Treatment of patients with acute recurrent tonsillitis due to group A beta-haemolytic streptococci: a prospective randomized study comparing penicillin and amoxycillin/clavulanate potassium.

The failure of penicillin to eradicate Group A beta-haemolytic streptococcal tonsillitis may be caused by beta-lactamase producing bacteria in the tonsillar tissue. A prospective randomized clinical study comparing the efficacy of penicillin-V potassium with amoxycillin plus clavulanate potassium (Augmentin) in the treatment of acute episodes of recurrent streptococcal tonsillitis was conducted. Twenty children were included in each group. Surface tonsillar cultures were obtained before therapy, ten days after termination of therapy, and then once every two months for up to one year. beta-Lact

[Rapid diagnosis of acute tonsillitis caused by streptococcus group A].

In general practice, a material of 202 patients seeking advice on account of difficulty in swallowing were included consecutively in an investigation with the object of assessing the value of a rapid test, Respiralex, for the diagnosis of group A streptococci as compared with culture. The true positive and negative diagnostic frequencies were 0.89 and 0.94, respectively. Absence of agreement was found most frequently when culture rendered poor growth. It is concluded that Respiralex is such a reliable method of examination in the diagnosis of group A streptococci, that it is recommended for us

Atrioventricular block complicating acute streptococcal tonsillitis.

A 38 year old woman presented with severe weakness, high fever, and sore throat. Physical examination showed follicular tonsillitis and bradycardia caused by a atrioventricular block. Within 24 hours a normal sinus rhythm was regained but slight transient ST-T changes compatible with myocarditis were evident. Throat culture grew Streptococcus haemolyticus group A.

Comparison of the microbiology of group A and non-group A streptococcal tonsillitis.

We studied the microbial flora of tonsils removed from 20 children who suffered from recurrent group A beta-hemolytic streptococcal (GABHS) tonsillitis and 20 who had tonsillar hypertrophy following recurrent non-GABHS tonsillitis. Similar polymicrobial aerobic and anaerobic flora were recovered from the cores of the tonsils in each group. beta-Lactamase-producing bacteria (BLPB) were recovered more often in the group with GABHS. This difference was due mostly to the lower incidence of beta-lactamase-producing strains of Branhamella catarrhalis and Bacteroides sp in hypertrophic tonsils follow

[Studies on plasma levels and clinical efficacy of rokitamycin in pediatrics].

Pharmacokinetic and clinical evaluations of rokitamycin (RKM, TMS-19-Q), a new macrolide antibiotic, were carried out. RKM was administered orally to 14 patients with congenital heart diseases before cardiocatheterization and angiography. Peak plasma levels of RKM were observed at 30 minutes after the administration at dosages of 5, 10, 15 mg/kg. Although the reason is not clear, there were great variations among plasma levels. Peak plasma levels of patients with relatively good absorption were high enough against bacteria such as beta-hemolytic Streptococcus, Mycoplasma pneumoniae and Chlamyd

The distribution of immunocompetent cells in the compartments of the palatine tonsils in bacterial and viral infections of the upper respiratory tract.

Employing a series of monoclonal antibodies directed against T cell subsets using the avidin-biotin complex method as the immunoperoxidase technique and using fluorescein-conjugated antisera directed against the major immunoglobulins, we have studied the distribution of immunocompetent cells in sections of tonsils from 21 patients with various inflammatory diseases of the tonsils, including Streptococcal tonsillitis, recurrent tonsillitis and tonsillitis associated with infectious mononucleosis. The following conclusions can be made in regard to our study. The percentage of T cells decreases i

Short-term treatment of streptococcal tonsillitis with ceftriaxone.

The short-term therapy of streptococcal pharyngotonsillitis and scarlet fever with ceftriaxone (Rocephin) is reported. Sixty children, in whom the clinical diagnosis was confirmed by rapid enzyme immunoassay and smear test, were divided into two randomized groups and treated with a single dose of 50 mg/kg ceftriaxone or 50 mg/kg ceftriaxone on 3 consecutive days. Clinical cure was obtained in 100% of the patients and pharyngeal sterilization in 95%, with no significant differences between the two groups.

Penicillin tolerance in beta-streptococci isolated from patients with tonsillitis.

Beta-streptococci isolated from patients with acute tonsillitis were tested for penicillin tolerance defined as an MBC/MIC ratio greater than or equal to 16. 11/18 strains recovered from patients with clinical treatment failure were tolerant to penicillin in comparison with 0/15 strains from successfully treated patients. The MBC/MIC ratio was less than 16 for all strains versus cefadroxil but above that ratio for many strains versus clindamycin, doxycycline and erythromycin. We suggest that penicillin tolerance may be one reason to treatment failures in individuals with streptococcal tonsilli

Twice-a-day regimen of erythromycin base is effective in the treatment of acute streptococcal tonsillitis.

A multicenter clinical trial on the efficacy and safety of a twice-a-day administration of erythromycin base was performed on 261 adolescent or adult patients with acute tonsillitis. The patients were randomized for each doctor separately into q.i.d. (4 x 250 mg) and b.i.d. (2 x 500 mg) groups. Throat swabs were cultured for group A beta-haemolytic streptococci before the treatment and, in positive cases (100 patients), 2-3 days after completing the treatment of at least 7 days. No significant difference in the efficacy between the two regimens was observed as judged by clinical evaluation or

Diagnosis of streptococcal tonsillitis in general practice by clinical assessment and by office culture of throat swabs on Streptocult.

In a one-year study 274 general practitioners performed office cultures on Streptocult of throat swabs from 2,699 patients with acute pharyngotonsillitis in order to identify those with beta-haemolytic streptococci in the throat. The finding of group A beta-haemolytic streptococci by laboratory culture of corresponding throat swabs was used as a reference when determining the diagnostic value of either clinical assessment or Streptocult culture for the identification of patients with streptococcal pharyngotonsillitis. Based on 2,150 cases, sensitivity was 70% and 84%, respectively; specificity

Clinical and laboratory findings in patients with acute tonsillitis.

In 82 patients with acute tonsillitis studied, beta-hemolytic group A streptococci were isolated from 30 (37%), and group C or G streptococci from 12 (15%). In the 40 patients with non-streptococcal tonsillitis there was a significantly higher isolation rate of pneumococci, H. influenzae and/or B. catarrhalis, as compared with those with beta-hemolytic streptococci. Patients were classified regarding clinical status according to standardized criteria as severe, moderate, or mild. The patients with group A streptococcal tonsillitis were significantly more often classified clinically as 'severe'

Concentrations of phenoxymethylpenicillin and cefadroxil in tonsillar tissue and tonsillar surface fluid.

Thirty patients who underwent elective tonsillectomy were given phenoxymethylpenicillin (0.8 g) or cefadroxil (1 g) at different times before operation. The concentrations of the antibiotics were analysed in serum, tonsillar tissue, fluid from the surface of the tonsils, and mixed saliva. The concentrations in tonsillar tissue for both drugs were much lower than the corresponding serum concentrations. This apparently low tissue accessibility could be ascribed to the limited intracellular penetration of beta-lactam antibiotics. For both antibiotics the concentrations in the tonsillar surface fl

Evaluation of beta-lactamase activity and microbial interference in treatment failures of acute streptococcal tonsillitis.

Out of 169 patients with streptococcal tonsillitis treated with phenoxymethylpenicillin, 13 (8%) developed a new clinical infection with the same streptococcal strain within 2 weeks of completing the therapy (clinical treatment failure) and 24 (14%) were clinically healthy but harboured the same streptococcal strain after treatment (bacterial treatment failure). Patients with clinical treatment failure showed beta-lactamase activity in their saliva pellet significantly more often than patients with bacterial treatment failure, healed streptococcal tonsillitis or non-streptococcal tonsillitis a
