulphadiazine/trimethoprim once daily in maxillary sinusitis: a randomized double-blind comparison with sulphamethoxazole/trimethoprim B.I.D.

In a randomized double-blind study fifty-four patients suffering from acute maxillary sinusitis were treated for 10 days with daily doses of sulphadiazine/trimethoprim (1 g) and sulphamethoxazole/trimethoprim (1.92 g), respectively. The efficacy was evaluated clinically at two follow-up visits. X-ray investigations were performed at admission and after the therapy. Of thirty-nine patients finally evaluated, thirty-seven showed a favourable result. After 6-8 days of therapy there was significant difference in cure rates in favour of sulphadiazine/trimethoprim (p less than 0.05) while the outcom

Treatment of acute maxillary sinusitis. V. Amoxicillin azidocillin, phenylpropanolamine and pivampicillin.

One hundred and seventy-five patients with clinical symptoms of acute maxillary sinusitis, the diagnosis confirmed radiologically by mucous membrane thickening and secretion, were consecutively allocated to four different treatment groups: amoxicillin, azidocillin, phenylpropanolamine, and pivampicillin. The cure course was followed up radiologically on days 5, 10, and 15. The outcome of treatment was somewhat more beneficial after 5 days for amoxicillin and after 10 days for azidocillin, but slightly less beneficial with phenylpropanolamine after 10 and 15 days. Acute maxillary sinusitis in children.

We sought to correlate the clinical, radiographic, and bacteriologic findings in maxillary sinusitis in 30 children who had both upper-respiratory-tract symptoms and abnormal maxillary radiographs. Cough, nasal discharge, and fetid breath were the most common signs, but fever was present inconsistently. Facial pain or swelling and headache were prominent symptoms in older children. Bacterial colony counts of greater than or equal to 10(4) colony-forming units per milliliter were found in 34 of 47 sinus aspirates obtained from 23 children. The most common species recovered were Streptococcus pn

Etiology and antimicrobial treatment of acute sinusitis.

Acute maxillary sinusitis is a disease of varied etiology. Over half of the cases are caused by Streptococcus pneumoniae and Hemophilus influenzae. Anaerobic bacteria account for another 10% of cases and these are usually of dental origin. The rest of the cases are caused by several other bacteria, each of which cause a small proportion. Rhinoviruses, influenza, and parainfluenza viruses also invade the sinuses and probably lead to secondary bacterial infection. Diagnosis of acute sinusitis on clinical grounds is difficult. Sinus transillumination and x-ray are the most valuable routine tests

Etiology and antimicrobial therapy of acute maxillary sinusitis.

Eighty-one adults with symptoms of acute sinusitis were studied by direct needle puncture and aspiration of the maxillary sinus (105 sinuses). Fifty-nine bacterial strains were isolated in titers of greater than or equal to 10(4) colony-forming units/ml; Streptococcus pneumoniae and Haemophilus influenzae accounted for 64% of the isolates. Other bacteria recovered included anaerobes (12%), Neisseria species (8.5%). Streptococcus pyogenes (3%), alpha-hemolytic Streptococcus (3%), non-group A beta-hemolytic Streptococcus (3%), Staphylococcus aureus (2%), Pseudomonas aeruginosa (2%), and Escheric

Bacteriology of maxillary sinusitis in relation to quality of the retained secretion.

The bacteriological findings in 200 patients with acute maxillary sinusitis are reported. It is concluded that the sampling technique--by antral aspiration--is highly significant in the evaluation of the bacteriological background of sinusitis, whereas the anerobic transport of the sample seems to be of less importance. By aspiration the purulent secretion can be properly separated from the non-purulent secretion. Pneumococci, H. influenzae and anaerobic bacteria can be isolated in about 90% of the patients with true sinus empyema, while sterile conditions are rare.

Large antral odontoma as the cause of acute maxillary sinusitis: report of case.

A case of a large compound odontoma in the right maxillary sinus causing acute maxillary sinusitis is reported. Seven large antral odontomas have been previously reported, none with the typical findings of acute sinusitis. Radiographs should always be considered in cases of recurrent maxillary sinusitis or pain, and the general practitioner should familiarize himself with lesions of the maxillary sinus that might be revealed in radiographic examination. Surgical excision is the preferred treatment for all odontomas, with no expectancy of recurrence. The odontoma described here was associated w

Liability of ultrasound in maxillary sinus disease.

For the first time liability of of ultrasonography in diagnosing paranasal sinus disease was controlled in 256 patients with acute maxillary sinusitis as compared to irrigation findings and in 150 patients with persistent maxillary sinus disease using x-rays, sinoscopy and sinumanometry as controls. Confidence in diagnosing the presence of discharge ranges up 90%. In cases of total opacity of the sinus, ultrasonography helps differentiating mucosal swelling, tumor and discharge.
