Spontaneous Intracranial Hypotension: A Commonly Missed Cause of Secondary Headache.

Spontaneous intracranial hypotension (SIH) is an uncommon but important cause of secondary headache that can closely mimic primary headache disorders, often leading to diagnostic delays. We present the case of a 50-year-old woman with a persistent, treatment-resistant orthostatic headache initially misdiagnosed as migraine, sinusitis, and perimenopausal symptoms. Brain MRI revealed a spontaneous subdural hematoma (SDH) along with classical features of SIH, while spinal MRI demonstrated a small longitudinal epidural fluid collection. Conservative management failed to relieve her symptoms; howev

Acute Exacerbations of Chronic Rhinosinusitis.

PURPOSE OF REVIEW: We aim to highlight recent advancements on the evolving chronic rhinosinusitis (CRS) phenotype: acute exacerbations of chronic rhinosinusitis (AECRS). We focused on studies that expanded the current understanding of its pathophysiology, patient characteristics, and disease burden. RECENT FINDINGS: Defining AECRS has been a topic of discussion for many years. A recent regulatory definition of AECRS in the literature incorporates a > 3 day requirement of worsened symptoms and an escalation of treatment. It is important not to rely on patient-reported rescue medication frequen

Unveiling dental diagnostic dilemmas: a national survey of US dentists.

BACKGROUND: In dentistry, very little is known about diagnostic errors (DEs) despite their critical role in assessing patient safety. Many DE cases significantly impact the patient’s quality of life and daily function without necessarily causing medical harm. The primary goal of this study was to describe dentists past experiences or observations of DEs and their associated characteristics. METHODS: We conducted a national cross-sectional study of US dentists (May-June, 2021). The primary outcomes were self-reported personal experiences of DEs and observations of DEs made by other dentists. Se

Rapidly Progressive Orbital Mucopyocele Mimicking Pre-septal Cellulitis: A Diagnostic Challenge With Sight-Threatening Implications.

We present the case of a male patient in his 60s referred by his GP with mild, painless swelling of the left eyelid, initially suggestive of pre-septal cellulitis. Within 24 hours, the condition progressed rapidly to acute proptosis with marked lower eyelid ectropion. Examination revealed complete ophthalmoplegia with limitation in all axes of extraocular movement. CT imaging demonstrated a left frontal-anterior ethmoid mucopyocele with erosion of the frontal and ethmoidal walls, extending into the extraconal space of the left orbit and abutting the medial rectus at its scleral insertion. The

A Red Eye in an Elderly Patient: A Case Report Highlighting the Importance of Urgent Diagnosis.

Anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitides are a distinct group of autoimmune disorders characterized by necrotizing inflammation of small blood vessels. Three distinct clinical entities are described, including eosinophilic granulomatosis with polyangiitis (EGPA), granulomatosis with polyangiitis (GPA), and microscopic polyangiitis (MPA). GPA often presents as a multi-system inflammatory disease frequently involving the sinuses, kidney, and lung tissue. Ophthalmologic involvement has been described in GPA and is highly important to recognize, especially if presenting

Sudden Diplopia Two Months After a Blowout Fracture: A Case Report of Orbital Tissue Adhesion Associated With Acute Sinusitis.

Blowout fractures (BOFs) frequently result in early-onset diplopia, which often resolves spontaneously. Delayed diplopia is rare, and there are no reported cases of its sudden onset following a prolonged asymptomatic period. Persistent diplopia significantly impacts quality of life. BOF-induced late-onset diplopia is rare. To date, no cases of late-onset or acute-onset diplopia have been reported, underscoring its exceptional rarity. This report discusses a unique case of sudden diplopia two months post BOF, which improved after surgical treatment. A 57-year-old man presented with a history of

Influence of Dental Intervention and Disease on Acute Invasive Fungal Rhinosinusitis Severity and Outcomes.

BackgroundAcute invasive fungal rhinosinusitis (AIFRS) is an aggressive and often fatal disease process that principally impacts immunocompromised patients. Maxillary dental trauma and infections have been associated with the development of maxillary sinus fungal balls, but the role of dental procedures/trauma in the pathogenesis of AIFRS remains poorly defined.ObjectiveThis study seeks to review a single-institutional experience with AIFRS and examine the association between dental events and AIFRS severity and outcomes.MethodsRetrospective review of 95 consecutive patients with biopsy-proven

Invasive Fungal Granuloma of the Skull Base Mimicking Spindle Cell Neoplasm in a Young Immunocompetent Patient: A Diagnostic Challenge.

Skull base lesions presenting with proptosis and sinonasal involvement in young patients are commonly attributed to vascular neoplasms such as juvenile nasopharyngeal angiofibroma. However, rare infectious etiologies such as invasive fungal granulomas may present with overlapping radiological features, leading to diagnostic challenges. We report a rare case of a 19-year-old immunocompetent male presenting with progressive bilateral proptosis and a large skull base mass involving the clivus, nasal cavity, and orbits. Initial imaging suggested a vascular neoplasm, and biopsy revealed a spindle c

Occult medial orbital wall trapdoor fracture inducing recurrent rhinogenic intraorbital abscess in a pediatric patient: a case report.

BACKGROUND: In pediatric populations, rhinogenic intraorbital abscess is frequently encountered, typically resulting from the direct extension of acute sinusitis. However, recurrent rhinogenic intraorbital abscess secondary to an occult medial orbital wall trapdoor fracture is extremely rare and often overlooked due to atypical trauma history and subtle imaging features. CASE PRESENTATION: An 11-year-old male pediatric patient experienced recurrent nasal congestion and rhinorrhea with redness, swelling, and pain of the left eye. He was first diagnosed with acute sinusitis, yet conventional an

Subdural Abscess Suspected to Have Developed From Acute Sinusitis via the Foramen Cecum: A Case Report.

The foramen cecum connects the intracranial space to the sinus cavities. However, it is usually blind-ended and typically closed by surrounding bone and tissue. The opening of this anatomical structure can provide a pathway for intracranial infection from acute sinusitis. However, no previous reports have documented the spread of acute sinusitis intracranially via the foramen cecum, to the best of our knowledge. In this case report, we report a case of subdural abscess that was thought to have developed from acute frontal sinusitis through the foramen cecum and provide a review of relevant lit

Pott's Puffy Tumor: A Case Report of a Complication of Rhinosinusitis in the Pediatric Age.

Pott's puffy tumor is a rare and often fatal complication of frontal sinusitis characterized by osteomyelitis and subperiosteal collection of the frontal bone. It is typically managed by surgical drainage combined with antibiotic therapy. We describe a previously well eight-year-old boy with forehead swelling following rhinosinusitis. A brain CT scan with contrast was diagnostic for Pott's puffy tumor with findings of acute frontal sinusitis with intracranial extension, subperiosteal collection, subgaleal inflammatory changes, and osteomyelitis. The patient was managed medically with intraveno

[A Case of Fungal Sinusitis and Preseptal Cellulitis Caused by Lichtheimia corymbifera in a Patient with Acute Myeloid Leukemia].

Lichtheimia corymbifera (formerly known as Absidia corymbifera) is a mold fungus belonging to the family Mucorales. Mucormycosis caused by L.corymbifera is a newly recognized and rare opportunistic infection in immunocompromised patients. Rapid diagnosis and treatment are crucial due to the high mortality rates, especially in patients with hematologic malignancies. Early clinical suspicion, surgical removal of necrotic tissue and appropriate antifungal chemotherapy constitute the main steps of treatment. Lipid formulations of Amphotericin B are the first-line treatment for mucormycosis. In thi

Virus-Induced Hypercoagulable State: A Mythical Cause for Cerebral Venous Sinus Thrombosis in an Adult Male.

Chickenpox is a common childhood disease that presents with erythematous rashes. In adults, varicella can cause complications with significant morbidity. Cerebral venous sinus thrombosis (CVST) is a life-threatening condition if not promptly treated. Here, we report a rare case of CVST complicating primary varicella-zoster virus infection. We present a case of a 34-year-old immunocompetent male with diffuse maculopapular rash and acute neurological deficit. Magnetic resonance venography revealed acute thrombosis involving the superior sagittal sinus, straight sinus, right transverse sinus, and

Acute invasive fungal sinusitis in a new diagnosis of type 1 diabetes mellitus.

A 32-year-old man diagnosed with type 1 diabetes mellitus presented to the ED following loss of consciousness and was ultimately diagnosed with acute invasive fungal sinusitis (AIFS). AIFS is a disease that most commonly affects people with immunocompromise such as those with a hematologic malignancy, diabetes, or HIV; those on immunosuppressant drugs posttransplant; and those on chemotherapy or long-term corticosteroids. In this case, the patient's diabetic ketoacidosis put him in a high-risk physiologic state. Following presentation, the patient was managed in the ICU to reverse his immunoco

Temporal patterns of antibiotic prescribing for sore throat, otitis media, and sinusitis: a longitudinal study of general practitioner registrars.

BACKGROUND: Antibiotics provide minimal benefit for sore throat, otitis media, and sinusitis. Antibiotic stewardship, with reduced prescribing, is required to address antibiotic resistance. As most antibiotic prescribing occurs in general practice and prescribing habits develop early, general practitioner (GP) trainees (registrars) are important for effective antibiotic stewardship. OBJECTIVES: To establish temporal trends in Australian registrars' antibiotic prescribing for acute sore throat, acute otitis media, and acute sinusitis. DESIGN: A longitudinal analysis of data from the Registrar
