Risk factors and prediction models for bronchiolitis obliterans after severe adenoviral pneumonia.

UNLABELLED: Severe adenoviral pneumonia (SAP) can cause post-infectious bronchiolitis obliterans (PIBO) in children. We aimed to investigate the relevant risk factors for PIBO and develop a predictive nomogram for PIBO in children with SAP. This prospective study analysed the clinical data of hospitalised children with SAP and categorised them into the PIBO and non-PIBO groups. Least absolute shrinkage and selection operator (LASSO) regressions were applied to variables that exhibited significant intergroup differences. Logistic regression was adopted to analyse the risk factors for PIBO. Addi

Burden of primary influenza and respiratory syncytial virus pneumonia in hospitalised adults: insights from a 2-year multi-centre cohort study (2017-2018).

BACKGROUND: Viral community-acquired pneumonia (CAP) is a potentially serious illness, particularly in adult patients with underlying chronic conditions. In addition to the most recent SARS-CoV-2, influenza, and respiratory syncytial virus (RSV) are considered the most relevant causes of viral CAP. AIMS: To describe the clinical features of hospitalised adults admitted for influenza-A/B and RSV pneumonia and analyse, according to aetiology, factors associated with non-invasive ventilation (NIV) failure and in-hospital death (IHD). METHODS: This was a retrospective and multi-centre study of a

Feasibility, safety, and efficacy of iron bioresorbable scaffold in neonates with duct-dependent pulmonary circulation.

INTRODUCTION: Bioresorbable stent has the theoretical advantage in the pediatric age group; however, experience in the literature is very limited. OBJECTIVES: This pilot study sought to evaluate the feasibility, safety, and performance of iron bioresorbable scaffold (IBS Angel™) as short-term palliation in lesions with noncomplex patent ductus arteriosus (PDA). MATERIALS AND METHODS: Nine neonates with duct dependent but dual-source pulmonary blood flow (PBF) were included. Major stent-related complications, in-hospital course, stent patency up to 6 months, and unplanned re-interventions wer

Effective deep learning approaches for predicting COVID-19 outcomes from chest computed tomography volumes.

The rapid evolution of the novel coronavirus disease (COVID-19) pandemic has resulted in an urgent need for effective clinical tools to reduce transmission and manage severe illness. Numerous teams are quickly developing artificial intelligence approaches to these problems, including using deep learning to predict COVID-19 diagnosis and prognosis from chest computed tomography (CT) imaging data. In this work, we assess the value of aggregated chest CT data for COVID-19 prognosis compared to clinical metadata alone. We develop a novel patient-level algorithm to aggregate the chest CT volume int

ECMO Long Haulers: A Distinct Phenotype of COVID-19-Associated ARDS With Implications for Lung Transplant Candidacy.

BACKGROUND: Studies indicate that the recovery from coronavirus disease 2019 (COVID-19)-associated acute respiratory distress syndrome may be slower than other viral pneumonia. There are limited data to guide decisions among patients who need extracorporeal membrane oxygenation (ECMO) support, especially the expected time of recovery and considering lung transplantation (LT). METHODS: This was a retrospective chart review of patients with COVID-19-associated acute respiratory distress syndrome placed on ECMO between March 1, 2020, and September 15, 2021 (n = 20; median age, 44 y; range, 22-62

Severe COVID-19 pneumonia in an intensive care setting and comparisons with historic severe viral pneumonia due to other viruses.

PURPOSE: Severe viral pneumonia is associated with significant morbidity and mortality. Recent COVID-19 pandemic continues to impose significant health burden worldwide, and individual pandemic waves often lead to a large surge in the intensive care unit (ICU) admissions for respiratory support. Comparisons of severe SARS-CoV-2 pneumonia with other seasonal and nonseasonal severe viral infections are rarely studied in an intensive care setting. METHODS: A retrospective cohort study comparing patients admitted to ICU with COVID-19 between March and June 2020 and those with viral pneumonias bet

Multiclass Convolution Neural Network for Classification of COVID-19 CT Images.

In the late December of 2019, a novel coronavirus was discovered in Wuhan, China. In March 2020, WHO announced this epidemic had become a global pandemic and that the novel coronavirus may be mild to most people. However, some people may experience a severe illness that results in hospitalization or maybe death. COVID-19 classification remains challenging due to the ambiguity and similarity with other known respiratory diseases such as SARS, MERS, and other viral pneumonia. The typical symptoms of COVID-19 are fever, cough, chills, shortness of breath, loss of smell and taste, headache, sore t

COVID-19 and corticosteroids: a narrative review.

It has been reported that corticosteroid therapy was effective in the management of severe acute respiratory syndrome (SARS) and the Middle East Respiratory Syndrome (MERS), and recently in coronavirus disease 2019 (COVID-19). Corticosteroids are potent anti-inflammatory drugs that mitigate the risk of acute respiratory distress syndrome (ARDS) in COVID-19 and other viral pneumonia, despite a reduction of viral clearance; corticosteroids inhibit the development of cytokine storm and multi-organ damage. The risk-benefit ratio should be assessed for critical COVID-19 patients. In conclusion, cor

Multi-branch fusion auxiliary learning for the detection of pneumonia from chest X-ray images.

Lung infections caused by bacteria and viruses are infectious and require timely screening and isolation, and different types of pneumonia require different treatment plans. Therefore, finding a rapid and accurate screening method for lung infections is critical. To achieve this goal, we proposed a multi-branch fusion auxiliary learning (MBFAL) method for pneumonia detection from chest X-ray (CXR) images. The MBFAL method was used to perform two tasks through a double-branch network. The first task was to recognize the absence of pneumonia (normal), COVID-19, other viral pneumonia and bacteria

Diffuse Alveolar Hemorrhage Secondary to Human Metapneumovirus Pulmonary Infection.

A 67-year-old man presented with a week of flu-like symptoms, hypoxia, and fever. Respiratory viral panel was positive for human metapneumovirus. Initial chest imaging showed left lower lobe opacification, suggesting a bacterial superimposed on viral pneumonia. Despite antibiotics, the patient became tachycardic and increasingly hypoxic, requiring 40 L high-flow nasal cannula. Repeat imaging demonstrated worsening of a left lower lobe process. Elective bronchoscopy with bronchoalveolar lavage revealed hemorrhage. Subsequent autoimmune, bacterial, and fungal workup was negative. The patient was

Profiles and predictive value of cytokines in children with human metapneumovirus pneumonia.

BACKGROUND: Human metapneumovirus (HMPV) is an important cause of respiratory tract infections in young children. Early innate immune response to HMPV is focused on induction of antiviral interferons (IFNs) and other pro-inflammatory cytokines that are critical for the formation of adaptive immune responses. To evaluate the predictive value of Th1/Th2 cytokines which include IL-2, IL-4, IL-6, IL-10, INF-γ and TNF-α in pneumonia caused by HMPV. METHODS: A retrospective study was performed among 59 pneumonia pediatric patients with HMPV infection and 33 healthy children as the control cohort, w
