Coexistence of pulmonary aspergillosis and cryptococcosis following treatment for SARS-CoV-2 infection in a kidney transplant recipient: a rare case report and literature review.

BACKGROUND: Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2)- associated pneumonia increases patients’ susceptibility to fungal superinfections, particularly among immunocompromised individuals. CASE PRESENTATION: A 60-year-old woman with history of kidney transplantation was admitted for recurrent fever and positive SARS-CoV-2 antigen on pharyngeal swab testing. The patient developed rapid-onset respiratory distress secondary to progressive pulmonary infection and received assisted mechanical ventilation. Microbiological sequencing and culture of bronchoalveolar lavage fluid and s

Invasive fungal infections after Bruton tyrosine kinase inhibitor treatment: a systematic review and meta-analysis.

The epidemiology of invasive fungal infections (IFIs) among patients receiving Bruton tyrosine kinase inhibitors (BTKIs) remains incompletely characterized. We conducted a systematic review and meta-analysis of 88 studies including 23 737 patients to evaluate the prevalence and risk factors for IFIs in this population. Among 16 studies that applied the European Organization for Research and Treatment of Cancer and the Mycoses Study Group Education and Research Consortium consensus definitions of IFIs, the pooled prevalence of proven/probable IFIs was 2.83% (95% confidence interval [CI], 1.97-4

The dove of peace is not peaceful : the ghost of aortic arch pseudoaneurysm.

BACKGROUND: Infectious pseudoaneurysms are rare, and those caused by Cryptococcus, an organism commonly found in pigeon feces, are exceptionally uncommon. This case highlights this severe vascular complication as a sequel of cryptococcal infection. CASE PRESENTATION: A 71-year-old male with a history of pigeon exposure presented with a persistent fever and was diagnosed with cryptococcal pneumonia and bacteremia. After successful initial antifungal treatment, a follow-up chest CT scan three months later revealed a pseudoaneurysm at the aortic arch. The patient underwent successful endovascular

Molecular Mechanisms of Mucormycosis Pathogenesis: Host-Pathogen Interactions and Immune Evasion.

Mucormycosis, triggered by fungi of the order Mucorales, represents a potentially fatal invasive mycosis, with death rates over 50% despite intensive therapy. The COVID-19 pandemic brought a sharp increase in cases, especially in individuals with diabetes mellitus and those undergoing immunosuppressive treatment, emphasizing significant gaps in our comprehension of disease pathogenesis. Emerging molecular studies have highlighted key virulence factors, such as the CotH family of invasins that facilitate endothelial invasion via interaction with glucose-regulated protein 78 (GRP78), complex iro

Fungal infections in diabetes mellitus.

PURPOSE: Diabetes mellitus is recognised as a global health problem and has increasingly been associated with fungal infections, as an independent risk factor. Diabetic patients are predisposed to both superficial as well as invasive fungal disease, and account for substantial morbidity and mortality. Recent pandemic of COVID-19 underlined the global problem of mucormycosis, however, the burden of fungal infections among diabetics has a much broader horizon. This review aims to summarise the spectrum of fungal infections encountered among diabetic patients, along with elucidating immunopathoge

Post-SARS CoV-2 infection laryngeal mucormycosis: a rare ail mimicking malignancy.

Laryngeal mucormycosis is an uncommon form of fungal laryngitis that typically affects immunocompromised individuals. We report a rare such case in a male in his 40s who had recently recovered from severe acute respiratory syndrome-coronavirus disease-19 (SARS CoV-2 infection-COVID 19) and presented with a 1-month history of worsening hoarseness, haemoptysis, and noisy breathing. A chewing gum-like subglottic necrotic mass was biopsied. The rarity of the diagnosis was aided by histopathological and microbial study of the mass, which confirmed mucormycosis. Management included surgical debulkin

Acute pulmonary histoplasmosis in a Belgian traveller returning from Suriname diagnosed with NGS: a case report.

We describe the case of a 48-year-old Belgian woman who developed fever, dry cough and dyspnoea shortly after returning from a two-week trip to Suriname. CT scan of the thorax revealed 2 nodular masses with surrounding ground glass alterations in the right lung as well as multiple hilar and mediastinal lymphadenopathies. Bronchoscopy with broncho-alveolar lavage and endobronchial ultrasound-guided transbronchial needle aspiration of thoracic lymph nodes were performed. Broncho-alveolar lavage fluid showed no abnormalities, including a negative PCR for Aspergillus spp., and negative cryptococca

Histoplasmosis with mediastinal lymphadenopathy in an immunocompetent municipal water department employee in Texas: a case report.

BACKGROUND: Histoplasmosis is an endemic mycosis that primarily affects the lungs, commonly found in the central and eastern United States, particularly the Ohio and Mississippi River valleys. While disseminated histoplasmosis (DH) is typically associated with immunocompromised individuals, rare cases have been reported in immunocompetent hosts following high-risk environmental exposures, particularly in high-risk occupational settings. CASE PRESENTATION: We report a case of a 22-year-old immunocompetent male employee of a municipal water department in Texas who presented with fever, pleuriti

Histoplasmosis in kidney transplant recipients.

Histoplasma capsulatum var. capsulatum is a thermally dimorphic fungus endemic in regions like the Midwestern USA, Africa, and Central/South America. It grows as a mold in the environment and as yeast in human tissues. The disease's severity and presentation depend mostly on the host's immunity. In immunocompromised individuals, it often causes progressive disseminated histoplasmosis (PDH), a severe systemic condition. PDH symptoms include pneumonia, enlarged spleen and liver, enteritis, pancytopenia, lymphadenopathy, skin and mucosal lesions, and joint pain, making the diagnosis a challenge.

Temporal trends and clinical outcomes of pediatric invasive fungal diseases: a ten-year retrospective study from a tertiary-care center in Thailand.

BACKGROUND: Invasive fungal diseases (IFDs) remain an important cause of morbidity and mortality among immunocompromised children. Changes in diagnostic criteria and increasing use of antifungal prophylaxis may influence the epidemiology and clinical presentation of pediatric IFDs. Longitudinal data from Southeast Asia remain limited. This study aimed to characterize temporal trends, clinical features, and outcomes of pediatric IFDs using the 2020 EORTC/MSGERC criteria. METHODS: We retrospectively reviewed children aged 0-15 years diagnosed with IFDs at a tertiary-care hospital in Bangkok, Th

[Invasive mycoses in France: what are the trends?].

The most frequent invasive mycoses in France are candidemia, Pneumocystis jirovecii pneumonia, and invasive aspergillosis. They occur primarily in immunocompromised patients. The risk of Pneumocystis pneumonia and cryptococcosis has notably decreased for people living with HIV through the past two decades. Patients with diabetes or an autoimmune disease represent an increasing proportion of these cases. Mortality of invasive mycoses is still high, particularly in cases of candidemia and invasive aspergillosis. Azole-resistant Aspergillus spp. isolates remain rare in France and the proportion o

Serious Fungal Diseases: Assessing the Impact of Training Clinicians and Laboratory Scientists in Nigeria.

BACKGROUND: Surveillance for serious fungal diseases (SFDs) relies on the capacity to recognize them and request and accurately perform diagnostic investigations. However, knowledge in resource-limited settings is suboptimal. This study evaluated the impact of a training intervention on the diagnosis and management of SFDs among clinicians and laboratory scientists in Nigerian tertiary hospitals. METHODS: Clinicians in 6 hospitals were purposively selected and trained in 6 SFDs using standardized modules developed by international medical mycology experts. A quasi-experimental design assessed

Immunocompromise and early-onset invasive pulmonary aspergillosis in viral pneumonia: a retrospective cohort study.

BACKGROUND: In the context of viral pneumonia, immunocompromised status represents a recognized risk factor for invasive pulmonary aspergillosis (IPA), its association with the timing of IPA diagnosis remains unclear. METHODS: In the present study, 261 patients hospitalized with viral pneumonia were consecutively enrolled and categorized as immunocompromised hosts (ICHs) or non-ICHs. Baseline characteristics, outcomes, and time to IPA diagnosis were compared. Cox regression was used to evaluate the association between immunocompromised status and adverse outcomes. Patients diagnosed with IPA

Candida in the lower respiratory tract induces barrier disruption in mice and predicts poor outcomes in mechanically ventilated humans.

Candida albicans (Calb) in the lower respiratory tract (LRT) is considered a rare cause of pneumonia, yet its frequent recovery from LRT secretions of mechanically ventilated patients associates with poor clinical outcomes. To determine whether Calb contributes to lung injury, we conducted a translational investigation spanning two independent, prospective human cohorts, murine models of lung injury, and in vitro assays. In critically ill patients, Calb was the most abundant fungus in LRT specimens, and its detection associated with increased markers of lung injury, prolonged mechanical ventil

Amphotericin B promotes respiratory viral entry by enhancing late endosomal maturation and fusion via glucocerebrosidase-mediated ceramide remodeling.

Respiratory viral infections, such as influenza and COVID-19, pose significant global health challenges. For patients with invasive pulmonary aspergillosis, a subsequent viral infection can lead to markedly worse clinical outcomes. Although amphotericin B (AmB) remains a cornerstone antifungal therapy, our investigation demonstrates that it paradoxically enhances the entry of influenza A virus and SARS-CoV-2. Mechanistically, AmB directly binds to and activates glucocerebrosidase, leading to ceramide accumulation and RAB7 upregulation in the late endosomes, thereby enhancing late endosomal mat

Invasive pulmonary aspergillosis among ICU patients with COVID-19, influenza, and preexisting host factors: a nationwide cohort study.

BACKGROUND: Previous studies on the association between coronavirus disease 2019 (COVID-19) and invasive pulmonary aspergillosis (IPA) were limited by their single-center nature and small patient population. Whether aspergillosis risk is related to COVID-19 or other factors remains unclear. This study aimed to determine whether COVID-19 among intensive care unit (ICU) patients is associated with an increased incidence of IPA compared with that among individuals with influenza or preexisting host factors (control). METHODS: This was a nationwide study performed using the Korean National Health

Effects of Antifungal Treatment in COVID-19-Associated Pulmonary Aspergillosis: A European Multicenter Cohort Study.

BACKGROUND: The management of COVID-19-associated invasive aspergillosis (CAPA) is still debated while cases continue to occur, and more and more frequently in vulnerable populations, stressing the importance of obtaining data on the treatment of this disease. Only small cohort studies and case reports have yet discussed this essential issue, and the data obtained are insufficient to make conclusions with confidence. RESEARCH QUESTION: Is antifungal treatment associated with lower 60-day mortality in patients with probable or proven CAPA? STUDY DESIGN AND METHODS: This study assessed the ass

Fatal AA-like bone marrow failure and invasive pulmonary aspergillosis after long-term pembrolizumab in squamous NSCLC: a case report.

Immune checkpoint inhibitors (ICIs) such as pembrolizumab have substantially improved outcomes in advanced non-small cell lung cancer (NSCLC), including squamous histology, but prolonged exposure may be complicated by immune-related adverse events (irAEs) and opportunistic infections. We report a 58-year-old man with advanced squamous NSCLC who achieved durable tumor control after six cycles of pembrolizumab plus platinum-based chemotherapy, followed by pembrolizumab maintenance monotherapy (18 cycles). During the later course, he developed severe bacterial pneumonia, invasive pulmonary asperg

DECTIN-1 stalk length determines binding ability, immune response, and survival in a mouse model of coccidioidomycosis.

Coccidioidomycosis is a leading cause of pneumonia in the endemic area of the Sonora desert and the central valley of California. The causative agent are the fungi Coccidioides immitis and C. posadasii. While most infections ultimately resolve without treatment, some infections spread to the periphery and cause serious and even fatal illness. Much work has shown that a T cell response with production of interleukin-17A and interferon ɣ is critical. Less work has focused on innate responses that ultimately set the stage for the adaptive response. Genetic analysis in both mice and man have shown
