Patients' and physicians' views on the management of acute maxillary sinusitis.

OBJECTIVES: To study the views of patients and physicians on the management of suspected acute maxillary sinusitis and on suggested changes in practice. DESIGN: Cross-sectional survey and focus group interviews of both patients and physicians. SETTING: Primary care in Finland. RESULTS: A total of 175 patients and 245 physicians replied to the survey (response rates 88% and 70%, respectively). Eight focus groups were conducted, four with patients (n = 22) and four with physicians (n = 20). Patients had less trust than physicians in the natural healing process of acute maxillary sinusitis: on

Use of pharmacodynamic end points in the evaluation of gatifloxacin for the treatment of acute maxillary sinusitis.

The relationship between drug exposure and the time course of antimicrobial effect at the primary infection site for acute maxillary sinusitis has not previously been explored. This single-center, open-label study quantified the time course of sinus sterilization, described gatifloxacin exposure at the infection site, and posed the hypothesis that the use of continuous and quantitative time-related end points may allow for better characterization of drug effect with fewer patients than traditional clinical trial approaches. Of the 12 enrolled patients, 10 were clinically evaluable, from whom 7

[Maxillary fungal sinusitis by Zygomicetos. Report of a case in an immunocompetent patient].

We report the clinical case of a 36 years old female without significant previous history who was diagnosed with an acute maxillary sinusitis with not responding to antibiotic treatment. After her admission we asked for a CT which showed an occupation in the left maxillary sinus with a calcified area inside suggesting a mycetoma. The patient was operated by Functional Endoscopy Sinusal Surgery (FESS) plus Caldwell-Luc techniques sending the obtained sample for microbiological study, that was reported as sinusal mycosis by Zygomicetos. The postsurgical evolution was satisfactory.

Lipid peroxidation and antioxidant enzyme activities in experimental maxillary sinusitis.

The aims of this study were to assess whether th e increased oxidative stress of acute maxillary sinusitis is reflected by tissue lipid peroxidation and whether the activities of selected antioxidant enzymes are altered during inflammation of the maxillary sinus mucosa. Unilateral rhinosinusitis was induced in 8 rabbits by instillation of 0.2 ml of a killed suspension of Staphylococcus aureus into the right maxillary sinus cavity; control instillation of saline solution into the left maxillary sinus cavity of the same rabbits was also performed. At 7 days post-treatment, mucosal samples were e

Acute rhinosinusitis in primary care: a comparison of symptoms, signs, ultrasound, and radiography.

In primary care, the diagnosis of acute maxillary sinusitis (AMS) is based most often only on symptoms and clinical examination. This practice leads to many false positive diagnoses and unnecessary use of antibiotics. Sinus ultrasound has been suggested as a means to improve the accuracy of AMS diagnosis. We studied the symptoms, signs, and ultrasound findings in patients with acute rhinosinusitis. Primary care physicians received small group teaching on sinus ultrasound technique. Sinus radiography was performed in a subgroup of patients, and it acted as reference standard. A total of 150 adu

Efficacy and tolerability of telithromycin for 5 or 10 days vs amoxicillin/clavulanic acid for 10 days in acute maxillary sinusitis.

Telithromycin (HMR 3647) is a new ketolide antimicrobial that was developed for the treatment of community-acquired respiratory tract injections. We conducted a randomized, double-blind, multicenter study to compare the clinical efficacy and safety of oral telithromycin, at 800 mg once daily for 5 or 10 days, with that of amoxicillin/clavulanic acid, at 500/125 mg three times daily for 10 days, in adults with acute maxillary sinusitis (AMS). A total of 754 patients with AMS of less than 28 days' duration were randomized to receive either telithromycin for 5 days followed by placebo for 5 days,

Levofloxacin: a review of its use in the treatment of bacterial infections in the United States.

UNLABELLED: Levofloxacin (Levaquin) is a fluoroquinolone antibacterial agent with a broad spectrum of activity against Gram-positive and Gram-negative bacteria and atypical respiratory pathogens. It is active against both penicillin-susceptible and penicillin-resistant Streptococcus pneumoniae. The prevalence of S. pneumoniae resistance to levofloxacin is <1% overall in the US.A number of randomised comparative trials in the US have demonstrated the efficacy of levofloxacin in the treatment of infections of the respiratory tract, genitourinary tract, skin and skin structures. Sequential intrav

[Unilateral exophthalmos during the course of cavernous sinus thrombosis: a case report].

Ophthalmopathy, particularly its infiltrative form, is considered a characteristic feature of Graves' disease. However many other also life-treating conditions may be manifested as unilateral exophthalmos and lead to diagnostic problems. A case of a sixty six-year old patient with asymmetric exophthalmos and rapidly decreasing vision acuity is reported. Thyroid eye disease was suspected due to clinical symptoms (diplopy, impaired right eye motion) and thickening of extraocular muscles in ultrasound imaging. Further follow-up and orbits as well as head MRI imaging led to the final diagnosis of
