Antibiotic Choice for Community-Acquired Pneumonia in Children With Medical Complexity.

BACKGROUND: There are no guideline recommendations for antibiotic choice for community-acquired pneumonia (CAP) in children with medical complexity (CMC). We compared broad- vs narrow-spectrum antibiotics in CMC hospitalized with nonsevere CAP. METHODS: This cohort study was conducted using target trial emulation within an academic health system. We included hospitalizations from January 2016 to April 2024 with International Classification of Diseases, 10th Revision (ICD-10) codes for CAP and complex chronic conditions among children aged 3 months to 21 years who received 3 or more days of an

[Recurrent Pneumococcal Infections in an Immunosuppressed Patient With Fatal Outcome].

BACKGROUND: Neoplasms greatly increase the risk of infection-the main causes of death are pneumonia and influenza infection. Streptococcus pneumoniae, the most common pathogen causing community-acquired pneumonia, is of great significance in patients suffering from hematological neoplasms, which have a 30 times higher incidence of invasive pneumococcal disease than the general population. Vaccination plays a key role in prevention. The Standing Committee on Vaccination (STIKO) recommends vaccination for adults over 60 and immunosuppressed adults. Only about 20% of these individuals are vaccina

Neutrophil Control of Immune Cell Recruitment and Acute Lung Injury in SARS-CoV-2-infected Mice.

Neutrophils are postulated to contribute to the severity of acute lung injury induced by SARS-CoV-2 infection. Neutrophils play both protective and damaging roles in bacterial and viral pneumonia. The present study examined the hypotheses that neutrophils play multiple roles in the pathophysiology of SARS-CoV-2 infection and that granule exocytosis is a contributor to immune cell recruitment and lung injury. Neutrophils were depleted beginning one day prior to, or one day after, infection of 8-10 month old C57BL/6 mice with a mouse-adapted SARS-CoV-2. In separate experiments neutrophil exocyto

Early diagnosis of Mycoplasma pneumoniae pneumonia: Development and validation of a predictive model based on clinical examination indicators.

BACKGROUND: Mycoplasma pneumoniae accounts for a significant proportion of pathogens responsible for community-acquired pneumonia (CAP). Identifying Mycoplasma pneumoniae pneumonia (MPP) is challenging but essential for guiding early and appropriate antibiotic use. METHODS: We developed a predictive model using data from hospitalized CAP patients diagnosed with either MPP or bacterial pneumonia between January and December 2023. Logistic regression and receiver operating characteristic (ROC) curve analyses were performed to explore associations between clinical features (age, symptoms, labora

Source-specific priming of lung NK cells by Pseudomonas LPS enhances resistance to bacterial pneumonia.

Bacterial pneumonia is a major cause of morbidity and mortality in elderly individuals, yet how mucosal innate immunity changes with age remains unclear. We detected bacterial DNA, particularly signatures of Pseudomonas, in the circulation of healthy elderly individuals, suggesting that chronic exposure to low-dose microbial components occurs during aging. Using murine models, we found that intratracheal delivery of a low dose of Pseudomonas aeruginosa lipopolysaccharide (LPS) strongly protected against bacterial pneumonia through a microbiota-dependent mechanism, whereas Escherichia coli LPS

Acinetobacter baumannii Nosocomial Meningitis Secondary to Neurosurgical Intervention: A Case Report.

Nosocomial meningitis is a serious complication of neurosurgical procedures, associated with high morbidity and mortality, as well as significant diagnostic and therapeutic challenges. Among the causative pathogens, Acinetobacter baumannii (A. baumannii) is particularly concerning due to its ability to survive for prolonged periods in the hospital environment, its opportunistic virulence, and, especially, its tendency to develop multidrug resistance to antimicrobial agents. Diagnosis is often difficult in the postoperative setting, particularly when the cerebrospinal fluid (CSF) is altered as

Evaluation of a catch-up strategy for the vaccination in patients with hepatitis C virus.

Vaccination in patients with advanced chronic liver disease (ACLD) is an essential part of their comprehensive healthcare. These individuals may have impaired phagocytic function and diminished production of opsonizing antibodies, resulting in increased susceptibility to bacterial infections, particularly pneumococcal pneumonia. Similarly, there is an increased risk of fulminant hepatitis due to hepatitis A and B viruses. The Ministry of Health updated specific vaccination recommendations for this group in 2018.

Plasmid-mediated regulation of chromosomal gene expression and virulence in Chlamydia psittaci.

UNLABELLED: Chlamydia psittaci (Cps) causes psittacosis, a zoonotic disease characterized by severe pneumonia and systemic infection, yet its pathogenic mechanisms remain poorly understood. While plasmids are established virulence determinants in C. trachomatis (Ct) and Chlamydia muridarum (Cm), their role in Cps pathogenesis is unknown. Here, we generated a panel of Cps transformants with individual deletions of plasmid genes (pgp1-pgp8) and characterized in vitro and in vivo. Genetic analysis revealed that Pgp1 and Pgp2 are required for plasmid maintenance, whereas Pgp3-Pgp8 are dispensable.

Bortezomib exhibited bacteriostatic activity and pro-inflammatory effect against Mycoplasma pneumoniae.

Mycoplasma pneumoniae (MP) is an important respiratory pathogen. Growth inhibition and regulation of inflammation in lung are key to the treatment of MP infections. Bortezomib, a proteinase inhibitor, was found to possess both functions in this study. Firstly, bortezomib showed dose-dependent bacteriostatic activity against MP in culture medium. Decreases in glucose catabolism (changes of pH) and colony-forming units (CFUs) were detected at 3 d post-incubation (bortezomib>10 nM). The MP cells incubated with bortezomib of 25 nM exhibited decreases in contents of protein and carbohydrate compari

Extracellular Vesicle-Bound Bacterial Toxin Pneumolysin Triggers Membrane Engagement and Damage Beyond Canonical Pore Formation.

Pneumolysin (PLY) is a cholesterol-dependent pore-forming toxin and a key virulence factor of Streptococcus pneumoniae, the leading cause of pneumonia worldwide in children aged below 5 years. At sublytic toxin doses, host cells shed PLY-laden extracellular vesicles (EVs) during the membrane repair response. However, it remains unclear how these toxin-bearing EVs engage and damage target cell membranes. Here, we combine molecular dynamics simulations, liposome fusion assays and cell-based experiments to elucidate the membrane interaction potential of vesicle-bound PLY. Simulations indicate tha

Infectious Diseases Society of America (IDSA) Position Statement: Why IDSA Did Not Endorse the Community-Acquired Pneumonia Guidelines 2025 Update.

The American Thoracic Society recently released updated community-acquired pneumonia (CAP) guidelines. The Infectious Diseases Society of America (IDSA) agreed with 8 of the 10 recommendations in the guidelines but declined to endorse the guidelines because they include recommendations for use of antibiotics in outpatients with comorbidities and inpatients with nonsevere CAP who test positive for respiratory viruses. It is noted in the guidelines that bacterial coinfections are common and that delaying antibiotics may be harmful. IDSA notes, however, that nondiscriminatory use of antibiotics f
