BACKGROUND: Pneumocystis jirovecii pneumonia (PCP) is a significant concern among kidney transplant recipients on immunosuppressants, yet differentiating it from other pneumonias remains challenging.
METHODS: This retrospective study included 101 kidney transplant patients diagnosed with PCP (n = 65) or other pneumonias (n = 36) from January 2019 to April 2021. Receiver operating characteristic (ROC) analysis was performed to evaluate diagnostic efficacy. Nine kidney transplant patients hospitalized with PCP or other pneumonias in matched groups were also analyzed.
RESULTS: 65 PCP patients w

The coronavirus disease (COVID-19) pandemic has been associated with an increased burden of secondary infections, including fungal infections of the lungs, especially in critically ill patients. However, it is unclear whether this excess risk persisted in the post-pandemic period after infection-control practices and clinical pathways were optimized. This single-center retrospective observational study included 1441 PCR positive COVID-19 patients hospitalized during the COVID-19 pandemic and after the pandemic. Microbiological investigations, fungal cultures, serum galactomannan antigen tests,

BACKGROUND: Occasionally, severe coronavirus disease 2019 (COVID-19) can lead to rapidly progressive interstitial lung disease (RP-ILD) by triggering or unmasking various myositis-specific autoantibodies (MSAs). Although dual positivity for anti-aminoacyl-tRNA synthetase (ARS) and anti-melanoma differentiation-associated gene 5 (MDA5) antibodies is extremely rare, its management is exceptionally complex when complicated by secondary infections. Identifying a specific anti-ARS subtype is often difficult, but clinical features such as amyopathic phenotypes and refractory lung involvement often c

BACKGROUND: The relationship between early-life infections and childhood cancer risk remains unclear despite its importance for risk stratification and clinical surveillance. METHODS: We conducted a nationwide population-based cohort study using the South Korean National Health Information Database. Children born between January 1, 2002, and December 31, 2006, were followed through December 31, 2019. Two analytic cohorts were defined: Cohort 1 (n = 1,996,174) evaluated the number of infection episodes during ages 0–4 years, with a restricted analysis of infections occurring between ages 2–4 ye

Mucormycosis, an aggressive fungal infection caused by members of the order Mucorales, progresses rapidly and is associated with high mortality, particularly in immunocompromised hosts such as patients with uncontrolled diabetes, transplant recipients or those with COVID-19-associated immunosuppression. Early diagnosis remains challenging with current clinical methods, yet it is essential to reduce mortality. This review examines the evolution of diagnostic strategies for mucormycosis, ranging from conventional techniques such as histopathology, culture and microscopy to advanced and emerging

The COVID-19 pandemic coincided with rising secondary bloodstream infections (BSIs) from multidrug-resistant organisms, including Candida auris. To assess candidemia trends, we conducted a retrospective analysis of blood culture isolates from public and private laboratories in South Africa taken during January 2019-June 2022. We evaluated weekly aggregated Candida BSI counts and COVID-19 cases using segmented regression within an interrupted time-series framework. In total, 15,393 candidemia cases were identified, 70% from the private sector. C. parapsilosis accounted for 39% of cases, whereas

SUMMARYThe emerging fungal pathogen Candidozyma auris (syn. Candida auris; C. auris) has attracted considerable attention from the scientific, clinical, and public health communities due to its multidrug resistance, environmental persistence, and high transmissibility. Since its first description in Japan in 2009, C. auris has spread rapidly worldwide, with a marked acceleration following the coronavirus disease 2019 (COVID-19) pandemic. As of December 2025, 84,941 colonization or infection cases have been reported across 82 countries spanning 6 continents. In this review, we summarize the cur

BACKGROUND: Alveolar macrophages (AMs) are central to host defense against Pneumocystis, mediating organism clearance while also contributing to lung inflammation. EphA2, a transmembrane receptor that binds fungal β-glucans, has emerged as a regulator of host-pathogen interactions, but its role in AM responses during Pneumocystis pneumonia (PCP) is unknown.
METHODS: We evaluated AM inflammatory responses to Pneumocystis β-glucans in the presence and absence of EphA2. In vivo studies used a CD4-depleted mouse model of PCP comparing wild-type and EphA2-deficient (EphA2-/-) mice. Lung cytokine r

Distinguishing Pneumocystis jirovecii pneumonia (PJP) from colonization (PJC) is crucial due to overlapping symptoms but different treatments. This study aims to evaluate whether peripheral blood parameters can serve as a non-invasive tool for distinguishing PJP from PJC. We retrospectively enrolled 174 patients with PJP and 61 with PJC from the First Affiliated Hospital of Sun Yat-sen University (April 2022-March 2024). Peripheral blood parameters were analyzed and compared between groups. Normally distributed variables were assessed using Student's t-test, while non-parametric data were anal

BACKGROUND/AIM: Pneumocystis jirovecii pneumonia (PCP) remains a life-threatening opportunistic infection in patients receiving chemotherapy and other immunosuppressive cancer treatments. Accurate identification of true PCP cases within real-world electronic health record (EHR) databases is essential for epidemiological research and optimization of prophylactic strategies in oncology practice. The aim of this study was to develop and validate a practical, EHR-based algorithm for reliably identifying PCP cases.
PATIENTS AND METHODS: This retrospective, single-center validation study used EHR d

Acute esophageal necrosis is a rare but often fatal condition characterized by diffuse black discoloration of the esophageal mucosa. While commonly associated with hemodynamic compromise, or infection, the pathogenesis is often multifactorial. We report a fatal case of acute esophageal necrosis in a man in his 50s with untreated long-standing hypertension and newly diagnosed end-stage renal disease. The patient presented with fatigue, loss of appetite, and respiratory distress and was admitted with pneumonia, pulmonary edema, and advanced renal dysfunction. After the initiation of hemodialysis

This case report describes a female in her late 80s who presented with progressive dyspnoea and marked hypoxia. Notably, the hypoxia was refractory to high-flow supplemental oxygen. Her medical history included myeloperoxidase (MPO) positive vasculitis, vitamin B12 deficiency with a baseline haemoglobin of 88g/dL and well-controlled hypertension. A venous blood gas showed a MetHb of 10.2% [< 2%], which was initially overlooked. She was admitted to the ward and remained refractory to supplementary oxygen. A subsequent arterial blood gas (ABG) revealed an oxygen saturation (SaO2) of 95% and a me

Pneumocystis jirovecii pneumonia (PCP) is a serious opportunistic infection in patients with systemic rheumatic disease (SRD). Although less common than in HIV infection, PCP in patients with SRD worsens rapidly and is associated with high mortality, highlighting the importance of early recognition and prevention. However, risk stratification, indications for PCP prophylaxis, and optimal dosing regimens remain uncertain in patients with SRD. This review summarizes current evidence on the risk factors for PCP in patients with SRD, particularly clinically relevant determinants such as disease-sp
