Comparison of Short- vs Long-Course Antibiotic Therapy for Children With Tracheitis Associated With an Artificial Airway.

OBJECTIVE: Limiting antibiotic days for treatment of tracheitis to the shortest, most effective duration is an important antimicrobial stewardship endeavor. The objective was to compare outcomes between patients who received a short course of antibiotics to those receiving a longer course. METHODS: This is a retrospective, cohort evaluation of patients admitted to the pediatric intensive care unit with an artificial airway and prescribed a course of antibiotics for at least 3 days for tracheitis. We compared the rate of re-treatment of tracheitis or development of new pneumonia requiring anti

GTS-21 Alleviates Acute Lung Injury by Enhancing GLP-1 Secretion and Regulating Alveolar Surfactant Proteins via α7nAChR Activation.

GTS-21, a selective α7-nicotinic acetylcholine receptor (α7nAChR) agonist, exhibits significant anti-inflammatory effects in acute lung injury (ALI), but its mechanisms remain incompletely understood. We hypothesize that its protective effects involve the regulation of Glucagon-like peptide-1 (GLP-1) and pulmonary surfactant protein (SP). P. aeruginosa-induced pneumonia (2.5 × 10⁵ CFU/mouse) and LPS-induced ALI (2.5 mg/kg) models via intratracheal instillation (IT) were used in WT, α7nAChR KO and GLP-1R KO mice. Lung injury was assessed via histopathology, cytokine quantification (IL-6, HMGB1)

Proteome-Mining and Chemical Activation of Hidden Bioactive Fragments as Antimicrobial Assemblies.

Proteolytic processing of precursor proteins liberates bioactive fragments, yet the systematic discovery of such peptides remains challenging. Here, we establish a fragment-mining strategy that combines in silico prediction with chemical tailoring to generate self-assembling antimicrobial peptides directly from protein sequences, including previously unannotated proteins. Self-assembly proved integral to activity: amphiphilic modification promoted nanonet formation and efficient bacterial membrane disruption, while rational glycosylation reprogrammed physicochemical properties to confer select

Perioperative selective decontamination of the digestive tract does not improve postoperative infectious complications after gastrectomy: a propensity score-matched analysis.

PURPOSE: Infectious complications remain a significant cause of morbidity after gastrectomy for gastric cancer. Selective decontamination of the digestive tract (SDD) is effective in colorectal surgery, but its role in upper gastrointestinal procedures is unclear. METHODS: We conducted a retrospective cohort study of patients undergoing open gastrectomy between 2014 and 2024. Patients treated with perioperative SDD (2018–2024) were compared with a matched historical cohort without SDD (2014–2018) using 1:1 propensity score matching (PSM). The primary endpoint was infectious complications. RESU

In vitro and in vivo antimicrobial activity of protoberberine alkaloids as novel therapeutic candidates against Mycoplasma hyopneumoniae.

UNLABELLED: Mycoplasma hyopneumoniae (M. hyopneumoniae, Mhp), the primary causative agent of swine enzootic pneumonia, poses significant threats to the pork industry, challenging food safety and sustainable development of the livestock industry. Here, four protoberberine alkaloids-epiberberine, jatrorrhizine, berberine, and coptisine-were identified by screening multiple natural compounds, and their anti-Mhp activity was evaluated. All alkaloids exhibited potent inhibitory effects against the virulent Mhp strain ES-2, with minimum inhibitory concentrations from 8 to 32 µg/mL. Their bactericida

A review on hypervirulent and multidrug-resistant Klebsiella pneumoniae: An emerging threat to human health.

Klebsiella pneumoniae has a long history of causing infections as an opportunistic pathogen. There is an upsurge in infections caused by K. pneumoniae, mainly due to its two pathotypes, classical K. pneumoniae (cKP) and hypervirulent K. pneumoniae (hvKP). Various infections associated with these pathotypes include pneumonia, urinary tract infections, bloodstream infections, and pyogenic liver abscesses. This review provides an update on the pathogenicity, virulence factors, mechanisms of virulence, emergence and dissemination of multidrug resistance, as well as the convergence of virulence and

Blood eosinopenia (≤30/µL) as an early predictor of respiratory failure in community-acquired pneumonia: A prospective multicentre study.

INTRODUCTION AND OBJECTIVES: Lower blood eosinophil counts have been associated with increased mechanical ventilation rates in patients with community-acquired pneumonia (CAP). However, the optimal eosinophil count threshold for identifying CAP patients at high risk of respiratory failure remains undefined. This study aimed to establish an optimal admission eosinophil count as a prognostic biomarker for respiratory failure in CAP. METHODS: This prospective, multicentre cohort study (PROGRESS) enrolled adult patients (≥18 years) hospitalised with community-acquired pneumonia (CAP). A receiver

OT-CATH, a conserved antimicrobial peptide in Otariidae species, exhibits potent activity against gram-negative bacteria.

The rapid emergence of multidrug-resistant gram-negative bacteria necessitates the development of effective antimicrobial agents to combat antimicrobial resistance. Here, we identified and characterized OT-CATH, a conserved cathelicidin-family antimicrobial peptide derived from Otariidae species. OT-CATH exhibited potent bactericidal activity against gram-negative bacteria, particularly extended-spectrum β-lactamase-producing Escherichia coli, with rapid killing kinetics, potent antibiofilm activity, and no detectable resistance after 40 serial passages. Mechanistically, OT-CATH bound lipopoly

Timing of corticosteroid initiation and severe in-hospital outcomes in pediatric Mycoplasma pneumoniae pneumonia: a propensity score-matched cohort study.

BACKGROUND: Among hospitalized children with Mycoplasma pneumoniae pneumonia (MPP) who received systemic corticosteroids, the association between treatment timing and subsequent in-hospital outcomes remains uncertain. We evaluated whether earlier initiation was associated with a more favorable hospital course. METHODS: We conducted a retrospective propensity score-matched cohort study at a tertiary pediatric center in China between January 2023 and August 2025. Children aged 1-18 years with confirmed MPP who received systemic corticosteroids 3-14 days after symptom onset were included. Sympto
