A Pott's puffy tumour as a late complication of a frontal sinus reconstruction: case report and literature review.

A Pott's Puffy Tumour (PPT) is a rare clinical entity, which, traditionally has been described as an acute abscess with periosteitis secondary to osteomyelitis of the external table of the frontal bone of the skull, complicating an acute frontal sinusitis. The aim of this article is to present a case of progressively evolving PPT, which emerged during the course of a common rhinitis, in a patient who, thirty years previously, had undergone a reconstruction of the frontal sinus involving osteosynthesis. The patient was treated with antibiotic therapy coupled with external access surgery using t

Intracranial complications of sinusitis: what is the role of endoscopic sinus surgery in the acute setting.

OBJECTIVE: Intracranial complications of sinusitis (ICS) are uncommon in the antibiotic era. The role of neurosurgical drainage of ICS is fairly well established, but the role for acute surgical intervention for the sinus disease is not well defined. We retrospectively reviewed our experience with ICS to see if we could identify the role and effectiveness of endoscopic sinus surgery (ESS) in the acute setting of ICS. METHODS: The study used a retrospective review of patients presenting to a tertiary care academic medical center for a 6-year period. RESULTS: Twenty-three patients were identif

[Osteoma of the frontal sinus leading to acute sinusitis with the complication of subdural empyema].

BACKGROUND: Osteomas of the paranasal sinuses are benign, slow-growing bone tumours with preferred localization in the frontal sinus. They often remain asymptomatic and are found by chance in x-ray examination. Clinical manifestations include headache and orbital complications. CASE REPORT: We describe the case of a 54-year-old male patient who, based on an osteoma of the frontal sinus, developed an acute frontal sinusitis complicated by subdural empyema. Therapy consisted of two operative interventions. CONCLUSIONS: Aetiogenesis of paranasal osteomas is still unclear. Inspite of their slow

Complications of acute frontal sinusitis: a retrospective study.

Even though being a rare condition in the era of antibiotic treatment, complications of acute frontal sinusitis still pose a potentially life-threatening problem. We present a clinical case series using a combined surgical approach. Within a 7 year period, all patients with a suppurative complication of an acute frontal sinusitis were included into this retrospective study and evaluated concerning histories, diagnostic and therapeutic procedures and achieved outcomes. Twelve patients (11-74 years) were identified; this corresponds to 0.3% of all patients that have undergone paranasal sinus sur

The prevalence of intracranial complications in pediatric frontal sinusitis.

OBJECTIVE: Intracranial extension of infection represents a serious complication of sinusitis but with no clearly documented prevalence. The frontal sinus with its unique anatomical characteristics, has been singled out as a catalyst for intracranial spread, but without solid evidence. Our objective was to determine the prevalence of intracranial complications in pediatric acute frontal sinusitis and to test the claimed association. METHODS: A retrospective chart review of all children (</=16 years of age) admitted over an 18 year period with sinusitis was undertaken in a tertiary children's

Subdural empyema as a complication of sinusitis in the pediatric population.

OBJECTIVE: Sinusitis is a rare cause of intracranial infection in children. While intracranial complications of sinusitis are rare, the morbidity and mortality remain high. Subdural empyema is recognized as the most common sinogenic intracranial complication. We undertook a review of our cases of subdural empyema and other intracranial complications of sinusitis over the past 8 years at a busy inner city hospital. Our intent was to identify factors that may predispose children to these serious complications. METHODS: A retrospective chart review was conducted using ICD-9 codes to identify ped

[Acute frontal sinusitis accompanied with extra-dural abscess (a case reported with literature reviewed)].

OBJECTIVE: To investigate the clinical characteristics of acute frontal sinusitis accompanied with extra-dural abscess. METHOD: A case of acute frontal sinusitis accompanied with extra-dural abscess diagnosed by CT was analyzed, and the relative literatures were reviewed. RESULT: Headache, high fever and failure to antibiotic therapy were the main three clinical characteristics. CONCLUSION: It is the most important thing to drainage the abscess except for antibiotic therapy for acute frontal sinusitis accompanied with extra-dural abscess. Antibiotic therapy cannot substitute for surgery.

Acute and chronic frontal sinusitis.

This review summarizes the recent literature published on the microbiology, diagnosis, and medical and surgical management of acute and chronic frontal sinus disease. Two retrospective studies investigated the microbiology of frontal sinusitis in patients that underwent sinus surgery. Streptococcus pneumoniae, Haemophilus influenzae, and Staphylococcus aureus predominated in acute infection in patients with acute frontal sinusitis, and S. aureus and anaerobic bacteria were commonly isolated in chronic sinusitis. Surgery is indicated to treat patients with acute and chronic sinusitis and their

Pituitary apoplexy: a pathologic entity from an otolaryngologist's view.

A case of pituitary apoplexy, which was initially misdiagnosed as 'acute frontal sinusitis', is reported. The presenting symptoms and signs of the patient were headache, moderate fever, left periorbital edema, marked tenderness over the left frontal sinus and purulent secretion over the left middle turbinate and nasopharynx. These clinical symptoms were wrongly perceived as complicated frontal sinusitis. The CT scan and the elective right carotid angiography showed a pituitary adenoma. Therefore pituitary apoplexy of a preexisting pituitary adenoma was diagnosed. The patient underwent surgical

Extensive subgaleal abscess and epidural empyema in a patient with acute frontal sinusitis.

Acute frontal sinusitis can be a serious condition because of its potential life-threatening complications. These complications, including spread of infection to the frontal bone and intracranially, require prompt diagnosis and intervention to avoid morbidity and mortality. We report a case of acute frontal sinusitis in a 16-year-old girl who presented with fever, severe headache, and vomiting of 3 days' duration. Generalized fluctuant swelling of the nasal root, and bilateral supraorbital and frontoparietal regions was noted. Computed tomography (CT) demonstrated left pansinusitis, extensive

Bacteriology of acute and chronic frontal sinusitis.

Aspirates of 15 acutely and 13 chronically infected frontal sinuses were processed for aerobic and anaerobic bacteria. A total of 20 isolates (1.3 per specimen) were recovered from the 15 cases of acute frontal sinusitis, 16 aerobic and facultative isolates (1.1 per specimen) and 4 anaerobic isolates (0.3 per specimen). Aerobic and facultative organisms alone were recovered in 13 specimens (87%), and mixed aerobic and anaerobic bacteria were recovered in 2 (13%). The predominant aerobic and facultative organisms were Haemophilus influenzae (6), Streptococcus pneumoniae (5), and Moraxella catar

Healing results of prolonged acute frontal sinusitis treated with endoscopic sinus surgery.

In a marked part of patients suffering from acute frontal sinusitis, the symptoms do not resolve after initial therapy (antibiotics and/or trephination). The prolongation of the healing or recurrences may be caused by persistent inflammation or by structural abnormalities at the nasofrontal connection. During the last decade endoscopic surgery (FES) has become a very useful method to restore the frontonasal drainage although long term results of this kind of population are rare. We evaluated the results of endoscopic surgery in two patient groups (total number of 50) both operated after prolon

A survey of contemporary management of frontal sinus disease in the United Kingdom.

Surgery for frontal sinus disease historically has required an external approach. With the advent of endoscopic sinus surgery, endoscopic treatment for various frontal sinus pathologies has been described. This survey examined how widespread the use of endoscopy in frontal surgery has become. We were interested both in the acute emergency situation and in the more chronic situations such as mucocoeles. The replies of 266 practising United Kingdom surgeons to a postal questionnaire were evaluated. While 45% of surgeons regularly practise functional endoscopic sinus surgery (FESS), approximately

Pott's puffy tumor treated by endoscopic frontal sinusotomy.

Pott's puffy tumor is a serious extracranial complication of frontal sinusitis. The formation of this entity is facilitated by the close anatomic relationship between the paranasal sinuses and the frontal bone. Furthermore, the rich diploic venous drainage of the region enhances the spread of the infection. We report on successful functional endoscopic frontal sinusotomy in a series of four cases of Pott's puffy tumor following acute frontal sinusitis, between the years 1994 and 1997. We emphasize the advantages of this approach over the external approaches as follows: the technique treats the

[Course changes in acute inflammation of the frontal sinus based on the indices of lipid peroxidation and on the activity of enzyme during biovibration therapy].

Acute frontal sinusitis was treated by therapeutic complex incorporating drainage of the frontal sinus, its lavage with antiseptic solution, mechanical low-frequency biovibromassage. The dynamics of the inflammation resolution in the frontal sinus reflected positive clinicoroentgenological changes, normalization of lipid peroxidation products in the blood and activity of the studied enzymes.

Pseudotumor cerebri as a presenting symptom of acute sinusitis in a child.

Pseudotumor cerebri is a clinical syndrome characterized by increased intracranial pressure in the absence of an intracranial tumor. It is most frequently diagnosed in obese young women, but it is also reported in children of all age groups, including infants. A variety of medical conditions have been suggested as possible etiologic factors, including several infectious diseases. This study presents a child with pseudotumor cerebri as the only presenting symptom of acute frontal sinusitis. The possible association between these two conditions should be investigated in cases of pseudotumor cere

Subdural empyema and blindness due to cavernous sinus thrombosis in acute frontal sinusitis.

In this era of antibiotics, the complications of acute sinusitis are much less frequently encountered. Although orbital complications are most common, intracranial complications carry a high rate of mortality and morbidity. We describe a case of acute frontal sinusitis with subdural empyema and blindness due to cavernous sinus thrombosis and carotid artery thrombosis with a discussion of treatment of these complications and the etiology of blindness in sinusitis.
