Preoperative Carriage of Respiratory Viruses and Acute Respiratory Distress Syndrome After Cardiac Surgery: Protocol for the VIRUS-ATTAC Prospective Cohort Study.

BACKGROUND: Cardiac surgery patients are at particular risk of postoperative acute respiratory distress syndrome (ARDS), with an estimated incidence of 5% to 10%. Cardiopulmonary bypass (CPB), ischemia-reperfusion injury, and blood product transfusions are well-recognized contributing factors. Asymptomatic carriage of respiratory viruses has been hypothesized to prime the lungs and potentiate ARDS development when combined with the pulmonary insults of cardiac surgery. However, no study has directly assessed the relationship between preoperative asymptomatic viral carriage and postoperative AR

Safety and Efficacy of Therapeutic Agents in Viral Pneumonia in Immunosuppressed Patients.

INTRODUCTION/OBJECTIVE: Immunosuppressed Patients (ISP) - defined by congenital or acquired deficiencies in systemic defense mechanisms - are disproportionately susceptible to viral respiratory infections. Agents such as SARS-CoV-2, Respiratory Syncytial Virus (RSV), influenza A and B, and Cytomegalovirus (CMV) cause significant morbidity in both immunocompetent and immunosuppressed individuals. Clinical manifestations range from mild influenza-like illness to severe pneumonia, respiratory failure, multiorgan dysfunction, and concurrent bacterial infections. METHODS: A narrative synthesis of

Establishment and preliminary evaluation of an RT-ERA combined with CRISPR/Cas12a assay for detection of H5 and H7 influenza viruses.

BACKGROUND/OBJECTIVES: Novel influenza A viruses (NIAV) infection is a zoonotic acute respiratory disease. The H5N1 and H7N9 subtypes cause high mortality, posing major public health threats. This study aimed to establish rapid, field-deployable subtype-specific nucleic acid detection assays for H5 and H7 by integrating reverse-transcription enzymatic recombinase amplification (RT-ERA) with CRISPR/Cas12a. Two separate reactions are required per sample. METHODS: Conserved hemagglutinin (H) sequences were retrieved from the NCBI database, and primers, probes, and crRNAs were designed using Prim

Lineage dynamics and risk factors underlying serotype 4 invasive pneumococcal disease in Spain.

BACKGROUND: The emergence of vaccine-covered serotypes causing invasive pneumococcal disease (IPD) is a serious concern worldwide. METHODS: We characterized all national serotype 4 IPD isolates from children and adults received at the Spanish Pneumococcal Reference Laboratory (2009-24). RESULTS: We investigated the unexpected rise of serotype 4 causing IPD in young adults after the COVID-19 pandemic, affecting elderly population in recent years. Epidemiological and genomic analysis confirmed that the rise started as an abrupt cluster of cases in Seville (Andalusia) in the year 2022 due to th

Educational background and vaccination beliefs in the Netherlands: A Bayesian Evidence Synthesis from four studies.

BACKGROUND: Although studies repeatedly show that higher educated individuals uphold more positive beliefs about vaccines than their lower educated counterparts, emerging evidence suggests that vaccine skepticism can also be found among highly educated individuals in certain contexts. Given these divergent perspectives on the relation between educational background and vaccine beliefs, the present study investigated (a) the magnitude of educational differences in vaccination attitudes and (b) educational differences in changes in vaccination attitudes over time after the COVID-19 pandemic. Higher SARS-CoV-2 infection burden among racialized individuals: evidence from Canadian serology data.

INTRODUCTION: COVID-19 infection risk has not been evenly distributed across racial groups, with exposure being shaped by social and structural factors. The emergence of highly transmissible variants (i.e., Omicron) dramatically increased infection rates. However, it remains unclear whether racial disparities in infection risk disappeared or persisted over the course of the pandemic. OBJECTIVE: To understand how SARS-CoV-2 infection risk differed between racial groups in Canada and whether those disparities changed with the Omicron variant. METHODS: We analyzed cross-sectional SARS-CoV-2 ser

Hyaluronan Ameliorates Viral Pneumonia in Mice and Humans by Inhibiting Transcription Factor E2F1.

Viral lung infections are a major cause of morbidity and mortality worldwide. Despite significant advances in vaccines and antivirals, there remains a tremendous need for broadly applicable treatments that can be utilized across viral infections. Before infecting epithelial cells, viruses interact with the epithelial glycocalyx, which contains high-molecular weight hyaluronan (HMWHA), a glycosaminoglycan that has beneficial effects in lung injury. In this study, we sought to determine the role of HMWHA in viral pneumonia. We infected mice with influenza or severe acute respiratory syndrome cor

Gaps between willingness and uptake of influenza and COVID-19 vaccines during the 2025-26 respiratory virus season in a U.S. adult cohort.

During the 2025-26 respiratory virus season, changes to COVID-19 vaccine eligibility, recommendations, and communication may have made vaccination follow-through especially challenging. Under-vaccination may reflect not only lack of willingness, but also breakdowns between willingness and uptake. We analyzed data from 3390 adults in the CHASING COVID Cohort who completed assessments in August 2025 and March 2026 to examine gaps between vaccine willingness and subsequent influenza and COVID-19 vaccination. Vaccine willingness was defined using prior-season vaccination and stated intention to va

Self-reported reactogenicity after COVID-19 vaccination with and without influenza vaccine coadministration during pregnancy in Canada.

BACKGROUND: Despite consistent evidence of safety and efficacy of immunization against COVID-19, vaccine hesitancy remains prevalent among pregnant individuals. Reactogenicity (post-vaccination inflammatory symptoms) has been cited as a significant factor in decreasing vaccine acceptance. The seasonal overlap in the vaccination schedule for COVID-19 and influenza provides an opportunity to increase vaccine accessibility and uptake while decreasing healthcare burden through coadministration. METHODS: The Canadian COVID-19 Vaccine Registry for Pregnant Women and Pregnant People (COVERED) is a n

The platform trial in COVID-19 priming and BOOsting (PICOBOO): The reactogenicity of nine licensed COVID-19 vaccines delivered in immunocompetent adults.

BACKGROUND: PICOBOO is a randomised, Bayesian adaptive trial evaluating immunogenicity and reactogenicity of licensed COVID-19 vaccines in immunocompetent individuals aged >12 years. We present reactogenicity data for nine vaccines administered as additional doses within adult strata based on age cohort and primary vaccination regimen. METHODS: Individuals who had not received a COVID-19 vaccine within three months were eligible. Participants were randomised within each stratum to receive a single vaccine dose. Reactogenicity events were recorded on days 1-7 following vaccination. Serious or

Increased risk of pulmonary embolism following SARS-CoV-2 activity in Ontario, Canada.

OBJECTIVES: SARS-CoV-2 infection is an established prothrombotic trigger, yet the population-level temporal relationship between circulating viral activity and pulmonary embolism (PE) remains poorly characterized. We aimed to evaluate the short-term association between respiratory viral activity and PE, accounting for specific temporal lags. STUDY DESIGN: Ecological time-series study. METHODS: We conducted a population-level time-series analysis of incident PE hospitalizations in Ontario, Canada, from 2011 to 2024. Using distributed lag non-linear models, we assessed associations between sta

A Multicenter Clinical Evaluation of Polymerase Chain Reaction Coupled With Quantum Dot Fluorescence Analysis and Quantitative Real-Time Reverse Transcription Polymerase Chain Reaction in the Diagnosis of Pathogens in Patients With Suspected Respiratory Tract Infections.

This multicenter clinical study evaluated 17 common pathogens in 1922 pharyngeal swab samples, comparing the diagnostic performance of polymerase chain reaction coupled with quantum dot fluorescence analysis (PCR-QDFA) with that of clinically routine quantitative real-time reverse transcription polymerase chain reaction (qRT-PCR). The results were validated using Sanger sequencing as the gold standard. Our results showed that among the samples with single-pathogen infections (1037 cases, 53.95%), the three most frequently detected pathogen were influenza A virus (IAV) (296 cases, 15.40%), seve

Respiratory virus dynamics in a tropical region: Insights from Yucatán, México (2018-2024).

The activity of respiratory viruses (RVs) displays large variability in tropical regions, posing challenges for public health response strategies. Data from most RVs in south-eastern Mexico remain limited, particularly in the Yucatan Peninsula, the largest tourism hub in the country. This retrospective study analyses the regional epidemiology of RVs in Merida, the largest city in the region, using laboratory test data from a local hospital (January 2018-April 2024). Test results of 143292 RVs were collected, including 121976 for SARS-CoV-2, 19355 for influenza A and B viruses, and 1961 for 17

Segmented filamentous bacteria reprogramming of alveolar macrophages limits postinfluenza bacterial pneumonia.

Respiratory viral infection induces depletion and dysfunction of alveolar macrophages (AMs), resulting in high-susceptibility to life-threatening bacterial pneumonia. Colonization of the intestine by segmented filamentous bacteria (SFB) reprograms AM to resist depletion. Hence, we examined whether SFB protected mice against secondary bacterial infection by Streptococcus pneumoniae, Haemophilus influenzae, or Staphylococcus aureus after influenza A virus (IAV) infection. SFB protected mice against these infections based on pathogen loads and disease symptoms. AM depletion and transplant indicat

The COVID generation: the neurodevelopmental consequences of in-utero COVID-19 exposure.

BACKGROUND: In historical viral epidemics, such as the H1N1 influenza and Zika viruses, prenatal exposures were correlated with risk for neuropsychiatric conditions in offspring. However, the long-term effects of prenatal COVID-19 viral exposure on offspring neurodevelopment are still being discovered. METHODS: We prospectively recruited mother-baby dyads during the COVID-19 pandemic, who had been exposed to the SARS-CoV-2 virus during pregnancy (2020-2022) into a longitudinal infant brain development study and compared them to a low-risk normative pre-pandemic cohort (2016-2019). Validation of a multiplex qRT-PCR assay for the detection of RSV, influenza A/B virus and SARS-CoV-2.

The significant burden of viral respiratory diseases necessitates rapid detection of key pathogens. Simultaneous testing for SARS-CoV-2, RSV, and influenza A/B as an initial step, followed by broader panels as needed, offers a cost-effective diagnostic strategy. This study aimed to validate a new commercial multiplex qRT-PCR assay (Diagnovital® RTA Laboratories, Turkey) for the simultaneous detection of these viruses.Analytical sensitivity was determined using Probit regression analysis on serial dilutions (10⁵–10¹ copies/ml) for each target virus. Specificity was evaluated with 120 negative s

Tailored Individual Follow-Ups Versus a One-Day Group Course in Patients With Long COVID (Post- COVID-19 Condition): Protocol for a Randomized Controlled Trial.

BACKGROUND: The high prevalence of patients with post-COVID-19 condition, also called long COVID, even among those with mild initial disease, may have a large impact on both the individual and society. Disability in everyday life, reduced health-related quality of life and work capacity, strain on the health care system, and substantial socioeconomic costs are associated with long COVID. More research to investigate the effectiveness of rehabilitation services is warranted. OBJECTIVE: This study aims to examine the effectiveness of tailored individual follow-ups versus a 1-day group course in
