A mini review of the pathogenesis of acute rheumatic fever and rheumatic heart disease.

Acute rheumatic fever (ARF) is an autoimmune disease caused by group A streptococcal infection. Recurrent episodes of ARF can lead to rheumatic heart disease (RHD), which is the leading cause of cardiovascular mortality in children worldwide, especially in low- and middle-income countries. Investigations into the etiology of ARF and RHD constitute a crucial milestone in the advancement of both preventive measures and therapeutic interventions. The purpose of this mini review is to delineate the etiology and pathophysiological mechanisms underlying ARF and RHD. Selective searches were conducted

Physicians' knowledge and adherence to Centor Criteria for preventing acute rheumatic fever in Jeddah, Saudi Arabia.

Acute rheumatic fever (ARF) is a delayed autoimmune complication of Group A Streptococcal (GAS) pharyngitis and remains a significant public health concern in regions such as Saudi Arabia. Timely treatment with antibiotics guided by the Centor criteria can prevent ARF, yet adherence to these recommended guidelines remains inconsistent among physicians in developing countries. This study aimed to assess the knowledge and adherence of physicians in Jeddah, Saudi Arabia, to the Centor criteria in managing streptococcal pharyngitis as a strategy to prevent ARF. A cross-sectional study was conducte

Streptococcal Pharyngitis Complicated by Clinically Significant Rheumatic Myopericarditis: A Case Report.

Acute rheumatic fever (ARF) is a complication of streptococcal pharyngitis that can present with cardiac, joint, skin, and neurological symptoms. Cardiac manifestations most often involve valvular dysfunction, but can also include myocarditis or pericarditis. Although advances in healthcare have reduced the prevalence of streptococcal pharyngitis, and subsequently ARF, individual cases and outbreaks can still occur. We present a case of rheumatic myopericarditis in a 60-year-old White male who initially presented to the emergency department with sore throat for 6 days. Initial workup was large

Prevalence of Group A Streptococcal Pharyngitis and Antibiotic Susceptibility in Paediatric Patients With Sore Throats in Gaborone, Botswana.

BACKGROUND: Group A β-haemolytic Streptococcus, also known as Streptococcus pyogenes, commonly causes childhood pharyngitis and can lead to severe complications like acute rheumatic fever and rheumatic heart disease. Timely penicillin treatment is vital in preventing these issues. However, data on the prevalence of Group A β-haemolytic Streptococcus pharyngitis in Botswana are limited, and there is no national surveillance for Group A β-haemolytic Streptococcus infections. OBJECTIVES: This study aimed to determine the prevalence of Group A β-haemolytic Streptococcus pharyngitis among children

Establishing the lowest penicillin concentration to prevent pharyngitis due to Streptococcus pyogenes using a human challenge model (CHIPS): a randomised, double-blind, placebo-controlled trial.

BACKGROUND: The in-vivo plasma concentration of penicillin needed to prevent Streptococcus pyogenes pharyngitis, recurrent acute rheumatic fever, and progressive rheumatic heart disease is not known. We used a human challenge model to assess the minimum penicillin concentration required to prevent streptococcal pharyngitis. METHODS: In CHIPS, a randomised, double-blind, placebo-controlled, human challenge trial, healthy adult volunteers were randomly assigned by a computer-generated random sequence to target steady-state penicillin plasma concentrations (placebo, 3, 6, 9, 12, or 20 ng/mL). Acute Rheumatic Fever in an Adult Female Patient Presenting With Chest Pain: A Case Report.

Acute rheumatic fever (ARF) is an autoimmune disease triggered by group A Streptococcus (GAS) infection, predominantly affecting children in low- and middle-income countries. Although its incidence has significantly decreased in the United States (US), ARF remains a major cause of morbidity and mortality worldwide due to complications like rheumatic heart disease (RHD). The condition arises from molecular mimicry, where the immune system targets host tissues after a GAS infection, leading to inflammation and potential damage to the heart, joints, and nervous system. Early diagnosis and treatme

A Neurological Curtain Unmasking Rheumatic Carditis in Early Adolescents: Two Illustrative Cases From a Tertiary Care Center in Maharashtra, India.

Group A β-hemolytic streptococcal pharyngitis, if inadequately treated, can trigger an autoimmune inflammatory response known as acute rheumatic fever (ARF). While ARF classically presents with migratory arthritis, carditis, and other Jones criteria, it may also manifest as Sydenham's chorea, a delayed neurological complication. In some patients, chorea can occur as the sole clinical feature, making diagnosis challenging. Importantly, these individuals may harbor subclinical carditis, valvular inflammation, and dysfunction detectable only by echocardiography, which, if missed, can progress to

Case Report: Periorbital Edema as an Overlooked Presentation of Epstein-Barr Virus.

INTRODUCTION: Infectious mononucleosis (IM), most commonly caused by Epstein-Barr virus (EBV), classically presents with fever, pharyngitis, and cervical lymphadenopathy. However, less common manifestations such as bilateral periorbital edema, known as Hoagland sign, may precede or obscure more typical features, complicating diagnosis. Although described in the literature for decades, Hoagland sign remains underrecognized in clinical practice, especially in pediatric and young adult patients. This oversight can lead to misdiagnosis, delayed testing, unnecessary antibiotic use, and patient dist

Characterization of Streptococcus pyogenes Strains from Tonsillopharyngitis and Scarlet Fever Resurgence, 2023-FIRST Detection of M1UK in Bulgaria.

Recently a resurgence of Streptococcus pyogenes infections has arisen, with concerns around the highly virulent M1UK lineage. Our aim was to characterize S. pyogenes, the immune responses it causes, and to determine the presence of the M1UK lineage in Sofia, Bulgaria. In our study, the infections were confirmed by culture testing or rapid antigen test. Identification was performed by MALDI-TOF and was followed up by antibiotic susceptibility testing (EUCAST). Virulence factors were identified using multiplex PCR and whole genome sequencing (WGS). Immune responses were measured through detectio

Streptococcal Toxic Shock Syndrome (STSS) Secondary to Monoarticular Septic Arthritis Leading to Multiorgan Failure in a Patient without Underlying Comorbidities: Emphasizing Early Diagnosis and Management Strategies.

BACKGROUND: Streptococcal Toxic Shock Syndrome (STSS) is a life-threatening condition caused by bacterial toxins. The STSS triad encompasses high fever, hypotensive shock, and a "sunburn-like" rash with desquamation. STSS, like Toxic Shock Syndrome (TSS), is a rare complication of streptococcal infections caused by Group A Streptococcus (GAS), Streptococcal pyogenes (S. pyogenes). Staphylococcus aureus is the most frequently isolated bacterial species associated with TSS. Risk factors for STSS include older age, skin wounds, recent viral infection with open sores, recent surgery, nasal packing

Localized Leg Erythema as the Primary Symptom of Scarlet Fever: An Atypical Presentation.

Scarlet fever is an acute infectious condition caused by Streptococcus pyogenes, commonly leading to dermatological complaints in children. Its classical presentation includes fever, sore throat, and diffuse erythematous macules starting on the trunk; however, rare cases may present primarily with skin manifestations. We report a case of a seven-year-old boy who presented with pruritic, localized leg rashes as the primary symptom of scarlet fever. A thorough physical examination revealed painless erythema of the soft palate. Due to a local outbreak of Streptococcus pyogenes infections, a rapid

Pediatric Non-Cystic Fibrosis Pulmonary Nontuberculous Mycobacterium Infections: A Global Population Based Study.

BACKGROUND: Nontuberculous mycobacteria (NTM) are Mycobacterial pathogens that cause pulmonary infections among children, particularly those with underlying lung conditions or immunosuppression. Clinical presentations include chronic cough, weight loss, and fatigue. Diagnosis involves clinical assessment, radiographic imaging, and microbiological confirmation, while treatment often requires prolonged, multidrug antibiotic regimens. This study aimed to analyze the epidemiology and clinical outcomes of pulmonary NTM infections in a non-cystic fibrosis pediatric population from four distinct age

Persistent Postauricular Swelling in a Pediatric Patient: Unraveling the Cat Scratch Disease.

The etiology of posterior auricular swelling can be infectious, congenital, oncologic, or autoimmune. The most common cause of swelling in this region is reactive lymphadenopathy. Among the infectious etiologies, Bartonella henselae is an often-overlooked cause in immunocompetent individuals due to its varied clinical features. This swelling may fail to resolve despite the use of standard antibiotics, thereby presenting a diagnostic challenge for clinicians. We present a healthy nine-year-old male with no significant medical history who presented with persistent, painful postauricular swelling

Capture of Group A Streptococcus by Open-Microfluidic CandyCollect Device in Pediatric Patients.

STATE THE PURPOSE: Obtaining high-quality samples to diagnose streptococcal pharyngitis in pediatric patients is challenging due to discomfort associated with traditional pharyngeal swabs. This may cause reluctance to go to the clinic, inaccurate diagnosis, or inappropriate treatment for children with sore throat. Here, we determined the efficacy of CandyCollect, a lollipop-inspired open-microfluidic pathogen collection device, to capture Group A Streptococcus (GAS) and compare user preference for CandyCollect, conventional pharyngeal swabs, or mouth swabs in children with pharyngitis and thei

Knowledge and Attitude of Rheumatic Fever and Rheumatic Heart Disease Among the Makkah City Population, Saudi Arabia.

Background Rheumatic heart disease (RHD) is a prominent sequela of rheumatic fever (RF) and the most common cause of acquired valvular disease worldwide. Patients develop RHD as a result of autoimmune reactions caused by an untreated group A Streptococcus (GAS) throat infection, resulting in significant valvular destruction. Objectives The current study aimed to assess the knowledge and attitude of RF and RHD among the Makkah city population in Saudi Arabia. Methods An observational cross-sectional study was conducted on a convenience sample of 1364 adult participants from Makkah city in Saudi

Spatiotemporal Trends in Group A Streptococcal Pharyngitis in the United States.

BACKGROUND: Group A Streptococcus (GAS) causes an estimated 5.2 million outpatient visits for pharyngitis annually in the United States, with incidence peaking in winter, but the annual spatiotemporal pattern of GAS pharyngitis across the United States is poorly characterized. METHODS: We used outpatient claims data from individuals with private medical insurance between 2010 and 2018 to quantify GAS pharyngitis visit rates across U.S. census regions, subregions, and states. We evaluated seasonal and age-based patterns of geographic spread and the association between school start dates and th

Fifteen-minute consultation: Group A streptococcal pharyngitis, diagnosis and treatment in children.

Group A streptococcus (GAS) is the most common bacterial cause of pharyngitis in children. GAS causes significant suppurative and non-suppurative complications including invasive GAS disease and acute rheumatic fever. This article describes the current epidemiology and clinical presentation of GAS pharyngitis and explores how diagnostic and treatment decisions differ globally. Several key decision support tools are discussed including international guidelines, clinical decision scores and laboratory tests along with the evidence for treatment choice and duration. With recent international repo

"Give Me Five": The Case for 5 Days of Antibiotics as the Default Duration for Acute Respiratory Tract Infections.

Acute respiratory tract infections (ARTIs) account for most antibiotic prescriptions in pediatrics. Although US guidelines continue to recommend ≥10 days antibiotics for common ARTIs, evidence suggests that 5-day courses can be safe and effective. Academic imprinting seems to play a major role in the continued use of prolonged antibiotic durations. In this report, we discuss the evidence supporting short antibiotic courses for group A streptococcal pharyngitis, acute otitis media, and acute bacterial rhinosinusitis. We discuss the basis for prolonged antibiotic course recommendations and recen
