Diagnosis and Treatment of Group A Streptococcal Pharyngitis in Children.

The purpose of this review is to summarize the current evidence regarding the management of streptococcal pharyngitis in children. This article aims to provide a valid support to discriminate streptococcal pharyngitis from viral cases and treat it appropriately to avoid the development of complications. Differential diagnosis based only on clinical features is not always easy. For this reason, different clinical scores were created to provide an accurate diagnosis. Microbiological tests are valuable tools as well, but their use is not recommended unanimously. Concerning treatment, all guidelin

Explainable AI decision support improves accuracy during telehealth strep throat screening.

BACKGROUND: Artificial intelligence-based (AI) clinical decision support systems (CDSS) using unconventional data, like smartphone-acquired images, promise transformational opportunities for telehealth; including remote diagnosis. Although such solutions' potential remains largely untapped, providers' trust and understanding are vital for effective adoption. This study examines how different human-AI interaction paradigms affect clinicians' responses to an emerging AI CDSS for streptococcal pharyngitis (strep throat) detection from smartphone throat images. METHODS: In a randomized experiment

[Clinical diagnosis of group A streptococcal pharyngitis and progress in the application of scoring systems].

Pharyngitis can be caused by various pathogens, including viruses and bacteria. Group A streptococcus (GAS) is the most common bacterial cause of pharyngitis. However, distinguishing GAS pharyngitis from other types of upper respiratory tract infections is challenging in clinical settings. This often leads to empirical treatments and, consequently, the overuse of antimicrobial drugs. With the advancement of antimicrobial drug management and healthcare payment reform initiatives in China, reducing unnecessary testing and prescriptions of antimicrobial drugs is imperative. To promote standardize

Swift Onset, Swift Recovery: Unusual Nonrheumatic Myocarditis in a Young Adult Post Group A Streptococcal Pharyngitis.

This case report highlights the unusual presentation and management of nonrheumatic myocarditis in a 24-year-old male, an age demographic not commonly associated with myocardial complications following Group A streptococcal pharyngitis. The patient, devoid of any prior medical history, manifested symptoms one day after being diagnosed with Group A streptococcal pharyngitis, a stark contrast to the typical progression of myocardial complications. The swift onset of symptoms and the patient's subsequent clinical presentation necessitated a comprehensive diagnostic approach. The patient's symptom

Stratification of Group A Streptococcal Pharyngitis Children Using Unsupervised Learning.

Background and objectives Group A Streptococcus (GAS) is the most frequent cause of bacterial pharyngitis, and it is advised to selectively use rapid antigen detection testing (RADT). Currently, the decision to perform this test is based on pediatricians' observations, but the criteria are not well-defined. Therefore, we utilized unsupervised learning to categorize patients based on the clinical manifestations of GAS pharyngitis. Our goal was to pinpoint the clinical symptoms that should prompt further examination and treatment in patients diagnosed with pharyngitis. Methods We analyzed catego

Amoxicillin vs. placebo to reduce symptoms in children with group A streptococcal pharyngitis: a randomized, multicenter, double-blind, non-inferiority trial.

UNLABELLED: The efficacy of antibiotic therapy for group A streptococcus (GAS) pharyngitis is debated. The role of antibiotics in preventing complications seems limited, with the main potential benefit being symptom duration reduction. Our study aimed to evaluate whether a placebo is non-inferior to amoxicillin in reducing fever duration. We randomized 88 children between 3 and 15 years of age presenting with acute symptoms of pharyngitis and a positive rapid antigen detection test for GAS to receive 6-day treatment with either placebo (n = 46) or amoxicillin (n = 42). The primary outcome was

A Unique Presentation of Bacterial Group G Streptococcus Myopericarditis.

Perimyocarditis and myopericarditis are inflammatory conditions of the pericardium and myocardium, often of idiopathic or infectious etiology, with viral infections being the most common. Nonrheumatic streptococcal myopericarditis (NSM) is a rare condition that can mimic acute myocardial infarction. This case report presents a 22-year-old male with no prior medical history who developed NSM following a streptococcal pharyngitis infection. The patient presented with pleuritic chest pain, tightness, and decreased exercise tolerance. Laboratory studies revealed elevated troponin levels and positi

Treatment of acute pharyngitis in children: an Italian intersociety consensus (SIPPS-SIP-SITIP-FIMP-SIAIP-SIMRI-FIMMG).

Sore throat represents one of the main causes of antibiotic overprescription in children. Its management is still a matter of debate, with countries considering streptococcal pharyngotonsillitis a benign and self-limiting condition and others advocating for its antibiotic treatment to prevent suppurative complications and acute rheumatic fever. Italian paediatricians frequently prescribe antibiotics on a clinical basis regardless of microbiological results. Moreover, broad-spectrum antibiotics are inappropriately prescribed for this condition. In this regard, an intersociety consensus conferen

Centor scores associated poorly with rapid antigen test findings in children with sore throat.

UNLABELLED: The Finnish Current Care Guideline recommends rapid antigen tests as the primary diagnostic tool for both adults and children with Centor score ≥ 3. We aimed to analyze the association of Centor score and rapid antigen test positivity of group A streptococcal pharyngitis (GAS) in Finnish children. We performed a retrospective single-center study from July 2019 to June 2022. We included all children aged 0-15 years based on ICD-10 diagnostic codes for acute pharyngitis. We manually extracted the data from the electronic healthcare records. We extracted the information on Centor scor

Implementation of molecular diagnostic testing for group A streptococcal pharyngitis: considerations and challenges with a focus on point-of-care environments.

INTRODUCTION: Rapid and accurate detection of group A streptococcus (GAS) pharyngitis allows for timely initiation of appropriate antibiotic treatment. This is important to prevent empiric antibiotic overuse while simultaneously lowering the risk of post-infection sequelae. Timely treatment may also reduce forward transmission, which could prevent cases of devastating invasive infections. The need for timely and accurate diagnosis of GAS pharyngitis has created an ideal environment for molecular diagnostic (MDx) testing. The high sensitivity of MDx tests mean no culture confirmation is require
