qacA is a key factor in heteroresistance to vancomycin in sequence type 5 methicillin-resistant Staphylococcus aureus isolates from pneumonia patients.

UNLABELLED: The presence of heterogeneous vancomycin-intermediate Staphylococcus aureus (hVISA) among methicillin-resistant Staphylococcus aureus (MRSA) strains has been associated with vancomycin treatment failure, especially in cases of MRSA pneumonia. This study aimed to analyze the molecular characteristics and risk factors for hVISA among MRSA strains isolated from pneumonia patients and to validate the most important risk factors. All the MRSA clinical isolates were collected in a multicenter, prospective, observational clinical trial from July 2012 to June 2020. The hVISA strains were v

Stiffness-Tunable Phenolic Nanocapsules Loaded with J-Aggregates for Near-Infrared II Imaging-Guided Phototherapy of Bacterial Infections.

Near-infrared II (NIR-II) imaging-guided photothermal therapy (PTT) represents a promising noninvasive strategy for treating bacterial infections. However, its efficacy is often limited by poor agent accumulation at infection sites and insufficient penetration into biofilms. Herein, we developed stiffness-tunable phenolic nanocapsules (NCs) loaded with NIR-II J-aggregates for enhanced biofilm phototherapy. Specifically, the NIR-II photothermal molecule of BTPTIC was synthesized and assembled with 8-arm-PEG-OH to form J-aggregates (BTPTIC@PEG J-aggregates). The BTPTIC@PEG J-aggregates were used

Rethinking pneumonia in terms of the lung microbiome.

PURPOSE OF REVIEW: This review aggregates, analyzes, and summarizes the current understanding of the lung microbiome as it relates to pneumonia. We will review the composition and function of a healthy lung microbiome and conceptualize dysbiosis associated with pneumonia. Finally, we discuss how the lung microbiome impacts the diagnosis, prognostication and pathogenesis, and recovery from pneumonia. RECENT FINDINGS: The most tangible benefit of studying the lung microbiome has been the identification of pathogenic organisms in suspected pneumonia; however, as there is a growing body of eviden

Effect of probiotic colonization on oral hygiene and ventilator-associated pneumonia in ICU patients: a triple-blind randomized controlled trial.

BACKGROUND: Mechanically ventilated patients are highly susceptible to poor oral hygiene and ventilator-associated pneumonia, mainly due to pathogenic bacterial colonization in the oral cavity. While antiseptic mouthwashes are commonly used, they may disrupt natural oral microbiota. Probiotics offer a potential alternative by promoting beneficial microbial colonization, enhancing oral hygiene, and reducing the risk of infection. This study aimed to evaluate the effect of probiotic colonization on oral hygiene and the incidence of ventilator-associated pneumonia in mechanically ventilated patie

Intranasal unadjuvanted PspA spikes lung-resident memory immunity after parenteral pneumococcal vaccination.

Intramuscular (IM) prime-boost vaccination with outer membrane vesicle (OMV)-delivered pneumococcal surface protein A (OMV-PspA) formulated with alum is safe but it compromises protection against respiratory Streptococcus pneumoniae (Spn) infections. To enhance pulmonary immunity, we employ a "prime-spike" strategy: IM OMV-PspA/alum prime-boost followed by an intranasal booster with adjuvant-free PspA. The immunization regimen demonstrates a favorable safety profile in mice and provided robust, nearly complete protection against Spn D39 (serotype 2) and A66.1 (serotype 3) strains during both p

Methicillin-resistant Staphylococcus aureus nasal swabs predict need for antibiotic coverage in a trauma population.

BACKGROUND: Methicillin-resistant Staphylococcus aureus (MRSA) nasal swab screening in a general intensive care unit population has been shown to have a high negative predictive value (NPV) and is used to guide antibiotic stewardship. Trauma populations may be more susceptible to hospital-acquired pneumonia. The purpose of this study is to assess the utility of the MRSA nasal swab in predicting MRSA pneumonia in a trauma population. We hypothesize that the NPV of MRSA nasal swabs in the trauma population will be sufficient to rule out MRSA ventilator-associated pneumonia and therefore withhold

The therapeutic potential of high-dose inhaled nitric oxide for antimicrobial effects: a narrative review and future directions.

Inhaled nitric oxide (iNO), long used as a selective pulmonary vasodilator, has demonstrated potential antimicrobial and antiviral properties when administered at high concentrations (> 20 parts per million, ppm). While definitive evidence is still lacking, this narrative review synthesizes the emerging clinical and mechanistic properties supporting high-dose iNO as a potential therapeutic strategy for lower respiratory tract infections, including drug-resistant bacterial pneumonias, COVID-19, nontuberculous mycobacteria, and bronchiolitis. We summarize safety data from laboratory studies, Pha

Diagnostic accuracy of chest radiograph versus computed tomography for suspected pneumonia in obese patients.

BACKGROUND: Obesity poses unique diagnostic challenges in medical imaging due to technical limitations that impair image quality. In emergency and urgent care settings, chest radiography (CXR) remains the first-line investigation for suspected pneumonia despite its reduced sensitivity in obese patients. This study aimed to compare the diagnostic yield of CXR and chest computed tomography (CT) in obese and non-obese patients with suspected pneumonia and to assess the impact of CT findings on antimicrobial management. METHODS: A retrospective chart review was conducted at a community-based teach

Semi-supervised multi-class pneumonia classification using a CNN-cascade forest framework.

Medical imaging diagnosis of pneumonia must be accurate at distinguishing between the various disease subtypes but must be resistant to sparse annotated evidence as well as cross-imaging variability. The current deep learning methods are mostly univariate and binary-classification, which limits their clinical use. As a solution to these shortcomings, this paper suggests a semi-supervised CNN-Enhanced Cascade Forest (CE-Cascade) scheme to classify multi-class pneumonia based on X-ray and CT images of the chest. Based on the approach proposed, a convolutional neural network will first be used to

A Clinically Silent Resistance Phenotype That Promotes Acinetobacter baumannii Survival During Colistin Therapy.

Acinetobacter baumannii is a major cause of multidrug-resistant nosocomial infections, particularly ventilator-associated pneumonia, for which therapeutic options are increasingly limited. Colistin, a polymyxin antibiotic, is a drug of last resort for A. baumannii, boasting high susceptibility rates. Yet, despite relatively low rates of breakpoint-defined colistin resistance, clinical outcomes are highly variable, and the bacterial strategies that enable survival during colistin therapy remain poorly understood. Here, we integrate supervised machine-learning-guided genomic prioritization with

Discovery of antimicrobial peptides targeting Acinetobacter baumannii via a pre-trained and fine-tuned few-shot learning-based pipeline.

Acinetobacter baumannii, a robust Gram-negative bacterium known for causing nosocomial infections, exhibiting multidrug resistance, and lacking antimicrobial peptides that target it, remains hard to treat. Here, we report a few-shot learning pipeline integrating classification, ranking, and regression modules. Each module is trained via a few-shot learning strategy involving pre-training and multiple fine-tuning steps, incorporating similar and true data for fine-tuning, to identify potent AMPs against Acinetobacter baumannii. This pipeline effectively scans complete libraries of hexapeptides,

A Simple Feeding Efficiency Test as a Screening Tool for Oral Function in Older Adults: A Cross-Sectional Study.

Background Aspiration pneumonia is a major public health concern among older adults in aging societies. Although various oral function tests are available, most assess only a single aspect of oral function and require specialized equipment. Therefore, a simple and comprehensive screening indicator applicable in routine clinical practice is needed. This study aimed to evaluate the potential usefulness of a newly developed feeding efficiency (FE) test as a screening indicator of oral function in older adults. Methods This cross-sectional study included older adults aged ≥65 years who visited a d

The influence of seasonal factors on postoperative bacterial infection in lung transplantation patients: a retrospective cohort study.

BACKGROUND: Lung transplantation is a critical intervention for patients with irreversible, end-stage lung diseases. Postoperative infection is a common complication of lung transplantation, significantly affecting the survival of patients. Thus, a greater understanding of the spectrum of pathogens and the possible factors related to postoperative infections will aid in the prevention and treatment of postoperative infections. This study examined the distribution and drug resistance of pathogens causing surgical site infections in lung transplant patients, as well as seasonal variations. METH
