Acute Pansinusitis Caused by a Retained Merocel Pack Forgotten for 50 Days: A Case Report With Histopathologic Insights.

Merocel, a polyvinyl acetal sponge, is a nonabsorbable nasal packing material widely used after sinonasal surgery because of its hemostatic efficacy and ease of placement. However, nonabsorbable packing is generally removed within 48 hours to reduce the risks of mucosal injury, infection, and adhesion. A 48-year-old man underwent endoscopic sinus surgery (ESS) for chronic rhinosinusitis at a secondary hospital. The Merocel pack was inadvertently retained for 50 days, resulting in acute pansinusitis with foul odor, postnasal drip, and facial pain. Attempts to remove the packing by traction and

Rapidly progressive subdural empyema secondary to acute pansinusitis complicated by superior sagittal sinus thrombosis in a young adult: a multidisciplinary case report.

Subdural empyema is a rare but life-threatening intracranial infection, most commonly arising as a complication of paranasal sinusitis. Early diagnosis and multidisciplinary treatment are essential to reduce morbidity and mortality. A 23-year-old previously healthy male developed a rapidly progressive subdural empyema associated with superior sagittal sinus thrombosis and severe sepsis. Clinical deterioration occurred within 24 hours of symptom onset. Cranial computed tomography revealed a left fronto-temporo-parietal subdural collection with mixed density and air inclusions, along with extens

Nasal septal abscess complicating acute pansinusitis in a child: A case report.

INTRODUCTION AND IMPORTANCE: Nasal septal abscess (NSA) is a rare complication of acute sinusitis in children. It is very difficult to find in the literature. NSA can have devastating complications. These include potentially life-threatening intracranial infection and cosmetic deformity of the nose. These complications, which affect not only functional but also vital prognosis, can be prevented by early diagnosis and treatment. CASE PRESENTATION: We report the case of a 13-year-old boy who presented to the emergency department with a 2-day history of bilateral eyelid oedema extending to the n

Pott's Puffy Tumor in Young Age: A Systematic Review and Our Experience.

Background: Pott's Puffy Tumor (PPT) in young-age patients is a rare clinical entity characterized by osteomyelitis of the frontal bone with a subperiosteal abscess collection. Previous reviews primarily consist of small, retrospective case series and anecdotal reports. This study aims to present the largest, most up-to-date systematic review of essential clinical findings, diagnostic modalities, microbiologic considerations, and treatment approaches for managing PPT in pediatric and adolescent populations. Methods: PubMed, Scopus, and Web of Science databases were systematically screened unti

[Acute rhinosinusitis in children requiring hospitalization between 2016 and 2022 - retrospective analysis].

Introduction: Rhinosinusitis is an inflammation of the mucous membrane of the nasal and paranasal sinuses with discharge and swelling. Hospitalisation may be necessary especially in complicated cases of acute bacterial rhinosinusitis. Objective: Our aims were a retrospective analysis of the pediatric cases hospitalized with acute rhinosinusitis in the Department of Otorhinolaryngology, Head and Neck Surgery of the University of Szeged and the determination of whether the COVID–19 epidemic had an influence on the proportion of cases requiring admission. Method: Demographic and clinical data wer

A superior ophthalmic vein thrombosis following an acute pansinusitis: A case report.

INTRODUCTION AND IMPORTANCE: Superior ophthalmic vein thrombosis (SOVT) is a very rare condition that can occur following either septic or aspetic causes. Early recognition and management is important in order to avoid vision-threatening and fatal complications. CASE PRESENTATION: A 28-year-old man presented with orbital cellulitis complicating acute pansinusitis. CT scan revealed ipsilateral SOVT. The patient received intravenous antibiotics and anticoagulation with strict ophthalmic monitoring. No orbital complications were recorded during the course of treatment. CLINICAL DISCUSSION: SOVT

Bilateral Longus Colli Abscesses as a Complication of Advanced Bacterial Rhinosinusitis.

Known complications of acute bacterial rhinosinusitis include retropharyngeal abscess, cavernous sinus thrombosis, intracranial abscess, and associated sequelae. We describe the case of a patient with longus colli abscess formation resulting from acute pansinusitis, complicated by bilateral cavernous sinus thrombosis in the setting of concurrent viral pneumonitis, severe sepsis, and a large area cerebral infarction. The bilateral longus colli abscesses were drained via stereotactic image-guided endonasal sinus surgery, yielding Staphylococcus hominis in culture. The described disease progresse

Complicated acute frontal sinusitis in a child presenting with acute ischemic stroke.

Acute stroke as a complication of sinusitis is exceedingly rare, and even more so in children. Here we present the case of a healthy 9-year-old male who had an acute stroke in the setting of severe acute pansinusitis. The patient was started on parenteral antibiotics and underwent urgent endoscopic sinus surgery and adenoidectomy, ultimately making a full neurologic recovery. Of the previously reported cases of stroke as a complication of sinusitis, all showed evidence of carotid artery pathology. This is the first report to our knowledge of acute sinusitis presenting as stroke with normal art

[Pott's puffy tumor: a rare complication of frontal sinusitis].

Pott's puffy tumor is a rare complication of frontal sinusitis characterized by swelling and edema in the brow due to a subperiosteal abscess associated with frontal osteomyelitis. Added complications are cellulitis by extension to the orbit and intracranial infection by posterior extension, with high risk of meningitis, intracranial abscess, and venous sinus thrombosis. Early diagnosis and aggressive medical or surgical treatment are essential for optimal recovery of affected patients. In the antibiotic age it is extremely rare, with very few cases described in the recent literature. A case i

Purulent pansinusitis, orbital cellulitis and rhinogenic intracranial complications.

Objectives: Acute pansinusitis is rarely seen in the maxillofacial surgery field, but often occurs in combination with orbital and intracranial involvement. Clinically this entity is of great importance, since it represents a severe disease with possibly disastrous consequences. Patients: Aetiology, diagnosis and therapy of acute pansinusitis and its complications were analysed in 36 patients treated surgically from 1987 to 1996. Results: Eighteen patients were aged between 3 and 21-years-old. Only eight suffered from pure pansinusitis, and three of an isolated purulent orbital infection. Of t

Sinusitis and ischemic stroke.

Acute sinusitis is a prevalent and generally uncomplicated infection that is normally resolved by medical therapy. However, severe neurological complications are known, and comprise of cerebral abscess, cavernous sinus thrombosis, meningitis, and epidural or subdural empyema. We report a case about a 10-year-old girl with a severe acute pansinusitis and ischemic stroke in the right lentiform nucleus and the anterior part of the right internal capsule. Possible explanations for this rare combination are discussed.

[Pansinusitis secondary to inflammatory myofibroblastic tumor].

Inflammatory myofibroblastic tumors are aetiologically enigmatic and biologically unpredictable lesions. The lungs are the organs of apparent predilection. These tumors have also been documented in a number of extrapulmonary sites including head and neck. Paranasal location is rare. We report a case of a maxillary location in a 10 years and 10 months old girl initiated as an acute pansinusitis.

Management of acute complicated sinusitis: a 5-year review.

Acute sinusitis is a relatively common problem; however, sinusitis associated with a complication is less frequent. Currently there is debate as to whether acute complicated sinusitis should be managed by frontal sinus trephine and sinus washout or by immediate frontoethmoidectomy/functional endoscopic sinus surgery. To assess the effectiveness of frontal sinus trephine in the management of acute complicated frontal sinusitis, we reviewed all patients admitted to Groote Schuur Hospital with acute pansinusitis (includes frontal, maxillary, and ethmoid) from 1989 to 1993. Eighty-seven patients w

Acute pansinusitis with bacteremia due to a beta-hemolytic group C streptococcus: Streptococcus milleri.

A 41-year-old truck driver had acute onset of weakness, severe headache and pain over the left side of the face, forehead, and orbital area. He was found to have acute pansinusitis. Blood cultures and culture of the sinus drainage yielded beta-hemolytic group C streptococcus: Streptococcus milleri. He recovered completely after treatment with cefazolin, surgical drainage and debridement, and outpatient cephalexin therapy. Beta-hemolytic streptococci are uncommon causes, and bacteremia is rare in acute sinusitis. Speciation of the streptococcus is important in determining the epidemiology and c

Subdural empyema of sinus origin in children.

Two children with subdural empyema of paranasal sinus origin presented as neurological emergencies. Both had severe acute pansinusitis with complicating subdural empyema. The need for a high index of suspicion in the proper clinical setting is mandatory even in the absence of confirmatory contrast studies. Surgical proof should be immediately followed by adequate treatment. The high mortality rate even in the antibiotic area calls for this approach.
