Extracellular Vesicle-Bound Bacterial Toxin Pneumolysin Triggers Membrane Engagement and Damage Beyond Canonical Pore Formation.

Pneumolysin (PLY) is a cholesterol-dependent pore-forming toxin and a key virulence factor of Streptococcus pneumoniae, the leading cause of pneumonia worldwide in children aged below 5 years. At sublytic toxin doses, host cells shed PLY-laden extracellular vesicles (EVs) during the membrane repair response. However, it remains unclear how these toxin-bearing EVs engage and damage target cell membranes. Here, we combine molecular dynamics simulations, liposome fusion assays and cell-based experiments to elucidate the membrane interaction potential of vesicle-bound PLY. Simulations indicate tha

Protective efficacy of bivalent anti-flagellin IgY against Pseudomonas aeruginosa infection in acute pneumonia and burn wound murine models.

The rising emergence of antimicrobial-resistant Pseudomonas aeruginosa strains necessitates effective therapeutic strategies like antibody-based immunotherapy. Flagellin is crucial in P. aeruginosa infection development. This study evaluated the antibacterial efficacy of bivalent immunoglobulin Y (IgY) raised against both A and B flagellins. IgY's immunoreactivity and specificity were examined via ELISA and immunoblot analysis. Functional assays, including motility, biofilm formation, and opsonophagocytic tests, examined the antibody's inhibitory effects on diverse bacterial functions. Murine

Pneumonia in Ghana: prevalence, mortality and antimicrobial resistance-a systematic review and meta-analysis.

Pneumonia is a major cause of morbidity and mortality in Ghana, especially among children <5 y of age and the elderly. However, comprehensive data on its prevalence and distribution remain limited. This systematic review addresses this gap by analysing the prevalence, regional variations, risk factors, antimicrobial resistance and mortality rates of pneumonia in Ghana. We searched PubMed, ScienceDirect, Web of Science and African Journals Online databases, including 21 studies with a total of 124 582 participants. A random-effects meta-analysis estimated pooled prevalence and subgroup differen

Evaluation of the timing of switch from intravenous to oral antibiotic therapy in community-acquired pneumonia during hospital admission: cohort study.

INTRODUCTION: Clinical guidelines recommend switching from intravenous to oral antibiotics in community-acquired pneumonia (CAP) once patients are clinically stable because it is safe and cost-effective. However, studies have reported low switching rates in clinical practice. This study aimed to assess current switch practices in the real-world setting. METHODS: The switch from intravenous to oral antibiotic therapy was evaluated in adults (≥18 years) admitted to hospital with CAP. The primary outcome was a switch within 72 hours (early switch). Secondary outcomes included duration of hospita

Infectious Diseases Society of America (IDSA) Position Statement: Why IDSA Did Not Endorse the Community-Acquired Pneumonia Guidelines 2025 Update.

The American Thoracic Society recently released updated community-acquired pneumonia (CAP) guidelines. The Infectious Diseases Society of America (IDSA) agreed with 8 of the 10 recommendations in the guidelines but declined to endorse the guidelines because they include recommendations for use of antibiotics in outpatients with comorbidities and inpatients with nonsevere CAP who test positive for respiratory viruses. It is noted in the guidelines that bacterial coinfections are common and that delaying antibiotics may be harmful. IDSA notes, however, that nondiscriminatory use of antibiotics f

We Dissent: Lessons From the 2025 Community-Acquired Pneumonia Guidelines.

Clinical practice guidelines profoundly influence patient care, making transparency, rigor, and fairness in their development essential. The 2025 community-acquired pneumonia (CAP) guideline update, developed by the American Thoracic Society (ATS) and initially co-sponsored by the Infectious Diseases Society of America (IDSA), included a recommendation to prescribe antibacterials for CAP upon detection of a respiratory viral pathogen. Here, we acknowledge the guidelines' strengths with respect to supporting shorter treatment duration but highlight the lack of supporting data for antibacterial

Preventing resistance development in infections by OXA β-lactamase-producing Pseudomonas aeruginosa: correlating clinical outcomes with hollow-fibre model input.

BACKGROUND: Infections caused by XDR Pseudomonas aeruginosa significantly limit treatment options. Although ceftolozane/tazobactam and ceftazidime/avibactam have emerged as promising alternatives, increasing resistance has been reported. This study describes three critically ill patients with ventilator-associated pneumonia caused by OXA-producing XDR P. aeruginosa that developed resistance to both agents during therapy. METHODS: Antibiotic exposure and resistance were monitored clinically and in a hollow-fibre infection model (HFIM) to evaluate different ceftazidime/avibactam and ceftolozane

Cotrimoxazole in the intensive care unit: an impossible task for the fixed association in real life?

BACKGROUND: Cotrimoxazole (trimethoprim/sulfamethoxazole) is a widely used antibiotic in ICU for the treatment of infections such as nosocomial or Pneumocystis jirovecii pneumonia. However, dosing in ICU patients is complicated by fluctuations in renal function, particularly acute kidney injury and augmented renal clearance, which affect drug elimination. This study investigated how renal function and body weight influence trimethoprim and sulfamethoxazole concentrations. PATIENTS AND METHODS: This retrospective bicentre observational study involved 116 ICU patients receiving cotrimoxazole th

Lack of S100A8 Impairs Lung-Protective Immunity Against Streptococcus pneumoniae.

BACKGROUND: We recently showed that S100A9 is indispensable for lung antibacterial immunity, making it crucial for the survival of pneumococcal pneumonia. However, the role of S100A8 in lung antibacterial immunity is ill-defined. METHODS: S100A8 levels in the bronchoalveolar lavage fluid (BALF) of patients with pneumonia were quantified by enzyme-linked immunosorbent assay. Wild type and S100A8 knockout mice were orotracheally infected with Streptococcus pneumoniae, and bacterial clearance, disease progression, lung histopathology, and leukocyte recruitment were analyzed at defined time point

Semi-Quantitative Detection of Respiratory Pathogens: A Systematic Review and Meta-Analysis of Results From the BIOFIRE FILMARRAY Pneumonia Panel and Culture.

This systematic review and meta-analysis compared bacterial semi-quantification of respiratory samples from the BIOFIRE FILMARRAY Pneumonia (PN) Panels with quantitative and semi-quantitative culture methods (qCMs). Fourteen studies comprising 1,654 samples were included. Across both bronchoalveolar lavage-like and endotracheal aspirate-like specimens, the BIOFIRE PN Panel reported consistently higher bacterial loads than qCMs, with pooled mean differences of 1.17 and 0.95 log, respectively. Discrepancies decreased as culture-reported bacterial burden increased. The concordance rate in identif

How do immunometabolites shape bacterial infections?

Metabolites generated by host and pathogen have a major impact on the severity and outcomes of infection. The metabolic response to infection shapes the nature and intensity of the immune response, both in bloodstream infections and, especially, in the pathogenesis of pneumonia. Some metabolites are closely linked to pro-inflammatory responses, whereas others act as immunomodulators in mitigating damage to the host, a common consequence of inflammation. Immunometabolites are also major factors in driving bacterial adaptation to the host, enabling pathogens acquired from environmental sources t
