Pathobiology of A/chicken/Hong Kong/220/97 (H5N1) avian influenza virus in seven gallinaceous species.

Direct bird-to-human transmission, with the production of severe respiratory disease and human mortality, is unique to the Hong Kong-origin H5N1 highly pathogenic avian influenza (HPAI) virus, which was originally isolated from a disease outbreak in chickens. The pathobiology of the A/chicken/Hong Kong/220/97 (H5N1) (HK/220) HPAI virus was investigated in chickens, turkeys, Japanese and Bobwhite quail, guinea fowl, pheasants, and partridges, where it produced 75-100% mortality within 10 days. Depression, mucoid diarrhea, and neurologic dysfunction were common clinical manifestations of disease

Influenza A-associated bronchiolitis obliterans organizing pneumonia mimicking Wegener's granulomatosis.

We describe a patient with bronchiolitis obliterans organizing pneumonia (BOOP) requiring respiratory support and treated with corticosteroids and cytoxan for presumed Wegener's granulomatosis (WG). The diagnosis of WG was based on clinical presentation and strongly positive stains for anti-neutrophilic cytoplasmic antibodies (cANCA). The results of an open-lung biopsy were consistent with BOOP. Although BOOP has previously been described as one of the pulmonary manifestations of WG, other more specific histologic features of WG such as capillaritis or necrotizing vasculitis were lacking. [Central hemodynamics and microcirculation in hypertensive patients with influenza or other acute respiratory viral infections].

Integral rheography and conjunctival biomicroscopy studied central hemodynamics (CH) and microcirculation (MC), respectively, in 37 hypertensive patients with influenza and acute respiratory viral infection (ARVI) and 38 normotensive patients with influenza and ARVI aged 41 to 94 years (group 1 and 2, respectively). In group 1 vs group 2 specific peripheral resistance was significantly higher, intravascular changes were more manifest (disseminated aggregation of red cells, slow blood flow with its incomplete block. It is shown that patients with influenza and ARVI had main functional circulato

Effect of experimental influenza A infection on systemic immune and inflammatory parameters in allergic and nonallergic adult subjects.

BACKGROUND: The economic impact and medical complication rate of viral upper respiratory infections are well documented, but many of the physiologic, inflammatory, and immune responses to respiratory viruses have only recently been investigated. A previous study demonstrated differential systemic immune and inflammatory responses in allergic rhinitis (AR) and nonallergic rhinitis (NAR) subjects during experimental infection with rhinovirus-39. OBJECTIVE: The purpose of this study was to compare selected systemic immune and inflammatory responses to experimental influenza A virus (FLU) challen

[Fulminant influenza pneumonia].

Severe community acquired pneumonia is a common cause of acute respiratory failure requiring ventilator support. Bacteria are by far the most common pathogens, necessitating initial coverage with antibacterial agents. When influenza virus is involved in the aetiology of severe pneumonia, its role in the pathogenesis is most often that of facilitating secondary bacterial pneumonia. The influenza virus itself can, however, cause a severe and rapidly evolving viral pneumonia. We describe two cases of fulminant pneumonia with multiorgan failure, in which extensive microbiologic and serologic diagn

Viral pathogens and clinical manifestations associated with acute lower respiratory tract infections in children of the Sudan.

BACKGROUND: Incomplete knowledge regarding the viral agents causing respiratory infections in children living in developing countries impedes diagnosis and management of patients. OBJECTIVES: To assess the role of viral pathogens in Sudanese children presenting with acute lower respiratory tract infections (ALRI). STUDY DESIGN: The study population consisted of patients presenting with ALRI at the Children's Emergency Hospital in Khartoum during 2 periods (December 1987 to April 1988 and September 1990 to March 1991). Identification of viral infections was based an antigen detection by immun

[The pulmonary manifestations in AIDS].

Clinical and paraclinical experience in HIV infection, though the time elapsed since the first observations is relatively short, begins to get typical outlines. In the case of AIDS, the lung is the main place of opportunistic infections, other inflammatory processes and neoplasia. The present work deals with six clinical cases with positive serum tests for HIV and secondary respiratory phenomena such as: Kaposi sarcoma, pneumonia with Pneumocystis carinii, tuberculosis, candidosis, pneumonia with common germs. Particular aspects of treatment and disease evolution are commented.

[Virological surveillance of acute respiratory tract illnesses of children in Morioka, Japan. I. Epidemiological patterns of infection with respiratory viruses over a 10-year period].

A virological surveillance of acute respiratory tract illnesses (RTI) of children in Morioka, Japan, was maintained from September 1973 to December 1983. Nasal and throat swabs were collected from 4,334 children with RTI. These patients consisted of 3,500 children (80.8%) with upper RTI (URTI) and 834 children (16.2%) with lower RTI. When these patients with URTI were classified by maximum temperature recorded into 4 groups (less than or equal to 37.0 degrees C, 37.1-37.9 degrees C, 38.0-38.4 degrees C, and greater than or equal to 38.5 degrees C), the number of patients in each group was 1,90

[The significance of a congenital enterovirus infection in the pathogenesis of transient intrainfectious nephropathy and interstitial nephritis detected in children with influenza-like and acute respiratory diseases].

The paper presents the data confirming the hypothesis on the involvement of the congenital enteroviral infection in etiology of chronic nephropathy manifesting in children in the presence of influenza-like and acute respiratory infections diseases. 100 relevant children were examined. Family history indicated a high risk of enteroviruses inheritance from mothers who were chronic carriers in 19 out of 20 children with pyelonephritis or interstitial nephritis (95%), in 17 out of 23 children with transitory nephropathy (73.9%) and 16 out of 57 patients without nephropathy (28.1%). Influenza A virus potentiates bacteria-induced histamine release. Examination of normal individuals and patients allergic to bacteria.

Influenza A virus was found to enhance basophil histamine release induced by Escherichia coli, Salmonella enteritidis, Staphylococcus aureus, Streptococcus pneumoniae and Streptococcus sanguis, but did not per se release histamine. This potentiating effect of the virus was seen both when the bacteria-induced mediator release was IgE-dependent (i.e. patient allergic to bacterium) and when the bacterium caused histamine release by a non-immunological mechanism independent of IgE (putative sugar-lectin mediated). Also histamine release induced by other immunological and non-immunological stimuli,

A 36 nucleotide deletion mutation in the coding region of the NS1 gene of an influenza A virus RNA segment 8 specifies a temperature-dependent host range phenotype.

Previously a spontaneous 36 nucleotide deletion in the coding region of NS1 was detected in the NS gene of a reassortant virus (CR43-3) recovered from a dual infection by the influenza A/Ann Arbor/6/60 cold-adapted (ca) mutant and wild-type (wt) influenza A/Alaska/6/77 (H3N2). The hemagglutinin, neuraminidase and NS genes were derived from the wild type virus parent while the other 5 genes were derived from the ca parent. The CR43-3 reassortant virus exhibited: (i) a host range (hr) phenotype, i.e. the reassortant replicated efficiently in avian cells in tissue culture but failed to grow in ma

[The 1985 influenza epidemic in a pediatric practice].

Because it is not possible to distinguish clinically influenza from other respiratory infections, virological methods have to be used to establish the influenza etiology. Nasopharyngeal swabs from 202 children with respiratory symptoms were taken. Influenza A virus (H3N2) was isolated from 44 children, influenza A virus (H1N1) from 61 children and influenza B virus from 13 children. The maximal activity of the two influenza A virus subtypes was different. The following features permitted the classification of 3 groups; monophasic fever greater than or equal to 38.5 degrees C (81.35%), biphasic

Clinical manifestations and consequences of influenza.

The spread of influenza virus through a community typically causes large increases in medical visits for febrile respiratory disease. Increased school absenteeism occurs early in the epidemic, and school children appear to be important for disseminating the virus. Industrial absenteeism, hospitalizations of adults and infants for pneumonia, and deaths due to pneumonia-influenza all tend to peak later in the epidemic. Although influenza infection rates are highest in persons of school age, hospitalizations and deaths occur primarily in infants and in the elderly, particularly among those with p

Mycoplasma pneumonia: a study on hospitalized American patients with pneumonia in Vietnam.

A prospective study on consecutively hospitalized pneumonia patients showed that 41.5% of 58 patients had a fourfold rise in the complement-fixation titer for Mycoplasma pneumoniae. Viral isolation techniques and serologic tests for influenza A1, A2 and B, parainfluenza 1 and 3, respiratory syncytial virus and the adeno virus group yielded only a single positive isolate for influenza A2. Serologic tests for melioidosis, leptospirosis, scrub, murine and epidemic typhus and psittacosis were all negative. The clinical manifestations were not distinctive for the positive M. pneumoniae patients whe

An outbreak of severe pneumonia due to respiratory syncytial virus in isolated Arctic populations.

A rapidly developing outbreak of pneumonia in young infants was documented in two isolated Artic populations in May 1972. These were studied virologically, serologically and clinically. In addition to the two stricken communities, one apparently unaffected with serious clinical illness and a fourth, in which are located the major hospital and airport in the eastern Arctic, were also studied. One hundred and twenty-four patients were studied serologically and 81 respiratory and other specimens were obtained for virus isolation from 40 of these patients. Clinical records were kept of the outbrea

[Characteristics of experimental para-influenza in Syrian hamsters infected by different routes].

The pathomorphological and virological study of peculiar features of parainfluenza infection caused by intranasal, aerosol and oral administration of the virus was carried out. The most marked lesions in the respiratory tract were observed in hamsters infected by the intranasal route, whereas with other routes of infection the intensity of lesions was minimal. Such differences in the pathomorphological manifestations of the process are most probably due to uneven distribution of the virus in the respiratory organs and to peculiar features of the immunological response of the animals.
