Towards credible digital twins for basic and preclinical research.

Atomoxetine (Strattera(R)) is a selective norepinephrine (noradrenaline) reuptake inhibitor that is not classified as a stimulant, and is indicated for use in patients with attention-deficit hyperactivity disorder (ADHD). Atomoxetine is effective and generally well tolerated. It is significantly more effective than placebo and standard current therapy and does not differ significantly from or is noninferior to immediate-release methylphenidate; however, it is significantly less effective than the extended-release methylphenidate formulation OROS(R) methylphenidate (hereafter referred to as osm

Perseverance: We Should Continue to Strive for Novel Antibiotics and Companion Diagnostics.

Since Fleming's discovery of penicillin, antibiotic resistance has remained an inevitable biological reality; however, our guidelines and diagnostic practices are not keeping pace with the rising medical need. This viewpoint uses a real family case of recurrent Group C streptococcal pharyngitis to illustrate these gaps in an outpatient setting. The situation is obviously more dire with life threatening infections. Addressing the fragile antibiotic pipeline requires updated, accessible guidelines, routine culturing upon relapse, and public education to support fair pricing and sustained investm

Temporal trends in pediatric group A streptococcal pharyngitis across the coronavirus disease 2019 era: A large retrospective cohort study.

ObjectıveThe coronavirus disease 2019 pandemic and associated non-pharmaceutical interventions, including school closure, mask use, and social distancing, substantially altered the epidemiology of pediatric respiratory infections. However, their impact on pediatric Group A Streptococcus pharyngitis remains incompletely understood. This study evaluated temporal trends in Group A Streptococcus positivity across the coronavirus disease 2019 era and factors independently associated with Group A Streptococcus positivity.MethodsIn this retrospective cohort study, pediatric patients aged 0-18 years p

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Evaluating pediatric antimicrobial dosing of β-lactam antibiotics for upper respiratory tract infections in emergency and primary care settings.

PURPOSE: Pediatric prescribing is often weight and indication specific and has been associated with high rates of error. The aim of this study was to evaluate outpatient β-lactam suspension dosing practices for pediatric upper respiratory tract infections (URIs), to characterize the rate of error and to describe common error types. METHODS: This retrospective cohort study was conducted at a community health system with 2 emergency departments and 20 outpatient family medicine practices. Pediatric patients treated from these settings with oral suspension formulations of amoxicillin, amoxicilli

A Rare Sequela of Untreated Streptococcus Pharyngitis: A Case Report.

Pharyngitis is commonly caused by the gram positive bacteria, streptococcus. Given the potential morbid complications of untreated streptococcal pharyngitis, antibiotics are critical. One of the rarer complications is pulmonary-renal syndrome (PRS), defined as rapidly progressive glomerulonephritis and diffuse alveolar hemorrhage. PRS is associated with high rates of morbidity and mortality, making swift diagnosis and treatment imperative for survival. More common causes of PRS are tied to autoimmune diseases. This case describes a novel progression of PRS caused by streptococcus. A 26-year-ol

Clinical Characteristics and Outcomes of Patients with Nonrheumatic Streptococcal Pharyngitis-Associated Myocarditis.

BACKGROUND: Reports of nonrheumatic streptococcal pharyngitis-associated myocarditis (SPAM) are rare, and its incidence, pathophysiology, and clinical features remain unclear. We evaluated the clinical course and outcome of patients diagnosed with nonrheumatic SPAM, with a particular focus on differentiating it from other etiologies of myocarditis. METHODS: Seventy-nine consecutive individuals (age 32 ± 9 years, 71 men) with clinically diagnosed SPAM were evaluated. None satisfied the revised Jones criteria for diagnosis of acute rheumatic fever. RESULTS: Average onset of symptoms prior to h

Acute Generalized Exanthematous Pustulosis in a Patient With Crohn's Disease Being Treated for Streptococcal Pharyngitis and Sternoclavicular Joint Abscess.

Acute generalized exanthemous pustulosis (AGEP) is a rare skin reaction most commonly due to an adverse drug reaction. The rash is characterized by the sudden onset of diffuse pustules on a maculopapular rash. We present a case of a 62-year-old female treated with cephalexin for streptococcal pharyngitis who was admitted with the characteristic rash over her chest, abdomen, arms, and legs that started four days after starting the antibiotic. The diagnosis was further confirmed with a punch biopsy of the affected skin. The rash quickly resolved after discontinuing cephalexin. The patient's cour

Feasibility of utilizing the Modified Centor Criteria in adult virtual care.

The Modified Centor criteria (MCC) is a validated clinical decision tool determining the need for testing in suspected Streptococcal pharyngitis. This study aims to understand the use of this tool to guide testing during remote evaluation. Patients with sore throats and no more than 3 days of symptoms were recruited from the emergency department and urgent care at an urban academic centre in 2019-2022. All patients enrolled were 18 years or older. Each participant had three MCC recorded, once in person and again by two different blinded telemedicine providers (TP). A total of 172 patients were

IgA Vasculitis (Henoch-Schönlein Purpura) in a Seven-Year-Old Female Child With Recurrent Respiratory Infections.

A seven-year-old female child presented with a purpuric rash, joint pain, and a recent history of streptococcal pharyngitis. Initial symptoms included fever, throat pain, and vomiting, for which she received amoxicillin. Following treatment, the patient developed a rash and joint pain, prompting further evaluation. The physical examination revealed erythematous, edematous, and purpuric lesions on the extremities, along with periarticular swelling in the knees and ankles. Laboratory investigations showed hematuria, raising concerns for differential diagnoses, including IgA vasculitis, acute urt

The Role of Physical Examination and Clinical Scores in Distinguishing Streptococcal Colonization from Pharyngitis in Pediatric Patients: Insights from a Common Clinical Scenario.

The accurate differentiation between asymptomatic carriage with group A streptococcus (GAS) and active streptococcal pharyngitis is a complex task with important clinical and public health implications. This work aims to highlight the key strategies necessary for optimizing the diagnostic and therapeutic management of pediatric pharyngitis. Clinical scores are essential tools for improving diagnostic accuracy. When combined with laboratory tests such as throat cultures and rapid antigen detection tests, these systems enable effective risk stratification of patients, supporting more precise tre

Prevalence of Asymptomatic Group A Streptococcus Carriage Based on Rapid Antigen Detection Test in Healthy Adults in Poland.

Background: Acute pharyngitis is one of the most prevalent disorders seen in general practitioners' consultations. Most cases of acute pharyngitis in adults are caused by respiratory viruses and are self-limited. However, clinical manifestations of viral pharyngitis can overlap with bacterial pharyngitis, mainly caused by group A Streptococcus (GAS). A rapid antigen test for GAS can help diagnose streptococcal pharyngitis, but misdiagnosing S. pyogenes infection in a patient with a viral condition can lead to inappropriate antibiotic use. Some patients with a sore throat due to a virus or othe

Patient Demographics Are Associated With Testing for Group A Streptococcal Pharyngitis in Children.

Among a retrospective cohort of 65 786 outpatient encounters for children >3 years with acute respiratory tract infection at an academic institution in 2023, 38 790 (59%) had a rapid group A streptococcal (GAS) test ordered. Testing rates differed significantly by race, ethnicity, insurance type, social vulnerability index, visit location, and clinician type, and children in historically underserved and socially vulnerable groups were less likely to undergo GAS testing compared to their social counterparts.

Rethinking Limp Diagnosis: Brodie's Abscess Linked to Streptococcal Infections.

BACKGROUND Brodie's abscess is a subacute osteomyelitis, most often seen in the long bones of children. In the emergency department (ED) these patients usually present with prolonged atraumatic limb pain and no signs of systemic infection. There is usually no known triggering factor for this infection. We describe a case of Group A Streptococcus (GAS) pharyngitis resulting in Brodie's abscess, which has not been reported previously. CASE REPORT A 6-year-old boy with 4 days of sore throat presented to his pediatrician and was treated with amoxicillin for a confirmed GAS pharyngitis. He presente

Challenges in the Diagnosis and Management of the Miller Fisher Variant of Guillain-Barré Syndrome: A Case Report.

Miller Fisher syndrome (MFS) is a rare variant of Guillain-Barré syndrome (GBS), characterized by the triad of ophthalmoplegia, ataxia, and areflexia, often following a bacterial or viral illness. This case report presents a 58-year-old Hispanic female patient with a preceding streptococcal pharyngitis who developed symptoms of bilateral facial nerve palsy and lower limb ataxia. Cerebrospinal fluid (CSF) analysis revealed albuminocytologic dissociation and, combined with her presentation of ophthalmoplegia, ataxia, and areflexia, supported the diagnosis of MFS. Magnetic resonance imaging (MRI)

From Throat to Limb: A Novel Case of Compartment Syndrome Following Group A Streptococcal Pharyngitis.

Acute compartment syndrome is a medical emergency caused by increased pressure within a closed fascial space, leading to tissue ischemia and potential limb loss or death if untreated. While typically secondary to trauma, rare cases have been associated with streptococcal infections. However, no documented case exists of compartment syndrome originating from Group A Streptococcus (GAS) pharyngitis. We present a 35-year-old female, with prediabetes, who presented to the emergency department with worsening right lower extremity (RLE) pain and edema following a febrile illness with pharyngitis. CLIA-Waived Group a Streptococcal Pharyngitis Test and Treat Programs in Community Pharmacies.

Acute pharyngitis is mostly viral in origin, but antibiotics are commonly prescribed to cover group A streptococcus (GAS) which is the cause in 5%-30% of pharyngitis episodes. Patients are increasingly utilizing community pharmacy test and treat programs to diagnose and treat acute respiratory infections. There are several Clinical Laboratory Improvement Amendment (CLIA)-waived point-of-care tests (POCT) available to quickly identify GAS. The objective of this article is to explore the 11 antigen and two molecular CLIA-waived GAS POCTs currently available in the United States (U.S.) and the st
