Molecular epidemiology of carbapenemase-producing gram-negative bacilli in diabetic foot infections: association with glycemic control.

BACKGROUND: Carbapenem-resistant Gram-negative bacilli are an increasing threat in diabetic foot ulcer (DFU) infections, limiting therapeutic options and worsening outcomes. However, data linking specific carbapenemase genes to clinical and metabolic factors remain limited. OBJECTIVES: To isolate Gram-negative bacilli from deep tissue biopsies of DFUs, determine carbapenem susceptibility by minimum inhibitory concentration (MIC) testing, detect major carbapenemase genes by multiplex qPCR, and evaluate their association with glycemic control and Wagner grade. METHODS: This cross-sectional stu

Research progress on detection technologies for Mycoplasma pneumoniae.

Mycoplasma pneumoniae (M. pneumoniae) is a primary pathogen responsible for community-acquired pneumonia (CAP), particularly prevalent among children and adolescents. The recent global resurgence of infection cases, coupled with the rapid dissemination of macrolide-resistant M. pneumoniae (MRMP), underscores the critical clinical need for rapid and precise diagnostic technologies. This article systematically reviews the evolutionary trajectory of detection technologies, covering the transition from traditional culture and serological testing to modern molecular diagnostic techniques. It critic

Case Report: When muscle edema is not a myositis flare: Legionella pneumonia complicated by rhabdomyolysis and acute kidney injury in long-standing polymyositis.

Legionella pneumophila is an important cause of severe community-acquired pneumonia and can produce extrapulmonary manifestations that obscure the primary diagnosis. Diagnostic error is especially likely in patients with established idiopathic inflammatory myopathy, because fever, myalgia, weakness, creatine kinase elevation and muscle edema may be attributed to autoimmune flare. We report a 70-year-old woman with a 25-year history of polymyositis receiving long-term corticosteroids and azathioprine who developed high fever, severe bilateral thigh pain and progressive proximal weakness after r

Cervical Scrofula Revealing Miliary Tuberculosis and Previously Undiagnosed Advanced HIV: A Case Report.

Cervical tuberculous lymphadenitis, or scrofula, may be the first manifestation of previously undiagnosed advanced HIV, even when concurrent miliary pulmonary tuberculosis produces few or no respiratory symptoms. A 25-year-old male immigrant from Eastern Europe presented with one to two weeks of progressive right cervical and jaw swelling, fever, night sweats, fatigue, and anorexia, without cough, dyspnea, or chest pain. Neck and chest computed tomography demonstrated necrotic bilateral cervical lymphadenopathy, supraclavicular and paratracheal adenopathy, and diffuse miliary pulmonary nodular

Development of a novel diagnostic tool for community-acquired pneumonia based on microbial community characteristics.

BACKGROUND: The lung microbiota plays a key role in pneumonia. A correct diagnosis in the early stage helps treatment and limits severity progression of community-acquired pneumonia (CAP). METHODS: There were 42 patients with CAP and 20 controls. Amplicon bacterial 16S sequencing from bronchoalveolar lavage fluid samples revealed indicator genera by linear discriminant analysis effect size, random forest and ROC analyses. The abundance of bacteria was combined with laboratory inflammatory indexes to construct logistic regression models for CAP diagnosis. The final predictive model with a lowe

Melioidosis in Asia: Hidden burden, epidemiological distribution, vaccine and therapeutic prospects, and public health priorities.

Melioidosis is an under-recognized tropical infectious disease caused by Burkholderia pseudomallei, an environmental pathogen that persists mainly in soil and surface water across tropical and subtropical regions. In Asia, the burden of melioidosis remains poorly defined because many cases are underdiagnosed, underreported, or clinically misclassified as tuberculosis, pneumonia, sepsis, or other bacterial infections. This narrative review examines the epidemiological distribution, transmission pathways, risk factors, vaccine and therapeutic developments, public health challenges, and future co

Invasive Infections Caused by Streptococcus pseudoporcinus: A Two-Case Series and Review of the Literature.

Streptococcus pseudoporcinus (S. pseudoporcinus) is a beta-hemolytic, Gram-positive, catalase-negative, nonmotile coccus that primarily colonizes the female genitourinary tract. Although it has historically been considered a colonizing organism, it has increasingly been associated with invasive infections. Its identification may be challenging because of its phenotypic similarity to other beta-hemolytic streptococci. We report two invasive infections caused by S. pseudoporcinus. The first occurred in a 59-year-old woman with cirrhosis who developed spontaneous bacterial peritonitis in the sett

Dual-mechanism adjuvant catharanthine restores β-lactam efficacy against Staphylococcus aureus infections.

Staphylococcus aureus (S. aureus) remains a major global health threat with high mortality, causing both community-associated and hospital-acquired infections. Consequently, resistance mechanisms, notably β-lactamase production and biofilm formation, are considered promising therapeutic targets. In this study, Catharanthine, a plant-derived alkaloid extracted from Catharanthus roseus (Madagascar periwinkle), is a potent adjuvant that restores the efficacy of β-lactam antibiotics against S. aureus. This study aimed to investigate resistance mechanisms, notably β-lactamase production and biofilm

The mechanism of lentinan in preventing and treating acinetobacter baumannii pneumonia via immunomodulation.

BackgroundAcinetobacter baumannii pneumonia (ABP) presents a clinical challenge due to drug resistance and immune evasion. The specific mechanism by which the natural immunomodulator lentinan (LNT) exerts anti-infective effects remains unclear. This study aimed to investigate whether LNT functions through the PINK1/Parkin-mediated mitophagy pathway.MethodsAn immunosuppressed mouse model of ABP was established, and the mice were treated with LNT alone or in combination with the mitophagy inhibitor mdivi-1. Therapeutic efficacy was evaluated through survival analysis. The bacterial load in lung

Closely related ST11 Klebsiella pneumoniae isolates with transient attenuation and re-emergence of carbapenem resistance during prolonged hospitalization: a case report.

BACKGROUND: Apparent transitions in carbapenem susceptibility among sequential Klebsiella pneumoniae isolates are often interpreted as strain replacement. However, phenotypic variation among closely related isolates may also produce similar phenotypic shifts, particularly in heavily treated patients. CASE PRESENTATION: We report a 91-year-old long-term hospitalized patient from whom three sequential ST11 Klebsiella pneumoniae isolates were recovered from blood and urine over a 3-month period. The initial bloodstream isolate (KPB1) exhibited a hypermucoviscous phenotype, KPC production, and ca

Nocardiosis: death from the ground.

Nocardiosis is a rare but potentially life-threatening infection caused by aerobic, filamentous, gram-positive bacteria of the genus Nocardia found in soil and water. Although traditionally found in immunocompromised patients, up to one-third of cases occur in immunocompetent individuals, contributing to frequent diagnostic delays. Historically fatal before the antibiotic era, outcomes have substantially improved with modern antimicrobial therapy and diagnostic advances. However, nocardiosis remains underrecognized due to its nonspecific presentation and ability to mimic more common diseases.

Narrative Review of the Bacterial Profile Involved in Severe COPD Exacerbations Complicated by Pneumonia in Patients Admitted to the Intensive Care Unit.

Chronic obstructive pulmonary disease (COPD) is a chronic progressive respiratory disease characterized by respiratory symptoms and airflow limitation and is one of the leading causes of morbidity and mortality worldwide. The evolution of the disease is associated with episodes of acute exacerbations (AECOPD), characterized by worsening symptoms and accelerated deterioration of lung function. Acute exacerbations of COPD have different severity, being mild, moderate or severe depending on how aggravated the patients' symptoms are, but they also depend on pulmonary functional and biological chan

PA4933 Mediates Dual Regulation of Oxidative Resistance and Virulence in Pseudomonas aeruginosa Pulmonary Infection.

Pseudomonas aeruginosa is a critical opportunistic pathogen requiring rigorous prevention and control, which triggers severe infections in immunocompromised populations. Bacterial membrane proteins serve as core regulatory molecules coordinating stress tolerance and virulence homeostasis during host colonization. Nevertheless, the physiological functions of numerous structurally homologous yet unannotated membrane proteins remain largely elusive. In this study, we identified a significantly upregulated hypothetical gene, PA4933, from published transcriptomic datasets of P. aeruginosa PA14 infe
