[Clinical implications of impaired microcirculation and hemodynamics in acute respiratory viral infections and their pharmacological correction].

Microcirculation, hemocoagulation and blood viscosity were studied in 377 of 1033 inpatients with influenza and other acute respiratory viral infections. The new data show that it is feasible to improve the course, outcomes and results of the treatment. Microcirculation disorders persist throughout the disease, but in convalescence, intravascular alterations are leading. The microcirculatory changes manifest themselves with advanced erythrocyte aggregation, activation of vascular-platelet and plasmic links of hemostasis, and high blood viscosity at low shift velocities. In the presence of conc

The pathogenesis and management of influenza virus infection in organ transplant recipients.

Infection with influenza viruses poses specific problems in adult and pediatric organ transplant recipients, including a higher rate of pulmonary and extra-pulmonary complications. Also, data suggest that influenza is associated with acute cellular rejection and chronic allograft dysfunction. The main strategy of influenza prevention has been influenza immunization in order to stimulate local and systemic antibodies. However, studies have shown that antibody response to inactivated influenza vaccine is decreased in all groups of organ transplant recipients. A live attenuated influenza virus va

Correlation between serum interleukin 6 and C-reactive protein concentrations in patients with adenoviral respiratory infection.

OBJECTIVE: To characterize adenoviral respiratory infection, we evaluated clinical features, laboratory findings and serum cytokine concentrations in patients with adenoviral infection and compared them with those in patients with influenza virus and respiratory syncytial virus (RSV) infections. METHODS: We enrolled 106 patients who had been diagnosed with acute viral respiratory infection caused by adeno-, influenza and respiratory syncytial viruses from January, 1995, through December, 1998. Forty-nine patients had adenovirus infection, 19 patients had influenza virus infection and 38 patie

Symposium: Helicobacter pylori and clinical risks--focus on gastro-oesophageal reflux disease.

BACKGROUND: Studies show that endangered work ability (EWA) can be maintained or restored through medical rehabilitation (MR). For patients, general practitioners (GP) represent an important point of access to MR in outpatient care. However, many different barriers and shortcomings hinder GPs in both timely detection of the need for MR and the recognition of its potentials for their EWA-patients. These are necessary if GPs are to adequately inform patients about MR options and successfully support applications for MR. This study describes the evaluation of a continuing medical education (CME)

Pathobiology of A/chicken/Hong Kong/220/97 (H5N1) avian influenza virus in seven gallinaceous species.

Direct bird-to-human transmission, with the production of severe respiratory disease and human mortality, is unique to the Hong Kong-origin H5N1 highly pathogenic avian influenza (HPAI) virus, which was originally isolated from a disease outbreak in chickens. The pathobiology of the A/chicken/Hong Kong/220/97 (H5N1) (HK/220) HPAI virus was investigated in chickens, turkeys, Japanese and Bobwhite quail, guinea fowl, pheasants, and partridges, where it produced 75-100% mortality within 10 days. Depression, mucoid diarrhea, and neurologic dysfunction were common clinical manifestations of disease

Influenza A-associated bronchiolitis obliterans organizing pneumonia mimicking Wegener's granulomatosis.

We describe a patient with bronchiolitis obliterans organizing pneumonia (BOOP) requiring respiratory support and treated with corticosteroids and cytoxan for presumed Wegener's granulomatosis (WG). The diagnosis of WG was based on clinical presentation and strongly positive stains for anti-neutrophilic cytoplasmic antibodies (cANCA). The results of an open-lung biopsy were consistent with BOOP. Although BOOP has previously been described as one of the pulmonary manifestations of WG, other more specific histologic features of WG such as capillaritis or necrotizing vasculitis were lacking. [Central hemodynamics and microcirculation in hypertensive patients with influenza or other acute respiratory viral infections].

Integral rheography and conjunctival biomicroscopy studied central hemodynamics (CH) and microcirculation (MC), respectively, in 37 hypertensive patients with influenza and acute respiratory viral infection (ARVI) and 38 normotensive patients with influenza and ARVI aged 41 to 94 years (group 1 and 2, respectively). In group 1 vs group 2 specific peripheral resistance was significantly higher, intravascular changes were more manifest (disseminated aggregation of red cells, slow blood flow with its incomplete block. It is shown that patients with influenza and ARVI had main functional circulato

Effect of experimental influenza A infection on systemic immune and inflammatory parameters in allergic and nonallergic adult subjects.

BACKGROUND: The economic impact and medical complication rate of viral upper respiratory infections are well documented, but many of the physiologic, inflammatory, and immune responses to respiratory viruses have only recently been investigated. A previous study demonstrated differential systemic immune and inflammatory responses in allergic rhinitis (AR) and nonallergic rhinitis (NAR) subjects during experimental infection with rhinovirus-39. OBJECTIVE: The purpose of this study was to compare selected systemic immune and inflammatory responses to experimental influenza A virus (FLU) challen

[Fulminant influenza pneumonia].

Severe community acquired pneumonia is a common cause of acute respiratory failure requiring ventilator support. Bacteria are by far the most common pathogens, necessitating initial coverage with antibacterial agents. When influenza virus is involved in the aetiology of severe pneumonia, its role in the pathogenesis is most often that of facilitating secondary bacterial pneumonia. The influenza virus itself can, however, cause a severe and rapidly evolving viral pneumonia. We describe two cases of fulminant pneumonia with multiorgan failure, in which extensive microbiologic and serologic diagn

Autoimmune insulitis and diabetes in the absence of antigen-specific contact between T cells and islet beta-cells.

Autoimmune diabetes develops following recognition of organ-specific antigens by T cells. The disease begins with peri-islet infiltration by mononuclear cells, proceeds with insulitis and becomes manifest with destruction of insulin-producing islet beta-cells. T cells are necessary to induce insulitis and diabetes, but it is not clear by what mechanisms they can do so, i. e. whether the T cells need to make antigen-specific contact with the beta-cell or whether other interactions are sufficient to induce beta-cell death. In the present study we have constructed chimeric mice in which the bone

[A case of influenza A virus associated encephalopathy with bilateral thalamic hemorrhage].

A three-year old girl was hospitalized in a semi-conscious state following a febrile convulsion. She did not recover despite treatment and died 16 days after admission. Influenza A virus (H3N2) was detected from a throat swab from the patient, and serum hemagglutinin-inhibiting antibodies to the virus elevated from less than 8 to 256. Brain CT revealed bilateral thalamic hemorrhage and peripheral low density. Subarachnoid hemorrhage was also observed thereafter. Based on clinical manifestations and neuroimaging, this patient was diagnosed as an atypical case of acute necrotizing encephalopathy

[Polypeptides from influenza virus, obtained in the presence of high concentrations of rimantadine].

FPV/Rostock obtained in the presence of remantadine (25 micrograms/ml) is studied. This variant of the virus is characterized by a decreased amount of M1-protein. Its buoyant density in urograffin density gradient is 1.20 gamma/cm3 vs. 1.18 and 1.175 g/cm3 for initial FPV and remantadine-resistant FPV, respectively. Purification and concentration decreases the hemagglutinating activity of the virus 5-10 times more intensely than that of initial and remantadine-resistant FPV. On the other hand, the production of virus-specific proteins and intracellular distribution of hemagglutinin in this vir

[Primary infection with varicella-zoster virus in risk groups].

INTRODUCTION: Chickenpox represents the primary form of Varicella-zoster virus (VZV) infection and appears most commonly in preschool and school children. The clinical course of chickenpox in immunocompetent children is mainly mild and complications are rare (1-5). Adults and immunocompromised patients are considered to be risk groups for development of serious and even life-threatening complications. The most frequent bacterial complications include secondary bacterial skin infections, angina, sinusitis, otitis and bronchopneumonia. Central nervous system complications, visceral dissemination

Antiviral activity of an immunomodulatory lipophilic desmuramyl dipeptide analog.

BCH-527, the lipophilic hydrochloride salt of octadecyl D-alanyl L-glutamine, was evaluated for efficacy against experimentally induced murine cytomegalovirus (MCMV), influenza A (H1N1) (IV-A), and Punta Toro virus (PTV) infections in mice. The compound was administered i.p. every other day for a total of 4 injections commencing 24 h previrus exposure. Doses ranged from 12.5 to 200 mg/kg per injection in the various experiments. The MCMV infection was significantly inhibited in two experiments by doses of 25-200 mg/kg, as manifested by increased numbers of survivors and decreased titers of vir

Nuclear import of microinjected influenza virus ribonucleoproteins.

Influenza virus ribonucleoproteins (vRNPs) devoid of the matrix protein (M1) were isolated and introduced into the cytoplasm of CHO and MDCK cells by microinjection. The injected vRNPs were found to be imported into the nucleus, and the RNA was transcribed. Their uptake into the nucleus was ATP-dependent, inhibited by antibodies to the nuclear pore complex, unaffected by the prior acidification of the vRNPs, and not inhibited by amantadine. The results showed that for productive infection, all the early stages of the viral entry pathway (receptor interaction, endocytosis, acid exposure, and me
