Primary Epstein-Barr virus infection with polyradiculitis: a case report.

BACKGROUND: Nervous system complications of primary Epstein-Barr virus (EBV) infection in adults are rare, but may occur with encephalitis, meningitis, myelitis, cranial and peripheral neuropathies, or radiculitis. CASE PRESENTATION: We describe an immune competent adult with a primary EBV infection complicated by lumbosacral polyradiculitis with pure radicular pain. Prior to the onset of radicular pain the 35-year-old woman had been suffering from infectious mononucleosis misdiagnosed for streptococcal tonsillitis. The diagnosis of primary EBV infection associated polyradiculitis was proven

[A child with convulsions of unknown origin: posterior reversible encephalopathy syndrome].

BACKGROUND: Posterior reversible encephalopathy syndrome (PRES) is a rare complication in children with post-streptococcal glomerulonephritis (PSGN). CASE DESCRIPTION: An 8-year-old boy was brought to the emergency department with seizures preceded by acute headache attacks and vomiting. On examination the boy was hypertensive with periorbital edema. Further investigation showed proteinuria, haematuria and intra-cerebral abnormalities. Recent history indicated streptococcal tonsillitis for which oral amoxicillin was prescribed in the preceding week. The diagnosis 'PRES consequent to PSGN' was

[Intoxication syndrome concomitant with recurrent streptococcal tonsillitis and its correction].

The objective of the present work was to study selected features of intoxication in the patients presenting with recurrent tonsillitis (RT) and managed by means of the combination of basal therapy with reamberine, a preparation showing the detoxication activity. A total of 70 patients with moderately severe recurrent streptococcal tonsillitis induced by infection with group A beta-hemolytic streptococci were available for the examination. Characteristics of endogenous intoxication were explored. The study has demonstrated that recurrent tonsillitis is associated with the development of the pro

Streptococcal tonsillitis and acute streptococcal myocarditis: an unusual combination assessed by cardiac magnetic resonance imaging and endomyocardial biopsy.

OBJECTIVES: Acute streptococcal tonsillitis is occasionally combined with myocarditis. Our aim was to examine patients with tonsillitis in whom myocarditis was suspected by using cardiac magnetic resonance imaging (MRI) and endomyocardial biopsy. METHODS: After prospective evaluation of 200 patients with tonsillitis, 17 men (median age, 23 years; age range, 18 to 29 years) were recruited for cardiac MRI because of a suspicion of myocarditis. Chest pain 3 to 5 days after tonsillitis was the main complaint in 15 patients, and atypical chest discomfort in 2 patients. We performed cardiac MRI inc

Peritonsillar abscess: treatment with immediate tonsillectomy - 10 years of experience.

CONCLUSION: Immediate tonsillectomy in patients with peritonsillar abscess is a safe and effective treatment that should be considered as an alternative to conventional incision and drainage. OBJECTIVE: To assess the efficacy, safety, and microbiology of immediate tonsillectomy over 10 years, in patients with peritonsillar abscess. METHODS: This was a retrospective study. We reviewed the clinical charts of patients diagnosed with peritonsillar abscess in Clínica Las Condes from September 2000 to August 2010, who were treated with immediate unilateral or bilateral tonsillectomy. The patients'

[Treatment of streptococcal tonsillitis with once-a-day amoxicillin: a meta-analysis].

INTRODUCTION: The objective of this systematic review is to determine if the treatment of streptococcal pharyngitis with a daily dose of amoxicillin is similar in effectiveness to other dosing schedules (every 6, 8 or 12 hours) of the same antibiotic or penicillin V. MATERIAL AND METHODS: Randomised clinical trials (RCT) comparing amoxicillin (one dose per day) compared to other dosages of amoxicillin (every 8-12 hours) or penicillin V (every 6, 8 or 12 hours). Search databases consulted: Medline, Central, EMBASE and Google Scholar. The results were combined using the risk difference (RD). We

Towards evidence-based emergency medicine: best BETs from the Manchester Royal Infirmary. Can inflammatory markers distinguish streptococcal from viral tonsillitis?

A short-cut review was performed to evaluate whether inflammatory markers such as C reactive protein (CRP), erythrocyte sedimentation rate (ESR), white cell count (WCC) and procalcitonin (PCT) are able to discriminate between streptococcal and viral tonsillitis, enabling a reduction in the overuse of antibiotics. Eight studies with a total of 1031 participants were found. The author, date and country of publication, patient group studied, study type, relevant outcomes, results and study weaknesses of these papers are tabulated. The clinical bottom line is that WCC, CRP and PCT levels are highe

Streptococcal tonsillitis as a cause of urticaria: tonsillitis and urticaria.

BACKGROUND: The primary role of infections in chronic urticaria (CU) is controversial. We hypothesised that streptococcal tonsillitis (ST) could be a primary cause of CU or acute recurrent urticaria (ARU). METHODS: Retrospective study of 14 outpatients observed between January 2000 and December 2009, with CU/ARU and clinical and/or laboratorial suspicion of an aetiopathogenic link with ST. Clinical history, objective examination and laboratorial study were looked for. Three groups were defined: spontaneous resolution of urticaria, resolution after tonsillectomy, and still symptomatic. Revolutionizing clinical microbiology laboratory organization in hospitals with in situ point-of-care.

BACKGROUND: Clinical microbiology may direct decisions regarding hospitalization, isolation and anti-infective therapy, but it is not effective at the time of early care. Point-of-care (POC) tests have been developed for this purpose. METHODS AND FINDINGS: One pilot POC-lab was located close to the core laboratory and emergency ward to test the proof of concept. A second POC-lab was located inside the emergency ward of a distant hospital without a microbiology laboratory. Twenty-three molecular and immuno-detection tests, which were technically undemanding, were progressively implemented, wit

Acquisition of complement factor H is important for pathogenesis of Streptococcus pyogenes infections: evidence from bacterial in vitro survival and human genetic association.

Streptococcus pyogenes (or group A streptococcus [GAS]) is a major human pathogen causing infections, such as tonsillitis, erysipelas, and sepsis. Several GAS strains bind host complement regulator factor H (CFH) via its domain 7 and, thereby, evade complement attack and C3b-mediated opsonophagocytosis. Importance of CFH binding for survival of GAS has been poorly studied because removal of CFH from plasma or blood causes vigorous complement activation, and specific inhibitors of the interaction have not been available. In this study, we found that activation of human complement by different G

Acute myopericarditis complicating acute tonsillitis: beware the young male patient with tonsillitis complaining of chest pain.

OBJECTIVES: We describe 11 cases of myopericarditis complicating bacterial tonsillitis. METHODS: We performed a retrospective study of all cases of myopericarditis treated at one hospital during the years 2005 and 2006. RESULTS: The patients were all young men. The average latency from the onset of throat pain to the onset of chest pain was 4.6 days. All patients complained of chest pain. The most common electrocardiographic finding was transient ST segment elevations. The levels of cardiac enzymes and troponin were elevated in all cases. CONCLUSIONS: Otolaryngologists should be aware of th

[Ormond's disease in 17-year-old girl: a case study].

Ormond's disease (RPF) is chronic inflammatory process characterized by intensive fibrosis and inflammatory cells infiltration in retroperitoneal space. RPF in most cases possess idiopathic origin and is rare in children. The histopathologic verification is needed for establishing a proper diagnosis of disease. In this study a case of RPF in 17-year-old girl was reported which caused many diagnostical and therapeutical difficulties. In our patient streptococcal tonsillitis was recognized as a primary cause of disease.

Acute upper respiratory infections in Western Australian emergency departments, 2000-2003.

OBJECTIVE: To describe the epidemiological and other characteristics of emergency department (ED) presentations diagnosed with acute upper respiratory infection (URI). DESIGN AND SETTING: A retrospective study of patients given an ED diagnosis of acute URI from July 2000 to July 2003 at any of the four metropolitan teaching hospitals in Perth, Western Australia. RESULTS: Acute URI accounted for 3.6% (95% CI, 3.5-3.7) of ED presentations, and 80.7% (95% CI, 80.1-81.3) of these were aged less than 15 years. The most common diagnosis was acute upper respiratory infections of multiple and unspec

Detection of Fusobacterium necrophorum subsp. funduliforme in tonsillitis in young adults by real-time PCR.

Throat swabs from 61 patients, aged 18-32 years, with non-streptococcal tonsillitis (NST) and 92 healthy controls were examined for the presence of Fusobacterium necrophorum DNA using a novel TaqMan-based real-time quantitative PCR assay for F. necrophorum subspecies. The assay was based on the gyrB subunit gene, and detected F. necrophorum DNA in 48% of patients with NST and in 21% of controls (p <0.001). F. necrophorum subsp. funduliforme was the only subspecies found in both patients and controls. The load of F. necrophorum DNA on swabs from patients with NST was significantly higher than t

Discrepancy between total white blood cell counts and serum C-reactive protein levels in febrile children.

Total white blood cell (WBC) counts and serum C-reactive protein (CRP) are used as inflammatory markers in febrile children. We studied the occurrence and clinical significance of discrepancy in these markers. From a 2-y period, we retrospectively reviewed the medical records of febrile children (> or =1 month of age) with WBC > or =15 x 10(9)/l and/or CRP levels > or =80 mg/l, as well as of children with lower values in both these parameters. WBC and CRP were discordant in 556 children and concordantly high in 194 children. A severe bacterial disease was presumed in 57% of children with conco
