The ENT Management of Acute Frontal Sinusitis With Intracranial Complications in Adult Patients.

OBJECTIVES: To describe how adult patients with suppurative intracranial complications of acute frontal sinusitis have been managed across three neurosurgical centers in the northwest of England, United Kingdom. METHODS: A retrospective analysis over a 5-year period investigating adult patients (> 18 years old) presenting with acute frontal sinusitis with intracranial complications. RESULTS: Forty two patients were identified. 76.2% were male with a mean age of 43.1 years. The most common presenting complaint was headache. Subdural collection was the most common intracranial complication. Th

Subdural Abscess Suspected to Have Developed From Acute Sinusitis via the Foramen Cecum: A Case Report.

The foramen cecum connects the intracranial space to the sinus cavities. However, it is usually blind-ended and typically closed by surrounding bone and tissue. The opening of this anatomical structure can provide a pathway for intracranial infection from acute sinusitis. However, no previous reports have documented the spread of acute sinusitis intracranially via the foramen cecum, to the best of our knowledge. In this case report, we report a case of subdural abscess that was thought to have developed from acute frontal sinusitis through the foramen cecum and provide a review of relevant lit

Pott's Puffy Tumor: A Case Report of a Complication of Rhinosinusitis in the Pediatric Age.

Pott's puffy tumor is a rare and often fatal complication of frontal sinusitis characterized by osteomyelitis and subperiosteal collection of the frontal bone. It is typically managed by surgical drainage combined with antibiotic therapy. We describe a previously well eight-year-old boy with forehead swelling following rhinosinusitis. A brain CT scan with contrast was diagnostic for Pott's puffy tumor with findings of acute frontal sinusitis with intracranial extension, subperiosteal collection, subgaleal inflammatory changes, and osteomyelitis. The patient was managed medically with intraveno

Osteomyelitis of Frontal Sinus: A Rare Sequelae of Acute Bacterial Sinusitis Associated With Anatomical Variations in the Frontal Sinus Drainage Pathway.

Frontal osteomyelitis is characterized by localized osteal inflammation of the frontal bone. This is a rare complication of acute frontal sinusitis. The present case is being reported to highlight the likely role of anatomical variations in frontal sinus drainage pathways in the causation of this complication apart from other known predisposing factors like young age and immunocompromised state. The patient initially presented with seizures, fever, and headache and was diagnosed with viral encephalitis. However, the symptoms gradually progressed to cause right eye swelling and an increase in t

Complicated acute frontal sinusitis in a child presenting with acute ischemic stroke.

Acute stroke as a complication of sinusitis is exceedingly rare, and even more so in children. Here we present the case of a healthy 9-year-old male who had an acute stroke in the setting of severe acute pansinusitis. The patient was started on parenteral antibiotics and underwent urgent endoscopic sinus surgery and adenoidectomy, ultimately making a full neurologic recovery. Of the previously reported cases of stroke as a complication of sinusitis, all showed evidence of carotid artery pathology. This is the first report to our knowledge of acute sinusitis presenting as stroke with normal art

Pott's Puffy Tumor.

Pott's puffy tumor is a potential complication of acute frontal sinusitis, characterized by subperiosteal abscess and osteomyelitis of the frontal bone. It can be managed with a combination of open and endoscopic sinus surgery and intravenous antibiotic therapy. In the current report, a 15-year-old male presented with a classic case of Pott's puffy tumor which was managed with bilateral ethmoidectomies, frontal sinusotomies, and frontal sinus trephination, resulting in discharge on intravenous antibiotic therapy and subsequent complete resolution of symptoms.

Pott's puffy tumour of the frontal and Parietal bones with subgaleal abscess as a complication of acute sinusitis - a case report.

Acute sinusitis is a frequent medical condition that can affect any age group and may lead infrequently to very serious life-threatening complications. These complications include the spread of infection to frontal and other cranial bones, meninges, and other intracranial structures that require urgent medical actions to prevent mortality and morbidity. We report the case of acute frontal sinusitis in a 14 years old male who presented with fever, severe headache, and fluctuant swelling of the nasal root, and right supraorbital and frontoparietal regions. Imaging studies demonstrated signs of l

Early experience with feasibility of balloon sinus dilation in complicated pediatric acute frontal rhinosinusitis.

BACKGROUND/OBJECTIVE: Complicated acute rhinosinusitis in the pediatric population is an uncommon problem that may affect the orbit or brain and is life-threatening. This condition requires surgical intervention with endoscopic sinus surgery for source control, and prior studies have demonstrated the safety of balloon sinuplasty in chronic frontal sinusitis. METHODS/RESULTS: We present our approach with a balloon sinus dilation hybrid procedure to resolve four distinct types of complicated acute frontal sinusitis in pediatric patients, including intracranial manifestations, intraorbital compl

Odontogenic Sinusitis-Associated Pott's Puffy Tumor: A Case Report and Literature Review.

Pott's puffy tumor (PPT) is a nonneoplastic complication of acute frontal sinusitis characterized by subperiosteal abscess (SPOA) formation and osteomyelitis. Although various clinical conditions are associated with frontal SPOA, cases associated with odontogenic sinusitis are rare. We report a recurrent case of frontal SPOA who had a previous history of craniotomy due to head trauma. The patient was presented with headache, facial swelling and diagnosed as frontal, ethmoid, and maxillary sinusitis with frontal SPOA, which is PPT. The patient was surgically managed by endoscopic sinus surgery

[A case report of epidural abscess caused by acute frontal sinusitis].

Epidural epidural abscess is usually secondary to acute frontal sinusitis and osteomyelitis of the frontal bone. Nose-derived intracranial connplications are less common than otogenic ones, and infections caused by nasal and sinus trauma, foreign bodies and surgery are more common than those caused by inflammation alone. The frontal lobe abscess caused by frontal sinusitis is more common in brain abscess with nasal intracranial complications.

Very painful acute frontal sinusitis revealing granulomatosis with polyangiitis.

INTRODUCTION: Granulomatosis with polyangiitis (GPA) is a necrotizing vasculitis of small and medium-sized vessels comprising inflammation of the vessel wall and perivascular and extravascular granulomas, frequently presenting in the form of chronic sinusitis. OBSERVATION: We report the case of a 27-year-old man who presented with very painful acute frontal sinusitis that was managed medically and surgically. The symptoms rapidly recurred despite treatment and CT scan demonstrated diffuse thickening of the sinus mucosa. Anti-proteinase 3 ANCA were positive. Biopsy of a pulmonary nodule confir

Subcutaneous emphysema after vigorous sneezing in the setting of acute frontal sinusitis.

INTRODUCTION: Subcutaneous emphysema [SCE] can develop due to traumatic, infectious, and spontaneous causes and usually localizes to the periorbital space. CASE: We present a case of an 18-year-old male with an 8-day history of migraine-like headaches followed by the acute onset of frontofacial swelling after vigorous sneezing. Radiologic and physical exam findings supported a diagnosis of frontofacial SCE in the setting of frontal sinusitis. DISCUSSION: A sneeze, although usually benign, causes a significant increase in intranasal pressure. When coupled with a history significant for facial

Impact of perioperative lidocaine infusion and bis monitorization on remifentanil dosage in hypotensive anesthesia.

BACKGROUND: Combination of local and regional anesthetic agents are widely used in emergency and surgical setting and the interaction between the medications used in general anesthesia and these local and/or regional anesthetic becomes a growing concern in current patient management system. The interaction between general anesthetic agents and the local anesthetic agents given epidurally, spinally, intravenously or intramuscularly and the effects of BIS monitorisation on combined propofol-remifentanil anesthesia are examined in several studies. In literature, there is no research investigating

Sinus headaches: avoiding over- and mis-diagnosis.

'Sinus headache' is a term used by many patients and primary-care physicians and, contrary to popular belief, sinus headaches are uncommon. Headaches that are due to sinusitis are confined to a minority of patients who have acute frontal sinusitis or sphenoiditis. The International Headache Society classification is robust in qualifying the term sinus headache and says that "chronic sinusitis is not validated as a cause of headache and facial pain unless relapsing into an acute stage". The vast majority of people who present with a symmetrical frontal or temporal headache, sometimes with an oc

Balloon sinuplasty in acute frontal sinusitis.

Balloon sinuplasty has been licensed for the treatment of chronic rhinosinusitis since 2006. Balloon dilation enlarges the sinus ostium by effecting tiny fractures of the surrounding bone and compressing soft tissue such that no mucosa is removed. We believe this to be the ideal technique for achieving drainage of an acutely infected frontal sinus when surgical intervention is required. We report the application of this new technique in a 32-year-old female with acute frontal sinusitis. The frontal sinus was successfully cannulated and drained. Post-operative irrigation was achieved using a mo
