Right Infectious Cavernous Sinus Thrombosis Complicated by Multiple Cerebrovascular Events in the Right Hemisphere.

Bacterial meningitis and infectious cavernous sinus thrombosis (CST) are both life-threatening central nervous system infections, often caused by sinusitis. While cerebrovascular complications are well-recognized in bacterial meningitis, their association with CST is rare. A 69-year-old man presented with a 19-day history of headache, followed by diplopia. On the day of admission, he developed left hemiparesis. On examination, he had mild right eye abduction impairment, dysarthria, mild left hemiparesis with left facial paralysis, and nuchal rigidity. At admission, brain computed tomography re

A Case of Kartagener's Syndrome Presenting With Severe Hypoxemia.

Kartagener syndrome is a rare hereditary disorder that follows an autosomal recessive pattern of inheritance. It falls under the group of primary ciliary dyskinesias (PCDs). It is typically characterized by a classic triad: bronchiectasis, persistent sinus infections, and a reversal of internal organ positioning known as situs inversus. Due to its non-specific symptoms and absence of a simple diagnostic test, Kartagener syndrome is frequently diagnosed late, often after significant respiratory damage has occurred. We report the case of a 15-year-old female patient who presented with recurrent

Evaluating the Role of Sinus Surgery on the Non-diseased Contralateral Side in Patients With Unilateral Sinus Disease: A Systematic Review.

Functional endoscopy sinus surgery (FESS) is a frequently employed surgical technique to treat patients with sinus diseases. However, limited studies are available that discuss the impact of sinus surgery on the non-diseased contralateral side. The present review was conducted to examine existing literature regarding the role of sinus surgery on the non-diseased contralateral side in patients with unilateral sinus disease (USD). We conducted a comprehensive systematic review using the Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA) guidelines. Relevant literature wa

Chemical Pneumonitis from a Swimming Pool Disinfectant Exposure in a 13-Year-Old Male: A Case Report.

While chlorine and other pool chemicals can be used for the purpose of disinfection and pool maintenance, they also serve as a potential respiratory hazard. We report the case of a 13-year-old boy who presented to the emergency department after experiencing immediate dyspnea, drooling, coughing, nausea, and vomiting following an exposure to newly added pool chemicals. The patient's medical history included intermittent asthma with a recent history of an acute exacerbation two weeks before the inhaled chemical exposure. The treatment course following pool chemical exposure consisted of corticos

Exogenous Testosterone and Estrogen Blocker-Associated Venous Thrombosis Presenting With Acute Aphasia: A Case Report.

Cerebral venous thrombosis (CVT) is an uncommon cause of stroke that often presents with vague or subacute neurological symptoms that make timely diagnosis challenging. We describe the case of a 52-year-old male patient who presented with an acute onset of aphasia, a left-sided headache, and visual field deficits. Initial non-contrast computed tomography (CT) imaging was unremarkable, but subsequent magnetic resonance venography (MRV) confirmed a thrombosis in the left transverse and sigmoid sinuses. The patient had been started on testosterone replacement therapy (TRT) and estrogen blockers s

Comparison of Simple Drainage Versus Drainage With Tract Destruction in the Management of Pilonidal Abscess.

INTRODUCTION: Pilonidal disease is an acquired condition primarily affecting the gluteal cleft and sacrococcygeal area, often complicated by the formation of sinuses and abscesses. Despite multiple surgical options, there is no universally accepted standard treatment. This study aimed to compare the outcomes of simple drainage alone versus drainage combined with sinus destruction using the same incision technique. METHODS: Between February 2019 and March 2024, a total of 480 patients with acute pilonidal abscesses received treatment. Of these, 235 underwent simple drainage (Group A), while 24

Osseous Sarcoidosis of the Clivus Causing Cranial Nerve VI Palsy: A Case Report.

Sarcoidosis is an inflammatory condition characterized by the presence of noncaseating, or nonnecrotizing, granulomas in one or more organ systems. It has a wide range of clinical presentations and often mimics other conditions, making it difficult to diagnose. Osseous sarcoidosis of the skull base is extremely rare and has not previously been reported in a young adult patient. A 20-year-old man with a prior diagnosis of chronic recurrent multifocal osteomyelitis and pulmonary nodules of unknown etiology presented with six weeks of intractable headache and eye pain, followed by acute-onset com

Diagnosis of Acute Isolated Sphenoid Sinusitis Based on a Positive Kernig Sign in a Pediatric Patient.

Acute isolated sphenoid sinusitis in children is relatively rare, often presents with atypical symptoms, and may result in intracranial complications; therefore, early diagnosis is essential. We report the case of a 14-year-old girl who presented with a headache and a positive Kernig sign. Computed tomography (CT) demonstrated fluid retention in the sphenoid sinus, and lumbar puncture showed no abnormalities in the cerebrospinal fluid (CSF). Based on these findings, she was diagnosed with acute isolated sphenoid sinusitis without intracranial complications. However, the patient showed a poor r

An Unusual Case of Cerebral Venous Sinus Thrombosis With Negative D-dimer Mimicking Giant Cell Arteritis.

Cerebral venous sinus thrombosis (CVST) is a rare, potentially life-threatening condition that presents with variable symptoms such as headache, visual disturbance, and seizures, often mimicking an arterial stroke or intracranial lesion. Diagnosis can be challenging, and delays may result in severe outcomes. Although an elevated D-dimer may suggest CVST, a negative result does not exclude it. We report a rare case of extensive CVST initially treated as giant cell arteritis (GCA) due to unilateral temporal headache, elevated CRP, and a negative D-dimer. A 62-year-old man presented with acute le
