Effect of meteorological parameters on acute laryngitis in adults.

The aim of this study was to investigate the influence of weather conditions on the frequency of acute laryngitis in adults. The medical records of 825 patients with a diagnosis of acute laryngitis obtained over a 5-year period were reviewed. Meteorological data reviewed included the daily values of 13 parameters. The monthly distribution of the disease was assessed. In addition, a detailed 2-part statistical analysis was performed as follows: (i) the relationship between each meteorological parameter and the frequency of the disease was investigated, using contingency tables; and (ii) the 182

[Inflammation and laryngitis].

UNLABELLED: 1. DISTINCT ENTITIES: Unlike inflammation or the middle ear or nasal or sinus cavities, there is no direct link between acute and chronic inflammation of the larynx. Chronic laryngitis is a distinct nosological entity. Patients with recurrent allergic laryngitis do not necessarily develop chronic laryngitis as is observed in patients with nasal allergy. 2. RESEARCH: There is no research data on infections of the larynx comparable to that available for the middle ear or the nasal cavities. This is probably related to the difficulty of accessing the laryngeal mucosa and the risks r

Endoscopically demonstrable esophageal changes after Helicobacter pylori eradication in patients with gastric disease.

OBJECTIVE: To compare the cost-effectiveness of preoperative testing strategies in women with stress incontinence symptoms, as although urodynamic testing is used to improve the diagnostic accuracy in women with incontinence, the clinical and economic consequences of different levels of testing have not been evaluated. MATERIALS AND METHODS: Decision analysis was used to evaluate basic office assessment (BOA) and urodynamic testing for women with stress incontinence symptoms who were candidates for primary surgical treatment. Costs were calculated using the Federal Register. Parameter estimat

Steroid therapy of acute ENT infections: rarely indicated.

(1) Apart from acute laryngitis in children, the use of steroids in acute ENT infections is not supported by clinical data. (2) A single dose of steroids (oral or intramuscular dexamethasone, 0.6 mg/kg) has only moderate efficacy in children with acute laryngitis, but it can hasten symptom relief. Available clinical data fail to show whether steroid therapy reduces the frequency of severe respiratory complications in this setting, or if it is helpful in minor cases. (3) There are no published data justifying the use of steroids as adjuvant treatment in other acute ENT infections, such as non a

24 hours prior to curtain.

A retrospective review was conducted of 40 singers presenting with acute voice problems prior to performance. The purpose of this study was to determine the reasons for seeking emergent voice treatment, the types of acute voice disorders, and the performance outcome. The patients were assessed by age, singing style, years of experience, chief complaint, laryngovideostroboscopic findings, and treatment regimens. The outcomes were classified as full, restricted, or no performance. The majority of patients were classical singers. Laryngovideostroboscopy frequently revealed a pattern of early glot

Serological evidence of pertussis in patients presenting with cough in general practice in Birmingham.

Three hundred and fifty-six patients in a large suburban practice (registered population 10,400), were diagnosed clinically with acute laryngitis/tracheitis or whooping cough (acute spasmodic cough of three weeks duration) between March 1996 and November 1997. Forty out of 145 who provided specimens for serological testing had evidence of recent infection with Bordetella pertussis. During the study a further 18 patients (mostly younger patients who presented early) had a diagnosis of pertussis confirmed by culture. Fifty-eight cases of pertussis in this population and time period was equivalen

An unusual cause of stridor-a case report.

Slridor in children is usually acute in onset and most commonly caused by infections such as acute laryngitis, laryngotracheobronchitis, and acute epiglottitis. Stridor due to foreign body is also not uncommon. The case reported is that of a large upper thoracic pre and paravertebral abscess presenting with stridor. Although a rare cause, it presents as a life threatening entity requiring urgent treatment.

[Continuous flow apnoeic ventilation via intratracheal oxygen insufflation].

BACKGROUND: In patients with disturbed gas-exchange (e.g. COPD) intratracheal oxygen insufflation (ITO2) improves oxygenation and reduces the minute ventilation. We use a bronchoscopic technique of intratracheal catheter placement in unintubated patients. In a patient with a pink-puffer emphysema after endoscopical insertion of the catheter ITO2 induced a "continuous flow apnoeic ventilation" (CFAV). CASE REPORT: A patient (female, 58 years) with a pink-puffer emphysema was admitted to the ICU with acute on chronic respiratory failure due to acute laryngitis. Because of laryngitis associated

Antibiotic prescribing for Canadian preschool children: evidence of overprescribing for viral respiratory infections.

Antibiotic resistance is associated with prior receipt of antibiotics. An analysis of linked computerized databases for physician visits and antibiotic prescriptions was used to examine antibiotic prescribing for different respiratory infections in preschool children in Canada. In 1995, 64% of 61,165 children aged <5 years made 140,892 visits (mean, 3.6 visits per child) for respiratory infections; 74% of children who made visits received antibiotic prescriptions. Antibiotics were prescribed to 49% of children with upper respiratory tract infection, 18% with nasopharyngitis, 78% with pharyngit

Acute laryngitis in the rat induced by Moraxella catarrhalis and Bordetella pertussis: number of neutrophils, dendritic cells, and T and B lymphocytes accumulating during infection in the laryngeal mucosa strongly differs in adjacent locations.

Infectious laryngotracheitis results in fulminant respiratory distress. During the disease, the subglottic mucosa is selectively infected and swollen, the reason for this preference being unknown. Therefore, in the present study the immunoreaction of the laryngeal mucosa was studied in the rat after inhalation of either heat-killed Moraxella catarrhalis (PVG rats) or application of viable Bordetella pertussis (BN rats). The number of neutrophils, macrophages, dendritic cells, and T and B lymphocytes was determined in the mucosa of the supraglottic, glottic, and subglottic area of the larynx as

Adult acute upper respiratory tract infections in Sicily: pattern of antibiotic drug prescription in primary care.

We performed an observational study of the antibiotic-prescribing behaviour of Sicilian general practitioners (GPs) in managing acute upper respiratory tract infections (URTIs). Seventy-six GPs from 25 towns, representing a patient population of 96,630, participated in the study between September 1995 and May 1996. These physicians issued 2038 antibiotic treatment courses for acute upper respiratory tract infections: 792 for acute pharyngitis, 531 for acute tonsillitis, 304 for acute laryngitis and tracheitis, 268 for suppurative and non-suppurative acute otitis media, 124 for acute sinusitis

Prolonged croup due to herpes simplex virus infection.

UNLABELLED: Herpes simplex virus (HSV) is an uncommon cause of acute laryngitis in immunocompetent patients since it mostly occurs in immunocompromised subjects. We present two previously healthy children with prolonged gingivostomatitis and stridor (lasting 3 and 4 weeks) in whom HSV-1 was isolated from subglottal ulcers. CONCLUSION: HSV should be considered a possible pathogen in cases of prolonged or atypical croup not only in immunocompromised or elderly patients but also in otherwise healthy children.

[Good clinical practice in nebulization].

A meeting on nebulization held in April 1997 defined good clinical practices. Guidelines that were proposed pertained to the following: pneumatic and ultrasonic nebulizers; delivering circuit, occluded or not, the choice of the tip being done according to the disease to treat and to the drugs to be delivered; various functions, depending on the type of nebulizer; the particle size, as it will indicate which disease may be treated: between 2 and 6 microns for bronchi, between 0.5 et 3 microns for lung, > 5 microns for ear, nose and throat diseases; compatibility between the type of nebulizer an

[Corticotherapy in ENT. Subglottic laryngitis in children, acute sinusitis, naso-sinusal polyposis, sero-mucous otitis].

Corticosteroid therapy is a key element in the treatment of acute subglottic laryngitis as well as certain types of sinusitis and seromucous otitis. Insufficient or lacking compensatory mechanism in the physiological regulation of the ENT mucosa favors the progression of these different conditions to chronic or life threatening situations, particularly for acute laryngitis. Corticosteroid therapy is one of the management steps; it should be adapted to each individual and of short duration. The goal of corticosteroid therapy is to decrease inflammation, provide pain relief and reestablish the p

[Clinical study of acute supraglottitis as a disease entity].

Adult supraglottitis is an acute inflammation of the supraglottic structures first reported by Shapiro et al. While multiple anatomical sites in the larynx and oropharynx are inflamed, the epiglottis is not always the most involved area. In this paper, we refer to "adult supraglottitis" as "acute supraglottitis" because pediatric supraglottitis is rare in Japan. There have been no reports of acute supraglottitis in Japan to date. We report a clinical study of 15 cases of acute supraglottitis. In addition, we investigated whether acute supraglottitis can be recognized as a special form of acute

Some aerodynamic and acoustic characteristics of acute laryngitis.

The purpose of the present study was to describe the effects of acute laryngitis on some aerodynamic, acoustic, and perceptual measures. Eleven subjects with diagnosed acute laryngitis due to upper respiratory infection were recorded during a laryngitic episode and 1 week to 10 days after amelioration of the laryngitic condition. Fundamental frequency, values, collapsed across the five vowels, were significantly reduced in the laryngitic compared with the normal speaking condition. The decrease in fundamental frequency associated with acute laryngitis suggests an increase in the mass of the vo

Moraxella catarrhalis in acute laryngitis: infection or colonization?

The complement phenotypes of Moraxella catarrhalis isolates obtained from adult patients with acute laryngitis were investigated using a microliter serum bactericidal assay and compared with those of other donor groups. Laryngitis isolates had a higher proportion (57%) of complement-resistant strains than did carrier strains from healthy 8- to 13-year-old schoolchildren (16%). The difference between these groups was statistically significant (chi2 [3 x 2 table] = 21.55; P < .001). The relatively frequent occurrence of the complement-resistant (virulence-associated) phenotype in adults with acu
