A novel CXCL8(3-72) K11R/G31P-BPI233 hybrid protein modulates neutrophil-driven inflammation and protects against carbapenem-resistant Acinetobacter baumannii infection.

Carbapenem-resistant Gram-negative bacteria (CRGNB) pose a severe global health threat. Carbapenem-resistant Acinetobacter baumannii (CRAB) is one of the most critical threats among CRGNB, and effective non-antibiotic therapeutic strategies are urgently needed. Bactericidal/permeability-increasing protein (BPI)-derived molecules display antibacterial and endotoxin-neutralizing activities against Gram-negative bacteria, whereas CXCL8(3-72) K11R/G31P (G31P), an analog of CXCL8 with the mutations of CXCL8 at K11 and G31 positions, could antagonize ELR-CXC chemokine-mediated neutrophil chemotaxis.

[Liver abscess caused by hypermucoviscous Klebsiella pneumoniae].

Hypervirulent Klebsiella pneumoniae (hvKP) represents an emerging highly virulent subtype, associated with cryptogenic liver abscesses and severe metastatic infections. First described in Asia in the 1980s, its global spread has been progressive, with a recent increase in reported cases in Argentina. It is characterized by a hypermucoviscous phenotype demonstrated by the "string test," capsular serotypes K1 and K2, and specific genetic determinants that confer resistance to phagocytosis and complement-mediated killing. We report the case of a 34-year-old man without major comorbidities who pre

FNR/ArcA-2,3-butanediol regulatory axis enhances glucose uptake and adhE expression in high alcohol-producing Klebsiella pneumoniae.

Previously, we found that Klebsiella pneumoniae is a high-alcohol producer, which contributes to the development of non-alcoholic fatty liver disease (NAFLD). However, the molecular mechanisms underlying its high-alcohol production remain unclear. By comparing biological phenotypes of K. pneumoniae with those of Escherichia coli, we found that the former is better at metabolizing high concentrations of glucose, exhibits faster growth rates, and produces higher levels of alcohol, with the 2,3-butanediol (2,3-BD) pathway being the key determinant underlying these differences. More importantly, s

Viral Respiratory Infections: Upper and Lower Respiratory Tract Infections.

Respiratory infections are a major health concern and are divided into upper and lower respiratory tract infections. Upper respiratory tract infections are mostly viral and self-limiting; billions of cases occur globally each year. Lower respiratory tract infections such as pneumonia and bronchitis can cause significant illness, especially in children, older adults, and individuals with comorbidities or immunocompromise. In 2021, lower respiratory tract infections caused more than 2 million deaths globally, mainly in low- and middle-income countries. Preventive measures include hand hygiene, m

Case report: Maternal engraftment of T-cells in a patient with Severe Combined Immunodeficiency due to DCLRE1C pathogenic variant - diagnostic and treatment challenges.

BACKGROUND: Severe combined immunodeficiency (SCID) is a rare but serious congenital condition caused by genetic defects that severely impair the development and function of T cells, B cells, and natural killer (NK) cells. Without early diagnosis and immune system reconstitution, SCID is usually fatal in infancy. The condition requires prompt recognition and hematopoietic stem cell transplantation (HSCT) to restore immune function. This report describes the first genetically confirmed case of SCID in Latvia - caused by a DCLRE1C gene pathogenic variant and diagnosed prior to the implementation

Genomic characterization of bla KPC transmission via Tn4401a and Tn4401l transposons across Enterobacteriaceae and ESKAPE in Ecuador.

Carbapenem resistance in Enterobacteriaceae poses a major threat to public health because of the limited therapeutic options. The dissemination of the bla KPC gene, which encodes KPC carbapenemases, typically occurs through Tn4401 transposons and plasmids that facilitate its transmission. In this study, we analysed 802 genomes of ESKAPE pathogens and other Enterobacteriaceae from Ecuador, including both clinical and non-clinical isolates, to determine the prevalence and genetic context of bla KPC-2 and bla KPC-3 and their associated mobile genetic elements. Whole-genome analysis revealed sever

Pentasystem embolism associated with transient antiphospholipid antibodies in severe M. pneumoniae pneumonia: a "Dual-Hit" case report and systematic review.

BACKGROUND: Thromboembolism is a critical complication of severe Mycoplasma pneumoniae pneumonia (SMPP) in children, yet its clinical spectrum and immunothrombotic mechanisms remain poorly defined. The "dual-hit" hypothesis-synergy between infection-induced inflammation and transient antiphospholipid antibodies (aPLs)-lacks integrated clinical validation. METHODS: We report a case of extensive multi-system thrombosis in a 6-year-old boy with SMPP. A systematic review of literature (January 2020-December 2025) identified 19 studies comprising 199 pediatric cases. RESULTS: In the index case, t

Blocking Extracellular Cyclophilin A-Mediated Adhesion Protects Against Bacterial Infection.

Bacterial pneumonia remains a leading cause of mortality and morbidity worldwide and is primarily managed through antibiotic therapy. However, the increasing threat of antimicrobial resistance underscores the urgent need for alternative strategies. In this study, we identify extracellular cyclophilin A (eCypA) as a key host factor that promotes the adherence of bacteria such as group A Streptococcus (GAS) and Streptococcus pneumoniae (S. P.). Using single-nucleus RNA sequencing of mouse lung tissue, we demonstrate that eCypA upregulates the expression of integrin/extracellular matrix adhesion

HIV-1 Tat protects macrophages from killing by the exoenzyme U from Pseudomonas aeruginosa.

UNLABELLED: People living with HIV (PLWH) have a higher risk of developing other diseases, such as bacterial pneumonia. Pseudomonas aeruginosa is a common cause of pneumonia in humans. The most virulent strains produce ExoU, which is a key effector of P. aeruginosa cytotoxicity. Upon injection into the target cell cytosol by the type III secretion system (T3SS), ExoU binds to PI(4,5)P2 on the inner leaflet of the plasma membrane. ExoU phospholipase activity then induces a loss of plasma membrane integrity, rapidly leading to cell death. HIV-1-Tat is secreted by HIV-infected cells, leading to n

Clinical utility of CT-based deep learning for differentiating pulmonary aspergillosis from bacterial pneumonia: a multicenter retrospective study.

BACKGROUND: Early differentiation of pulmonary aspergillosis (PA) from bacterial pneumonia (BP) remains challenging in respiratory practice because of overlapping clinical and computed tomography (CT) features. Conventional microbiological and serological tests may be invasive, delayed, or insufficiently sensitive in selected patients. This study aimed to develop and externally validate CT-based deep learning (DL) models as adjunctive tools for differentiating PA from BP. METHODS: This multicenter retrospective study included patients diagnosed with PA or BP at The First Affiliated Hospital o

Microbe Profile: Acinetobacter baumannii: persist and resist.

Acinetobacter baumannii is a Gram-negative opportunistic pathogen associated with a wide variety of healthcare-associated infections, including ventilator-associated pneumonia, urinary tract infections, bacteraemia and wound infections. It adopts a 'persist and resist' pathogenic strategy to establish infection in the host. It can form robust biofilms and survive in a desiccated state for months at a time, driving its persistence in the hospital-built environment. It also has a remarkable capacity to rapidly develop antimicrobial resistance, with >90% of contemporary clinical isolates being mu

Incidence and Risk Factors of Post-Colonoscopy Infections in Liver Transplant Candidates: A Retrospective Cohort Study.

BACKGROUND AND STUDY AIMS: Liver transplant candidates, especially those with cirrhosis are more prone to infections, potentially precipitated by semi-invasive procedures like colonoscopy. Limited data exist on infection rates post-colonoscopy and risk factors in this population. We aimed to estimate the incidence of post-colonoscopy infections and explore potential associated risk factors in patients awaiting liver transplantation. PATIENTS AND METHODS: This retrospective Belgian single-centre cohort study included 141 adult liver transplant candidates undergoing screening colonoscopy (2019-

Integrative multi-omics reveals CUL4B as a modulator of macrophage pyroptosis and innate defense against Staphylococcus aureus.

Staphylococcus aureus (SA) causes a broad range of invasive human diseases, increasingly complicated by antibiotic resistance. Macrophages (MΦ) play a central role in innate immunity, yet key aspects of their interaction with SA remain incompletely understood because of the pathogen's diverse immune evasion strategies. Using multi-omics and systems-level analytics, we identified the E3 ubiquitin ligase cullin 4B (CUL4B) as a host factor that modulates MΦ responses to SA infection. CRISPR-mediated deletion of CUL4B in human THP-1 MΦ enhanced intracellular bacterial killing, reduced cell death,

Inhibition of Endothelial Phosphodiesterase 2A Attenuates Post-infarct Inflammation.

After acute myocardial infarction (AMI), endothelial barrier dysfunction facilitates cardiac and sometimes remote pulmonary immune cell infiltration. Unraveling molecular pathways can provide targets for anti-inflammatory therapies. It is known that during bacterial pneumonia, tumor necrosis factor alpha (TNF-α)-driven endothelial up-regulation of the dual phosphodiesterase (PDE) 2A, reducing cyclic nucleotide levels, fosters lung hyperpermeability. We studied whether PDE2A also participates in endothelial hyperpermeability after AMI. In mice, 2 days after experimental AMI, cardiac and pulmona
