Comparison of baloxavir/marboxil treatment and treatment with other anti-influenza agents in adult patients hospitalized for influenza: An internet-based, prospective, multicenter, cohort study in Japan.

INTRODUCTION: Influenza remains a huge clinical issue, but new anti-influenza agents, such as baloxavir/marboxil (BXM), have recently become available in Japan. The objective of this study was to clarify the clinical differences of hospitalized patients treated by BXM and other anti-influenza agents (non-BXM), such as neuraminidase inhibitors (NAIs). METHODS: The clinical data of patients who were severely ill with influenza and required hospitalization were gathered and analyzed between September 2019 and August 2024 (6 influenza seasons) using an internet surveillance system. From them, pat

Candida glabrata (Nakaseomyces glabratus) as a component of aspiration pneumonia in a dog with megaesophagus.

Candida glabrata is a yeast that is a commensal of mucosal surfaces and can cause opportunistic infection in several species. Unlike other Candida species, it is commonly resistant to azoles. Candida pneumonia has been reported in humans, with unclear prevalence, but is very rare in dogs. This report describes an 11-year-old spayed female Dogo Argentino dog with megaesophagus that was managed with mechanical ventilation for aspiration pneumonia. The dog had been treated previously with omeprazole and amoxicillin-clavulanic acid for 2.5 wk. Airway cytology showed inflammation and numerous yeast

Prophylactic Effect of Trimethoprim-sulfamethoxazole on Severe Infections in Idiopathic Inflammatory Myopathy.

Idiopathic inflammatory myopathy (IIM) is commonly treated with glucocorticoids and other immunosuppressants, which substantially increase the risk of opportunistic infections, including Pneumocystis jirovecii pneumonia (PJP). Therefore, trimethoprim-sulfamethoxazole (TMP/SMX) is frequently used as prophylaxis against PJP in patients with IIM. In addition to its role in PJP prevention, TMP/SMX has been reported to reduce the risk of severe infections in other immunosuppressed populations. We evaluated the association between TMP/SMX use and the risk of severe infections other than PJP in IIM.

Pulmonary fungal infections in the age of biologics and climate change.

PURPOSE OF REVIEW: This review examines the evolving epidemiology of invasive fungal infections, with emphasis in pulmonary presentations, in the context of climate change and the expanding immunomodulatory therapy use. RECENT FINDINGS: Fungal infections represent a growing global health threat, with epidemiological patterns increasingly extending beyond traditional immunocompromised populations. Climate-driven thermal adaptation and geographic range expansion of endemic fungi, particularly Coccidioides and Histoplasma species, are exposing immunologically naive populations to infection. The

Antimicrobial use and documented infection among hospitalized adults in South American acute care facilities during the coronavirus disease 2019 (COVID-19) pandemic.

BACKGROUND: Despite low bacterial and fungal infection rates, increased antimicrobial use (AU) among hospitalized patients has been reported during the Coronavirus Disease 2019 (COVID-19) pandemic. We evaluated whether COVID-19 was a driver of AU and documented bacterial or fungal infection. METHODS: We conducted a retrospective cohort study in two hospitals each in Argentina, Brazil, and Chile. We included hospitalized adults with and without COVID-19 admitted during the pandemic (March 2020-February 2021) as well as a cohort admitted prior to the pandemic (March 2019-February 2020) with sim

Association between low-dose trimethoprim-sulfamethoxazole blood concentration and clinical outcomes in non-HIV patients with Pneumocystis jirovecii pneumonia: a retrospective cohort study.

BACKGROUND: Trimethoprim-sulfamethoxazole (SMX-TMP) represents the first-line treatment for non-human immunodeficiency virus (non-HIV) patients with Pneumocystis jirovecii pneumonia (PJP). However, the relationship between SMX peak serum concentrations (Cmax) and clinical outcomes of PJP is not clear yet. This study aim to investigate the impact of initial Cmax of SMX on clinical outcomes non-HIV patients with PJP who were predominantly treated with low-dose SMX-TMP, and to evaluate related adverse drug reactions (ADRs), providing evidence for optimal clinical medication strategies. METHODS: A

Real-world clinical effectiveness of trimethoprim-sulfamethoxazole for primary prophylaxis of pneumocystis pneumonia in non-hodgkin lymphoma patients treated with rituximab.

There are no definitive clinical practice guidelines regarding the necessity and dosage of trimethoprim-sulfamethoxazole (TMP/SMX) prophylaxis for Pneumocystis jirovecii pneumonia (PJP) in individuals undergoing rituximab therapy. This retrospective study evaluated the effectiveness and safety of various TMP-SMX prophylactic dosing regimens over a 1-year period in 690 patients with non-Hodgkin lymphoma treated with rituximab at a university hospital in Thailand from 2013 to 2022. Out of these patients, 622 (90.1%) received TMP/SMX, with a mean duration of prophylaxis of 265.7 days (SD 85.66).

Biologics Use in Eosinophilic Lung Disease: Controversies and Consensus.

Significant advances in the management of diseases driven by eosinophilic inflammation in the age of biologics provide us with an untapped opportunity to extend these highly effective treatments to other lung diseases that have eosinophils as a central effector cell. Currently, rare diseases such as chronic eosinophilic pneumonia and allergic bronchopulmonary aspergillosis (ABPA) are treated with oral corticosteroids (OCS) and often require long courses of exposure to high doses of OCS. In this era of increased awareness of OCS toxicity and a heightened interest in OCS stewardship, we are obli

Genomic Characterization of Candida spp. Highlights a Persistent, Azole-Resistant C. parapsilosis Clone Circulating in a Tertiary Care Hospital During the First COVID-19 Wave.

Yeasts belonging to the Candida genus typically reside on the mucosal surface and within the respiratory and gastrointestinal tract as commensals. Under conditions of host vulnerability, they can act as opportunistic pathogens, leading to various forms of candidiasis, including candidemia. Such infections can be particularly problematic when caused by isolates that exhibit resistance to antifungal drugs, which are becoming more prevalent in many regions. One hundred and seven samples of Candida spp. were isolated from patients with candidemia in the hospital San Matteo in Pavia (Italy) over a

Candidemia in adult patients with alcoholism: a retrospective cohort study in Finland from 2007 to 2016.

BACKGROUND: Candidemia is a life-threatening, typically nosocomial infection, which often occurs with critical illness and immunosuppression. Alcoholism has not been associated with candidemia risk or outcomes. However, alcoholism is a common comorbidity in hospitalised patients, excess alcohol consumption has detrimental effects on the immune system, and alcoholism is associated with poor outcomes from bacteraemia, sepsis and bacterial pneumonia. We aimed to investigate the patient and disease characteristics, treatment, outcomes, and Candida species distribution in patients with alcoholism.

Successful Outcome of Rhino-Orbital-Cerebral Mucormycosis With Extensive Intracranial Disease in a Patient With Poorly Controlled Diabetes.

Rhino-orbital-cerebral mucormycosis is a life-threatening infection typically requiring complete surgical debridement of necrotic tissue for survival. The authors have reported a case of invasive mucormycosis in a 22-year-old female with severe diabetic ketoacidosis and recent COVID-19 infection. The patient developed extensive intracranial disease, including cavernous sinus thrombosis and internal carotid artery occlusion, which precluded complete surgical source control without extensive morbidity. Facing such circumstance, a multidisciplinary team pursued an aggressive, individualized multi

Pulmonary fibroblast activation during Aspergillus fumigatus infection enhances lung defense via immunomodulation and tissue remodeling.

Aspergillus fumigatus is the etiologic agent of invasive aspergillosis, a life-threatening fungal pneumonia initiated by the inhalation of conidia into the lung. If the conidia are not cleared, they secrete large quantities of hydrolytic enzymes and toxins as they grow, causing extensive pulmonary damage. Fibroblasts are central mediators of tissue repair in many organs, but their functional response to pulmonary damage caused by A. fumigatus remains unexplored. Here, we employ cell lineage tracing, targeted cell ablation, and single-cell RNA sequencing to monitor fibroblast dynamics upon expo

Development of a time-dependent prediction model for the risk of Pneumocystis jirovecii pneumonia in ANCA-associated vasculitis.

OBJECTIVES: In patients with ANCA-associated vasculitis (AAV), Pneumocystis jirovecii pneumonia (PJP) is a potentially fatal complication. As treatment intensity typically changes over time, PJP risk also changes, making fixed prophylaxis strategies suboptimal. We aimed to develop a time-dependent prediction model to guide individualized decisions on PJP prophylaxis. METHODS: We conducted a retrospective cohort study using a nationwide claims database. Patients newly diagnosed with AAV between 2015 and 2024 were included. Candidate predictors included demographics, comorbidities and monthly u
