[A case of paroxysmal sympathetic hyperactivity following hypoxic brain injury with ‍difficult treatment].

The patient was a woman in her 20s. In June of a certain year, she experienced cardiopulmonary arrest due to acute epiglottitis and was admitted to the ICU and required mechanical ventilation. From the first day of hospitalization, she exhibited generalized seizures and was diagnosed with acute symptomatic seizures following hypoxic brain injury. Treatment with diazepam and levetiracetam was initiated, leading to symptom resolution. However, around the seventh day of hospitalization, she began experiencing episodic limb muscle hypertonia and fever. Based on diagnostic criteria, she was diagnos

A Biomimetic Scaffold for Single-Stage Reconstruction of Load-Bearing Segmental Bone Defects.

INTRODUCTION: Croup leads to signs of upper airway obstruction, and must be differentiated from acute epiglottitis, bacterial tracheitis, or an inhaled foreign body. Croup affects about 3% of children a year, usually between the ages of 6 months and 3 years, and 75% of infections are caused by Parainfluenza virus. Symptoms usually resolve within 48 hours, but severe infection can, rarely, lead to pneumonia, and to respiratory failure and arrest. METHODS AND OUTCOMES: We conducted a systematic review and aimed to answer the following clinical questions: What are the effects of treatments in ch

Association of Early Steroid and Antibiotic Therapy With Airway Outcomes in Adult Epiglottitis: A 10-Year Multicenter Retrospective Cohort Study.

BACKGROUND: Adult epiglottitis is an uncommon but potentially life-threatening condition requiring rapid recognition and airway-focused management, yet evidence regarding early medical therapy is limited. We evaluated temporal trends in adult epiglottitis and examined associations between early corticosteroid or antibiotic therapy and clinically important outcomes. METHODS: We conducted a retrospective cohort study of adults (≥ 18 years) presenting to U.S. emergency departments with acute epiglottitis between 2014 and 2024 using the TriNetX Research Network. Cases were identified using the IC

Epiglottic Abscess in an Adult Male: A Case Report.

Acute epiglottitis in adults is an uncommon but potentially life-threatening condition that can rapidly progress to airway compromise. Epiglottic abscess represents a rare and severe complication associated with increased morbidity and diagnostic challenges. We report a case of a 36-year-old male admitted to the emergency department in Belgium with progressive odynophagia, dysphonia, and hypersalivation despite prior oral antibiotic therapy. Laboratory investigations revealed a marked inflammatory response, including leukocytosis and elevated C-reactive protein. Contrast-enhanced computed tomo

Airway Decision-Making in Acute Epiglottitis: A Case Report of Planned Tracheostomy in a Can Ventilate but Cannot Intubate (CVCI) Scenario.

Acute epiglottitis is a life-threatening emergency characterized by rapid airway obstruction and challenging airway management due to severe supraglottic edema. We report the case of a 22-year-old man who presented with acute epiglottitis complicated by extensive neck edema, inspiratory stridor, and progressive respiratory distress. Despite multiple attempts, endotracheal intubation failed because of severe epiglottic swelling obscuring the laryngeal inlet. However, effective bag-mask ventilation was maintained, resulting in a "can ventilate but cannot intubate" (CVCI) scenario rather than a "

Active Emergency Management of Adult Acute Epiglottitis: An Analysis of 570 Cases.

OBJECTIVE: Acute epiglottitis, which is primarily caused by bacterial infection, can quickly progress to life-threatening airway obstruction. The first 24 h after symptom onset is a critical window for management. However, few studies have evaluated the effect of emergency management during this period or have identified the factors that influence treatment outcomes. SUBJECTS AND METHODS: This retrospective study includes a cohort of patients diagnosed with acute epiglottitis between December 2012 and December 2022. Patients are categorized based on whether they received improved emergency ma

Simultaneous purulent pericarditis and acute epiglottitis from Haemophilus influenzae in an immunocompetent adult: a case report.

OBJECTIVES: Haemophilus influenzae can cause severe and invasive disease. While purulent pericarditis and epiglottitis are known to be rare complications of Haemophilus influenzae, we report a rare case of their simultaneous occurrence. METHODS: We report a 70-year-old female who presented with a sore throat and dyspnea to the emergency department, was found to have epiglottitis, and then rapidly deteriorated, developing shock and acute respiratory failure. A point-of-care ultrasound was performed, revealing a pericardial effusion with tamponade. RESULTS: Emergent pericardiocentesis yielded

Emergency Cricothyroidotomy for Acute Epiglottitis in a Patient With Super-Super Obesity: A Case Report.

Acute epiglottitis is uncommon in adults but can obstruct the airway within hours. Although few patients require airway intervention, those who do present one of the highest-risk intubation scenarios in clinical practice. Obesity further complicates physiological or technical challenges of airway management. It shortens safe apnea time, weakens the seal of a facemask or supraglottic device, hides the neck landmarks needed for a surgical airway, and can make imaging of the neck uninterpretable. A 39-year-old man presented with two days of sore throat, dysphagia, and a muffled voice. He had asth

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Refractory and Relapsing Laryngeal Edema Possibly Associated With Chronic Tonsillitis and Mycoplasma Infection, Requiring Reintubation and Tracheostomy.

Relapsing epiglottitis has rarely been reported, and its etiology is not well established. A 44-year-old previously healthy Japanese man presented with a quickly progressing choking sensation. He had been experiencing refractory and relapsing laryngeal edema and probably acute epiglottitis (three episodes within 2 weeks), with rash and elevated pancreatic amylase. The patient required immediate intubation. After the initial extubation, he required reintubation and a subsequent tracheostomy. Antibiotics, glucocorticoid, and antihistamines were administered, and he finally recovered with the tra

Epiglottitis in Patients Treated for Acute Leukemia: Case Series and Systematic Review of the Literature.

<p>Introduction: Acute epiglottitis is a medical emergency characterized by inflammation of supraglottic structures of the larynx and epiglottis. The clinical characteristics and course of this complication in acute leukemia (AL) are largely unknown. We present a case series and a systematic review of the literature of adult patients with AL and acute epiglottitis. Case Presentation: Four patients with acute myeloid leukemia (age 38-61 years) were diagnosed with acute epiglottitis during their intensive treatment course. One patient presented with epiglottitis at the end of induction, two at t

Acute epiglottitis in an older patient: a case report and review of the literature.

BACKGROUND: Acute epiglottitis is a rapidly progressive infection of the supraglottic structures that can potentially be life-threatening. Although its incidence has decreased in pediatric populations owing to widespread vaccination, it remains a diagnostic and therapeutic challenge in adults, where clinical presentations may be subtle, and airway compromise can occur with little warning. The case of a 64-year-old white European female patient is presented. CASE PRESENTATION: The case of a 64-year-old white European female patient is presented. Delayed Diagnosis of an Adult Epiglottic Abscess: A Case Report.

Distinguishing acute epiglottitis from other benign causes of sore throat can be challenging in the adult age group, and failure to do so can lead to an airway-related death. We report the case of a 65-year-old man with type 2 diabetes who initially presented with sore throat, odynophagia, and fever, and was twice discharged with presumed pharyngitis after outpatient and emergency department evaluations. Within 24 hours, he returned with worsening odynophagia and inability to swallow medications. Despite stable vital signs and absence of stridor and drooling, laboratory testing revealed leukoc

A Rare Case of Adolescent Epiglottitis Secondary to Streptococcus dysgalactiae Septicemia.

Streptococcus dysgalactiae (S. dysgalactiae)is a relatively uncommon pathogen in the pediatric populations, often overshadowed by Streptococcus pyogenes (S. pyogenes)in causing diseases such as cellulitis, severe seep tissue necrotizing infections, and streptococcal toxic shock syndrome. This case report presents the case of a 15-year-old recent immigrant male patient from Egypt who developed an extensive neck infection with complications from S. dysgalactiae septicemia. Initially misdiagnosed as viral pharyngitis, the patient was later admitted with high fevers, dysphagia, and progressive res

Purpura Fulminans as a Rare Manifestation of Invasive Haemophilus influenzae Disease: A Case Report.

Haemophilus influenzae (Hi) is a bacterium usually found in the upper respiratory tract of humans. Though it is recognized as a naturally occurring element in the human bacterial reservoir, Hi infections have the potential to be severe and even fatal, particularly when they result in conditions such as meningitis or epiglottitis. Because of this, Hi invasive infections are considered a reportable disease in Portugal. We report a case of a 58-year-old female, chronically adrenally suppressed on long-term steroids, who developed an invasive Hi type b infection that led to purpura fulminans and m
