[Streptococcal tonsillitis: failure of penicillin therapy].

Group A beta-haemolytic streptococci (GAS) have remained sensitive to penicillin, but penicillin V therapy does not eradicate GAS in 5-30% of cases of GAS tonsillitis. Multiple factors contribute to treatment failure, the foremost among these being lack of compliance with the prescribed therapy, tolerance of GAS towards the killing effect of penicillin, inactivation of penicillin by bacterial beta-lactamases, and absence of bacteriocin-producing non-haemolytic streptococci from the oral flora. The effect of local immunity should be further assessed. It is unlikely that environmental sources of

Interfering alpha-streptococci as a protection against recurrent streptococcal tonsillitis in children.

Recurrent streptococcal tonsillitis/pharyngitis is a great problem, especially in certain epidemiological situations. Patients treated with antibiotics often have a disturbed normal throat flora and may lack, e.g., alpha-streptococci known in vitro to have an interfering activity against group A streptococci. Thirty-one patients with recurrent streptococcal tonsillitis were given antibiotics for 10 days. At the end of this treatment they were sprayed in their mouths with four selected alpha-streptococcal strains known to have strong growth inhibiting activity in vitro against most beta-strepto

Alpha-streptococci as supplementary treatment of recurrent streptococcal tonsillitis: a randomized placebo-controlled study.

Recurrences are a common finding after antibiotic treatment of acute group A streptococcal tonsillitis. This has been attributed to several factors, among others a disturbed normal throat flora and especially a lack of alpha-streptococci. It thus seems logical in patients with recurrent streptococcal tonsillitis, to restore the normal alpha-streptococcal flora by reimplantation of alpha-streptococci. This was performed in a double blinded, randomized, placebo-controlled study. 36 patients with recurrent streptococcal group A tonsillitis were treated with antibiotics followed by either placebo

Penetration of penicillin V to tonsillar surface fluid in healthy individuals and in patients with acute tonsillitis.

In the treatment of group A streptococcal tonsillitis, as the bacteria are located on the epithelial surface, an important determinant of outcome is the concentration of penicillin in extracellular tonsillar surface fluid. Accordingly, we investigated the concentration of penicillin in serum, and penetration to tonsillar surface fluid and saliva in nine patients with acute group A streptococcal tonsillitis and in nine healthy controls. Among the healthy subjects, despite high serum penicillin concentrations (mean, 2.04 micrograms/ml), there was no penetration to tonsillar surface fluid or to s

[Autoimmune reactions in diseases of streptococcal etiology].

In this work 98 cases of streptococcal tonsillitis and 361 cases of erysipelas were studied. As revealed in this study, in relapsing forms of streptococcal infection antibodies to cross-reacting antigens of the infective agent appear in the blood. The synthesis of antibodies to the basal layers of skin epithelium was accompanied by an increase in the concentration of medium-size immune complexes, 0-cell population and by a decrease in the number of T suppressors. The mechanism of the development of the autoimmune process in relapsing forms of streptococcal infection is discussed.

Penicillin tolerance in group A streptococci and treatment failure in streptococcal tonsillitis.

Penicillin tolerance in Streptococcus pyogenes has been suggested as a possible cause of therapeutic failure in streptococcal phryngitis treated with penicillin. In 144 patients with acute group A streptococcal tonsillitis treated with phenoxymethyl penicillin 12.5 mg per kg body weight b.i.d. for 10 days the same T-type was recovered after treatment in 21%. The recovery rate was higher for non-tolerant strains, 23%, than for tolerant strains, 10% (p greater than 0.05). Of patients with a non-tolerant strain 17% had both clinical and bacterial treatment failure in comparison with 5% infected w

An evaluation of a commercial co-agglutination test for the diagnosis of group A streptococcal tonsillitis in a family practice.

The result of a rapid co-agglutination test was compared with the clinical diagnosis of group A streptococcal tonsillitis, a laboratory culture from the throat serving as the correct result. The study was carried out in a family practice, with two GPs partaking: it included 105 consecutive patients. The outcome of the rapid test was significantly correlated to the degree of growth determined by throat culture, and it was superior to the clinical judgement. The statistical analysis of the results of the rapid test revealed (95% confidence limits in brackets) a specificity of 97% (91-100), sensi

Acute nonrheumatic perimyocarditis complicating streptococcal tonsillitis.

Streptococcal infection is a rarely recognized cause of nonrheumatic perimyocarditis. We report a case of a young patient who developed acute perimyocarditis as manifested by diffuse electrocardiogram changes, and markedly elevated cardiac enzymes, concurrently with streptococcal tonsillitis. Despite the dramatic presentation, the patient recovered uneventfully. We conclude that streptococci can involve the heart also directly by a bacterial component or toxin, and not only through a delayed immunologic mechanism as in rheumatic fever. Further studies are necessary to accurately determine the

[Nuclear fragmentation of neutrophilic granulocytes in patients with angina].

The informative value of the neutrophilic segmentation index (NSI) was estimated in 77 patients with primary streptococcal tonsillitis and in 40 normal young subjects. This index was found reduced at the peak of the disease in all the patients, this decrease depending on the disease severity. During convalescence the sooner NSI recovered, the milder was the disease course. This permits recommending NSI as an objective laboratory criterion permitting assessment of the severity of bacterial tonsillitis and the efficacy of its treatment.

[Acute tonsillitis: clinical symptoms; bacteriologic culture and rapid test as deciding criteria for the use of antibiotics].

In order to prevent late sequelae from an untreated streptococcal pharyngitis all patients with streptococcal tonsillitis/pharyngitis have to be treated with antibiotics, preferably penicillin. A correct diagnosis by clinical criteria is only achievable in 72% of patients with streptococcal pharyngitis. Additional criteria such as diagnosis by culture are, therefore, mandatory. Time is a major disadvantage of traditional culture methods. Slide agglutination tests show an acceptable sensitivity of 92% and a specificity of 93%. Due to the possibility of false negative test results, with conseque

[Use of immunoenzyme analysis for determining the persistence and biodegradation of antigens in the cell wall of group A streptococci in patients with rheumatism].

A sandwich ELISA system was used for the study of the identification of antigenic group A polysaccharide determinants by antibodies in a suspension of streptococcus cells with varying degrees of lysis. Antibodies were able to identify less than 0.01% of antigenic A polysaccharides determinants in intact streptococcal cells. It is concluded that A polysaccharide is a marker of the destroyed group A streptococcus cells. The circulation of group A polysaccharide was examined in the sera of patients with rheumatism and acute streptococcal tonsillitis. Group A polysaccharide was detectable more fre

Acute tonsillitis in young men: etiological agents and their differentiation.

Group A beta-hemolytic streptococci were found in 38% of 257 young men in military service with acute tonsillitis. Of the 108 patients tested also for viral antibodies, 42% showed a 4-fold rise in antibody titers. Adenovirus was the most frequent (31%) nonstreptococcal agent, followed by Epstein-Barr virus (6%), and influenza virus (5%). Non-group A streptococci were isolated in about the same proportion (18%) as in healthy control subjects. Other data also suggested that these bacteria were carried and not true infecting organisms. Group A streptococci and adenovirus occurred in mixed infecti

Streptococcal tonsillitis and acute nonrheumatic myopericarditis.

Two young men with streptococcal tonsillitis had acute myopericarditis mimicking myocardial infarction. Subsequently, 264 consecutive army conscripts hospitalized with a fever and sore throat underwent throat cultures. Group A streptococci were found in 84 patients. When these patients were systematically screened by serial electrocardiography for myocarditis, one case of probable asymptomatic myocarditis was diagnosed. None had signs of acute rheumatic fever. All three patients received penicillin therapy when the signs of myocarditis appeared. Their recovery was good.

Microbiology of obstructive tonsillar hypertrophy and recurrent tonsillitis.

A qualitative and quantitative analysis of the tonsillar surface and core of children with recurrent streptococcal tonsillitis and children with obstructive tonsillar hypertrophy was performed. No qualitative difference was found within the two population groups. Haemophilus influenzae and Bacteroides melaninogenicus were the most prevalent beta-lactamase-producing isolates in both groups. Staphylococcus aureus had the highest rate of beta-lactamase production on the tonsillar surface of children with recurrent tonsillitis, while Streptococcus pyogenes was more prevalent in the tonsillar surfa

Medical treatment of non-streptococcal recurrent tonsillitis.

Forty patients with a history of recurrent non-beta-hemolytic streptococcal tonsillitis (RNST) participated in a prospective randomized study comparing penicillin and clindamycin in the treatment of acute non-group A streptococcal infection. The efficiency of each antibiotic was evaluated according to its ability to alleviate acute infection and prevent recurrence. Surface tonsillar cultures were obtained both before and ten days after the termination of therapy, and specimens were processed for aerobic and anaerobic bacteria.
