Eosinophilic granulomatosis with polyangiitis (EGPA) is an anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitis, with myeloperoxidase antibodies in 40% of cases. Proteinase 3 (PR3) positivity is rare and may overlap with granulomatosis with polyangiitis (GPA). We present a 43-year-old male with recurrent sinusitis, epistaxis, neuropathic pain, and acute kidney injury, with histopathology of kidney biopsy showing EGPA despite the absence of asthma, which is rare. His PR3 positivity and clinical presentation mimicked GPA, highlighting the diagnostic challenges of vasculitides. We re

Frontal sinus mucoceles are benign expansile lesions that may progressively erode the surrounding bone and extend toward the orbit or intracranial compartment. When expansion occurs anteriorly, however, they may present as an apparently superficial forehead swelling and be mistaken for a cutaneous abscess. A 77-year-old woman with chronic sinusitis developed recurrent left paramedian frontal swelling over several months, with only transient improvement following antibiotic treatment. The lesion was initially interpreted as a cutaneous abscess and incised in an outpatient setting. Clear, nonpur

BACKGROUND: Headaches after venous sinus stenting (VSS) for idiopathic intracranial hypertension (IIH) are very common and challenging to manage. We evaluated the safety and efficacy of middle meningeal artery (MMA) lidocaine infusion to mitigate post-VSS cephalalgia.
METHODS: We retrospectively reviewed 50 consecutive patients with medically refractory IIH who underwent VSS at a single institution (February 2024-January 2026). Seven patients received bilateral MMA infusion of 50 mg of 1% lidocaine immediately following stent deployment; 43 patients served as controls. The primary outcome was

IMPORTANCE: Balloon sinus dilation (BSD) is a tool for treating patients with chronic rhinosinusitis or recurrent acute rhinosinusitis. The use of BSD has increased exponentially since its introduction, with concerns for potential misuse. Expert consensus reports that most patients who are candidates for BSD do not need to have all 6 sinuses dilated (bilateral frontal, maxillary, and sphenoid).
OBJECTIVE: To determine the percentage of patients who are undergoing 6-sinus BSD, evaluate factors associated with this practice, and explore outlier practice patterns when performing 6-sinus BSD.

BACKGROUND: We aimed to estimate the burden of antibiotic-nonsusceptible nonbacteremic pneumonia and sinusitis due to Streptococcus pneumoniae (pneumococcus) in US adults (aged ≥18 years).
METHODS: We estimated antibiotic-nonsusceptible pneumococcal sinusitis and nonbacteremic pneumonia incidence as products of pneumococcal pneumonia and sinusitis incidence rates, serotype distribution, and serotype-specific antimicrobial nonsusceptibility prevalences from 2016 through 2019. Nonsusceptibility was considered by antibiotic class and guideline-recommended agents. We estimated pneumonia and sinus

BACKGROUND: Post hoc analyses of clinical trials have characterized dupilumab's adverse effects, yet the real-world impact in chronic rhinosinusitis with nasal polyps (CRSwNP) and asthma is not well described. This study aims to characterize the risks of lymphoma, cardiovascular events, eosinophilia, joint pain, inflammatory arthritis, and sleep apnea in dupilumab-treated CRSwNP and/or asthma patients compared to those not taking dupilumab, and to other biologics.
METHODS: This retrospective cohort study used TriNetX, a de-identified database containing over 100 million patient records. Demog

BACKGROUND: Acute sinusitis is common in general practice. Although typically self-limiting, antibiotics are frequently prescribed despite guideline recommendations to restrict use. It remains unclear whether antibiotic treatment reduces subsequent health care use or work absence.
OBJECTIVE: To assess how initial treatment with or without antibiotics for acute sinusitis is associated with subsequent health care use and work absence, and to compare phenoxymethylpenicillin (PcV) versus other antibiotics.
METHODS: Nationwide registry-based observational cohort study of adults with acute sinusit

Intracranial complications secondary to acute sinusitis in pediatric patients are rare but potentially life-threatening. Initially, the symptoms are nonspecific, which can result in a delayed diagnosis. If a patient presents with persistent fever and severe headache, an intracranial complication should be suspected and promptly investigated using imaging techniques such as contrast-enhanced computed tomography (CT) and magnetic resonance imaging (MRI). Here, we describe a case in which both a frontal epidural abscess and an interhemispheric subdural empyema developed following acute rhinosinus

Isolated sphenoid sinusitis is an uncommon entity, accounting for a small proportion of paranasal sinus disease. Its deep anatomical location and proximity to the optic nerve, cavernous sinus, and orbital apex often result in delayed diagnosis, particularly when typical nasal symptoms are absent. In poorly controlled diabetics, fungal infection of the sphenoid sinus can progress rapidly, presenting initially with ophthalmic deficits rather than sinonasal complaints. This series describes three female patients between 50 and 53 years of age, all with long-standing type 2 diabetes mellitus with

Cerebral venous sinus thrombosis (CVST) is an uncommon cause of stroke and can present in ways that closely resemble arterial transient ischemic attack or ischemic stroke. Early recognition is important because management differs from typical stroke pathways, and initial noncontrast head CT may be normal. We report a 63-year-old male patient with a history of remote pulmonary embolism, pulmonary hypertension, and a family history of venous thromboembolism who presented with several weeks of headache and neck pain followed by acute left-sided weakness. Initial computed tomography (CT) imaging s

BACKGROUND: Invasive sinonasal and skull base fungal infections represent life-threatening conditions with increasing incidence. Early diagnosis and correct delineation of disease extent using imaging modalities are crucial for reducing morbidity and mortality.
AIM AND OBJECTIVES: The aim and objectives of the study are to analyze and compare the computed tomography and magnetic resonance imaging features of acute and chronic invasive sinonasal and skull base fungal infections, correlate imaging findings with microbiological and histopathological findings, and evaluate post-treatment imaging

Cerebral venous sinus thrombosis (CVST) is a rare but potentially serious cerebrovascular disorder caused by thrombosis of the cerebral venous sinuses, which can result in raised intracranial pressure, cerebral edema, and hemorrhagic complications. The clinical presentation is highly variable, often leading to diagnostic delays despite advances in neuroimaging. This study aimed to describe the clinical, demographic, and epidemiological characteristics of patients with CVST, identify associated risk factors and comorbidities, and evaluate complications and long-term outcomes. In addition, the s

Brain abscesses are rare but potentially life-threatening infections that require prompt recognition and treatment. Although they typically occur in patients with identifiable risk factors, they can occasionally present in otherwise healthy individuals. Sinusitis is a recognized source of intracranial infection, but its progression to a brain abscess in immunocompetent patients is uncommon and may be clinically subtle. We report the case of a 21-year-old previously healthy male who presented to the emergency department (ED) with a non-specific bifrontal headache. He was initially diagnosed wit

Extranodal natural killer (NK)/T-cell lymphoma is a rare malignant tumor, more prevalent in Asia and Latin America, and associated with Epstein-Barr virus infection. Its diagnosis is challenging due to its clinical presentation, which may mimic chronic sinusitis, dacryocystitis, or granulomatous diseases. This lymphoma follows a rapid and aggressive course, with a poor prognosis. We report the case of a 71-year-old man presenting with nasal obstruction, clear rhinorrhea, and facial heaviness. Initially, acute dacryocystitis and periorbital cellulitis were suspected and treated with antibiotics

A sinus of Valsalva aneurysm (SOVA) is a rare cardiac anomaly characterized by dilation of one of the aortic sinuses, most commonly the right coronary or noncoronary sinus. A SOVA can be clinically silent or present acutely when rupture occurs or more gradually with progressive left-to-right shunting and high-output heart failure. Etiologies of SOVA include congenital failure of the aortic media and annulus to fuse, or an acquired SOVA from endocarditis, trauma, or connective tissue disorders. SOVAs are typically diagnosed by transthoracic echocardiography (TTE), and most patients undergo a su

Influenza increases the risk of secondary diseases, but other than pneumonia, many of these diseases (e.g., sinusitis, otitis media, acute myocardial infarctions) are not consistently considered in estimates of influenza burden. We used the Merative Marketscan database (2001-2019) and time-series methods to identify age-specific categories of diseases that were temporally associated with patterns of influenza activity. Next, we estimated hypothetical reductions in the incidence and costs of these diseases if influenza incidence were reduced. Of 282 different disease categories evaluated, 23 (8

OBJECTIVE: To assess whether biologics (dupilumab, mepolizumab, and omalizumab) used for conditions other than chronic rhinosinusitis with nasal polyps (CRSwNP) reduce the frequency of acute bacterial rhinosinusitis (ABRS) episodes.
METHODS: This retrospective cohort study utilized the All of Us Research Program database to identify patients treated with dupilumab, mepolizumab, or omalizumab, excluding those with CRSwNP. A control group matched by race, gender, and comorbidities (asthma, allergic rhinitis, atopic dermatitis, smoking status) was created using propensity score matching (1:1 rat
