An interpretable radiomics-machine learning model for early risk stratification of invasive fungal infections in community-acquired pneumonia: a dual-center study.

To develop and validate an interpretable HRCT radiomics-based machine learning model for early risk stratification and identification of invasive fungal infection (IFI) in patients with community-acquired pneumonia (CAP). A total of 570 CAP patients who underwent HRCT from July 2022 to August 2024 in Center 1 and Center 2 were recruited. A VB-net pneumonia automatic segmentation algorithm was employed. Three models, a radiomics model (HRCT-derived radiomics features), a clinical model (clinical variables), and a combined model (integrating both), were developed. The performance of the model wa

Amplified inflammatory and immune responses in viral-associated pulmonary aspergillosis.

BACKGROUND: Viral-associated pulmonary aspergillosis (VAPA) is a severe complication of viral pneumonia (VP) that is associated with pronounced inflammatory amplification, immune dysregulation, and increased mortality. However, systemic metabolic and immune response patterns accompanying VAPA remain incompletely understood. METHODS: Plasma samples were obtained from 35 patients with viral pneumonia (VP group) and 20 with viral-associated pulmonary aspergillosis (VAPA group). An integrated multi-omics strategy combining data-independent acquisition (DIA)-based proteomics and untargeted metabol

Incidence, mortality and risk factors in COVID-19-associated pulmonary aspergillosis (CAPA): an umbrella review and meta-meta-analyses (2020-2025).

INTRODUCTION: COVID-19-associated pulmonary aspergillosis (CAPA) has emerged as a severe and potentially underrecognised complication in patients with SARS-CoV-2 infection, particularly in intensive care units (ICUs). Existing reviews show substantial heterogeneity in diagnostic criteria, incidence estimates, and reported risk factors, limiting reliable interpretation of the overall evidence base. MATERIALS AND METHODS: this umbrella review synthesised systematic reviews and meta-analyses published between 2020 and 2025 to clarify the incidence, mortality, and risk factors associated with CAP

Diagnosis of fungal respiratory infections in immunocompetent children.

Fungal respiratory infections are rare in immunocompetent children, and their recognition remains challenging since clinical features are often subacute and overlapping with other causes of respiratory disease. A structured diagnostic approach is fundamental to facilitating earlier diagnosis and prompt targeted therapy, thereby improving outcomes. Fungal spores are ubiquitous in the environment and fungal colonisation of skin and mucosal membranes is common, hence differentiation of infection from colonisation is key to an accurate diagnosis and a prudent approach to management. The diagnosis

Pneumocystis pneumonia in non-HIV immunocompromised patients in 2026: evolving therapeutic standards.

INTRODUCTION: Pneumocystis jirovecii pneumonia (PJP) remains a life-threatening lower respiratory tract infection associated with respiratory failure in patients with impaired T-cell immunity. Its incidence is increasing in non-HIV patients, who typically experience a more severe disease course and poorer outcomes than HIV patients. Keeping a high level of suspicion for PJP when confronted to an immunocompromised patient presenting with an acute respiratory failure (ARF) is essential to start promptly the treatment and thereby reduce associated morbidity and mortality. AREAS COVERED: The mech

Construction and validation of a predictive nomogram model for invasive fungal infections in sepsis patients with severe pneumonia in the ICU.

BACKGROUND: Invasive fungal infections (IFI) represent a serious complication in critically ill sepsis patients with severe pneumonia, contributing to increased mortality and prolonged hospitalization. Early prediction of IFI remains challenging due to the lack of specific clinical tools. This study aimed to develop and validate a predictive nomogram for IFI risk in this high-risk population. METHODS: A total of 1,890 sepsis patients with severe pneumonia admitted to the ICU of the primary center were retrospectively enrolled from Shaanxi Provincial People's Hospital. Patients were randomly d

Candidemia during the COVID-19 pandemic in a Brazilian tertiary care hospital: Comparison of episodes with and without COVID-19.

The epidemiology of candidemia changed during the COVID-19 pandemic, but direct comparisons of episodes with and without coronavirus disease 2019 (COVID-19) remain limited. We performed a retrospective cohort study of consecutive adult patients with candidemia diagnosed from February 2020 through February 2023 at a Brazilian tertiary-care teaching hospital. Demographic, clinical, microbiological, treatment, and outcome variables were compared according to COVID-19 status, and predictors of 30-day mortality were evaluated among treated patients. Among 106 episodes in 103 patients, 36 (34.0%) oc

Antifungal prophylaxis and preemptive therapy to prevent COVID-19-associated pulmonary aspergillosis in patients requiring advanced respiratory support: A real-world cohort study in China.

OBJECTIVES: COVID-19-associated pulmonary aspergillosis (CAPA) is a frequent and life-threatening complication in critically ill patients. However, the role of antifungal prophylaxis and preemptive therapy remains unclear. This study aimed to evaluate the effects and safety of these strategies. METHODS: This single-center, retrospective study included patients with COVID-19 who required advanced respiratory support (high-flow nasal cannula, non-rebreathing mask, mechanical ventilation, or extracorporeal membrane oxygenation) between November 2022 and December 2023. The primary outcome was the

Oral wash PCR for the diagnosis of Pneumocystis jirovecii pneumonia in non-HIV immunocompromised hosts: a diagnostic accuracy study.

We assessed the Pneumocystis jirovecii PCR accuracy on non-invasive oropharyngeal wash (OW) specimens for the diagnosis of Pneumocystis jirovecii pneumonia (PJP) in non-HIV immunocompromised patients. In this retrospective study, 134 patients with suspected PJP undergoing both OW and deep respiratory sampling were included. PJP was defined by composite criteria. OW P. jirovecii PCR showed a sensitivity of 61.5% (95% confidence intervals [CI] 42.5-77.6) and a specificity of 97.2% (95% CI 92.2-99.1). Positive and negative predictive values were 84.2% and 91.3%, respectively. These results sugges

Determinants of fungal infection and hospital readmission risk in interstitial pneumonia with autoimmune features: associations with vitamin D and pirfenidone.

BACKGROUND: Fungal infections significantly compromise the prognosis of patients with interstitial pneumonia with autoimmune features (IPAF). However, the specific immune-related risk factors and their impact on clinical stability remain poorly defined. This study aimed to identify independent predictors for fungal infection and early readmission to optimize risk stratification. METHODS: We conducted a retrospective analysis of 98 patients meeting the 2015 European Respiratory Society/American Thoracic Society (ERS/ATS) IPAF classification criteria. Fungal infections were confirmed through cl

Paradoxical Role of Glucocorticoids in Severe Pneumocystis jirovecii Pneumonia Among Renal Transplant Recipients: Case Series.

BACKGROUND Severe Pneumocystis jirovecii pneumonia (PJP) in renal transplant recipients (RTRs) can rapidly progress to acute respiratory distress syndrome (ARDS) and is associated with high mortality. Glucocorticoids (GCs) play a paradoxical role, constituting a risk factor for infection and a trigger for immune reconstitution inflammatory syndrome upon withdrawal; they may also serve as a therapeutic agent for lung injury. We evaluated the efficacy of a standardized triple-therapy regimen designed to address this paradox. CASE REPORT We analyzed 7 RTRs admitted to the intensive care unit (ICU

Epidemiology and outcomes of invasive fungal infections in patients with mature T-cell and NK-cell lymphomas.

BACKGROUND: Invasive fungal infection (IFI) causes poor outcomes in haematological malignancies, but data in mature T-cell and NK-cell lymphomas (T/NKCL) are scarce. We aimed to define epidemiology and outcomes of IFI in this population. MATERIALS AND METHODS: This retrospective study enrolled adult patients with T/NKCL between 2019 and 2024. Proven and probable IFI were defined according to the 2020 EORTC/MSGERC criteria. Multivariable logistic regression was used to identify factors associated with IFI and death. RESULTS: Among 203 patients, 43 (21%) developed 52 IFI episodes, including in

Integrated diagnostic strategies for Pneumocystis pneumonia: bronchoalveolar lavage qPCR cycle threshold and serum β-D-glucan.

OBJECTIVES: Pneumocystis jirovecii pneumonia (PCP) is a severe opportunistic infection increasingly affecting HIV-uninfected immunocompromised patients. Although bronchoalveolar lavage fluid (BALF) PCR is highly sensitive, distinguishing infection from colonisation remains challenging. We evaluated the diagnostic performance of BALF quantitative PCR (qPCR) cycle threshold (Ct) values (BioGX, BD MAX) and an integrated diagnostic strategy combining qPCR and serum β-D-glucan (BDG) (Toxinometer MT-6500 system). METHODS: We conducted a retrospective diagnostic accuracy study including all BALF P.

The Role of 1-3-Beta-D-Glucan in Predicting Pneumocystis jirovecii Pneumonia in Immunocompromised Children.

Serum 1,3-β-D-glucan (BDG) showed high negative predictive value (~99%) for Pneumocystis jirovecii pneumonia (PJP) in 106 non-human immunodeficiency virus immunocompromised children, but modest positive predictive value at low prevalence. These exploratory findings suggest BDG may help exclude PJP, pending prospective validation.

X-linked hyper-IgM syndrome presenting as severe Pneumocystis pneumonia in a 6-month-old infant: a case report.

BACKGROUND: X-linked hyper-immunoglobulin M (XHIGM) syndrome is a rare primary immunodeficiency caused by mutations in the CD40 ligand gene (CD40LG), characterized by defective T-cell-dependent B-cell class-switch recombination. Patients typically present with recurrent infections in early childhood, but diagnosis is often delayed due to heterogeneous clinical manifestations and the fact that serum IgM levels may remain within the normal range in a significant proportion of patients. Here we report a case of XHIGM in an infant whose diagnostic journey began with recurrent lymphadenitis and cul

Unanticipated global emergence and evolution of Pneumocystis jirovecii mutations and resistance driven by a human-targeted drug.

Nearly all known cases of antimicrobial resistance have been linked to agents that directly target microbes. This paradigm overlooks other drivers of microbial evolution and resistance. Here, we report compelling evidence of resistance-associated mutations in Pneumocystis jirovecii, a high-priority fungal pathogen designated by WHO as having unknown resistance. These mutations emerged under selective pressure from mycophenolic acid (MPA), a widely used immunosuppressant not used for Pneumocystis pneumonia (PCP) treatment. A potent inhibitor of inosine monophosphate dehydrogenase (IMPDH), MPA h

Immune dysregulation in Pneumocystis jirovecii infection and the development of a machine learning-based diagnostic model.

BACKGROUND: Pneumocystis jirovecii pneumonia (PJP) is a life-threatening opportunistic infection in immunocompromised patients. A major clinical challenge is the inability of current microbiological diagnostics (e.g., quantitative polymerase chain reaction [qPCR]) to reliably distinguish PJP from Pneumocystis jirovecii colonization(P. jirovecii colonization). OBJECTIVE: This study aimed to develop and validate a diagnostic model for differentiating PJP from P. jirovecii colonization by analyzing 17 core model input predictors that integrate pathogen burden and host immune response profiles.

Aerosolized Pentamidine Is an Effective Prophylaxis for Pneumocystis jiroveci Pneumonia in Adults Undergoing Allogeneic Hematopoietic Cell Transplantation.

Male infertility (MI) and male sexual dysfunction (MSD) can often coexist together due to various interplay factors such as psychosexual, sociocultural and relationship dynamics. The presence of each form of MSD can adversely impact male reproduction and treatment strategies will need to be individualized based on patients' factors, local expertise, and geographical socioeconomic status.
