Perianal streptococcal dermatitis. The possible protective role of alpha-streptococci against spread and recurrence of group A streptococcal throat infection.

OBJECTIVE: To follow the spread of beta-haemolytic streptococci group A (GAS) within a family and examine the protective activity of normally occurring alpha-streptococci against GAS tonsillitis. DESIGN: Follow up of recurrent GAS throat infection within a family. SETTING: Intra familial spread of GAS. PATIENTS: A family of four, the mother suffering from recurrent streptococcal tonsillitis and a son with perianal streptococcal dermatitis. RESULTS: The strain of the GAS found in the perianal region of the boy was identical with that found in the throat of his mother. She had recurrent stre

Interference by aerobic and anaerobic bacteria in children with recurrent group A beta-hemolytic streptococcal tonsillitis.

OBJECTIVE: To compare the frequency of recovery of aerobic and anaerobic bacteria with interfering capability of group A beta-hemolytic streptococci (GABHS) in the tonsils of children with and without a history of recurrent GABHS pharyngotonsillitis. PATIENTS AND METHODS: Tonsillar cultures were taken from a group of 20 children with and 20 without history of recurrent GABHS pharyngotonsillitis. RESULTS: Eleven aerobic and anaerobic isolates with interfering capability with GABHS were recovered from 6 (30%) of the 20 children with recurrent GABHS, and 40 such organisms were isolated from 17

[Sore throat in general practice. Minimizing diagnosis--preventing superfluous use of antibiotics].

Sore throat usually is caused by viral pharyngitis, in about 15 to 30% by group A beta-haemolytic streptococci. Based on current concepts a guideline for the management in general practice is developed. If the typical symptoms of streptococcal pharyngo-tonsillitis are present--serious sore throat, fever more than 38.5 degrees C, purulent tonsillar exsudate, painful cervical lymphadenopathy, lack of cough or rhinorrhea--without any other diagnostic procedure penicillin is given for ten days. Only in cases of doubt throat swabs are taken for rapid diagnostic test and culture. The implementation

Tonsillitis and chronic psoriasis.

Although the relationship between streptococcal tonsillitis and acute guttate psoriasis is well recognized, its relationship to chronic forms of psoriasis is less established. In order to explore this further, the authors questioned 35 patients with severe psoriasis and 35 age-matched eczema controls about their history of significant sore throats (i.e. requiring a GP visit) and any resultant worsening of the skin condition. A third of the psoriasis patients reported recurrent sore throats which worsened their skin condition. This was true of only one (3%) of the 35 eczema controls. The author

Etiologic aspects of chronic urticaria.

BACKGROUND: Urticaria is a common disease that is always a challenge to the dermatologist due to its evasive etiology. PATIENTS AND METHODS: One hundred and seven chronic urticaria patients were studied. Routine laboratory investigations were performed and Helicobacter pylori (H. pylori) immunoglobulin G (IgG) antibody determinations, autoimmune reactivity, infections, allergies, and hyperreactivities were investigated. RESULTS: Pathologic findings were seen in 92 patients. Concomitant diseases suggesting autoimmune reactivity were detected in nine patients and, in 16 patients, infections in

[Plea for an evaluation of beta hemolytic streptococcal tonsillitis: from diagnosis to treatment].

There is no justification to maintain a systematic antibiotic treatment of all tonsillitis as currently practiced in France. Indeed, it is today possible to exclude the streptococcal origin of tonsillitis with a fair probability using a rapid diagnosis test. It is therefore mandatory that these tests are made available to the French practitioner. Additionally, alternatives to the classical 10-day penicillin V treatment of streptococcal tonsillitis must be seriously considered: shorter duration treatment with oral cephalosporins, macrolides or amoxicillin have proved to be as efficient and of l

[Acute tonsillitis: towards a new therapeutic strategy].

There is a need for revising the current practice for treatment of acute tonsillitis in France, i.e., systematic antibiotic treatment. There are three main reasons for this revision: 1) group A betahemolytic streptococcus is involved in only 20% of acute tonsillitis (80% being viral); 2) rheumatic fever has become very rare; 3) efficient rapid diagnostic tests are now available, allowing a selection of patients with streptococcal tonsillitis who must be treated.

Acute nonrheumatic myopericarditis associated with group A hemolytic streptococcal tonsillitis in a male ICU-nurse.

We present the case of a 38-year-old man who developed acute myopericarditis, mimicking acute myocardial infarction, as manifested by electrocardiographic, echocardiographic alterations and elevated cardiac enzymes complicating Lancefield group A beta-hemolytic streptococcal tonsillitis. After receiving oral penicillin, the clinical recovery was complete. Fever, tachycardia and chest discomfort resolved within a few days. Furthermore, enzyme levels and C-reactive protein returned to normal within eight days.

Circulating levels of cytokines in poststreptococcal glomerulonephritis.

Cellular immune mechanisms are important in the pathogenesis of acute glomerulonephritis, as underlined by recent demonstrations of cytokine activity in the urine and in the renal tissue of some of these patients. Therefore, we decided to study circulating levels of IL-6, TNF alpha and PDGF measured by ELISA in 12 patients with acute poststreptococcal glomerulonephritis (PSGN) on admission, at the time of discharge from the hospital, 45 days and 3 months later. We also studied 9 patients with acute streptococcal pharyngitis without nephritis, during acute infection, 8 and 21 days later and 20

Azithromycin: 3-day versus 5-day course in the treatment of respiratory tract infections in children. Croatian Azithromycin Study Group.

A total of 371 children, aged 6 months to 12 years, with acute otitis media, acute sinusitis, streptococcal tonsillitis/pharyngitis, or pneumonia were included in an open, multicenter study. Among them, 192 children were randomized to receive azithromycin for 3 days (10 mg/kg daily), and 179 for 5 days (10 mg/kg on day 1 and 5 mg/kg on days 2-5). The overall clinical cure rate was 95.7% and 96.1%, and bacteriological eradication rate 90.1% and 94.2% in the 3-day and 5-day groups, respectively. Side effects, mostly mild gastrointestinal disturbances, were observed in 5.3% of children from the 3

Comparison of clarithromycin suspension and amoxycillin syrup for the treatment of children with pharyngitis and/or tonsillitis.

Twenty-three general practices in the UK randomised 229 paediatric patients (aged 1-12 years, body weight > 8 kg) with clinical evidence suggestive of streptococcal tonsillitis and/or pharyngitis in this physician-blind study. Patients received either clarithromycin suspension (7.5 mg/kg twice daily) or amoxycillin syrup (125 mg/kg three times daily body weight < 25 kg, or 250 mg/kg three times daily body weight 25 kg) for 7 days and were followed up 3-8 days post treatment and 21-28 days later. Clinical and microbiological assessments were made at each visit. A total of 189 patients (98 on cl

Five-Day Spiramycin vs Seven-Day Penicillin V in the Treatment of Streptococcal Tonsillitis in Children.

Because of the frequency of penicillin allergies in children receiving β-lactam antibacterial agents, the macrolides are frequently chosen as alternatives in patients with group A β-haemolytic streptococcal (GABHS) infections. Spiramycin, amacrolide widely used in paediatrics, achieving remarkably high tonsillar tissue concentrations, was evaluated in this study in comparison with penicillin V (Phenoxymethylpenicillin). 298 children aged 1.5 to 14 years with acute tonsillitis and a positive rapid antigen test for GABHS were randomised to receive either a 5-day course of spiramycin 100 000 IU/k

Interaction between oral alpha-streptococci and group A streptococci in patients with tonsillitis.

The incidence of oral alpha-streptococci with inhibitory activity against group A streptococci, as a defense mechanism against bacterial infection in the oral cavity, was investigated in 141 patients with streptococcal tonsillitis. The study population included both children (n = 79) and adults (n = 62). Infection by group A streptococci appeared to be more common in children than in adults, as the detection rates of inhibitory alpha-streptococci in healthy children (29.7%), as well as pediatric patients with tonsillitis (14.9%), were lower than those in adults (63.0%; p < .01). It is possible

Effect of penicillin or cefprozil therapy on tonsillar flora.

The effect on the tonsillar bacterial flora of antimicrobial therapy with penicillin or a second-generation cephalosporin (cefprozil) was studied. Sixty children scheduled for elective tonsillectomy because of recurrent group A beta-haemolytic streptococcal tonsillitis participated in a prospective randomized study that divided them into three groups. One group received no therapy, and the others were given either penicillin or cefprozil for 10 days prior to surgery. The core of the patients' tonsils was cultured for aerobic bacteria. Group A beta-haemolytic streptococci (GABHS) were isolated

Streptococcal tonsillitis and its association with psoriasis: a review.

Lancefield group A streptococcal upper respiratory tract infections are well known to be precursors of a number of disease processes. That they frequently herald a first attack of guttate psoriasis or a reactivation of chronic plaque psoriasis is well recognized, though this is perhaps more true among dermatologists than otolaryngologists. This paper briefly summarizes the historical background, recent research into, and current understanding of the connection between the two pathological phenomena.

6-day amoxicillin versus 10-day penicillin V for group A beta-haemolytic streptococcal acute tonsillitis in adults: a French multicentre, open-label, randomized study. The French Study Group Clamorange.

We compared the efficacy and safety of amoxicillin 1 g b.i.d. for 6 days and penicillin V 1 MU t.i.d. for 10 days in the treatment of group A beta-haemolytic streptococcal (GABHS) tonsillitis in adults. 342 patients with acute tonsillitis and a positive test for streptococcal antigen were included in a multicentre, randomized, open-label study; 338 patients were evaluable in an intention-to-treat analysis, and 234 with a positive throat swab culture in a per-protocol analysis. In the per-protocol analysis, the GABHS eradication rate at the end of treatment was comparable in the two groups: 92%

[Importance of anaerobic bacteria in recurrent childhood non-streptococcal tonsillitis].

BACKGROUND: The presence of anaerobes in the oropharyngeal flora was already known, but they were infrequently involved in infections, except for specific diseases (abscesses, Vincent's angina). The involvement of anaerobes in more frequent by observed oropharyngeal diseases has been recently proposed. METHODS: We analyzed the tonsillar flora of 22 healthy children (control group) and 54 children diagnosed of recurrent tonsillitis. We compare the clinical response of children with recurrent tonsillitis to two treatments: penicillin, and penicillin plus metronidazole. RESULTS: We isolated Str

A double-blind, multicenter comparative study of two regimens of clindamycin hydrochloride in the treatment of patients with acute streptococcal tonsillitis/pharyngitis.

In a double-blind, prospective, randomized, multicenter study, 164 patients with a clinical and bacteriologic diagnosis of acute streptococcal tonsillitis/pharyngitis were enrolled to compare the efficacy and safety of two regimens of clindamycin. A rapid identification test of Group A beta-hemolytic streptococci (GABHS) was used to initiate the therapy; however, a positive tonsillar/pharyngeal culture was required at pretreatment to determine if the patient was assessable. Another culture was repeated at least 2 days after the 10 days of drug therapy. From 164 patients enrolled (mean age, 27.
