Otitis media and otitis media with effusion are among the most common childhood illnesses and contribute a great deal to health care costs. The cause of otitis media is multifactorial. Eustachian tube dysfunction, bacterial or viral infection of the middle ear, and nasal inflammation resulting from allergic rhinitis or upper respiratory infection are acknowledged contributing factors. Data from epidemiology studies indicate that 25% to 40% of upper respiratory infections in children younger than 3 years are accompanied by an episode of otitis media, 40% to 50% of children older than 3 years wi

OBJECTIVE: Each year, influenza and its complications account for 10,000-40,000 excess deaths in the United States. Over 80% of these deaths occur among the elderly. The Advisory Committee on Immunization Practices for the US Public Health Service and others recommend that all persons 65 years of age and older as well as other persons with high-risk conditions receive annual influenza vaccination. Despite these recommendations, 40% or more of high-risk persons fail to receive influenza vaccine. Among the barriers to successful immunization efforts are persistent uncertainties regarding the eff

Respiratory syncytial virus (RSV), the most important cause of lower respiratory disease in infants and young children, is rarely considered among the causes for community-acquired lower respiratory infection in adults. All noninstitutionalized adults hospitalized with community-acquired pneumonia in two Ohio counties were evaluated between December 1990 and May 1992. Fifty-three (4.4%) of 1195 adults admitted during the RSV seasons and 4 (1.0%) of 390 in the off-season had serologic evidence of RSV infection, making RSV one of the four most common pathogens identified. RSV-infected patients h

Viral etiologic agents of acute lower respiratory tract infections were studied from November, 1990, through April, 1994, in Korean children. From 712 children who visited or were admitted to Seoul National University Children's Hospital because of acute lower respiratory tract infections, 804 nasal aspirates were collected; viral agents were detected by virus isolation and virus antigen was detected by indirect immunofluorescent staining. One or more viral agents were identified in 369 (45.9%) cases; of which 3.3% were mixed infections. The pathogens identified were respiratory syncytial viru

BACKGROUND: Influenza vaccine effectiveness evaluations were carried out among the elderly, as part of a demonstration established to estimate the value of including influenza vaccination as a covered Medicare benefit.
METHODS: Cases hospitalized with pneumonia and influenza-related diagnoses during November through April were identified and group matched to randomly selected community controls. Data were collected from cases and controls on influenza vaccination status and other factors which could have confounded the association between vaccination and hospitalization. A community-based inf

We reviewed data collected between January 1987 and December 1989 on the etiology of acute respiratory infections (ARI) among 827 children in two low-income communities and a hospital in Rio de Janeiro. Respiratory syncytial virus was identified in 38% of cases of ARI, influenza A virus in 1%, parainfluenza 3 virus in 1%, and multiple viruses in 1%. Respiratory syncytial virus was most prevalent among hospitalized children, with seasonal increases in the late fall and winter. The principal bacterial pathogens were Staphylococcus aureus, gram-negative bacteria, Streptococcus pneumoniae, and alp

Continuous surveillance of influenza was carried out in Pune between 1978 and 1990. Most of the cases were identified during investigation of 16 outbreaks of influenza in Pune over this period. The majority of cases were children. Ten of the outbreaks occurred during rainy seasons. A total of 290 isolates consisting of several antigenic variants of influenza type A (H3N2), type A (H1N1), and type B viruses were isolated from throat/nasal swabs that were processed in chick embryos and MDCK cell culture and identified using the haemagglutination inhibition test. These variants circulated every y

The etiology of acute pneumonia was studied in 596 pediatric inpatients at Chiba Municipal Kaihin Hospital between January 1990 and December 1991. A pathogen was identified in 389 (64.4%) episodes of pneumonia. Evidence of bacterial infection was present in 167 (28.8%) episodes, viral infection in 178 (29.9%) and Mycoplasma pneumoniae infection in 89 (14.9%). The major bacterial pathogens were H. influenzae 117 (19.6%), S. pneumoniae 51 (8.6%), M (B). catarrhalis 24 (4.0%). RS virus was the most common respiratory virus. The peak age of the patients was 7 months to 2 years old. For bacterial p

During the winter of 1989-1990, influenza type A(H3N2) circulated widely, causing excess morbidity and mortality nationwide. From November through April, 1989-1990, hospitalized cases of pneumonia and influenza occurring among noninstitutionalized individuals 65 or more years of age were identified by 20 acute care hospitals in southern lower Michigan. These cases were group matched on age, sex, race, and zip code to randomly sampled, community-based controls from a comprehensive listing of Medicare beneficiaries residing in the study area. Self-reported data were collected from cases and cont

This study, conducted at Goroka Hospital from January 1983 to June 1985, examined the viruses identified in nasopharyngeal aspirates (NPA) and urines collected from 716 hospitalised children with moderate or severe pneumonia, in NPA from 170 children with mild pneumonia treated as outpatients and in NPA from a control group of 428 children attending the outpatient department of Goroka Hospital suffering from minor ailments other than upper or lower respiratory tract infections. One or more viruses were identified from 68%, 51% and 43% of children with moderate or severe pneumonia, mild pneumon

Totals of 58,661,000 acute respiratory disease (ARD) cases, 1,376,651 bronchitis and pneumonia complications, and 93,042 deaths from influenza, bronchitis, pneumonia or chronic pulmonary affection were notified during 11 years of ARD surveillance from 1975 to 1986. All ARD seasons started with the first phase in September-December; this increase in morbidity was caused chiefly by adenoviruses, parainfluenza viruses, rhinoviruses and M. pneumoniae. Second wave of ARD morbidity occurring in January-April used to be explosive and was associated with an influenza epidemic in 9 of the 11 seasons; o

To determine whether respiratory virus infections (URI) are associated with exacerbation of nephrotic syndrome (NS) in childhood, a prospective two-winter study of 32 children with NS was done. We obtained pre- and post-season viral serologic studies, biweekly nose and throat viral cultures, daily urinalysis, biweekly telephone follow-up for URI and renal complaints, and clinical assessments as indicated. When a URI occurred, viral cultures were done weekly if the child was at home and twice weekly if hospitalized. Sixty-one URIs occurred; the agent was identified in 33 (51.6%) (respiratory sy

Data from an 11-year study of acute lower respiratory tract illness were analyzed to provide added insight into the causes and patterns of occurrence of pneumonia in an outpatient pediatric population. Over 80% of all episodes of pneumonia occurred in children less than seven years of age; the peak attack rate was observed in 2-4-year-old children. A virus or Mycoplasma pneumoniae was isolated from 24% of children with pneumonia; 57% of all episodes occurred during outbreaks for which a viral or mycoplasmal cause could usually be identified. Respiratory syncytial virus (RSV), parainfluenza vir

Acute respiratory infections have been found to be a common cause of morbidity in young adults in the tropical setting of the Philippines, with incidence rates equalling those of colder northern countries. Influenza outbreaks have occurred as part of a worldwide pattern and parainfluenza infections are common, as revealed by serological tests on such students. About half of the respiratory infections could be identified etiologically. In contrast, clinical infections from group A haemolytic streptococci, Mycoplasma pneumoniae, adenoviruses, and infections due to Epstein-Barr virus (infectious

Cases of Hong Kong influenza did not appear in Czechoslovakia until the middle of January 1969, although cases had occurred in neighbouring Poland in December 1968.Altogether 426 strains of A2 influenza virus were identified between the 5th and 22nd weeks of 1969 and their biological and antigenic properties were investigated. The strains isolated were highly sensitive to non-specific inhibitors present in horse, guinea-pig and human sera: in human sera, the inhibitors could be removed by means of receptor-destroying enzyme but not by treatment with KIO(4). The haemagglutinin of the new varian
