Environmental lead exposure and clinical infectious respiratory disease in a sample of urban and disadvantaged children.

BACKGROUND: Early-life environmental lead exposure can adversely impact the development of the immune system and increase the susceptibility to infectious respiratory illnesses, but evidence regarding childhood respiratory outcomes remains inconclusive. OBJECTIVE: To examine the association between childhood blood lead concentrations and diagnosis of streptococcal pharyngitis or croup in an under-resourced, low-income sample of children residing in Philadelphia, PA. METHODS: This retrospective cohort study included medical record data on 13,221 children ≤14 years as part of the Temple Univer

The Perilous Journey of Patients With Primary CNS-HLH to Bone Marrow Transplant: A Case Report.

Hemophagocytic lymphohistiocytosis (HLH) is a rare but life-threatening syndrome of immune dysregulation with an incidence of 1.2-1.5 cases per million children annually. Curative therapy for primary HLH requires hematopoietic stem cell transplantation (HSCT). A subset of patients develop central nervous system (CNS) involvement, which is associated with high morbidity and mortality yet remains poorly characterized. We describe two patients with genetically confirmed primary HLH (PRF1 mutations with impaired perforin activity) who developed CNS disease. Both presented to the emergency departme

Myocarditis Following Invasive Group A Streptococcus Infection: Important Differential Diagnostic Distinctions.

Nonrheumatic streptococcal pharyngitis-associated myocarditis presents as acute myocarditis within days of Group A Streptococcus pharyngitis, typically resolving with antibiotics and minimal sequelae. It is thought to result from direct bacterial toxin injury rather than autoimmunity. Little epidemiologic data regarding its incidence and prevalence exist, and standardized diagnostic criteria are lacking. In contrast, acute rheumatic fever develops 2-4 weeks after a Group A Streptococcus pharyngitis and is autoimmune-mediated. Diagnosis is based on the revised Jones criteria, and only carditis

Unmasking the unexpected: a rare case of rheumatic heart disease.

BACKGROUND: Rheumatic heart disease is a systemic immune condition that develops from acute rheumatic fever. Rheumatic heart disease remains prevalent throughout the world, and this case highlights an unusual presentation of the disease. CASE SUMMARY: An otherwise healthy 7-year-old Caucasian girl from a suburban Midwest United States community presents with several weeks of headaches, nausea, and vomiting. After an unremarkable initial evaluation, a systolic murmur was noted during a general paediatrics clinic visit. An echocardiogram revealed severe mitral regurgitation, and further finding

Implementation and evaluation of a pragmatic community streptococcal treatment programme to improve rheumatic heart disease primary prevention in Uganda.

INTRODUCTION: Detection and treatment of streptococcal pharyngitis in children reduces acute rheumatic fever by 70-80%. However, investment in primary prevention is limited in rheumatic heart disease (RHD) endemic countries due to lack of diagnostic capacity. We conducted a pragmatic public health trial (Rheumatic Heart Disease Community Streptococcal Treatment Program (RESET)) to determine the effect of an integrated group A streptococcal (GAS) education and treatment programme on the echocardiographic burden of RHD among children aged 5-15 years in Uganda. METHODS: We implemented our interv

Sydenham's Chorea as the Sole Presenting Symptom of Acute Rheumatic Fever in a 14-Year-Old Boy: A Case Report.

BACKGROUND: Rheumatic chorea is an uncommon but time-honored neurological manifestation of acute rheumatic fever that usually presents weeks to months after an untreated streptococcal pharyngitis. When it presents alone, that is, without other manifestations of the disease, it could easily be overlooked or delayed in its diagnosis. In such a scenario, it is uniquely presented as an isolated primary presentation of rheumatic fever, which underscores the challenge of diagnosis and the importance of early recognition. CASE PRESENTATION: A 14-year-old boy presented with automatic, aimless, and ar

Infectious mononucleosis with ampicillin rash and a positive rapid streptococcal antigen test: a case report.

Infectious mononucleosis (IM), caused by Epstein-Barr virus, often mimics group A streptococcal (GAS) pharyngitis and may result in an ampicillin rash when aminopenicillins are administered. We report a 22-year-old woman with fever, sore throat, lymphadenopathy, and a generalized erythematous rash after a 7-day course of amoxicillin-clavulanate. Although a rapid streptococcal antigen test was positive, non-tender lymphadenopathy and absence of left shift in the white blood cell count suggested viral infection, and the result was considered consistent with a carrier state rather than true GAS i

Antibiotic therapy in adult patients with infectious mononucleosis in the prehospital period - a single-center experience.

Introduction: Some viral pharyngitis can mimic streptococcal pharyngitis. The clinical similarity of infectious mononucleosis (IM) and streptococcal pharyngitis makes differential diagnosis based on the clinical picture alone tricky, and patients with IM may be treated empirically with antibiotics (ATBs).

Aim: To assess prehospital ATB use in adult patients with IM admitted to the hospital. In addition, to compare antibiotic-treated individuals and patients not treated with ATBs using selected demographic, clinical, and laboratory param

Streptococcal pharyngitis, infectious mononucleosis, or both? A diagnostic dilemma in pediatric pharyngitis.

Infectious mononucleosis (IM) is a common cause of pharyngitis in children, particularly among adolescents. IM is a self-limiting condition, and differentiating it from other causes of pharyngitis that require treatment is key to its management. One such condition is group A streptococcal (GAS) pharyngitis, which can be complicated by acute rheumatic fever if left untreated. In this report, we present a case of suspected IM with a positive GAS rapid antigen detection test, highlight the evidence surrounding these two conditions, and discuss the clinical dilemma of performing GAS testing in pat

2025 Clinical Practice Guideline Update by the Infectious Diseases Society of America on Group A Streptococcal Pharyngitis: Risk Assessment Using Clinical Scoring Systems in Children and Adults.

This publication represents the first part of an update to the clinical practice guideline on the diagnosis and management of group A streptococcal (Streptococcus pyogenes or GAS) pharyngitis, developed by the Infectious Diseases Society of America (IDSA). Diagnosis of GAS pharyngitis by clinician judgment alone is unreliable, and unselective testing incurs cost and inconvenience for individuals at low risk of having GAS infection. Clinical scoring systems have been used to quantify the probability of a positive GAS throat culture based on standardized criteria such as the presence of fever; t

Clinical and genomic characteristics of hypervirulent Klebsiella pneumoniae isolated from throat culture suggesting an emerging causative agent of pharyngitis.

Hypervirulent Klebsiella pneumoniae (hvKp) is rarely implicated in non-invasive infections. We report eleven cases of pharyngitis with whole genome sequencing of throat culture isolates identifying hvKp. Most cases (64%) occurred in the summer months of June to September. Cases were geographically diverse and only one patient reported recent travel outside of the continental US. No patients were diabetic or immunosuppressed, though five patients (45%) were prescribed inhaled or intranasal corticosteroids. Clinical presentation was indistinguishable from viral or streptococcal pharyngitis. All

Smartphone-based Detection of Group A Streptococcal Pharyngitis in Ugandan Children: A Pilot Study.

Prompt diagnosis of group A streptococcal pharyngitis is essential for primary prevention of acute rheumatic fever and rheumatic heart disease, yet affordable point-of-care diagnostics remain limited in low-resource settings. We conducted a prospective study among Ugandan children 5-15 years of age presenting with sore throat, comparing smartphone-acquired oropharyngeal videos analyzed using an artificial intelligence-based image recognition model-trained and validated using combined data from Ugandan and US-based cohorts-with microbiologic culture as the reference standard. Among 82 children

Efficacy and safety of 24 antibiotics for group A streptococcal pharyngitis: a network meta-analysis of 64 randomized controlled trials.

BACKGROUND AND AIMS: Penicillin remains the first-line therapy for group A streptococcal (GAS) pharyngitis. However, broad-spectrum antibiotics continue to be widely prescribed in clinical practice. This study aimed to evaluate the relative efficacy and safety profiles of antibiotics for GAS pharyngitis. METHODS: Literature published through March 2026 was searched. Efficacy outcomes included early and late bacterial eradication, clinical response, and bacteriological recurrence, whereas safety was assessed according to adverse events. Per-protocol data were extracted, and a Bayesian network

A Nationwide Review of the Utilization of Rapid Antigen Testing for Group A Streptococcus.

Background Group A streptococcus (GAS) is the leading cause of bacterial pharyngitis in children and adolescents, accounting for up to 40% of all cases of pharyngitis in children aged 5 to 15 years. The global incidence of GAS pharyngitis and invasive GAS infections has increased over the past few years. In Malta, Mater Dei Hospital houses the only dedicated pediatric emergency department and is also the only health care facility offering free GAS rapid antigen detection tests (RADTs). As per hospital protocol, the Centor criteria are used to assess the likelihood of GAS pharyngitis. Method St

Streptococcal Pharyngitis in Paediatric Primary Care: Clinical Characteristics, Recurrences, Complications, and Antibiotic Management.

INTRODUCTION: Group A streptococcal (GAS) pharyngitis is the most common bacterial infection in childhood. National and international guidelines consistently recommend narrow-spectrum beta-lactams as first-line therapy; however, variability in outpatient management remains substantial. This study aimed to characterize the clinical features, recurrence patterns, and complications of paediatric group A streptococcal pharyngitis in primary care. METHODS: We performed a prospective, multicenter observational study coordinated by the Italian Society of Primary Care Paediatrics from August 2024 to

Streptococcus pyogenes carriage is associated with distinct in vivo transcriptional states but not carrier-specific genotypes.

Group A Streptococcus (GAS) is a major human pathogen causing extensive global morbidity. While the asymptomatic carrier state challenges clinical management and contributes to community transmission, genomic, and transcriptomic analyses of naturally acquired human GAS carriage have been lacking. In this study, samples are collected from individuals before and after treatment for acute GAS pharyngitis. Selected isolates undergo whole-genome sequencing and RNA sequencing, providing transcriptional profiling of GAS directly from human-derived oropharyngeal swabs. Genomic analysis reveals no cons

Post-streptococcal uveitis syndrome in a middle-aged adult patient.

Post-streptococcal uveitis syndrome (PSUS) is a rare immune-mediated condition typically affecting children and young adults following group A Streptococcus infection. We describe a 53-year-old woman who developed bilateral panuveitis following documented streptococcal pharyngitis, representing one of the oldest reported cases of PSUS. The patient presented with photophobia, blurred vision and floaters, with examination revealing bilateral anterior segment inflammation, cystoid macular oedema, optic disc oedema and retinal vasculitis. Post-streptococcal Hemophagocytic Lymphohistiocytosis in an Immunocompetent Adult.

Secondary hemophagocytic lymphohistiocytosis (HLH) is a rare and life-threatening hyperinflammatory syndrome that can occur after acute infection, predominantly caused by viruses. Due to its nonspecific presentation and clinical overlap with conditions like sepsis, HLH is frequently underrecognized in adults. We present the case of an immunocompetent adult admitted with undifferentiated shock, which rapidly progressed to multi-organ failure. Upon further investigation, the patient was diagnosed with HLH, and a comprehensive infectious workup revealed streptococcal pharyngitis as the precipitat

Adult scarlet fever: Case report and epidemiology.

A 30-year-old woman presented to the ED with a generalized rash, fever, and tachycardia. A detailed review of systems and physical examination revealed symptoms and signs of streptococcal pharyngitis. PCR testing confirmed the diagnosis of streptococcal infection and scarlet fever. Scarlet fever is generally a disease of children, but episodic outbreaks are associated with the disease in adults, sometimes with significant complications. It is important for Emergency Medicine clinicians to be aware of this childhood illness presenting in adults.
